Anxious Anna Anna is a thirty-four year-old white female. Currently she is married with four children and works full-time as a homemaker. Her non-verbals (picking at nails and clothes, very talkative) indicated a moderate level of anxiety regarding the appointment. The client perceives her primary difficulty at this time to be marital issues including betrayal and a desire to forgive and submit. Anna was raised by a maternal grandmother. She left home at the age of sixteen, to escape her father’s sexual abuse. Anna married her current husband in 1992, although they had already had a child together before that time. Neither Joe nor Anna were Christians when they met, and she briefly described a history of personal alcohol abuse, as well her husband’s drug and alcohol abuse and an escalating pattern of pornography addiction. Both Anna and her husband were recently saved at a Pentecostal Church; a decision that Anna believes dramatically changed their lives. ‘Recently’ (2 years ago) Anna was made aware of her husband’s affair with a seventeen year old. She states this is the first she knew of his adulterous behavior, although she describes his long standing patterns of staying out all night, finding hickeys and following him to bars where he would leave with other women. She states that she is very angry with him about this behavior, although she believes that this was an isolated incident. When discovering that she was aware of the affair Joe also became physically violent with Anna, but she states that this was an isolated incident as well. Anna’s goal for seeking counseling was to learn how to control her anger toward her husband and gain coping skills for the situation. She desires to learn how to forgive him for his past actions and learn how to submit to him. On her intake papers Anna describes herself as a ‘kind, loving, affectionate, meek shy and blessed’ person’; her husband believes she has low self-esteem and little confidence. Currently Anna is on Oxyeontin for pain management; Flexoral for muscle spasms and Relafen for arthritis. She currently suffers from Fibromyalgia and Osteo-arthritis. The client indicated prior counseling at SMH in 89-90 for Anorexia; she also has a brief history of lesbian behavior. On the phone, as well as during our initial meeting Anna presented as a very chaotic and angry person. She arrived at the session approximately twenty minutes late after getting lost on the way – apologizing profusely when I went to greet her. Once inside the office she asked if she could start the session with prayer, then got on her knees and touching her forehead to the ground and prayed for approximately five minutes, primarily asking the Lord to help her become a more submissive wife to her husband. It was extremely difficult to end the session, with our time lasting almost twenty minutes over the hour; she was not prepared to pay at that time. She subsequently missed her follow-up appointme.

Hi, and thanks for attending this case presentation. My name is Dr. Stephen Brewer and I am a licensed
clinical psychologist in San Diego, California and Assistant Professor of Psychology and Applied
Behavioral Sciences at Ashford University. Today, I will be sharing with you the story of Bob.
Presenting problem
Bob Smith is a 36-year-old man who came to me approximately six months ago with concerns about his
career choice and life direction. He did not have any significant psychiatric symptoms, besides some
understandable existential anxiety regarding his future. Bob was cooperative, friendly, open, and
knowledgeable about psychology during our first few sessions together. I noticed that he seemed
guarded only when talking about his family and childhood experiences. To confirm his identity, I checked
his driver’s license to ensure his name was indeed Bob Smith and that he lived close by in a mobile home
in Spring Valley. Given his relatively mild symptoms, we decided to meet once a week for supportive
psychotherapy so he could work through his anxieties. I gave him a diagnosis of adjustment disorder
with anxiety.
History
Here’s some background on Bob to give you a sense of who he is.
Family
Bob grew up as an only child in Edmonton, Canada, in a low-income, conservative, and very religious
household.
He shared that his father was largely absent during his childhood, as he spent most of the week residing
north of Edmonton, where he worked as a mechanic in the oil fields near Fort McMurray. On weekends,
Bob’s father would return home and spend as much time as possible with his family. Bob described his
father as warm, caring, and a hard worker. His father reportedly died one year ago.
Bob’s mother was described as a strict, rule-based woman who had a short temper and was prone to
furious outbursts over trivial matters. She worked in Bob’s junior high as a janitor, which meant that Bob
often crossed paths with his mother at school, where she would often check up on him. During Bob’s
high school years, Bob’s mother got a new job as a high school librarian.
At 18, Bob moved to San Diego to study psychology at San Diego State University. He lived in the dorms
for his first few years, where he easily made friends and joined a fraternity. Bob maintained contact with
his parents, but ceased all contact when his mother suggested she would move to San Diego to be closer
to him. He graduated with a 3.2 GPA and began working for the county as a psychiatric technician. He
worked as a psych tech for 14 years and described it as “fun at first, but it got boring and predictable
after a while.”

Bob shared that he has a medical doctor that he visits once every few years for his routine physical. He

