In compliance with the provisions of Article 53 (1) of R. A. 9520; Sections 2 and 7, Rule 8, Rules
and Regulations Implementing certain Provisions of R.A. 9520, and CDA Memorandum Circular No.
2012-09, presented below are the required annual regular reports for submission by cooperatives
to the Cooperative Development Authority, including the period of compliance, accountable
officers and the amount of penalty for delay of submission:
