Evaluation of Breast Cancer in Reference to Skin ChangesQUESTJOURNAL
Introduction:- Breast cancer is the most commonly occurring female cancer in the world which is more than double that of the second ranked cancer i.e. cervical cancer. Breast cancer accounts for 23% of all cancer deaths. It is the most frequent cancer death in developing countries of the world. Mammary skin changed in breast carcinoma is categorized as advance stage in breast cancer classification. In the present study we evaluated the correlation of macroscopic, microscopic and no skin changes with axillary lymph node using histologic factor dermal lymphatic involvement. Materials and methods: prospective study was conducted on 42 breast cancer admitted patients of different age groups. Based on degree of skin involvement patients were placed into four groups, i.e. clinical stageT1 toT4. All groups were compared on the basis of percentage of patients involvement according to T1, T2, T3 and T4 stage, tumor size, histopathological dermis and epidermis involvement, dermal lymphatic invasion, tumor size and tumor subtype. Results: Majority of the patients with skin (dermis and epidermis) infiltration by the tumor (94.4%) were of T4 stage (along with dermal lymphatic invasion). Majority of the patients with only dermal lymphatic involvement (87.5%) without dermis and epidermis infiltration were of T2 stage. Conclusion: In our study, most of the patients of stage T2 and T3 with dermal lymphatic invasion had involved node when studied by routine histologic technique even though they were not clinically palpable. The identification of characteristics of the primary tumor like dermal lymphatic invasion that are associated with nodal metastases should encourage the surgeon to perform a more extensive axillary lymph node dissection and the pathologist to use methods of examining the nodes that increase the likelihood of finding metastatic disease. From this we can conclude that patients in early stage breast cancer may also have metastatic axillary lymph nodes. Dermal lymphatic invasion may be regarded as the precursor of nodal involvement, and all patients with nodal involvement can be assumed to have lymphatic invasion in the primary tumors. However the converse may not be true, not all patients with lymphatic invasion have nodal involvement.

Ovarian cancer is relatively common but serious and has a poor prognosis. The aim of this study is to highlight the epidemiological, diagnostic, therapeutic and evolutionary aspects of this malignant pathology managed at the Bejaia university hospital center. This is a retrospective and descriptive study over a period of 3 years (2019 - 2022) carried out on 20 patients who developed ovarian cancer. The average age of the patients was 50 years old, 53.23% of whom were over 45 years old. The CA-125 blood test was positive in 18 out of 20 patients. The tumors were discovered on ultrasound in 87.10% of cases and at laparotomy in 12.90%. Total hysterectomy with bilateral adnexectomy was the most performed procedure (64.52%). The early postoperative course was simple. 15 patients underwent second look surgery (16.13%) for locoregional recurrences. Epithelial tumors were the most frequent histological type (93.55%), including 79% in the advanced stage ( IIIc -IV) and 21% in the early stage (Ia- Ib ). Adjuvant chemotherapy was administered in 80% of patients. With a median follow-up of 36 months, 2 patients were lost to follow-up. The evolution was favorable in 27.42% and in 25.81% deaths occurred late postoperatively. Ovarian cancer is not common but serious given the advanced stages and the high rate of late postoperative deaths which were largely observed in patients deprived of adequate neoadjuvant or adjuvant chemotherapy.

Survival Analysis of Determinants of Breast Cancer Patients at Hossana Queen ...Premier Publishers
Breast cancer is one of the most severe diseases in the world and become the public’s ever day’s agenda in both developed and developing countries. The primary goal of this study was to identify the determinants of survival time of breast cancer patients at Hossana hospital, south Ethiopia. Kaplan-Meier estimation method and a new two-parameter probability distribution called hypertabastic are introduced to model the survival time of the data. A simulation study was carried out to evaluate the performance of the hypertabastic distribution in comparison with popular distribution with the help of R and SAS statistical software Packages. One-fourth (25%) of the total patients survived for only 2 days. 31(35.2%) were censored, and 55(62.5%) were died. Hypertabastic survival model was found to be best fitting to the breast cancer data and age, level of education, family history, breast problem before, High fat diet, child late age, early menarche, late menopause were significant risk factors for the death of breast cancer patients. Awareness has to be given for the society on causes of breast cancer and screening test and early detection policies for most risky groups has to be established.