Aaron is a 24-year-old, unmarried, heterosexual, Caribbean immigrant male who is experiencing symptoms of anxiety and depression. Aaron reports no history of mental health treatment nor any medical or legal problems. He admits to social drinking but denies use of illegal substances. He lives alone in a room he rents above the restaurant where he works. He works 24 hours a week as a waiter, has few friends, and is a part-time student at a local university where he is working on an undergraduate degree in biology. Aaron came to speak with me, a university counselor, because he is having difficulty concentrating and finding the motivation to study. Aaron denied any thoughts or plans of suicide or homicide and stated he felt hopeless and nervous.
In the first session, Aaron struggled with sustaining eye contact, presented as preoccupied, and was indifferent to the attempts to engage him in the intake process. When asked what he thought precipitated counseling, Aaron said that he had a difficult relationship with his parents who, he stated, “are not supportive and could care less” about him. He also reported that his younger brother was killed not long ago. When asked what he wanted to work on in counseling, he said that he wanted to address why his family was so “messed up.” Subsequent sessions explored Aaron’s perspective on his family, the strained relationship between Aaron and his parents, and the loss of his sibling.
During one session, Aaron said his parents had always favored his younger brother and overlooked his criminal involvement, which had been a source of conflict between Aaron and his parents for years. While it had not been confirmed, Aaron suspected his brother’s death was related to gang involvement. Aaron shared that his academic interests and achievement had been ignored by his parents and had never been a source of interest for them.
In a subsequent session, Aaron stated that he had always felt disconnected and different from his parents and brother. Aaron’s family immigrated to the United States from Guyana when Aaron was 8 years old and his brother was 2 years old. His parents brought only his brother and left Aaron with his grandmother, informing him they would bring him over when they were settled. Seven years later, at the age of 15, he joined his family. Aaron reported that reuniting with his family after all that time was difficult. Aaron had always felt rejected by his parents because they did not bring him to the United States with his brother. He experienced a void in his relationship with his parents and his brother, and he felt there was an unspoken alliance between his parents and his younger brother that he did not share. Aaron said that he was often made fun of by them for not losing his accent and for his use of their culture’s traditions and customs. Dear Judge Kirk:
Sue Jones is a 52 year old Caucasian female who was referred by the Court for a guardianship evaluation.
Dr. Betty Rubble interviewed Ms. Jones at Anytown Nursing Home on January 1, 2016 for approximately 105 minutes. She was administered the Independent Living Scales on that date.
Prior to the commencement of this evaluation and psychological testing, Ms. Jones was advised of the nature and purpose of the evaluation. Ms. Jones was informed that the resulting report was not confidential, and that information obtained could be included in the report that would be submitted to the Court. She was aware this information was not related to treatment, but rather for her current case. Ms. Jones was provided this information both orally and in a written format. She stated that she understood the information provided to her, including the limits of confidentiality and her rights concerning the evaluation.

SOCIAL HISTORY: Ms. Jones reported that she was born on January 1, 1963 and reared in Kentucky. She said her father worked as a security guard and died eight years ago, while her mother worked as a waitress and died five years ago. She identified having a “good” relationship with her parents. Ms. Jones said she has two brothers with whom she has an “all right” relationship, as well as one older maternal half sister that she “[doesn’t ] get along with at all.” Ms. Jones stated that one of her brother has been diagnosed with bipolar disorder, and noted her brothers and her father had difficulties with alcoholism. She denied any childhood history of abuse and reported that she ran away from home at 17 years of age when she became pregnant.
Ms. Jones reported that she lived independently until two years ago when she was placed in a nursing home. She said she remains in a nursing home against her will because the court has appointed her a legal guardian due to her alcoholism. She would like to return to her home of Nowhere, California, where her cousin lives. She said that she talks with her cousin regularly on the phone, but acknowledged that she has not seen her in many years. She does not want a guardian and would like to make her own decisions.
According to her legal guardian, Ms. Flintstone, prior to her nursing-home placement, Ms. Jones was in sober housing. That home had staff present on site, but Ms. Jones continued to drink alcohol and visit hospital emergency rooms to obtain opiates.
EDUCATION HISTORY: Ms. Jones stated that she last completed the 9th grade and had “all right” grades. She said that she was not diagnosed with any learning.

Case Study of Donna Little - Chapter 5
Donna Little is a 39-year-old Indian woman who has a history of substance misuse and has struggled with reunification with her adolescent children over the last 6 years. She was in residential school from the age of 6 to 16 years old. She has a history of domestic violence in her previous relationships. Donna was the youngest of four children in her family. Her parents, siblings, and herself were raised in the same small northern reservation. Both her parents had gone to residential school in the early 1950s, as did her grandfathers and grandmothers on both sides of her family system in the late 1910s. Donna was raised in an environment of violence and mayhem in her early childhood, which she has talked about quite extensively in counseling. Although her parents abused alcohol, she emphasizes repeatedly that her family was quite ceremonial and participated in the big drum feast and singing within the community. When Donna was 6, an Indian agent wearing a red, white, and black checkered jacket gave her candy and took her to the residential school. She never had the opportunity to say good-bye to her mom and dad, who died of tuberculosis while she was in the residential school. Donna reflects on her residential school experience with a despondent look. While in the residential school, she had only one friend she could count on. Her siblings, who were also at the school, were older and thus not allowed to play with her or sleep near her at the residence dorms. This created an incredible loneliness that Donna did not know how to fill, and often she would use alcohol to help numb that pain. She did not like to drink, but it helped her to stop her thinking badly about the past. Donna was a victim of sexual abuse in the residential school, primarily by the Roman Catholic priest who was in charge. The first time she was assaulted she was 7; the last assault occurred right before she ran away at age 16. When Donna had attempted to tell the head nun in charge of her dorm what was happening to her, she was beaten severely, to the point of unconsciousness. Donna recalls it was her friend, Sue, who nursed her back to health. Donna describes her life as difficult. She went home to her community, only to find a partner who turned out to be as violent toward her as her father was to her mother. She loves her children and cares for them deeply. She breast-fed her three children and still today can feel that connection to them. When her children were taken from her home after the last time her husband beat her, she spiraled out of control. Dear Judge Kirk:
Sue Jones is a 52 year old Caucasian female who was referred by the Court for a guardianship evaluation.
Dr. Betty Rubble interviewed Ms. Jones at Anytown Nursing Home on January 1, 2016 for approximately 105 minutes. She was administered the Independent Living Scales on that date.
Prior to the commencement of this evaluation and psychological testing, Ms. Jones was advised of the nature and purpose of the evaluation. Ms. Jones was informed that the resulting report was not confidential, and that information obtained could be included in the report that would be submitted to the Court. She was aware this information was not related to treatment, but rather for her current case. Ms. Jones was provided this information both orally and in a written format. She stated that she understood the information provided to her, including the limits of confidentiality and her rights concerning the evaluation.

