This document discusses ecosystem thresholds and the precautionary principle as they relate to One Health. It proposes four mechanisms to support a science-policy process that creates healthy environments: 1) Develop political consensus, 2) Foster collaborative, knowledge-based work, 3) Focus resources effectively, and 4) Communicate honestly. Case studies show ecosystem responses at different phosphorus thresholds, demonstrating impacts on water quality, cyanobacteria levels, and human health. The conclusion emphasizes observing nature, creating healthy environments for health and well-being, and taking action now through orientation and progress within a short time scale.

Endocrine Disruptors and their impact on Adult onset disorders is a document about endocrine disrupting chemicals (EDCs) and their effects on human and animal health. It discusses how EDCs are used in plastics, fertilizers, and other products but do not decompose easily and contaminate the environment. EDCs can negatively impact both animal and human health and studying their health effects provides a model for the "one health" approach that addresses social, environmental, business, and political factors related to environmental contaminants.
This document summarizes Israel's emergency response system and procedures for a mass toxicological event. It outlines key details about Israel's population and hospital infrastructure. The emergency response is led by MDA (Magen David Adom) and involves over 1,500 paid staff and 11,450 volunteers operating out of regional dispatch centers. The document describes clinical procedures for assessing and treating victims of organophosphate intoxication in the field. It emphasizes proper personal protective equipment, decontamination, transport, and training to safely manage a large-scale toxic gas or chemical exposure event.
This document discusses contract bonus systems that could encourage small-scale broiler farmers in Bali, Indonesia to adopt biosecurity practices. The authors analyzed 6 broiler farm contracts and found that contracts providing higher market price bonuses and performance-based bonuses provided better returns to cover biosecurity costs. Contracts 3, 5 and 6 offered the highest bonuses for practices like mortality rate and feed conversion ratio, allowing farmers to receive over $2.73 for every $1 invested in biosecurity. These contracts were identified as best rewarding farmers and providing incentives to reduce disease risks through improved biosecurity.

Climate change is projected to increase uncertainty for Mediterranean farming systems in Oristano, Italy. The analysis found that under a near future climate scenario:
(1) Total agricultural revenue and net income are projected to decline by 3.9% and 2% respectively;
(2) Irrigated farms under water user associations face revenue declines up to 4.2% while rainfed farms see smaller 1.7% declines;
(3) Adaptations like more drought tolerant crops, improved irrigation efficiency, and sustaining rainfed agriculture can help address increased climate variability and uncertainty.

The document discusses concepts related to ensuring a safe, secure, and sustainable energy supply. It introduces the concepts of risk assessment, resilience management, security of supply, sustainability, and multi-criteria decision analysis. It then presents a case study from the EU SECURE project that used these concepts to evaluate policy scenarios according to various environmental, economic, social, and security indicators. The study found that global climate policy scenarios generally performed best, though they were vulnerable to certain shocks like nuclear accidents or carbon capture failures. Overall policies that reduced fossil fuel use and led to greater diversification of energy sources and imports improved sustainability and security.

Congestive Heart failure is caused by low cardiac output and high sympathetic discharge. Diuretics reduce preload, ACE inhibitors lower afterload, beta blockers reduce sympathetic activity, and digitalis has inotropic effects. Newer medications target vasodilation and myosin activation to improve heart efficiency while lowering energy requirements. Combination therapy, following an assessment of cardiac function and volume status, is the most effective strategy to heart failure care.
Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

The biomechanics of running involves the study of the mechanical principles underlying running movements. It includes the analysis of the running gait cycle, which consists of the stance phase (foot contact to push-off) and the swing phase (foot lift-off to next contact). Key aspects include kinematics (joint angles and movements, stride length and frequency) and kinetics (forces involved in running, including ground reaction and muscle forces). Understanding these factors helps in improving running performance, optimizing technique, and preventing injuries.
