Clinical Trial of Homoeopathic Preparations of Amyleum Nitrosum, Azathioprine, Cocainum Muriaticum and Cyclosporine in HIV Disease- a study report

This document summarizes a clinical trial conducted by the Central Council for Research in Homoeopathy in New Delhi between 1998-2003 that evaluated the effectiveness of homeopathic preparations of Amyleum Nitrosum, Azathioprine, Cocainum Muriaticum and Cyclosporine in treating 237 HIV-infected individuals. The results showed that many participants experienced emotional and physical stability and avoided opportunistic infections despite declining CD4 counts. While CD4 counts did not universally improve, some participants did experience increases. The study concluded that homeopathic medicines may help asymptomatic HIV patients and those with CD4 counts over 500, but results were variable.

The severity and mortality of COVID 19 cases has been associated with the Three category such as vaccination status, severity of disease and outcome. Objective presently study was aimed to assess the severity and mortality among covid 19 patients. Methods Using simple lottery random method 100 samples were selected. From these 100 patients, 50 patients were randomly assigned to case group and 50 patients in control group after informed consents of relative obtained. Patients in the case group who being died after got COVID 19 whereas 50 patients in the control group participated who were survive after got infected from COVID 19 patients. Result It has three categories such as a Vaccination status For the vaccination status we have seen 59 patients were not vaccinated and 41 patients was vaccinated out of 100. b Incidence There were 41 patients were vaccinated whereas 59 patients were not vaccinated. c Severity In the case of mortality we selected 50 patients who were died from the Corona and I got to know that out of 50 patients there were 12 24 patients were vaccinated whereas 38 76 patients were non vaccinated. Although for the 50 control survival group total 29 58 patients were vaccinated and 21 42 patients was not vaccinated all graph start. Conclusion we have find out that those people who got vaccinated were less infected and mortality rate very low. Background: Acute respiratory infection is a common disease in children. Most cases were due to upper respiratory tract infection. Early intervention and prompt treatment of acute respiratory infections are the easiest ways to prevent complications. Objective of the study: to determine the indications, frequency, and types of antibiotics used in hospitalized paediatric patients Messellata General Hospital , Messellata, Libya and to evaluate whether the prescribed antibiotics were based on the isolation of organism and their sensitivity. Study Design: Descriptive observational hospital based study. Results and discussion: A total of 200 child patients were included over 6 months of study period, in whom antibiotics were prescribed at the time of admission. The majority were between < 2 and 8 years of age. Fever was the commonest symptom. Out of 200 encounters for patients with various acute respiratory infections, acute pharyngotonsillits were (62.5%), followed by acute laringitis (26.5%). Acute pneumonia represented by (11%) of the total acute respiratory infection cases. Penicillins were the most commonly prescribed antibiotics for acute pharyngotonsillitis among children patients (40.8% of prescriptions), followed by cephalosporins (36.0%) and aminoglycosides (23.2%). A high percentage (59.1%) of children patients diagnosed with acute pneumonia was treated with cephalosporins, whereas (27.3%) of children patients with acute pneumonia were treated with penicillins. However, only (13.6%) of children patients with acute pneumonia often treated with aminoglycosides antibiotics. In case of acute laryngitis, the antibiotic prescription rates were as follow: Penicillins (58.5%), Cephalosporis (30.2%) and aminoglycosides (11.3%).