Adenocarcinoma of the pancreas (PDAC) accounts for 2.4% of all cancers diagnosed and is the fourth leading cause of cancer death, with almost equal rates of incidence and mortality [1]. By 2030, pancreatic cancer is projected to be the second leading cause of cancer-related death [2], surpassing breast, prostate and colorectal cancer. The overall survival at 5 years of around 7.2% as the majority of patients present with advanced disease at diagnosis. Patients with localized disease are treated with surgery, with or without neoadjuvant chemotherapy/ radiotherapy, followed by adjuvant chemotherapy. The majority (around 80%) of patients are treated only with chemotherapy as they have an advanced disease. Patients are treated in the first line with gemcitabine-abraxane or Folfirinox and with Naliri plus 5FU in the second line. There have been few clinical advances in PDAC treatment over the last 20 years and chemotherapy is the only treatment option available for the majority of patients. These tumours are also resistant to many targeted therapies such as anti-EGFR therapy like cetuximab [3] due to the presence of a KRAS mutation in the majority of primary tumors. Personalized medicine strategies have not yet been established in pancreatic cancer as in other more common tumour types. Thus, novel anti-tumour strategies are an important clinical need in order to improve survival rates.

The current study was carried out in District Abbottabad aimed to determine the common urinary
tract infections in local community to determine the epidemiology of significant diseases in asymptomatic patients
of renal disorder. In this study a total of 1000 urine samples were examined during 3rd February to 1st April 2015
from patients attending Ayub Teaching Hospital Abbottabad by using dipstick and microscopic analysis of urine.
There were 638 females and 362 males patients examined during this period. The range of age groups is between
1.5 years to 80 years. Results of this study was reported as Pyuria 11%, Proteinuria 21.1%, Hematuria 10.4%,
Epithelial Cells 8.2%, pH 7.8 %, Granular casts 7.3%, Triple phosphate 6.6%, Calcium oxalate 6.4%, Glycosuria
6.3%, Bacteria 6.2% and mucous 4.1%. This study concludes that routing urinalysis should be performed for all
individuals to diagnose the asymptomatic diseases that will help in simple therapeutic measurements as urinalysis
is a simple step to determine the root of Urinary tract disorders.

N.I.C.U. WITH gestational age < 36 weeks with respiratory complaints were enrolled. Chest x-ray was
obtained within few hours of admission and lung sonography was performed within 24 hours. Follow – up

Aim: To study the clinico-haematological profile malaria in a rural hospital of Tripura.
Material and methods: A cross-sectional hospital-based study was done from at Kulai District
Hospital,Tripura. This hospital based cross sectional study was done on 60 confirmed cases of falciparum
malaria (either by peripheral smear or rapid diagnostic test) admitted in Kulai District Hospital. A case sheet
proforma was prepared and data (demographic profile,clinical feature, investigation, treatment, and
complication) from all indoor patients was collected and analyzed.
Result: Out of 60 patients, 40(66.6%) were males and 20 (33.4%) were females. Most of the patients were
between the age group 21-40 years with the highest prevalence between the age group of 21-30. Fever was the
most common symptom. Anemia was present in 42(70%) patients, out of which 6(10%) patients had severe
anemia. Thrombocytopenia was present in 36(60%) patients.Abnormal liver function tests were observed in
26(43.3%) subjects while abnormal kidney function tests were observed in16(26.6%) patients. All the 60
patients received Artemisinin based antimalarial drugs.
Conclusion: Early detection, prompt management, and adequate supportive therapy may reduce mortality due
to falciparum cerebral malaria.

Lempuyang gajah (Zingiber zerumbet (L.) Smith), lempuyang pahit (Zingiber amaricans BL.), and
lempuyang wangi (Zingiber aromaticum Vahl.) are used as traditional medicine (jamu) in Indonesia. It is also
used for treatment of microbial infections, helps to increase appetite and stimulate digestion in chickens.
Information on their uses are available, but only limited in the scientific data on their bioactivity. The study was
conducted on the antibacterial effect of organic extracts of these plants with Mycoplasma gallisepticum as the
agent of chronic respiratory disease in chickens. Juice and extracts of fresh and dried rhizome are evaluated
through the disc diffusion assay and minimum inhibitory concentration. Oxytetracyclin (30 µg) are used as
standards. All extracts are individually exhibited as antibacterial activity against Mycoplasma gallisepticum (7
± 0.11 mm to 21 ± 0.86 mm). The minimum inhibitory concentration (MIC) determination of plants extracts are
ranged from 7.8 mg/ml to 31.2 mg/ml. The preliminary results suggested promising antibacterial properties of
wild ginger from Indonesia, and probably could be used in management of chronic respiratory disease in
chickens.