SOCIAL HISTORY: Ms. Jones reported that she was born on January 1, 1963 and reared in Kentucky. She said her father worked as a security guard and died eight years ago, while her mother worked as a waitress and died five years ago. She identified having a “good” relationship with her parents. Ms. Jones said she has two brothers with whom she has an “all right” relationship, as well as one older maternal half sister that she “[doesn’t ] get along with at all.” Ms. Jones stated that one of her brother has been diagnosed with bipolar disorder, and noted her brothers and her father had difficulties with alcoholism. She denied any childhood history of abuse and reported that she ran away from home at 17 years of age when she became pregnant.
Ms. Jones reported that she lived independently until two years ago when she was placed in a nursing home. She said she remains in a nursing home against her will because the court has appointed her a legal guardian due to her alcoholism. She would like to return to her home of Nowhere, California, where her cousin lives. She said that she talks with her cousin regularly on the phone, but acknowledged that she has not seen her in many years. She does not want a guardian and would like to make her own decisions.
According to her legal guardian, Ms. Flintstone, prior to her nursing-home placement, Ms. Jones was in sober housing. That home had staff present on site, but Ms. Jones continued to drink alcohol and visit hospital emergency rooms to obtain opiates.
EDUCATION HISTORY: Ms. Jones stated that she last completed the 9th grade and had “all right” grades. She said that she was not diagnosed with any learning.

Case Study of Donna Little - Chapter 5
Donna Little is a 39-year-old Indian woman who has a history of substance misuse and has struggled with reunification with her adolescent children over the last 6 years. She was in residential school from the age of 6 to 16 years old. She has a history of domestic violence in her previous relationships. Donna was the youngest of four children in her family. Her parents, siblings, and herself were raised in the same small northern reservation. Both her parents had gone to residential school in the early 1950s, as did her grandfathers and grandmothers on both sides of her family system in the late 1910s. Donna was raised in an environment of violence and mayhem in her early childhood, which she has talked about quite extensively in counseling. Although her parents abused alcohol, she emphasizes repeatedly that her family was quite ceremonial and participated in the big drum feast and singing within the community. When Donna was 6, an Indian agent wearing a red, white, and black checkered jacket gave her candy and took her to the residential school. She never had the opportunity to say good-bye to her mom and dad, who died of tuberculosis while she was in the residential school. Donna reflects on her residential school experience with a despondent look. While in the residential school, she had only one friend she could count on. Her siblings, who were also at the school, were older and thus not allowed to play with her or sleep near her at the residence dorms. This created an incredible loneliness that Donna did not know how to fill, and often she would use alcohol to help numb that pain. She did not like to drink, but it helped her to stop her thinking badly about the past. Donna was a victim of sexual abuse in the residential school, primarily by the Roman Catholic priest who was in charge. The first time she was assaulted she was 7; the last assault occurred right before she ran away at age 16. When Donna had attempted to tell the head nun in charge of her dorm what was happening to her, she was beaten severely, to the point of unconsciousness. Donna recalls it was her friend, Sue, who nursed her back to health. Donna describes her life as difficult. She went home to her community, only to find a partner who turned out to be as violent toward her as her father was to her mother. She loves her children and cares for them deeply. She breast-fed her three children and still today can feel that connection to them. When her children were taken from her home after the last time her husband beat her, she spiraled out of control. Donna has had long periods of abstinence, has a home in her community that is well cared for, and now has a partner who loves her deeply. He lives alone in a room he rents above the restaurant where he works. He works 24 hours a week as a waiter, has few friends, and is a part-time student at a local university where he is working on an undergraduate degree in biology. Aaron came to speak with me, a university counselor, because he is having difficulty concentrating and finding the motivation to study. Aaron denied any thoughts or plans of suicide or homicide and stated he felt hopeless and nervous.
In the first session, Aaron struggled with sustaining eye contact, presented as preoccupied, and was indifferent to the attempts to engage him in the intake process. When asked what he thought precipitated counseling, Aaron said that he had a difficult relationship with his parents who, he stated, “are not supportive and could care less” about him. He also reported that his younger brother was killed not long ago. When asked what he wanted to work on in counseling, he said that he wanted to address why his family was so “messed up.” Subsequent sessions explored Aaron’s perspective on his family, the strained relationship between Aaron and his parents, and the loss of his sibling.

Joan and Francine had been together for 30 years, and while Francine personally identifies as a lesbian, she never came out to her family or to her colleagues. When speaking to all but her closest confidantes, Francine referred to Joan as her “best friend” or her “roommate.” Francine’s bereavement was therefore complicated because she did not feel she could discuss the true nature of her partnership with Joan. She felt that there was little recognition from her family, and even some of her close associates, of the impact and meaning of Joan’s death to Francine. There is a history of alcohol abuse in Francine’s family, and Francine abused alcohol from late adolescence into her mid-30s. However, Francine has been in recovery for several decades. Francine has no known sexual abuse history and no criminal history.
Francine sought counseling with me for several reasons, including an ongoing depressed mood, a lack of pleasure or enjoyment in her life, and loneliness and isolation since Joan’s death. She also reported that she had begun to drink again and that while her drinking was not yet at the level it had been earlier in her life, she was concerned that she could return to a dependence upon alcohol. Francine came to counseling with several considerable strengths, including a capacity to form intimate relationships, a successful work history, a history of having maintained her sobriety in the past for many years, as well as insight into the factors that had contributed to her current difficulties.
During our initial meetings, Francine stated that her goals were to feel less depressed, to reduce or stop drinking, and to feel less isolated. In order to ensure that no medical issues were contributing to her depression symptoms, I referred Francine to her primary care physician for an evaluation. Francine’s physician did not find any medical cause of her symptoms, diagnosing Francine with moderate clinical depression and recommending that Francine begin a course of antidepressant medication. Francine was reluctant to take medication and first wanted to try a course of counseling.
In order to help Francine meet her goal of reducing her depression symptoms, I employed a technique called behavioral activation (BA), which is drawn from principles of cognitive behavioral therapy and helps to reengage people in pleasant physical, social, and recreational activities. We began with a small initial goal of having Francine dedicate at least 5 minutes of each day to an activity she found pleasant or rewarding. Over the following weeks, we increased the time. The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature
death. We discussed the strengths that he and Carol demonstrated
in staying together and working out their p.

Aaron came to speak with me, a university counselor, because he is having difficulty concentrating and finding the motivation to study. Aaron denied any thoughts or plans of suicide or homicide and stated he felt hopeless and nervous.
In the first session, Aaron struggled with sustaining eye contact, presented as preoccupied, and was indifferent to the attempts to engage him in the intake process. When asked what he thought precipitated counseling, Aaron said that he had a difficult relationship with his parents who, he stated, “are not supportive and could care less” about him. He also reported that his younger brother was killed not long ago. When asked what he wanted to work on in counseling, he said that he wanted to address why his family was so “messed up.” Subsequent sessions explored Aaron’s perspective on his family, the strained relationship between Aaron and his parents, and the loss of his sibling.
During one session, Aaron said his parents had always favored his younger brother and overlooked his criminal involvement, which had been a source of conflict between Aaron and his parents for years. While it had not been confirmed, Aaron suspected his brother’s death was related to gang involvement. Aaron shared that his academic interests and achievement had been ignored by his parents and had never been a source of interest for them.
In a subsequent session, Aaron stated that he had always felt disconnected and different from his parents and brother. Aaron’s family immigrated to the United States from Guyana when Aaron was 8 years old and his brother was 2 years old. His parents brought only his brother and left Aaron with his grandmother, informing him they would bring him over when they were settled. Seven years later, at the age of 15, he joined his family. Brandon has previously participated in therapy to address challenging behaviors, including physical aggression, difficulty following rules at home and school, and using inappropriate language with sexual overtones toward female peers. Brandon and his mother report that they ceased participating in therapy in the past after there was no change in Brandon’s behavior. Brandon’s teachers have suggested that his behaviors are similar to those of peers with attention deficit hyperactivity disorder, but his mother has declined educational or psychological testing because she does not want her son to be labeled and is unsure if she agrees with the use of psychotropic medication with children.
Brandon began attending trauma-focused treatment after demonstrating an increase in argumentative behavior and minor property destruction at home. His mother reported that the majority of undesired behaviors were initiated during interactions with her boyfriend. Brandon’s use of physical aggression has not increased in school; however, a female peer recently reported him for using sexually explicit language toward her. Brandon admitted to using inappropriate language toward the female peer but appeared to have a limited understanding of what the phrases used meant. Darian also denies smoking or regular alcohol or drug usage. He has been drunk a few times, but Eboni reports that he does not think it is a problem. Eboni and Darian became sexually active soon after they started dating, and they were using withdrawal for birth control.
Eboni’s mother, Darlene, is 34 years old and also biracial African American/Caucasian. She works as an adminis trative assistant for a local manufacturing company. Eboni has lived with her mother and her maternal grandmother, May, from the time she was born. May is a 55-year-old African American woman who works as a paraprofessional in an elementary school. They still live in the same apartment where May raised Darlene.
Darlene met Eboni’s father, Anthony, when she was 17, the summer before their senior year in high school. Anthony is 34 years old and Caucasian. They casually dated for about a month, and after they broke up, Darlene discovered she was pregnant and opted to keep the baby. Although they never married each other, Anthony has been married twice and divorced once. He has four other children in addition to Eboni. She visits her father and stepmother every other weekend. Anthony works as a mechanic and pays child support to Darlene.
Recently, Eboni took a pregnancy test and learned that she is 2 months pregnant. She actually did not know she was pregnant because her periods were not always consistent and she thought she had just skipped a couple of months. Eboni immediately told her best friend, Brandy, and then Darian about her pregnancy. He was shocked at first and suggested that it might be best to terminat.

