Sabanal, Kalamansig, Sultan Kudarat. It was founded through the initiative of Mr.
Guialuson S. Musa, Al Haj, Public Schools District Supervisor, Mr. George M.
Astillero, Teacher-In-Charge of Kubo Banday Elementary School, with the huge
support of Hon. Rolando P. Garcia, Municipal Mayor, Hon. Erlinda L. Comaling,
Punong Barangay, Hon. Jojo U. Apang, Barangay Indigenous Peoples Mandatory
Representative (IPMR) and the donor of the 20, 000 sq. meter school site, and Mr.
Edwin M. Blowong, PTA president, who provided lumbers for the construction of
makeshift classrooms.

The school started as an extension class of Kubo Banday Elementary School
offering Grades 1 to IV with thirty-five (35) enrollees in the First Semester of School
Year 2018-2019. Pupils from other schools transferred in the Second Semester
yielding sixty-seven enrollees of Kinder to Grade VI. There were two teachers
assigned namely: Mr. Michael Lyod Matos (regular-permanent and the Officer-In-
Charge) and Mr. Kevin A. Bendulo (Local School Board Teacher). Mr. Norman S.
Sarino was the school ancillary. During its first half year of operation as an extension
class, classes were conducted at the house of Mr. Jojo U. Apang while makeshifft
classrooms were being constructed.
In School Year 2019-2020, Mr. Jeraldin C. Superio was assigned in the
school as regular-permanent teacher. On September 11, 2019, the school was
approved to operate as regular elementary school and named as Datu Bak-Bak
Apang Elementary School with School ID 136943, several months after the regional
validation. Mr. George M. Astillero was the cluster Teacher-In-Charge. It was on
December 9, 2019 when Mr. Michael Lyod Matos assumed the position as Teacher-
In-Charge.
The school applied for its conversion to Integrated School after months of its
approval as advised by the validator during regional validation. Through the initiative
of Mr. Guialuson S. Musa, Al Haj, Public Schools District Supervisor, Mr. George M.
Astillero, Teacher-In-Charge of Kubo Banday Elementary School, Mr. Michael Lyod
Matos, Teacher-In-Charge, with the huge support of Hon. Rolando P. Garcia,
Municipal Mayor, and Hon. Erlinda L. Comaling, Punong Barangay, the application
was approved. On August 28, 2020, the Central Office finally confirmed its
conversion from Datu Bak-Bak Apang Elementary School to Datu Bak-Bak Apang
Integrated School with School ID 501902 having twenty-three Grade VII students
and now, having a grade eight levels.

II. GOALS AND OBJECTIVES
This manual shall serve as a guide of the school in activating their
contingency plan to minimize the impact of COVID-19 resurgence in the school
community. This shall ensure continuity of learning if a member of the school environ
becomes a close contact, suspect, probable, or confirmed COVID-19 case or if a
school lockdown is needed due to a resurgence of COVID-19 cases within or outside
the school. Moreover, it contains case scenarios that may affect school operations,

and the necessary response of each stakeholder to Prevent, Detect, Isolate, and
Treat COVID-19. It also contains strategies to reintegrate school operations to
ensure the safe resumption of face-to-face classes. The response and strategies in
this manual are the minimum requirements for each school but may be enhanced to
be culturally and geographically responsive to the needs of the community. It is
recommended to review and update the contingency plan regularly, and as the need
arises, according to the most recent risks associated with COVID-19 and the latest
guidelines from the Department of Health (DOH).
All the contents of the manual were designed to respond to the complex and
rapidly changing COVID-19 situation in the school setting. However, this may be
used as a basis to craft contingency plans to respond to health-related crises, if in
case a similar scenario occurs in the future.
A. Introduction
The school has followed a decision model and contingency plan for
reclosing and reopening the school in case of COVID-19 resurgence in the
community.
The Department of Education continuously facing great challenges in
delivering educational services caused by COVID-19 pandemic. Due to General
Health and Safety Protocol classes in school were compromised. Despite of the
current situation brought to us by this pandemic, the Department of Education is
eager and determined to continue delivering quality, accessible, relevant education.
Thus, the school year 2020-2021 shifted to different distance learning delivery
modalities up to the present school year as mandated by the Inter-Agency Task
Force (IATF).
Distance learning modalities include Online Learning, Modular Distance
Learning, Blended Learning, Radio-Based Instruction, and TV-Based Instruction.

