Understanding and implementation of outcome based practice within my work setting.
Outcome based practice is to deliver a service which is measured by the outcomes of
the support delivered.
From the initial stages of referral it is made clear what outcomes are expected to be
achieved or worked towards. An individual s care co ordinator will draw up a pen
picture representing the individual and disclosing details of the support that they
require and a summary of their character, within this pen picture, required outcomes
will be stated and it s down to us to detail how we can meet these outcomes within

It is of upmost importance that the systems and processes used are in favour of the
promotion of well being. Many professional parties involved in the care of an
individual have many different systems and many different processes so to avoid
confusion I will focus on some applicable to our organisation.
The initial care needs assessment has a huge impact on the individual s well being
as first contact with a service which will be playing a huge part in the delivery of
your care can be quite daunting. It is important to make the individual feel valued,
empowered and in control of their own care package. A good care needs assessment
will reflect the individual s wants, dreams, aspiration s and how they want their life to
be, it will highlight their true needs as well as their strengths.
The review process should have the same result as the above and should ensure that
changes are monitored, progress mapped and new goals and targets are set, it is
imperative that the care plan gives a true and current reflection of the individual in
order for them to receive the correct support.

Great Lakes Region for a number of years. Water quality can be anything that affects
the benefits humans derives from water. It can include drinking water, food,

In Mary Oliver s Crossing the Swamp , the speaker compares a swamp to the
struggles and hardships in life that fight against her. The speaker repeatedly contrasts
herself to the swamp to emphasize the vastness of the swamp, conveying how the
speaker understands the hardships that come with life. The speaker describes the
swamp as a wet thick cosmos [...] гѓјthe nugget . The paradox compares the vast
cosmos with a small golden nugget, contrasting how the experience the speakeris

In August Wilson s play, The piano lesson, the author takes his readers on a journey
through the life of the Charles family. Sister and brother battle each other over a
piano that tells their family s history. Through examining their history, Wilson uses
setting and symbolism to convey his personal belief in the importance of one s
ancestral family roots and the significant impact it has in a person s everyday life.
The author uses setting to introduce the significance in the ancestry of the Charles
family and further examine the troubling state of the current generation. Wilson
grew up in a non wealthy colored district in which he set most of his plays (Fisher,
James), so there is no surprise that Wilson s play has black roots that are relatable to
his past ,also acting as salute to people of color (Gantt, Patricia). His strong
connection to African American roots are why censors ,who have criticized
Wilsons work, applaud him on his capability to make interesting characters and pull
from African American folklore (Bloom, Harold), and in doing so, he shared much
of his native craft and technique (Tracie, Guzzio). A man owns the Charles family,
whose grandparents had ownership of his grandparents, and he happens to also be
man who held the title of Robert Stutter (Bryer, Jackson R., and Mary C. Hartig,
Wilson 1.2.8). In the play, a major flashback occurs on a slave plantation where the
older Charles family must to pick cotton (2.1.7). The setting is critical to the family
when it comes to the readers understanding of the present Charles s past, as their
grandparents were slaves which in turn made life for the future generations a lot
harder. Although Bernice lives in Pittsburgh (1.1.5), it is still hard for colored people
to make a living and buy food from white only markets, so instead, their parents teach
them things such as how to kill a chicken (2.2.10). Wilson uses setting to show that
even though Bernice lives in a different environment than that of which her family
roots originate, their individualities interrupt and cross within her own (Lise,
Kidergurd), and the physical things, such as her skin color, still affect her present life.

empathy as the feeling that you understand and share another person s experiences
and emotions: the ability to share someone else s feelings (Empathy, n.d.). Studies
show that already one year old children engage in situations in which an adult needed
help to accomplish an action overcoming problems. These results also show that
infants readily provided help to adults in most conditions in which they are able to
interact. Additional studies suggest that children s prosocial behaviors become more
differentiated and less dependent on communicative cues with increasing age (Paulus
et.al., 2015, pg. 55). These situations are important in the development of empathyin
children because it allows the child to interact with others and begin the stages of
empathy. Martin Hoffman s theory of empathy includes information that cognitive
development is directly related to the development of empathy (Johansson, 2008, pg.
36). As children grow and interact with others, levels of empathy become evident.
Hoffman suggests that there are four levels of empathy in children:
Global empathy In the first year children may match the emotions they witness (e.g.,

adolescents in the United States. In 1999, 13% of children aged 6 to 11 years and
14% of adolescents aged 12 to 19 years in the United States were overweight. This
prevalence has nearly tripled for adolescents in the past 2 decades ( Fact Sheet ). A
key cause for concern is that overweight adolescents are at higher risk of medical
conditions such as hyperlipidemia, glucose intolerance, hypertension, and sleep

Furthermore, vending machines are available to adolescents throughout the day.
The availability of vending machines gives students a good alternative to the
cafeteria. The cafeteria is usually crowded and hence vending machines allow
students to skip the lines and grab a quick bite. Also some students may not be able
to get to the cafeteria before it closes for various reasons; vending machines
provide them with the convenience. They also allow for students to grab a drink
after vigorous exercise. However, a problem with all this is that it may disrupt
regularly scheduled meals. This in turn greatly disturbs the natural biorhythms of