John is a 38-year-old male of Euro-American heritage. He has never been married, and
he has one brother whom he rarely sees. John is a long-distance truck driver, and he
spends the vast majority of his time living out of his vehicle driving across the country.
He has a place he calls “home,” but he spends only about a week there every few
months. As a result, John has few close friends and no significant romantic relationship
in his life. John has also alienated several family members and close friends from whom
he has borrowed substantial amounts of money that he has not paid back.
John was referred by his employer’s EAP following an issue while at work. John had
lied about his time log and location, as he had stopped to gamble at a casino in
Mississippi during a cross-country run. He was put on probation for this infraction, and
ordered to seek counseling services. Though John denies having a “gambling problem,”
he admits that he does enjoy gambling and engages in both in-person and online
gaming whenever he has an opportunity. John also admits that he has lost a lot of
money to gambling, though rationalizes this with “at least I was having fun.” He also
notes that his gambling has cost him past relationships and prevented him from
reaching financial goals such as home ownership and retirement planning. John shares
that “it would be different if I was married or something. It’s just me, so who cares if this
is how I spend my money?”
Donna
Donna is a 40-year-old transgender female of Korean American heritage. She is the
manager of a popular local restaurant and club, and is well respected at her job. She
has come to counseling at the insistence of her boyfriend, who has expressed worry
over her increasing drug use. Donna also admits that she and her boyfriend have been
fighting a lot lately, and she is worried he might be cheating on her. In fact, she admits
that a recent altercation became physical, although she blames it on her “temporary
meth rage.”
Donna shares very openly about her use of methamphetamine, although she insists that
she has it “under control.” She states that in her 20s, she used a lot of recreational
drugs, but for a number of years after remained completely abstinent. She claims she
currently uses meth “just every now and then” because she likes the rush and energy it
gives her. She minimizes its role in the altercation with her boyfriend, stating that she
had used a little too much that day but “that never happens.” While Donna tends to
minimize the role and significance of her drug use, she states, “Maybe I could use a
little help getting my temper under control.”

Client presented in the emergency room (ER) having been brought in the previous night by her parents. Following an argument with her parents, L cut her right wrist. L's mother reported that L started screaming rapidly and became physically violent toward her prior to cutting her own wrist.
Psychological Data
L is a 17-year-old Hispanic female who resides in Pennsylvania with her mother, father, and older sister. She is in 11th grade at the local public school. L appeared to be of average to above-average intelligence, as she was able to respond to numerous questions in an articulate and intelligent manner. She was well versed about world history and current affairs. Her mother confirmed that she has done well in school, maintaining a B+ average and participating in various school activities (e.g., chorus, school paper) until last year. L slowly dropped out of many activities she liked in the past. Her mother noticed about 8 months ago that L had also begun having difficulty doing schoolwork. Erratic behavior arose during episodes when L also became irritable and explosive. During these repeated episodes, she became quite defiant, cut classes, had to be placed in school detention, and had even assaulted the principal. L has numerous friends and believed she can relate to all types of people. She has a boyfriend who adores her, but she said she doesn't feel the same about him. The school counselor confirmed that L is outgoing, popular, and smart; but during these episodes she became another person, one who is very violent and difficult.
Medical History
A physical examination by a staff doctor revealed superficial cuts on L’s left and right wrist. The cuts appeared to be a few weeks old. There were cigarette burns on her right wrist that looked to be approximately one week old. In questioning L about the cigarette burns, L responded, "I just wanted to see how it felt—now I know." When questioned about old cuts on her left wrist, she responded, "I don't want to talk about it." L weighs 103 pounds and is 5’ 6” tall. L denied any dieting or fasting, but her mother noticed over this past year that her weight has dropped. Substance Abuse History L denied any drug or alcohol use. When she was questioned regarding such, her response was "I could do drugs if I wanted to. I don't want to, because it’s dumb." Family History L’s mother is 42 years old and works as a secretary for a large telephone company. Her father is 49 years old and operates a small landscaping business. Both are U.S. citizens, with a cultural background from Guatemala of which they are proud. Both have 2 a high school education. L’s sister is considerably younger, aged 8.

