I've been fortunate in my career to have direct exposure to successful CEOs through CIO consulting advisory, speaking to CEO peer groups such as Vistage and Renaissance Forums, participating in prestigious CEO summits, serving as a member of executive steering committees, as a certified corporate director and as a former member of the National Association of Corporate Directors (NACD).
For most CEOs that have come up through the CFO and COO ranks, IT can be viewed as an overly complex, hard to understand cost only center. IT is incredibly complex to lead and technology continues to change at a dizzying pace. IT can sometimes be seen as an inhibitor and not a partner that drives innovation for top line results while improving processes and that positively impact the bottom line.
I've noticed some common IT management problems that occur for CEOs that can be easily addressed through the use of best practices. I can't cover all the best practices in this article but I can highlight some key observations based upon my experience reporting to and collaborating with CEOs.

slide3:Simply put, it’s putting structure around how organizations align IT strategy with business strategy, ensuring that companies stay on track to achieve their strategies and goals, and implementing good ways to measure IT’s performance. It makes sure that all stakeholders’ interests are taken into account and that processes provide measurable results. An IT governance framework should answer some key questions, such as how the IT department is functioning overall, what key metrics management needs and what return IT is giving back to the business from the investment it’s making.
slide 5:• Strategic alignment: Linking business and IT so they work well together. Typically, the lightning rod is the planning process, and true alignment can occur only when the corporate side of the business communicates effectively with line-of-business leaders and IT leaders about costs, reporting and impacts.
• Value delivery: Making sure that the IT department does what’s necessary to deliver the benefits promised at the beginning of a project or investment. The best way to get a handle on everything is by developing a process to ensure that certain functions are accelerated when the value proposition is growing, and eliminating functions when the value decreases.
• Resource management: One way to manage resources more effectively is to organize your staff more efficiently—for example, by skills instead of by line of business. This allows organizations to deploy employees to various lines of business on a demand basis.
• Risk management: Instituting a formal risk framework that puts some rigor around how IT measures, accepts and manages risk, as well as reporting on what IT is managing in terms of risk.
• Performance measures: Putting structure around measuring business performance. One popular method involves instituting an IT Balanced Scorecard, which examines where IT makes a contribution in terms of achieving business goals, being a responsible user of resources and developing people. It uses both qualitative and quantitative measures to get those answers.
One of the most daunting challenges organizations face in making decisions on what technology is needed to fully enable the business to achieve its strategy and objectives. The key is ALIGNMENT.

The success of an HIT systems implementation initiative will depend on many things going well. We’ve highlighted a few of the key processes that you will need to engage in, such as strategic planning, user needs analysis, and training. There are many other areas that will need to be considered, and to help you keep up with all of the things to consider you should seek out resources tailored to the particular setting and type of HIT system you will be deploying (such as the DOQ-IT resource highlighted later for EHR implementation in a doctor’s office setting).Some general key success factors that will apply to most systems acquisition projects include the readiness of the organization, availability of a champion, perceived usefulness of the system, and organizational teamwork. Is the organization ready for the new system? Do they understand what the system can do? Are they prepared to spend the time and effort to investigate the options and choose a system? Are they prepared to devote the time necessary for implementing a new system? This, by the way, is never insignificant and should be carefully planned for. You or your users should expect for work to be MORE difficult as a new system is implemented and integrated into the organization, since there is often an overlap period as old business processes are phased out, and new ones are brought in, and everyone struggles to learn. However, there is a light at the end of the tunnel, and the more you prepare users to understand exactly what the implementation process will be, and convince users of the usefulness of the new system, the more likely you are to have a satisfied workforce.Finally, do you have a champion who is behind the project and committed to its success, and is this champion available to provide needed time to the project? Does your organization demonstrate good teamwork? This is particularly important for smaller organizations, since successful change often occurs more organically from within the ranks rather than from the top down. Making connections with people and ensuring their commitment to the project is more important than the technology and its capabilities.So, as you embark on a systems acquisition project, ask yourself these questions. If you can’t answer them in the affirmative, your project may struggle.
Other challenges to adoption and integration of HIT include issues about funding. While the HITECH incentive payments to meaningful users of qualified EHRS have taken some of the wind out of this argument, there is still push-back. So quote “Officials at CMS estimate the EHR system adoption or upgrades of existing systems to meet federal standards will cost on average about $54,000 per physician employee in a practice, while annual maintenance will cost an average of $10,000 per physician employee”. And that comes from the Psychiatric News; May 7, 2010 ; Volume 45 Number 9 Page 6; from the American Psychiatric Association. So many clinicians feel that they’re going to get stuck with the tab, but the major benefactors are going to be insurance companies and the government. So this may have some element of truth to it – although I return to my mantra of “you can’t manage what you can’t measure and you can’t measure that which is trapped in paper data cemeteries”. Eventually we may all benefit and I would hope that the majority of the benefit attributes to the patient.So as mentioned earlier workflow and culture are two key barriers to adoption. Culture eats strategy for lunch.There’s also a fair amount of turnover in healthcare. So turnover is one of the things that leads to lower adoption.
