• Building Trust- Creating a climate for repairing broken rapport/
  discussing the situation
  • "instantly, she became very angry with me and started shouting about breaking
    confidentiality"
• Assessing safety- Primary Focus!
  • Very pale, tired, needed to lie down, not eating well, no vitamins, not seeing a
    doctor, tumultuous relationship with her mother, past worries about her living
    stability
• Assess her current family situation, living situation, and
  support system
• Help her understand our role in confidentiality- emphasize that the
  counselor cares
• Help her understand the benefits/ our desire to help her seek support
• Plan for disclosing to family- Give her one day to tell her mother and
  explain that we will tell ourselves if she does not
• Seek additional resources/referrals
• Person-Centered Theory
 • Empathy, Unconditional Positive self-regard.
   Non-judgmental. You want to attune to her
   and help her explore in a safe environm-
   ent her own thoughts, fears and options.
• Humanistic Theory (Satir)
 • Address the difficult issues and seek
   options through inner self discovery.
   Help the client live in the present with
   a new sense of courage and awareness.
• Cognitive Behavioral Therapy
• A.1.a – Primary obligation to the student
• A.1.e – Promote welfare of student & collaborate
    to create an action plan
•   A.2.a – Limits of confidentiality
•   A.2.d – Balance obligation to student with parent’s
    rights
•   A.2.e – Discuss method & timing of breach
•   A.7 – Confidentiality breach when student poses
    danger to self or others
•   B.1.a/f – Respect parent’s rights & collaborate

•   B.1.a – Discuss how, when, and with whom
    information is to be shared

    without ethical rationale
•   B.1.d – Limits of confidentiality

    serious/foreseeable harm

•   In Loco Parentis
    •   “In place of the parent”—Act in the best
        interests of the student
1)   Beneficence: To do good in her case by
     thinking of how to take care of her physical and
     emotional needs rather than just focusing on the
     rapport
2)   Nonmaleficence: Though by telling and not
     telling both have adverse consequences, the
     counselor must look at which choice will produce
     the least harm
3)   Autonomy: Helping her to independently care
     for herself
4)   Justice: Assessing her at-risk situation with the
     same intentionality despite the fear of backlash
• 1. Give her one day to tell her mom and if she doesn't,
  you will call home.
   • Not bringing to the mom’s attention right away that
     her daughter may be doing harm to herself
• 2. Let her know you are going to break confidentiality
  and call her mom right away.
   • Breaking confidentiality will probably make her upset
     and you may lose the rapport you have with her
• 3.Keep it confidential and work with her alone on her
  options
   • She may continue doing harm to herself and doesn’t
     have anyone to support her
• Multicultural
  • Discrepancies between counselors or students
    coming from collectivist vs. individualistic
    cultures
    • Counselor may not understand her independent
      approach.
      • Minimizing the students feelings
• Gender
  • Male to female student interaction
    • Student may not be inclined to discuss her
      pregnancy with a male counselor.
•   Formative
    • bring Tara back in to discuss on a scale, how
      willing she is to disclose her situation to her
      mother
    • check in with Tara the next day to see whether or
      not she has told her mother
    • Scaling: How stressed are you feeling about your
      situation?
•   Summative
    • Miracle Question: If everything was perfect the
      next day, what would your life look like?