Working With the Aging: The Case of Francine
Francine is a 70-year-old, Irish Catholic female. She worked for 40 years as a librarian in an institution of higher education and retired at age 65. Francine has lived alone for the past year, after her partner, Joan, died of cancer. Joan and Francine had been together for 30 years, and while Francine personally identifies as a lesbian, she never came out to her family or to her colleagues. When speaking to all but her closest confidantes, Francine referred to Joan as her “best friend” or her “roommate.” Francine’s bereavement was therefore complicated because she did not feel she could discuss the true nature of her partnership with Joan. She felt that there was little recognition from her family, and even some of her close associates, of the impact and meaning of Joan’s death to Francine. There is a history of alcohol abuse in Francine’s family, and Francine abused alcohol from late adolescence into her mid-30s. However, Francine has been in recovery for several decades. Francine has no known sexual abuse history and no criminal history.
Francine sought counseling with me for several reasons, including an ongoing depressed mood, a lack of pleasure or enjoyment in her life, and loneliness and isolation since Joan’s death. She also reported that she had begun to drink again and that while her drinking was not yet at the level it had been earlier in her life, she was concerned that she could return to a dependence upon alcohol. Francine came to counseling with several considerable strengths, including a capacity to form intimate relationships, a successful work history, a history of having maintained her sobriety in the past for many years, as well as insight into the factors that had contributed to her current difficulties.
During our initial meetings, Francine stated that her goals were to feel less depressed, to reduce or stop drinking, and to feel less isolated. In order to ensure that no medical issues were contributing to her depression symptoms, I referred Francine to her primary care physician for an evaluation. Francine’s physician did not find any medical cause of her symptoms, diagnosing Francine with moderate clinical depression and recommending that Francine begin a course of antidepressant medication. Francine was reluctant to take medication and first wanted to try a course of counseling.
In order to help Francine meet her goal of reducing her depression symptoms, I employed a technique called behavioral activation (BA), which is drawn from principles of cognitive behavioral therapy and helps to reengage people in pleasant physical, social, and recreational activities. We began with a small initial goal of having Francine dedicate at least 5 minutes of each day to an activity she found pleasant or rewarding. Over the following weeks, we increased the time. Francine’s treatment progress was monitored through weekly completion of the Patient Health Questionnaire (PHQ-9) in order t.

Jean (pseudonym) was diagnosed with schizophrenia. She reported having very little memory of her childhood. Her mother was severely depressed and, on several occasions throughout Jane’s early childhood, the mother spent several months at a time hospitalized for her depression. When her mother returned home, she would at times “lose control” and start beating on the children. Jean’s father left her mother when Jean was only three years old. Jean was eventually removed from the home and placed in foster care. She moved around from one foster care parent to another, and finally came to live with a pastor and his wife who raised her in a warm, loving family. Unfortunately, Jane started “looking for love in all the wrong places.” Jane became promiscuous and eventually got pregnant. This was a great disappointment to the pastor and his wife. The pastor’s wife told Jean that she was extremely disappointed with her and that she was embarrassed to have her in her home. Not long thereafter, Jean secretly got an abortion. She then ran away with a young man to a hippie commune in California (during the late 60s). Jean is now 55 years old and has been in and out of counseling her entire life. Francine is a 70-year-old, Irish Catholic female. She worked for 40 years as a librarian in an institution of higher education and retired at age 65. Francine has lived alone for the past year, after her partner, Joan, died of cancer. Joan and Francine had been together for 30 years, and while Francine personally identifies as a lesbian, she never came out to her family or to her colleagues. When speaking to all but her closest confidantes, Francine referred to Joan as her “best friend” or her “roommate.” Francine’s bereavement was therefore complicated because she did not feel she could discuss the true nature of her partnership with Joan. She felt that there was little recognition from her family, and even some of her close associates, of the impact and meaning of Joan’s death to Francine. There is a history of alcohol abuse in Francine’s family, and Francine abused alcohol from late adolescence into her mid-30s. However, Francine has been in recovery for several decades. Francine has no known sexual abuse history and no criminal history.Francine sought counseling with me for several reasons, including an ongoing depressed mood, a lack of pleasure or enjoyment in her life, and loneliness and isolation since Joan’s death. She also reported that she had begun to drink again and that while her drinking was not yet at the level it had been earlier in her life, she was concerned that she could return to a dependence upon alcohol. Francine came to counseling with several considerable strengths, including a capacity to form intimate relationships, a successful work history, a history of having maintained her sobriety in the past for many years, as well as insight into the factors that had contributed to her current difficulties.During our initial meetings, Francine stated that her goals were to feel less depressed, to reduce or stop drinking, and to feel less isolated. In order to ensure that no medical issues were contributing to her depression symptoms, I referred Francine to her primary care physician for an evaluation. Francine’s physician did not find any medical cause of her symptoms, diagnosing Francine with moderate clinical depression and recommending that Francine begin a course of antidepressant medication. Francine was reluctant to take medication and first wanted to try a course of counseling.In order to help Francine meet her goal of reducing her depression symptoms, I employed a technique called behavioral activation (BA), which is drawn from principles of cognitive behavioral therapy and helps to reengage people in pleasant physical, social, and recreational activities. We began with a small initial goal of having Francine dedicate at least 5 minutes of each day to an activity she found pleasant or rewarding. Over the following weeks, we increased the time. The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature
death. We discussed the strengths that he and Carol demonstrated
in staying together and working out their p.

