This document discusses hospital pharmacy practices in Bangladesh and other developed countries. It begins by defining hospital pharmacy as dealing with procurement, storage, compounding, dispensing, manufacturing, testing, packaging and distribution of drugs within a hospital setting. It then outlines the objectives, functions, ideal location and structure of a hospital pharmacy. The current situation of hospital pharmacy in Bangladesh is described as still being in its early stages. Key differences between hospital and community pharmacies are provided. Finally, examples of developed hospital pharmacy practices in countries like the US, UK, Norway and Japan are contrasted with the slower implementation in developing nations like Bangladesh.

The prostate is an exocrine gland of the male mammalian reproductive system
It is a walnut-sized gland that forms part of the male reproductive system and is located in front of the rectum and just below the urinary bladder
Function is to store and secrete a clear, slightly alkaline fluid that constitutes 10-30% of the volume of the seminal fluid that along with the spermatozoa, constitutes semen
A healthy human prostate measures (4cm-vertical, by 3cm-horizontal, 2cm ant-post ).
It surrounds the urethra just below the urinary bladder. It has anterior, median, posterior and two lateral lobes
It’s work is regulated by androgens which are responsible for male sex characteristics
Generalised disease of the prostate due to hormonal derangement which leads to non malignant enlargement of the gland (increase in the number of epithelial cells and stromal tissue)to cause compression of the urethra leading to symptoms (LUTS

MANAGEMENT OF ATRIOVENTRICULAR CONDUCTION BLOCK.pdfJim Jacob Roy
Cardiac conduction defects can occur due to various causes.
Atrioventricular conduction blocks ( AV blocks ) are classified into 3 types.
This document describes the acute management of AV block.
Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

6. What should be the Location of Hospital Pharmacy?
➢ Located in Hospital premises so that patient and stuff can easily approach
it.
➢ In multi-storeyed building of a hospital, the pharmacy should be
preferably located on ground floor especially the dispensing unit.
➢ It should be located in such a way that there is continuous flow of men
and materials.
Structure of a Hospital Pharmacy:
7. REQUIREMENTS AND ABILITIES
Hospital pharmacy is an important department of any hospital. It should have
competent persons. Well trained in the profession of pharmacy. Hospitals mainly
depend upon the services of diploma holders in pharmacy. The head of hospital
pharmacy department should be a post-graduate in Pharmacy preferably in
Pharmaceutics, Pharmacology or Hospital Pharmacy. He acts as a coordinator for
the pharmacy and non-pharmacy staff in the hospital. He reports to the
administrator and interacts with other medical departments.
Current Situation of Hospital Pharmacy Practice in Bangladesh:
In the recent year, Hospital Pharmacy Practice has become an important part in
the health care system worldwide. In Bangladesh, this system is still in the primary
stage. This current work reveals the real scenario of the Hospital Pharmacy
Practice in Bangladesh. To evaluate the actual condition, repeated visiting was
done personally with proper authority those who are specialist and responsible
for conducting the hospital pharmacy practice in some selected private hospitals
of Bangladesh. By the survey, data was collected about the services, facilities and
the problems of Hospital Pharmacy. This system is very much essential for the
proper health care system of the country and also for the employment of large
number of competent pharmacists. So, it is the time for the pharmacist, health
professional, entrepreneurs and government to come forward to establish
standard Hospital Pharmacy setting throughout the country.
8. Here is a chart which gives information about a research on current situation
of hospital pharmacy of four hospitals situated at Bangladesh.
Hospital A & B are situated in Dhaka, C is in Bogra & D is in Rajshahi.Among
them only D was government hospital and the rest are Privately owned.

9. What is the difference between Hospital Pharmacy & Community Pharmacy?
Although many of the tasks of a hospital pharmacist are similar to a community
pharmacist who works in a community setting, there are some distinctive differences.
These include:
• Increased interaction with prescribers and other health professionals
• Greater input in prescribing decisions about drugs and administration
• Larger team of pharmacists working together in the same institution
• Better access to medical records of patients
However, the patient contact for hospital pharmacists is often much less when
compared to a pharmacist working in the community. Hospital pharmacies also
usually stock a large variety of medications, including those for specialized purposes
that may only be indicated in a controlled medical setting.
Hospital Pharmacy in Developed Countries: Developed countries such as USA, UK,
Switzerland, Norway, UAE have realized the importance of pharmaceutical care service delivery in
hospitals and implemented clinical pharmacy programs. Empirical studies reveal that
implementation of clinical pharmacy services reduce the length of hospital stays and produce
significant cost savings, both medication costs and total inpatient expenditures. These developments
have the potential to lead to improved patient quality of life, better use of healthcare resources and
a reduced rate of mortality.
IN USA There are about 60,000 full-time equivalent (FTE) pharmacists providing
inpatient services in nonfederal short-term hospitals. 17 (Hospitals employ
approximately an equal number of pharmacy technicians.) About one-fourth of all
10. actively practicing pharmacists in the United States work in hospitals. In 1957 the
total number of hospital pharmacists were 4850 full-time.
IN Norway, there are 902 community pharmacies and 32 hospital pharmacies.
In CHINA, hospital pharmacies have the responsibilities to guarantee the safe and
effective use of drugs within the hospital and to do research. Even though Chinese
factories manufacture many pharmaceutical products, the hospital pharmacy must
manufacture many more. Hospitals have established Agreed Prescription programs,
whereby drugs are manufactured in large batches, repackaged, and distributed to
other hospital pharmacies in the area if needed.
In JAPAN, Japanese Society of Hospital Pharmacists (JSHP), is an organization whose
membership comprises pharmacists and other professionals who work at hospitals, clinics
and long-term care facilities. The JSHP carries out a variety of activities with the aim of
contributing to the promotion of the health and welfare of patients and the general public,
not only through the members’ voluntary efforts to enhance their professional skills and
knowledge, but also through the provision of high-quality pharmacotherapy.
Here is a list of Number of Hospital Pharmacies in Developed Countries:
11. Conclusion:
Hospital pharmacists are a vital part of the healthcare team. Working in either
the NHS or private hospitals, being a hospital pharmacist means you’re part of
a team where the focus is firmly on patients. Hospital Pharmacy is an important
subject for any country. Hospital pharmacy maintains health care facilities at a
higher level. Without hospital pharmacy a country can’t progress that much in
healthcare department. There is good implementation of hospital pharmacy in
Developed countries though developing countries are a bit slower. Many of the
developing countries don’t even know about it. Implementation of Hospital
pharmacy in developing countries are a bit slow and under developed countries
don’t even think about it but on Developed countries there is a better
implementation of Hospital pharmacy.