distance learning made more challenging especially on the part of the young
learners. These learners opted to do home schooling with their parents or guardians
as facilitators of learning. There is no doubt on the ability of the parents and the
learners to adapt in the new normal education, however, issues on instruction is one
of the expected problems and the modules cannot guarantee in building the
fundamental skills (Manila Times, 2020).
The present education system has encountered challenges in delivering
services caused by COVID-19 pandemic, where holding of classes in schools were
compromised due to prohibitions following the implementation of General Health and
Safety Protocols such as convergence and face-to-face contact. Despite the present
situation, the Department of Education is adamant to continue delivering quality,
relevant and accessible education. Thus, the school year 2020-2021 shifted to
different distance learning delivery modalities until the present school year as per the
mandate and advice of the Inter-Agency Task Force (IATF).
Distance learning modalities include Online Learning, Modular Distance
Learning, Blended Learning, Radio-Based Instruction, and TV-Based Instruction.

parents in the field of modular distance learning. To take a step in addressing the
abovementioned concerns and problems, the agency is looking forward to expanding
the limited face to face classes by the selected schools in the country who could
meet the standards set by the department and by the health agencies in
collaboration of the Local Government Unit in the locality.
The expansion of face-to-face classes marked a significant kick for a new
normal learning for both the teachers and the learners. Yet, along with this plan of
implementation, tight coordination with the concerned agencies is indeed a hand for
effectivity and progress. Datu Bak-Bak Apang Integrated School, as one of the
schools delivering limited face to face, is committed to deliver education services to
the learners following the health standards and protocols set by authorities.
B. Goal
The goal of this Contingency Plan is to provide effective, efficient, timely and
well-coordinated response mechanisms in the event of the infection of COVID-19
among the students and personnel of Datu Bak-Bak Apang Integrated School. These
mechanisms will help to protect lives and restore immediate needs of the affected
entities.
C. General Objectives
This document was developed by Datu Bak-Bak Apang Integrated School
SDRRM Team and School COVID-19 Task Force. It aims to:
1. align with the Harmonized National Contingency Plan for the Covid-19 Pandemic
of the National Disaster Risk Reduction and Management Council (NDRRMC);
2. establish coordination and communication mechanism among schools, districts,

V. ACTIVATION AND DEACTIVATION
A. Activation
This Contingency Plan shall be activated and implemented based on the risk
situation of the COVID-19 pandemic in SDO-SK having reference to the DOH official
declaration of Hotspot or outbreak or a surge in number of COVID 19 cases. The
activation is in concomitance with the local RIATF response. Afterwards the SDS
directs the School Heads who shall mobilize and implement necessary response and
mitigation actions at the division level down to the school level.

GENERAL GUIDELINES FOR INITIAL RESPONSE
If an individual in the school shows symptoms, becomes a close contact of a
confirmed case, or tests positive for COVID-19, it is critical to have a timely initial
response to ensure little to no spread of the virus among other members of the
school community. There are 3C’s that stakeholders must remember in responding
to any school situation related to COVID-19.
1. Coordinate Tasks
Other members of the school community may feel worried and become uneasy if
any situation related to COVID-19 arises in the school. As a means of preparation,
the school head must clearly identify the role of each teaching and non-teaching
personnel and provide the necessary training for their actions to be well-coordinated.

2. Collect Information
To assist health authorities and the local government unit in contact tracing, the
immediate responding health personnel or designated clinic teacher must collect the
following information of the individual who shows symptoms or is reported to be: a
close contact of a confirmed case:
a) Who: name of individual gender, age, grade level or occupation, the section
included in or handling
b) When: date, if in school (i) time of entry to school, (ii) time of isolation in the
clinic if at home (i) time of report
c) Where: school name, school address, the home address of the individual (if at
home during the time of the report)
d) What: symptoms experienced by the individual (if any)
e) How: details on where or how the individual was infected, or who is the
confirmed case to whom the individual became a close contact.
3. Contact authorities, other school personnel and families
All concerned stakeholders must be well informed to avoid the possible
transmission of the virus, and to guide the school community in the necessary
actions in case there will be a need to transition to a distance learning modality.