Aaron is a 24-year-old, unmarried, heterosexual, Caribbean immigrant male who is experiencing symptoms of anxiety and depression. Aaron reports no history of mental health treatment nor any medical or legal problems. He admits to social drinking but denies use of illegal substances. He lives alone in a room he rents above the restaurant where he works. He works 24 hours a week as a waiter, has few friends, and is a part-time student at a local university where he is working on an undergraduate degree in biology. Aaron came to speak with me, a university counselor, because he is having difficulty concentrating and finding the motivation to study. Aaron denied any thoughts or plans of suicide or homicide and stated he felt hopeless and nervous.
In the first session, Aaron struggled with sustaining eye contact, presented as preoccupied, and was indifferent to the attempts to engage him in the intake process. When asked what he thought precipitated counseling, Aaron said that he had a difficult relationship with his parents who, he stated, “are not supportive and could care less” about him. He also reported that his younger brother was killed not long ago. When asked what he wanted to work on in counseling, he said that he wanted to address why his family was so “messed up.” Subsequent sessions explored Aaron’s perspective on his family, the strained relationship between Aaron and his parents, and the loss of his sibling.
During one session, Aaron said his parents had always favored his younger brother and overlooked his criminal involvement, which had been a source of conflict between Aaron and his parents for years. While it had not been confirmed, Aaron suspected his brother’s death was related to gang involvement. Aaron shared that his academic interests and achievement had been ignored by his parents and had never been a source of interest for them.
In a subsequent session, Aaron stated that he had always felt disconnected and different from his parents and brother. Aaron’s family immigrated to the United States from Guyana when Aaron was 8 years old and his brother was 2 years old. His parents brought only his brother and left Aaron with his grandmother, informing him they would bring him over when they were settled. Seven years later, at the age of 15, he joined his family. Brandon is a 12-year-old, Caucasian male who currently resides with his mother and her boyfriend. Six years ago, Brandon disclosed that his father had repeatedly sexually abused him between the ages of 4 and 6. Brandon’s mother called law enforcement immediately after the disclosure, and his father has been incarcerated since. Brandon has previously participated in therapy to address challenging behaviors, including physical aggression, difficulty following rules at home and school, and using inappropriate language with sexual overtones toward female peers. Brandon and his mother report that they ceased participating in therapy in the past after there was no change in Brandon’s behavior. Brandon’s teachers have suggested that his behaviors are similar to those of peers with attention deficit hyperactivity disorder, but his mother has declined educational or psychological testing because she does not want her son to be labeled and is unsure if she agrees with the use of psychotropic medication with children.
Brandon began attending trauma-focused treatment after demonstrating an increase in argumentative behavior and minor property destruction at home. His mother reported that the majority of undesired behaviors were initiated during interactions with her boyfriend. Brandon’s use of physical aggression has not increased in school; however, a female peer recently reported him for using sexually explicit language toward her. Brandon admitted to using inappropriate language toward the female peer but appeared to have a limited understanding of what the phrases used meant. Brandon’s mother noted during intake that she is concerned that her son will become a violent sexual offender or a pedophile and noted that his use of sexual language was likely the start of sexual behavior problems.

Darlene met Eboni’s father, Anthony, when she was 17, the summer before their senior year in high school. Anthony is 34 years old and Caucasian. They casually dated for about a month, and after they broke up, Darlene discovered she was pregnant and opted to keep the baby. Although they never married each other, Anthony has been married twice and divorced once. He has four other children in addition to Eboni. She visits her father and stepmother every other weekend. Anthony works as a mechanic and pays child support to Darlene.
Recently, Eboni took a pregnancy test and learned that she is 2 months pregnant. She actually did not know she was pregnant because her periods were not always consistent and she thought she had just skipped a couple of months. Eboni immediately told her best friend, Brandy, and then Darian about her pregnancy. He was shocked at first and suggested that it might be best to terminat.

John
John is a 38-year-old male of Euro-American heritage. He has never been married, and
he has one brother whom he rarely sees. John is a long-distance truck driver, and he
spends the vast majority of his time living out of his vehicle driving across the country.
He has a place he calls “home,” but he spends only about a week there every few
months. As a result, John has few close friends and no significant romantic relationship
in his life. John has also alienated several family members and close friends from whom
he has borrowed substantial amounts of money that he has not paid back.
John was referred by his employer’s EAP following an issue while at work. John had
lied about his time log and location, as he had stopped to gamble at a casino in
Mississippi during a cross-country run. He was put on probation for this infraction, and
ordered to seek counseling services. Though John denies having a “gambling problem,”
he admits that he does enjoy gambling and engages in both in-person and online
gaming whenever he has an opportunity. John also admits that he has lost a lot of
money to gambling, though rationalizes this with “at least I was having fun.” He also
notes that his gambling has cost him past relationships and prevented him from
reaching financial goals such as home ownership and retirement planning. John shares
that “it would be different if I was married or something. It’s just me, so who cares if this
is how I spend my money?”
Donna
Donna is a 40-year-old transgender female of Korean American heritage. She is the
manager of a popular local restaurant and club, and is well respected at her job. She
has come to counseling at the insistence of her boyfriend, who has expressed worry
over her increasing drug use. Donna also admits that she and her boyfriend have been
fighting a lot lately, and she is worried he might be cheating on her. In fact, she admits
that a recent altercation became physical, although she blames it on her “temporary
meth rage.”
Donna shares very openly about her use of methamphetamine, although she insists that
she has it “under control.” She states that in her 20s, she used a lot of recreational
drugs, but for a number of years after remained completely abstinent. She claims she
currently uses meth “just every now and then” because she likes the rush and energy it
gives her. She minimizes its role in the altercation with her boyfriend, stating that she
had used a little too much that day but “that never happens.” While Donna tends to
minimize the role and significance of her drug use, she states, “Maybe I could use a
little help getting my temper under control.”

