A 25 year old female presented with features of right heart failure. She was treated for pulmonary tuberculosis in the past and completed treatment four years back. Contrast enhanced CT scan of the thorax was performed which revealed chronic calcific peri-carditis with a narrow necked left ventricular apical free wall aneurysm (Figure 1a & 1b). Small contrast filled out-pouching involving the ventricular apex (bold black arrow) showing a narrow neck (thin black arrow). Reduced size of ventricles with prominence of both atria is seen.

We present the case of a 49-year-old male who was referred from Emergency department with worsening
breathlessness, chest tightness for last 24 hours. He had a background history of Asthma and Hypertension. Initial ECG revealed symmetric T wave inversions in anterior leads and found to have raised
troponin levels. Patient was diagnosed and treated as NSTEMI. While waiting for his coronary angiogram
he underwent echocardiogram whose findings were consistent with right sided impairment. Differential
diagnosis of Pulmonary Embolism has been made and CT Pulmonary Angiogram done that confirmed
diagnosis of Pulmonary Embolism. Early advice from the Respiratory team was sought and patient was
treated with rivaroxaban. After a hospital stay, he made a remarkable recovery.
Infective Endocarditis as a Complication of Ventriculoatrial Shunting for Hyd...asclepiuspdfs
Our aim is to present a rare cause of tricuspid valve infective endocarditis (IE) in grown-up age due to cerebrospinal fluid shunt-associated infection. A 32-year-old woman, with a history of hydrocephalus that was treated with ventriculoperitoneal (VP) shunt at the age of 4, was admitted to a hospital due to fever. The VP shunt was replaced several times due to dysfunction and replaced with ventriculoatrial (VA) shunt 3 months before admission. Transesophageal echocardiogram revealed two separate VA catheters in the right atrium, with two floating echo formations, one attached to the tip of one catheter and the other to the anterior leaflet of tricuspid valve. Blood cultures grew methicillin-susceptible Staphylococcus aureus. Computed tomography scan showed bilateral pneumonia. The patient was treated with antibiotics followed by partial extraction of the VA shunt. After 8 weeks, the patient was discharged, without signs of infection. Two months later, she was readmitted due to fever, echocardiographic signs of catheter infection, and septic pulmonary embolization. Complete extraction of VA catheter was done and treatment was continued with antibiotics with complete recovery. Early diagnosis and optimal management that combines both conventional and surgical approaches is crucial for reducing the high embolic risk, risk of complications, and mortality risk.
Guillain–Barré syndrome after acute myocardial infarction: A rare presentationApollo Hospitals
The association of acute coronary syndrome with any immunological mediated polyradiculopathy like Guillain–Barré syndrome is very rare. We report such a rare association of acute myocardial infarction and Guillain–Barré syndrome. Our patient underwent primary angioplasty successfully, but developed respiratory failure while in hospital. While the difficulty in weaning off from ventilator a suspicion of neuromuscular disease was made. The further investigations, including nerve conduction study confirmed a diagnosis of Guillain–Barré syndrome. Despite treatment, the patient died secondary to multi-organ dysfunction. Our case is 4th reported in the literature without use of any thrombolytic agent for such association.
Worsening Tension Pneumocephalus from Late Post-traumatic Ventriculo-bronchia...asclepiuspdfs
The objective of the study was to report a case of tension pneumocephalus presenting as status epilepticus and outcome of treatment following emergency hyperbaric oxygen therapy. The data were collected from electronic medical record. The study was a case report. The data were extracted from medical record review and literature search. A 41-year-old male presented with status epilepticus and was found to have pneumocephalus within the cerebral venous sinuses. Before presentation he was complaining of intermittent hemoptysis attributed to a lung injury from a remote trauma due to a stab wound in the chest. At the time of his chest injury, he underwent multiple operations. His recovery was complicated by formation of left ventricular aneurysm and ventriculopleural fistula which was successfully repaired 5 years before presentation. Before determining the exact etiology of pneumocephalus, the patient was emergently treated with hyperbaric oxygen therapy (HBOT) to help with the management of intractable status epilepticus. During the HBOT therapy, the patient developed hemodynamic instability and the therapy was aborted. Repeat computed tomography (CT) scan showed worsening pneumocephalus with massive brain swelling and herniation. An echocardiogram showed bubbles crossing the left ventricle to the aorta. A CT thorax showed evidence of communication between the left ventricle and lung parenchyma at the site of the Gore-Tex confirming a ventriculo-bronchial fistula. Despite aggressive measures to control intracranial hypertension, the patient deteriorated and was declared brain dead. In cases of pneumocephalus where the exact cause is not well documented, an extensive investigation is recommended to ascertain the etiology before the institution of hyperbaric oxygen therapy.

Dear Dr. Kornbluth and Mr. Gorenberg,
The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.

1. AN UNUSUAL CASE OF LUNG COLLAPSE IN A PATIENT WITH ARTERIOVENOUS FISTULA

Background: Arteriovenous fistula (AVF) for hemodialysis
can become hyperdynamic and have local or systemic
complications. We present a very unusual complication of
such an AVF.
Case Report: A 40-year-old male with End Stage Renal
Disease on maintenance hemodialysis for three years
presented with epistaxis, hemoptysis, accelerated
hypertension (HTN) and breathlessness.
Examination showed a well functioning radio-cephalic
arteriovenous fistula with swelling and palpable thrill in the
entire right arm. There were distended veins over chest
wall (Fig1& 2). He was managed with anterior nasal packing
and anti-hypertensive treatment. His condition deteriorated
and oxygen saturation dropped. Chest radiograph (CXR)
showed complete collapse of right lung (Fig 3). He was
intubated and ventilated. Bronchoscopy revealed bilateral
clots; right main bronchus was occluded. (Fig 4) Contrast
enhanced CT scan revealed an enhancing mass lesion
partially occluding the right main bronchus (Fig 5&6).
Angiogram was suggestive of a highly vascular lesion in
the right main bronchus fed by bronchial artery, with
bronchial artery to pulmonary vein fistula (Fig 7).
Vascular Doppler showed hugely increased flows in the AVF
& enormously dilated veins. There was no thrombosis or
stenosis. A possible link between hyper-dynamic AVF and
pulmonary arterio-venous fistulous communication was
considered.
The AVF was surgically ligated with resultant dramatic
decrease in swelling of the right arm. (Fig8) Repeat
bronchoscopy two days later showed no vascular tumor or
fresh bleed. Clot clearance and full expansion of the right
lung was achieved. CT pulmonary angiogram five days after
AVF ligature also showed fully expanded lung with no
evidence of any vascular communication (Fig9).
Conclusion: It appears that in addition to the aspiration of
epistaxis, the right lung collapsed due to a vascular pseudo-
tumor, the result of a bronchial artery to pulmonary vein fistulous
communication, latent fistulas, which are known to open in hyper
dynamic circulatory flows.