The purpose of this study was to propose an extraction method rapid, efficient and cost of natural dye from
waste products from the food industry, such as the peels of eggplant, from which it was extracted, isolated and
purified the nasunin,a colored molecule in red-fuchsia.
Nasusin was tested on 58 patients to evaluate the potential sensitizing effect on the skin. The results demonstrate
that allergenic effects are negligible and therefore the nasunin can be used as a colorant in various industrial

Abstract : In Albania, many people erroneously think that tuberculosis (TB) is a disease of the past-an illness
that no longer constitutes a public health threat. Surveillance is an integral part of tuberculosis (TB) control.
Albania has a highTB notification rate and there are doubts about underreporting. The evolution of the
incidence of tuberculosis is presented, together with more detailed figures over the period 1998-2009. These
figures were obtained by the monthly forms (called 14/Sh) compared with the individual notification data.
Objective: To examine the distribution and sources of increased tuberculosis (TB) morbidity and reporting
system deficiencies in the Albania from 1998 through 2009. Metodology: The study is descriptive one conductet
during the period 1998-2009. The statistical analysis is based on data reported from regional level (regional
epidemiological departments) to the central level (Public Health Institute). Results: The main findings were:
discordance between the collected data (individual form) and reported data (monthly form); tuberculosis
incidence rate shows little oscillations which ranges from 6.67 to 9.2 cases/100.000 population; 50% of the
regions show a lack of information on the confirmation of diagnosis and laboratory examination type used for
confirmation. Conclusion: TB disease in high-risk populations where it is difficult to detect, diagnose, and treat;
limitations of current control measures and the need for new tests and treatments, including an effective
vaccine; improving information system, regulation of individual form and personnel training.
Total Phenol and Antioxidant from Seed and Peel of Ripe and Unripe of Indones...iosrphr_editor
Study on total phenol and antioxidantactivity ofsugar apple fruits of various solvent, part of fruits, and level of ripening. Solvent extraction used were 80% (v/v) methanol, 50% (v/v) acetone, boiling water, and 50% (v/v) ethanol. Part of fruits thatbeen used for samples were seed and peel which are normally by products of sugar apple processing, level of ripening were unripe, and ripe sugar apple fruits. Total phenol was determined by Folin-ciocalteau method. Total antioxidant was quantified by 1,1-diphenyl-2-picrylhydrazyl(DPPH) method.Therewas a difference in type of solvent, part of fruits, and level of ripeningon total phenol and antioxidant concentration of sugar apple fruits. Seeds have higher total phenol concentration than peels of this fruits. Unripe sugar apple fruits have higher total phenol and antioxidant than ripe fruit. The best solvent for phenol extraction was ethanol 50%butthe best solvent for antioxidant extraction was acetone 50%.

Non-steroidal anti-inflammatory drugs are the most widely used "over the counter" medication all over the world despite their complications in different major organs. Present studies envisaged for knowing the occurrence and severity of adverse drug reactions from NSAIDs in different ethnic communities of Sikkim. A cross sectional study was undertaken in the medicine outpatients department of a secondary and tertiary care hospital. The patients belonging to Nepalese, Bhutias, Lepchas ethnic communities and others community (settlers from other parts of India) were included to analyzed the data based on the age and gender, ethnicity and ADRs, drugs and ADRs. Severity assessment was done using Hartwing and Siegel scale and causality assessment by Naranjo scale. Total 109 cases of ADRs, predominating in female were detected. Nepalese were the most affected and Gastrointestinal tract (GIT) being the most affected organ in them. Diclofenac showed maximum number of ADRs in all the communities. Maximum number of cases occurred on single day use (40.36%) of drugs. All the cases were belonging to the "possible category" and the maximum being the mild (72.48%) in nature. It is advisable to consider the ethnic/racial differences equally with other factors, to improve the safety and efficacy of a drug.

STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

ABSTRACT: A survey was performed from July 2012 through September 2012 on randomly selected cancer
patients at three well-known hospitals in Dhaka city, Bangladesh, by the help of a structured questionnaire. A
total of 60 patients, 27 male and 33 female were involved in this study with the ratio of 1:1.2.Cancer prevalence
was much noticeable in middle to old age where lung and stomach cancers stood in the top level (37.04%),
followed by thyroid cancer (25.93%), sarcoma (14.81%), male genital tract cancer (7.41%), eye cancer
(7.41%), duodenum cancer (3.7%) and rectum cancer (3.7%) in male. In female patients, ovary cancer ranked
the top (39.39%), followed by breast cancer (27.27%), rectum cancer (18.18%) and duodenum cancer (9.09%).
This survey reflected that the primary and foremost means of cancer treatment in Bangladesh are surgery,
chemotherapy and radiotherapy which can be used alone or in combination. From this study we found that,
60% of cancer cases were being treated with chemotherapy alone and 20% cases with the combination of
chemotherapy and radiotherapy. Only a limited number of pharmaceutical companies manufacture anticancer
drugs in Bangladesh and many of the required anticancer drugs are imported.