a) Contact the identified health authority (e.g., barangay health station, rural
health unit) to cooperate regarding the appropriate measures such as tracing
and quarantine, or referral to a hospital if needed.
b) Contact other school personnel (from the different set of classes per week) in
case there will be a need to shift to a distance learning modality. A teacher
must also be informed and guided if he/she shall be a substitute teacher in
case a fellow teacher becomes unable to physically report to school for a long
period due to being infected with COVID-19 or other acceptable reasons.
c) Contact the family of the individual who showed symptoms or identified to be
a close contact of a confirmed case of COVID-19. The family must also be
informed regarding the actions that will be undertaken by the school, as well
as the protocols that must be observed at home as advised by health
authorities.
d) Contact the families of individuals from the same class as the person who
showed symptoms of COVID-19 and inform them of the protocols that must
be observed, as well as the possible shift to distance learning modality.
e) Contact families from other classes from the set of classes in the same week
as the person who showed symptoms of COVID-19, as well as those from the
set of classes in the other week to inform them of the current school situation.

the shared responsibility of each stakeholder in preventing the transmission of the
virus, the actions that must be undertaken by the school to coordinate with each

the barangay was announced by the LGU.

There is a resurgence of COVID-19 in multiple barangays in the
municipality where the school is located, and there is a change in
the risk classification of the municipality. The barangay where the
school is located is not adjacent to the barangays with COVID-19
resurgence. There are no known cases in the specific barangay of
the school.
Scenario 1. A household member of a learner or a school personnel shows
symptoms of COVID-19 but has not been tested. The learner or personnel is
not showing symptoms of COVID-19.

substitute teacher who will temporarily conduct distance

STRATEGIES TO TRANSITION BACK TO FACE-TO-FACE LEARNING
MODALITY
During a school lockdown, it is important to continue engaging learners in
meaningful activities to ensure the continuity of learning. However, some learners or
school personnel may need additional support in transitioning back to distance
learning and managing their emotions after learning about the situation in the school
or in the community. During this period, the support from the family and other school
personnel shall play a critical role in adapting to the change in environment and
coping with the resurgence of COVID-19 cases.
1. The school local hotline/help desk shall remain open for parents/guardians in
need of assistance in the distance learning of their child/children. Teachers may
utilize this platform to encourage parents/ guardians in their role as learning
facilitators at home and to provide additional support to learners in their
independent study.
2. The school shall establish a cleaning and maintenance routine to ensure that all
school facilities are organized and functional until the resumption of face-to-face
classes.
3. The school may perform critical administrative tasks on-site provided that they
are given permission by the LGU.
4. The school shall provide the required personal protective equipment to non-
teaching personnel who shall weekly conduct the cleaning and maintenance
routine of the school, and those who will need to perform administrative duties

compensated for physically reporting to school during the school lockdown.
5. The guidance office shall remain operational to ensure that psychosocial support
and services shall be made available for all the members of the school
community. In the absence of a guidance office, affected learners and school
personnel shall be provided with Psychological First Aid (PFA) by trained
teaching and nonteaching personnel in schools. They shall also be assisted for
referral to the counselling services provided by the Schools Division Office, if
further interventions are needed.
6. The school shall provide the appropriate support needed by the non-teaching
personnel while they are unable to physically report to school on a daily basis.
7. After the school lockdown, the school shall seek the consent of
parents/guardians through a written form for the participation of their
child/children in the resumption of face-to-face classes.
8. For the first five school days, the first hour shall be devoted to
discussion/facilitation of modules related to mental: health, facilitated by their
respective classroom advisers or designated teachers.

For immunocompromised, do RT-PCR testing on the 10th day. If RT-PCR

until such time that the RIATF/DOH will be back to “white alert” status or the new
normal. At this point, the operation is already terminated. The trigger for deactivation
of this CP will also be based on the declaration of the Secretary, RDs, SDSs and
School Heads upon the recommendation of IATF and other authorized government
agencies.
C. Non-Activation
When the response and policies being implemented by the RIATF/DOH is enough to
contain the spread of the COVID 19 virus and that the risk remains to be in control,
the Contingency Plan will not be activated. Depending on the nature of the
hazard/event, the CP may be maintained as a “continuing plan” or a plan that can
still be utilized for future use in case the same hazard will occur.

Education is committed to find ways for learning opportunities to be provided to give
hope and stability, contribute to the normalization of activities, and facilitate
development of our learners and bring normalcy to their lives.