Researchers Stop Vitiligo Progression! New hope for millions worldwide.
Dr. Igor Korobko, Chief Scientific Director of the non-profit Vitiligo Research Foundation (http://www.vrfoundation.org/), has reported a major breakthrough in vitiligo therapy development at the 10th EADV Spring Symposium in Krakow, Poland. Data released on May 24, 2013 from VRF's preclinical study of a Pharmsyntez drug, sold under the trademark Neovir, has shown positive results in arresting vitiligo lesion progression in 73.3 percent of patients with an active form of vitiligo. In four of the 60 patients taking part in the study, vitiligo lesions re-pigmented significantly.
Sodium oxodihydroacridinylacetate (ODHAA) is the active ingredient in Neovir, with an immunomodulatory capacity that is currently being used to normalize impaired immune system functions for various conditions. Serum tumor necrosis factor-alpha (TNF-alpha) and interleukin-6 (IL-6) levels were used as predictive and end-point markers during the trials. The treatment was well tolerated by patients, and no side effects were observed.
Researchers from the VR Foundation study did not compare the drug with a control placebo or other treatment modality, and it is, as yet, unclear how long the effects will last, though there are signs that for many patients, it could be up to a year or more. It may also be used to prevent vitiligo relapse after stopping its progression with an annual administration of the drug.
It is also not clear yet whether the drug will work for segmental vitiligo, which is often seen as a distinctly different form of the disease.
This study, and others on immune system drugs, will perhaps be the most closely watched items in the Vitiligo Research Foundation’s drug re-purposing research activities.
The Foundation’s CEO, Yan Valle, noted that today scientists know the molecular cause of 4,000 diseases, but treatments are available for only 250 of them. Systematic drug development is lagging behind for many rare and complex diseases. Drug re-purposing offers some effective and cost-efficient solutions.
Mr. Valle went on to add, “We are focused on off-label testing of approved drugs that are already on the market, and thus are already available to patients immediately. Neovir is cheaply available without prescription across the former Soviet Union, it is very affordable and has an excellent long-standing safety profile,”.
At the moment it is still unclear when this drug will be available in the United States or European Union.
Coming a month before World Vitiligo Day (http://25june.org/) on June 25, it is another positive sign that there is hope on the horizon for vitiligo sufferers.

Multiple sclerosis is a chronic inflammatory neurodegenerative disorder which directly affects Central Nervous System CNS . People with MS suffer with an episodic reversible memory loss during the initial stages and later it leads to the neurological deterioration. Number of research and studies has been done on the natural compounds and phytochemical compounds in order to develop the particular drug for the treatment of MS in vivo andin vitro. The present study focuses on the inhibitory effect of Rosmarinic acid against the effect of CD2 Associated protein with the help of Molecular Docking. Molecular Docking basically screens the ligand and the target protein and shows the interaction between them on the basis of the minimum binding affinities and drug likeliness properties. In our research, docking was performed between CD2 Associated protein and selected ligands with the help of docking software. Ligands were selected on the basis of their minimum Binding affinities and finally by their drug likeliness properties. Rosmarinic acid BA 5.6 was the resultant ligand of our recent study. It showed the perfect interaction with CD2 Associated protein. Therefore, we may conclude that Rosmarinic acid may act as a compound which may be used as a drug for the treatment of multiple sclerosis fromfurther in vitro and in vivostudies in future. Antibiotic resistance is a form of drug resistance whereby certain sub-populations of a microorganism, usually a bacterial species, are able to survive after exposure to one or more antibiotics; pathogens resistant to multiple antibiotics are considered multidrug resistant (MDR) and the issue is the subject of ongoing investigation these days. The present study observed the rationality of antibiotics prescribed in an outpatient setting in a tertiary care hospital.
A prospective cohort study was conducted for 2 months on the patients who were prescribed antibiotics in a tertiary care hospital in outpatient setting. The prescriptions of patients were evaluated with respect to the medical condition of the patient. A total of 500 prescriptions were evaluated for rationality out of which 240 prescriptions (48%) were rational and 260 prescriptions (52%) were found to be irrational. The misuse of antibiotics continues to haunt the health care system of Pakistan and pose a threat of pandemic of bacterial resistance due to irrational use. Interventions by clinical pharmacists in the given situation are the need of the hour.

Clinical Safety and Side Effects of Intra Dermal regimen of Tissue culture Anti-rabies Vaccine-Rabies is 100% fatal but preventable disease. WHO recommends Tissue culture Anti-rabies Vaccines for post exposure treatment but this prophylaxis becomes expensive. So for reducing the 1/6th cost of this prophylaxis intradermal ARV regime was also recommended. But again there is a question mark for balance between cost effectiveness and safty so this cross sectional study was carried out in year 2013 on 654 recipients of Purified Chick Embriyo Cell Vaccine (PCECV) anti-rabis vaccine (ARV) at Anti Rabies Clinic (ARC) of a tertiary-care teaching hospital (SMS) at Jaipur, Rajasthan. Side effects were observed during the follow up visits on days 3, 7 and 28. Though all the recipients complained of local side effects at site of inoculation but these symptoms were relieved by simple administration of paracetamol and ceterizine orally. The side effects (local symptoms) noted on First dose were local itch (4%), local pain (3.8%), low grade fever (2.1%) and the local signs noted are local induration (22.3%), local erythema (1.2%). Same pattern of sign and symptoms were observed in D3 and D7 dose of injection but in decreased frequency. None of the cases had anaphylaxis or regional lymphadenopathy. Thus, this cost effective way of treating the animal bite cases using PCECV in Intra Dermal Rabies Vaccination (IDRV) is recommended to deal with the burden of animal bite cases for the prevention of Rabies in India.

The global HIV AIDS pandemic is still dangerous due to high incidence and high mortality rate. There are many factors associated with the immunological response beside treatment with medical practice. This paper aimed to assess the correlation of immunological responses and trust in physician measures in adult patients living with HIV AIDS. A cross sectional was studied to find the association between CD4 T cell changes and interpersonal trust in patient physician relationships. This study was carried out 93 aldult patients male accounted for 83.9 who were newly diagnosed with HIV AIDS infection at outpatient clinics, Tropical Diseases Hospital from October 2018 to August 2019. The mean TCD4 increased by 324.7 cells µl and 379.7 cells µl, respectively, compared to the original of 161.7 cells µl. Almost all patients had their immunity recovered after 3 and 6 months and the majority of patients have achieved virological success after 6 months of treatment. Factors associated with early immune recovery are youth, lower secondary education and single status. The majority of patients had truly trust in physician after 1 month and factors associated with trust in physician scale are male and employed. In Spearman rank correlation, CD4 T cell recovery and the trust present a strong relationship with p=0.008. Our finding is that the initial immunity recovery was strongly influenced by the interpersonal trust between the patients and the healthcare workers. Antibiotic resistance is a form of drug resistance whereby certain sub-populations of a microorganism, usually a bacterial species, are able to survive after exposure to one or more antibiotics; pathogens resistant to multiple antibiotics are considered multidrug resistant (MDR) and the issue is the subject of ongoing investigation these days. The present study observed the rationality of antibiotics prescribed in an outpatient setting in a tertiary care hospital.
A prospective cohort study was conducted for 2 months on the patients who were prescribed antibiotics in a tertiary care hospital in outpatient setting. The prescriptions of patients were evaluated with respect to the medical condition of the patient. A total of 500 prescriptions were evaluated for rationality out of which 240 prescriptions (48%) were rational and 260 prescriptions (52%) were found to be irrational. The misuse of antibiotics continues to haunt the health care system of Pakistan and pose a threat of pandemic of bacterial resistance due to irrational use. Interventions by clinical pharmacists in the given situation are the need of the hour.