I. INTRODUCTION
Cancer is one of the leading causes of death worldwide [1]. The World Health Organization (WHO)
reported the cancer disease accounting for 8.2 million deaths in 2012 [2, 3] and also predicted that 12 million of
all deaths by 2030 worldwide will be due to cancer [2]. In major cases predominantly breast, colorectal,
prostate, lung, stomach and liver cancers cause the most cancer deaths each year [4]. However, the most
frequent types of cancer differ between men and women, lung cancer in men and cervical and breast cancer in
women constitute about 38% of all cancer in Bangladesh [5] and in most cases cervical cancer patients come for
diagnosis and treatment at very late stage [6]. In the European Union, the estimated numbers of new cases of
cancer were approximately 1.4 million in males and 1.2 million in females, and around 707,000 men and
555,000 women died from cancer in the same year [4]. According to WHO, at least 30–40% of all cancer deaths
are preventable and the rates are extending as more people live to an old age and as much lifestyle change
originate in the developing world [7]. About 30% of cancer deaths are due to the five leading behavioral and
dietary risks which are high body mass index, low fruit and vegetable intake, lack of physical activity, tobacco
use and alcohol use [8]. Smoking alone is estimated to have caused 21% of global cancer deaths and about 70%
of global lung cancer deaths [9]. Cancer rates are predictable to further increase if nothing changes. Some large
surveys related to cancer are being conducted worldwide based on various sorts of comparative data on patients
from different parts of the globe [10] but there is no population based cancer registry in Bangladesh for reliable
data on cancer incidence, prevalence and mortality [11].This study is aimed to define the characteristics and
distribution of types of cancer in Bangladesh through a prospective, cross-sectional, multicenter survey of
cancer patients. We are also attempted to present the cancer treatment patterns in different hospitals and to
reflect current condition of available anticancer drugs in Bangladesh.
II. MATERIAL AND METHODS
The study was conducted on 60 cancer patients at three well-known hospitals in Dhaka city,
Bangladesh: National Institute of Cancer Research and Hospital (NICRH), Ahsania Mission Cancer & General
Hospital (AMCGH) and Japan Bangladesh friendship Hospital (JBFH) between July and September in the year
of 2012 on both the indoor and outdoor patients having radiological evidence or clinical evidence of malignancy
from the hospitals. The study was a statistics based cross-sectional survey. This survey was done by a

