This is the first installment of the hitchhiker's guide to Health Level Seven. When complete this presentation will provide a concise overview of the history, operational framework, and standards of Health Level Seven (HL7). It is intended to be a guide to those seeking to engage in the HL7 standards development effort or to be consumers of HL7 products and services.
The presentation is being written and posted in five iterations. This particular installment introduces HL7 the organization and HL7 the portfolio of health informatics standard.
FHIR is the latest standard to be developed under the HL7 organization. Pronounced 'Fire' , FHIR stands for Fast Healthcare Interoperability Resources. I think it's the most interesting standard to have come out of HL7 since the original HL7 protocol.

Abstract HL7 (Health Level 7) is a standard developed for
exchanging incompatible healthcare information generated
from programs or devices among heterogenous medical
information systems. At present, HL7 is growing as a
global standard. However, the HL7 standard does not
support effective methods for treating data from various
medical sensors, especially from mobile sensors. As
ubiquitous systems are growing, HL7 must communicate
with various medical transducers. In the area of sensor
fields, IEEE 1451 is a group of standards for controlling
transducers and for communicating data from/to various
transducers. In this paper, we present the possibility of
interoperability between the two standards, i.e., HL7 and
IEEE 1451. After we present a method to integrate them
and show the preliminary results of this approach.

Introduction
HL7 is a messaging standard for exchanging medical
information that is becoming a world standard. Among
medical information such as a text or an image, HL7 deals
with text information (HL7 Organization. [http://www.hl7.
org]; HL7 Version 2.5.1 Messaging Standard). Recently, as
the ubiquitous era is approaching, a new type of informa-
tion, i.e., streaming data, has appeared. These data usually
come from sensors from various networks such as wireless
LANs, wired LANs, or cellular networks and increasingly
have important role in e-healthcare, u-healthcare, and other
health systems. Since various wireless networks are widely
spread and the performances of many devices such as
PDAs, notebooks, or cellular phones improve, the use of
sensors for patients will become popular as well, and thus
the quality of life for patients will improve. Sooner or later,
HL7 should include information from mobile patients,
mobile devices, and mobile sensors.
In the area of electrical and mechanical engineering,
sensors have been used for a long time to check and
monitor the status of machines and devices. In these areas,
there has been a group of standards IEEE 1451 which deal
with various aspects of sensors, such as the definition of
sensors and actuators, the format of data sheets, and how to
connect and disconnect the sensors from a system. Also,
these standards deal with communication protocols. Re-
cently, the advent of various sensors such as wireless
sensors has led the IEEE to develop a complete set of
standards that deal with these types of sensors [1–5]. The
family of IEEE 1451 standards will become a global
standard in all areas that use sensors.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.