Clinical Safety and Side Effects of Intra Dermal regimen of Tissue culture Anti-rabies Vaccine-Rabies is 100% fatal but preventable disease. WHO recommends Tissue culture Anti-rabies Vaccines for post exposure treatment but this prophylaxis becomes expensive. So for reducing the 1/6th cost of this prophylaxis intradermal ARV regime was also recommended. But again there is a question mark for balance between cost effectiveness and safty so this cross sectional study was carried out in year 2013 on 654 recipients of Purified Chick Embriyo Cell Vaccine (PCECV) anti-rabis vaccine (ARV) at Anti Rabies Clinic (ARC) of a tertiary-care teaching hospital (SMS) at Jaipur, Rajasthan. Side effects were observed during the follow up visits on days 3, 7 and 28. Though all the recipients complained of local side effects at site of inoculation but these symptoms were relieved by simple administration of paracetamol and ceterizine orally. The side effects (local symptoms) noted on First dose were local itch (4%), local pain (3.8%), low grade fever (2.1%) and the local signs noted are local induration (22.3%), local erythema (1.2%). Same pattern of sign and symptoms were observed in D3 and D7 dose of injection but in decreased frequency. None of the cases had anaphylaxis or regional lymphadenopathy. Thus, this cost effective way of treating the animal bite cases using PCECV in Intra Dermal Rabies Vaccination (IDRV) is recommended to deal with the burden of animal bite cases for the prevention of Rabies in India.

The global HIV AIDS pandemic is still dangerous due to high incidence and high mortality rate. There are many factors associated with the immunological response beside treatment with medical practice. This paper aimed to assess the correlation of immunological responses and trust in physician measures in adult patients living with HIV AIDS. A cross sectional was studied to find the association between CD4 T cell changes and interpersonal trust in patient physician relationships. This study was carried out 93 aldult patients male accounted for 83.9 who were newly diagnosed with HIV AIDS infection at outpatient clinics, Tropical Diseases Hospital from October 2018 to August 2019. The mean TCD4 increased by 324.7 cells µl and 379.7 cells µl, respectively, compared to the original of 161.7 cells µl. Almost all patients had their immunity recovered after 3 and 6 months and the majority of patients have achieved virological success after 6 months of treatment. Factors associated with early immune recovery are youth, lower secondary education and single status. The majority of patients had truly trust in physician after 1 month and factors associated with trust in physician scale are male and employed. In Spearman rank correlation, CD4 T cell recovery and the trust present a strong relationship with p=0.008. Our finding is that the initial immunity recovery was strongly influenced by the interpersonal trust between the patients and the healthcare workers. Immunotherapy for cancer has had two main approaches that have lead to clinical applications. The first is stimulating immune responses to tumor cells with cytokines or cellular immunotherapy and the second is blocking tumor immune evasion and the associated inhibition of T-cell activation with antibodies to the CTLA-4 receptor, PD-1 receptor or PD-L1. At OncBioMune, we have taken a different approach and have developed therapeutic cancer vaccines that are a combination of tumor antigens (whole cells or proteins) with biological adjuvants (the cytokines IL-2 and GM-CSF). This study is a Phase 1a/1b clinical trial of a PSA/IL-2/GM-CSF vaccine in recurrent prostate cancer in hormone-naïve and hormone-independent patients. Major inclusion criteria include adenocarcinoma of the prostate, rising serum PSA and no measurable disease. Phase 1a examines the rate of dose limiting adverse events (DLAEs) in an initial course of 6 vaccinations (“induction vaccination”). The Phase 1b examines the rate of DLAEs with a continued coarse of an additional 6 vaccinations (“maintenance vaccine”). All patients will receive intradermal injections of the PSA/IL-2/GM-CSF vaccine at weeks 1, 2, 3, 7, 11, and 15. In an additional 28 patients the six maintenance vaccines will alternate IL-2 and the complete vaccine (PSA/IL-2/GM-CSF) at weeks 23, 27, 31, 35, 39 and 43. To date, twelve of twenty patients in the Phase 1a portion of the trial have received at least one vaccine injection and ten patients have received all 6 vaccines. Seven of the ten patients that have received 3 vaccines had increased responses to PSA in a lymphocyte blastogenesis assay and five of the nine patients had an increase in their response after 6 vaccines. None of the patients vaccinated in the Phase 1a portion have had a DLAE and enrollment continues in the Phase 1a.