structured questionnaire which was developed through multi-stages of cross check and analysis and was used to
collect data from the patients. Questions were asked to the patient and finally answers of the patient were
inserted into the data collection form.
III. RESULTS
About 98 cancer patients were selected for the survey but only 60 of them were agreed to answer the
questionnaire, response rate was 58.8%. Out of 60 cancer patient we have got 27 male and 33 female in this
study (male: female ratio- 1:1.2, Table 1). Majority of the patients were from the Dhaka division. In this study
the cancer incidence was much higher at the old age 50-59 years (23.33%), followed by middle age 30-39
(21.66%) whereas cancer was least prevalent in pediatric and teenage patients that was only 5.0% (Table 2, Fig
1) in both cases. With respect to family income, we found that the majority (about 21.67%, Table 3) of patient’s
family income (range 30,000-34,000 taka only) was not sufficient for the treatment of cancer in Bangladesh.
Regarding the educational status, the incidence and prevalence of cancer were much in illiterate patients (total
30.00%, male 25.93% & female 33.33%, Table 4). Among the 60 cancer patient, there were 13(21.67%) ovary
cancer, 12(20.00%) lung and stomach cancer, 9(15.00%) female breast cancer, 7(11.67%) thyroid cancer,
7(11.67%) rectum cancer, 4(6.67%) sarcoma, 4(6.67%) duodenum cancer, 2(3.33%) eye and 2(3.33%) male
genital tract cancer patient. With respect to the prevalence, ovary cancer (39.39%) topped in women and lung &
stomach cancer (37.04%) in men (Table 5).
The main approaches of cancer treatment were surgery, chemotherapy and /or radiotherapy, used alone
or in combination. We found that 60% (36, 21 female and 15 male) of patient were being treated with
chemotherapy alone. In 20% of cancer cases chemotherapy was followed by radiotherapy and 11.6% cases were
being treated with radiotherapy whether triple therapy (chemotherapy, radiotherapy & surgery) treated cancer
patient were insignificant in the study (Table 6 and Fig 2). With respect to the high treatment cost and hospital
facilities, most of the patients were not satisfied (58%) in taking treatment from different hospitals (Fig 3). The
most important matter related to cancer treatment is the use of anticancer drug. In Bangladesh, there are a few
pharmaceutical companies that manufacture various groups of anticancer drugs. Among them Beacon
Pharmaceutical and Techno Drugs ltd. are in leading position. Beacon pharmaceutical produces diverse classes
of anticancer drugs pricing from 100 taka to 10,000 taka and Techno Drugs Ltd. produces significant numbers
of anticancer drug pricing from 95 taka to 12000 taka (Table 7). But the total needs of anticancer drugs are
much more. The rest of the required medicines are imported from different foreign countries which lead to an
increase in the price of the anticancer drug. In this study the entire chemotherapy treated patient had experienced
a number of chemotherapy induced side effects which were managed with suitable medications.
IV. DISCUSSION
This study was conducted during the period of July to September 2012 on the cancer patients in three
hospitals of Dhaka city. For this purpose, a questionnaire was used to collect data from 60 cancer patients
having radiological or clinical evidence of malignancy. The survey reflected some distinct picture on the cancer
in Bangladesh. The age range of the sampled patients were 0 years to 70 years above and a total 23.33% (Table
2, Fig 1) of the patients having cancer with the range of 50-69 years. This data supports the facts that cancer is a
disease of adult and old age [12, 13]. According to this study, there were exactly 33 female cancer patients
among the 60 patients, which represent 55 % of the total patients that indicates women are in the vulnerable
condition of the total cancer occurrence (Table 1). But a population based survey is greatly needed to enlighten
the exact scenario of cancer in Bangladesh. Most people are affected with cancer because of illiteracy,
ignorance, scarcity of cancer consciousness, religious superstitions, inadequate diagnosis availability in most
cancer center and poor socio-economic status in Bangladesh [10]. Our survey also found that lack of education
can cause cancer incidence, in our study 30% of the total patients were uneducated, and 23.33% patients passed
secondary level and only 10% completed graduation (Table 4). Globally the three most common cancers are
lung, breast and colorectal [14]. Our study also reflected that mostly occurred cancer type in Bangladesh are
stomach and lung cancer (37.04 %) in male and ovarian (39.39%) and breast cancer (27.27%) in female patients
(Table 5). According to this survey, surgery, chemotherapy and /or radiotherapy are the three main strategies of
cancer treatment and 60% of the total patients were being treated with chemotherapy alone (Table 6, Fig 2).
Most focusing segment of our survey was to find out the anticancer drugs which are manufactured within our
country as well as to see how many drugs are being used in our hospitals. So through our afford we got 22
different anticancer drugs that are available in the market manufactured by the local pharmaceuticals companies
which are “Beacon Pharmaceutical” And “Techno Drugs Ltd”(Table 7).

V. CONCLUSION
Cancer treatment is a long process and very much expensive. This survey reflects the overall picture of
cancer and its treatment in Bangladesh which include patients’ condition (age, sex, educational status and
income range), types of cancer, strategies of treatment and the cost of the treatment. This study reveals that mass
education, adoption of a healthy lifestyle (diet and exercise), tobacco abstinence, health consciousness and early
diagnosis of disease are much important to fight against serious health ailments such as cancer. One of the most
focusing segments of our survey was to keep an eye to the availability of anticancer drugs as well as to visualize
how many drugs are being used in hospitals. Only a few companies manufacture anticancer drugs in Bangladesh
which cannot fulfill the total drug requirement for the treatment of cancer within the country and the rest of the
requirement is imported from different foreign countries which lead to an increase in the expense of cancer
treatment. Government should offer enough facilities to conduct research and increase manufacturing scopes of
the anticancer drugs locally. If the local companies get more funds, many of them will come ahead to
manufacture anticancer drugs, which will aid to enrich our local pharmaceutical market. The government should
emphasize on the development of specialized cancer center. These centers can also provide rehabilitation and
palliative care for cancer patients to relieve their suffering. If we follow the efficient and decent way, we
suppose it can shrink the current condition of cancer diseases.