IV. Policy Statement
In accordance with the principles of the Department of Education, Datu Bak-
Bak Apang Integrated School of Division of Sultan Kudarat adopted the set of School
Required Health Standards and Protocols to ensure the protection of the health,
safety and well-being of learners, teachers and personnel, and to prevent the further
transmission of COVID-19. This policy facilitates their safe return in schools.
V. Required Health Standards Framework
In accordance with DOH Guidelines on the Risk-Based Public Health
Standards for COVID-19 Mitigation (DOH AO No. 2020-0015) and the Guidelines on
the Required Health Standards in Basic Education Offices and Schools (DepEd
Order No. 14 s.2020) and consistent with the Basic Education Learning Continuity
Plan (BE-LCP), the School Required Health Standards framework is mainly
composed of four COVID-19 Mitigation Objectives such as a) Increase Physical and
Mental Resilience, b) Reduce Transmission, c) Reduce Contact, d) Reduce Duration
of Infection.
Additional references consulted in the development of this framework include
the recommendations of Grade Chairmen, Nurse, Dentist and the Principal
considering the well-being of learners, teachers, and personnel in the school. The
four COVID-19 Mitigation objectives are explained in the DepEd Order No. 14 s.2020
and the specific measures for the standards are enumerated in these guidelines.

VI. School Guidelines for Health Standards and Protocols
A. GENERAL HEALTH AND SAFETY PROTOCOLS
I. Upon Entering the School
1. Practice physical distancing at all times (at least 1 meter apart).
2. Undergo temperature check through thermal scanner in the school entrance.
3. Clean hands frequently by using alcohol-based hand rub/disinfectants or by
proper handwashing with soap and water.
4. Observe proper use of face masks at all times. Both nose and mouth must be
covered.
5. Use tissue or inner portion of elbow when sneezing/coughing to cover nose and
mouth. Do not cover the mouth with hand.
6. Attend to the initial orientation on the respiratory etiquette and other protective
measures.
7. Smoking and vaping are prohibited in school.
II. Classroom Layout & Structure
1. Number of seats inside the classroom must not exceed from the number of
maximum learners in the classroom.
2. Seats must be 1-2 meters apart.
3. There should be working electric fan/s inside the classrooms.
4. Open all windows and doors at all times.
5. Follow the markers, stickers & signages inside the classrooms to manage traffic
system and maintain physical distancing.

4. The school gymnasium, Pascal and AP parks are designated as areas for
queue of visitors.
5. Always follow the stickers or markers in queuing in using the restrooms, library
or visiting offices in the school.
6. School Personnel vehicles are only allowed at the designated school parking
area.
7. Students, parents/ guardians and visitors’ vehicle are not allowed inside the
school.
IV. Hygiene, Sanitation & Disinfection
1. Always use alcohol or hand sanitizers, before and after, using item/s that you
don’t personally own.
2. Dispose your face mask and tissue in your bag or in yellow-labeled trash
bins/cans.
3. Every Friday afternoon is scheduled for disinfection of the entire school.
4. Every Friday afternoon, designated personnel must collect the infectious waste,
placed it on a leak-proof yellow plastic bag or container and bring it to the MRF
for disinfection.

13.Bring your own water and snacks (merienda) because canteens are not
allowed to sell any product yet.
14.Must present a CCTS ID (with latest picture) upon entering the school.

4. Check the maintenance of proper sanitation and hygiene facilities, restrooms,
hand washing station in the classroom/ office.
5. Check the availability of tissue paper/ towel, trash bins, adequate water and
soap and the hand sanitizers/ alcohol-based solutions/ other disinfectants in the
classroom/ office.
6. Facilitate mental health topic on first five school days.
7. Serve as guidance counsellor in the classroom by providing counselling
services and psychosocial support to the learners.
8. Promote “school-life balance” by engaging the learners for at least 30 minutes
of daily physical activities.
9. Report learners that needed further Psychosocial First aid and Mental Health
intervention to the Office of the Guidance Counselor.
10.Monitor the strict observance of the general health and safety protocol by the
learners.
11.Secure a locator slip from the principal or assistants to the principal in going out
of the school.
12.Ensure the cleanliness and disinfection of the classroom/ office before and after
class.
D. SCHOOL CLINIC AND HEALTH SERVICE
1. Provide health assessment and physical examination as needed.
2. Conduct appropriate intervention, first aid, or treatment.
3. Ensure proper management of symptoms including rest at home.
4. Notify the family member or guardian of the learner or the school personnel
who show flu-like symptoms.
5. Facilitate referral and follow-up to appropriate health facilities with coordination
to the school personnel.