INTRODUCTION
The human immunodeficiency virus (HIV) targets the immune system and weakens people's defense against many infections and some types of cancer that people with healthy immune systems can fight off. As the virus destroys and impairs the function of immune cells, infected individuals gradually become immunodeficient. Immune function is typically measured by CD4 cell count.
The most advanced stage of HIV infection is acquired immunodeficiency syndrome (AIDS), which can take many years to develop if not treated, depending on the individual. AIDS is defined by the development of certain cancers, infections or other severe long-term clinical manifestations.
Since HIV was first identified almost 30 years ago, remarkable progress has been made in improving the quality and duration of life for people living with HIV disease.
HIV or human immunodeficiency virus and acquired immunodeficiency syndrome is a chronic condition that requires daily medication.
HIV- 1 is a retrovirus isolated and recognized as the etiologic agent of AIDS.

Introduction
C. S. is a 33-year-old, married, African American male who presented to the ED for progressively worsening body aches, abdominal pain, and swelling and draining in the peri-rectal and perineal area. Patient stated he “developed a pimple on his buttocks a week ago and it broke open today”. Patient also stated his “weakness and pain have been worsening over the past week”.
The only medical history consisted of hypertension and insulin dependent diabetes diagnosed four years ago, but patient reports he has not been taking insulin for at least one week. Patient is employed full-time and denies any family medical history, allergies, or alcohol, tobacco, or drug use. Patient was diagnosed with diabetic ketoacidosis (DKA) and peri-rectal abscess. The process of drug discovery and development is a complex and multi-step endeavor aimed at bringing new pharmaceutical drugs to market. It begins with identifying and validating a biological target, such as a protein, gene, or RNA, that is associated with a disease. This step involves understanding the target's role in the disease and confirming that modulating it can have therapeutic effects. The next stage, hit identification, employs high-throughput screening (HTS) and other methods to find compounds that interact with the target. Computational techniques may also be used to identify potential hits from large compound libraries.
Following hit identification, the hits are optimized to improve their efficacy, selectivity, and pharmacokinetic properties, resulting in lead compounds. These leads undergo further refinement to enhance their potency, reduce toxicity, and improve drug-like characteristics, creating drug candidates suitable for preclinical testing. In the preclinical development phase, drug candidates are tested in vitro (in cell cultures) and in vivo (in animal models) to evaluate their safety, efficacy, pharmacokinetics, and pharmacodynamics. Toxicology studies are conducted to assess potential risks.
Before clinical trials can begin, an Investigational New Drug (IND) application must be submitted to regulatory authorities. This application includes data from preclinical studies and plans for clinical trials. Clinical development involves human trials in three phases: Phase I tests the drug's safety and dosage in a small group of healthy volunteers, Phase II assesses the drug's efficacy and side effects in a larger group of patients with the target disease, and Phase III confirms the drug's efficacy and monitors adverse reactions in a large population, often compared to existing treatments.

Involves a cascade effect through G-proteins and second messengers, leading to depolarization and action potential generation in the olfactory nerve.
Quality of a Good Odorant:
Small (3-20 Carbon atoms), volatile, water-soluble, and lipid-soluble.
Facilitated by odorant-binding proteins in mucus.
Membrane Potential and Action Potential:
Resting membrane potential: -55mV.
Action potential frequency in the olfactory nerve increases with odorant strength.
Adaptation Towards the Sense of Smell:
Rapid adaptation within the first second, with further slow adaptation.
Psychological adaptation greater than receptor adaptation, involving feedback inhibition from the central nervous system.
Primary Sensations of Smell:
Camphoraceous, Musky, Floral, Pepperminty, Ethereal, Pungent, Putrid.
Odor Detection Threshold:
Examples: Hydrogen sulfide (0.0005 ppm), Methyl-mercaptan (0.002 ppm).
Some toxic substances are odorless at lethal concentrations.
