3. Introduction
•Compilation of Selected preparation by Hospital is called a Hospital formulary
•It reflect current clinical judgment of medical staff.
•It provides for obtaining, prescribing, dispensing, and administering both branded and
generic drugs.
•In Indian hospital formulary provide by N.F.
• In some Hospital do not favour a Formulary system.

4. Define formulary
It is a continually revised compilation of pharmaceuticals & medicine used in hospital by medical
staff that reflects clinical knowledge.
OR
It is important document of the hospital containing a collective list of drugs which is used by
interne & fellow doctors reflecting current clinical judgement.
Reason for need of it
1)It outline the purpose of organization, functions & scope of hospital formulary
2) It provide guidance regarding selection, procurement, storage, distribution, use, safety procedure
& other matterrelating to drugs in hospital
3) It provides the staff the basic therapeutic information about each approved drug product.
4) It delivers the information on hospital policies & procedure pertaining to use of drug.
5) It provides special information about drug dosing, rules & abbreviations used in a hospital.
6) It provide information about antidote, emergency situation
7) It acts as guide for pharmacist, nursing staff & doctors.
5. Guiding Principles while Using a Formulary
1. The medical staff of the hospital shall appoint a P and T committee and outline its scope,
purpose, organization , and function.
2. The formulary system will be sponsored by medical staff.
3. The medical staff shall adopt the written policies and procedures of the formulary system
.
4. Drug should be included in the formulary by their nonproprietary names and should be
prescribed by the same.
5. When there is no formulary then the pharmacist has to follow physician’s prescription
.They can consult the physician when the prescribed brand is not available
6.The management of the hospital shall inform all the medical and nursing staff about the
existence of the formulary system, procedure of operation.
7. Provision shall be made or the use of drugs not included in the formulary, by the medical
staff.
8. The pharmacist shall be responsible for specification as to the quality, quantity, and
source of supply of all drugs used in the diagnosis and treatment of patients.
9. Limiting the no of drugs available from pharmacy can produce proper patient care and
financial benefits. These benefits can be greatly increased by using generic equivalents
6. The first step in the development of a formulary Is to form PTC.
Then they decide on the following.
1.What type of publication is most suitable for the hospital.
a. A hospital own formulary or
b. A simple list of drug or
c. A purchase formulary service
2. Fixation of rules by PTC for admission of formulary.
3. If formulary is to be prepared, decision must be taken on its content.
a .Rx writing
b. Use of drugs.
c. Table of metric weights and apothecary and household equivalents.
d.Section on calculation of doses for children.
e. pharmacological index
4. The type of format of the formulary.
a. Size
b. Loose leaf or bounded
c. Printed or mimeographed.
d. Extent of categorization and indexing
How is it Prepared ?
7. Contents of the Formulary:
1)Information of drug product: Include list of branded & generic drugs
With their formulation, dosage strength, route of administration, cost information etc.
2) Index to drug product listing: a) For branded & generic drugs index- Proper reference to
page number in each entry.
b) Therapeutic/ Pharmacological index—According to Therapeutic category.
3) Name & titles of members of PTC.
4) Details of hospital policies & procedure regarding drug use, & request for entry of new drug in
formulary.
5) List of approved product used in hospital.
6) Prescription writing-important section for young physicians.
7) Appendix: a) Norm gram of Body weight, height, BP. b) Posological table. c) List of items from
CSS d) List of poisons & their antidotes e) List of emergency drugs f) Tables of DI. g) ADR repor
h) Pharmacokinectic dosing & monitoring information. i) Metric conversion scales & tables.
8. Guiding principles for Admission or deletion of drugs in the formulary
Following criteria should be taken into consideration for admission or deletion of drugs in Formula
1.Whether the medical staff considers the drug to be of proved clinical value based on their
experience with it.
2. Drug must be recognized by the Pharmacopoeias and formularies approved under Drugs And
Cosmetic Act and Rules there under.
3. The mfg of the drug should have the license under Drug and Cosmetic rules and has not been
punished for any serious offence under any law of drug and medicines.
4. No drug or preparation of secret composition will be admitted in the formulary
5. No drug or preparation containing many ingredients shall be admitted if the similar therapeutic
effect can be obtain by the use of single ingredient preparation