6. Ensure the availability of Emergency Health kits.
7. Clinic In-charge & other clinic personnel must record student’s health status,
development, including immunization checks to prevent outbreak (prone-
vaccine- preventable diseases).
8. Availability of foods in school canteen is not yet allowed.
9. Areas designated for Private Screening for learners who showed symptoms:
Gate 1 – at the back of SBM room, Gate 2 – at the Science Park.
10.The library will be the temporary designated area for learners who been
managed in the clinic and awaiting referral to proper health authorities and
facility.
E. VISITORS & STAKEHOLDERS
1. Prioritization of non-face-to-face communications and coordination through
available platforms unless in times of emergency.
2. Accomplish the logbook information for necessary entrance approved by school
for recording and tracing.
3. If the subject must be done face-to-face, the visitor is required to schedule an
appointment before they will be entertained at school.
4. Visitors must wait at designated queuing areas; the school gymnasium, Pascal
and AP parks, wherein a school personnel will process their inquiry.
F. SCHOOL ACTIVITIES AND EVENTS
1. Conduct of physical or face-to-face large gatherings and activities shall only be
allowed if approved by the Local IATF.
2. Travel of learners, teachers and personnel shall be limited only to the most
critical or essential as determined by the principal and approved by the Division
Office.

G. SECURITY SERVICE
1. Ensure that you’re physically fit in rendering school services.
2. Manage traffic system at the school gate
3. Operate the thermal scanner in the entrance.
4. Report to the school management any untoward/ suspicious incident.
5. Observe the protocol in giving information or feedback to the management.
6. Perform the task or duty responsively with courtesy.
7. Ensure that the logbook is properly accomplished.
8. Check the availability of the soap in washing areas and alcohol or hand
sanitizer.
9. Ensure the conduct of hand washing, sanitation, disinfection of the parents,
guardians, visitors and stakeholders in the entrance.
10.Refer to the school personnel those learners and visitors who have fever during
thermal scan.
H. PARENTS/ GUARDIANS
1. Entrance of parents/ guardians for unreasonable purpose is discouraged.
2. Stay in the holding area inside the campus while waiting for the learners.
3. Accomplish the logbook information for necessary entrance allowed by school
for recording and tracing.
4. Observe health standard, proper behavior/ conduct in the holding area.
5. Keep the learners at home when he/she is sick.
6. Report any cases of COVID-19 that occurs in the household.
7. Help in providing mental health care and create positive environment for the
learners.

VII. Support Mechanisms for the KCS Required Health Standards and Protocols
The School Required Health Standards and Protocols shall be supported by the
following mechanisms:
1. The principal shall continue to lead the overall efforts of the school in
addressing the challenges through policy recommendations and strategy
development, consultation and delegation of works to the teachers and
personnel, and monitoring of the compliance of standards.
2. The school places great emphasis in the importance of maintaining clear
communications, consultation, and coordination with learners, teachers,
personnel, parents, and other education stakeholders in this time of
uncertainty.
3. The school clinic will continue to operate and manage the Preventive Alert
System in Schools (PASS), monitor the canteen to ensure the availability of
nutritious foods and ensure the availability of health kit in school.
4. The Water Sanitation and Hygiene (WASH) in School (WinS) shall ensure the
effective adoption of the proper handwashing and respiratory hygiene and
other safety precautions.
5. The School Disaster Risk Reduction and Management (DRRM) will develop
the contingency and response plan, disseminate of relevant preventive and
safety information, support and response.
6. The School Maintenance shall ensure that Materials Recovery Facilities
(MRF) is set up for proper waste segregation.
7. All Classrooms and Offices shall display Information, Education and
Communication (IEC) materials containing the key message on health and
safety.

VIII. BUDGET AND FINANCE
Funding requirements to implement the provisions set forth in this policy
guidelines shall be charged to available of funds through MOOE and other sources
of fund of the school. Utilization of funds for this purpose shall be subject to
applicable procurement, accountings and auditing rules and regulations.
IX. EFFECTIVITY/TRANSITORY PROVISIONS
This policy shall take effect immediately upon approval.