L appeared to be of average to above-average intelligence, as she was able to respond to numerous questions in an articulate and intelligent manner. She was well versed about world history and current affairs. Her mother confirmed that she has done well in school, maintaining a B+ average and participating in various school activities (e.g., chorus, school paper) until last year. L slowly dropped out of many activities she liked in the past. Her mother noticed about 8 months ago that L had also begun having difficulty doing schoolwork. Erratic behavior arose during episodes when L also became irritable and explosive. During these repeated episodes, she became quite defiant, cut classes, had to be placed in school detention, and had even assaulted the principal. L has numerous friends and believed she can relate to all types of people. She has a boyfriend who adores her, but she said she doesn't feel the same about him. The school counselor confirmed that L is outgoing, popular, and smart; but during these episodes she became another person, one who is very violent and difficult.
Medical History
A physical examination by a staff doctor revealed superficial cuts on L’s left and right wrist. The cuts appeared to be a few weeks old. There were cigarette burns on her right wrist that looked to be approximately one week old. In questioning L about the cigarette burns, L responded, "I just wanted to see how it felt—now I know." When questioned about old cuts on her left wrist, she responded, "I don't want to talk about it." L weighs 103 pounds and is 5’ 6” tall. L denied any dieting or fasting, but her mother noticed over this past year that her weight has dropped. Substance Abuse History L denied any drug or alcohol use. When she was questioned regarding such, her response was "I could do drugs if I wanted to. I don't want to, because it’s dumb." Wellcome Israel is a pharmaceutical company operating in Israel. Wellcome is being acquired by Glaxo and soon will become Wellcome-Glaxo. All employees and management were extremely surprised by the takeover and are having a hard time processing it. The main them and problem that will be noted in this post is the lack of communication, direction and vision.
Glaxo-Wellcome is on the way to becoming one of the largest pharmaceutical companies in the world. There is an apparent hostile takeover in progress that no one including Wellcome management is aware of. As with any change there are periods of uncertainty, fear, and anxiety. The main focus is a general manager Ofra Sherman and her team who works for Wellcome Israel through Promedico. This takeover was done in a manner that is very disturbing in the fact that there was no communication and very little guidance throughout the entire process.
What they should have done is really quite simple and that is communicate. As I mentioned before change is a very anxious time and can lead to stress and poor work performance for those involved. “Good communication throughout every stage of the change process is vital to keep everyone informed and motivation levels high.” (Kirke 2012) This takeover shouldn’t have been a surprise to Wellcome and they should have been upfront with employees about what was going to occur and then once the takeover by Glaxo started the communication should have ramped up. I think one of the best ways to have handled this would have been to have teams from both companies setting up meetings so there was a continuous flow of information.

The progression of the reaction will be observed using a pH meter and a titration curve will be created using the experimental data. You will start with a sample containing only the acid and indicator and slowly add your standardized base. A titration curve is simply a plot of the pH of an acid versus the volume of base added, or vice versa. The titration curve gives a good description of how an acid-base reaction proceeds. The pH will start out low and acidic, then increase as it approaches the equivalence point, where the concentration of acid equals that of the base. Then as the solution becomes more basic, it will slowly rise and level off as an excess amount of base is added. Note that the equivalence point is slightly different from the endpoint of a titration. The endpoint is when the indicator changes color. This does not always correspond to the equivalence point.

A host-based firewall essentially works closing and/or opening ports in a
computer. The engine behind firewalls is built with Access Control Lists (ACL).
Network-based firewalls are implemented in device-specific appliances and

or deny them.
Ports are layer-4 address specified in TCP/IP protocol suit that identify
networking processes running in clients and servers.

computer” due to sources only being available in electronic form. CAATs and CAATTs improve
efficiency and effectiveness of audits. A variety of standards and statements were issued because
new guidance was necessary once the growth of IT affected the nature, timing, and extent of audit
procedures. Continuous auditing is the key to improving the efficiency and effectiveness of audits.
Author’s Theme: Auditors are advised to use CAATs to gather evidence so that they can increase
the efficiency of inspection and analytical review. One of the many statements released on IT, SAS
94, stressed that IT’s impact on internal control is a result of the nature and extent of the system’s
complexity rather than the size of the firm, which is why auditing through the computer is
important when testing controls. Auditors can pinpoint the risk areas and thus have a better idea of
what to inquire when questioning management; improving the quality of the evidence and thus the
audit. Importance: Advancement in information technology has a direct impact on business
processes and the audit. Increasingly, auditors are required to perform audits in computerized
environments; therefore, additional standards are required to ensure that financial statements
auditors continue to perform high quality audits. Conclusion/Opinions: (1) The author concludes
that auditing through the computer is important when testing controls because of the impact IT has
on internal control. GAAS field work #2 supports this because the IT system is a part of the entity’s
environment, and especially, when it is complex, the auditor must audit through the computer to
adequately assess internal control risk. (2) Using CAATs can increase the efficiency of audit
procedures. SAS No. 106 indicates that CAATs allow auditors to inspect electronic evidence directly.
(3) Auditors should use data analysis from audit software in order to pinpoint the risk areas and
gain a better idea of what to ask management. AS # 5 states that risk assessment underlies the
entire audit process, including the determination of significant accounts and relevant assertions,
selecting controls to test, and determining the evidence necessary.

The Dropbox deliverables include the following.
1. Include a zipped file with all the files from your Visual Basic project (see directions in Doc Sharing on how to collect and zip files.)
2. Create a single MS Word document and include the following.
· For each lab, copy and paste your code directly into the MS Word file.
· Include screenshot(s) of your test data with test results. Enter enough data to demonstrate that all your code has been tested.
· Include another screenshot of the Visual Basic build output messages. Check your build output to make sure you have a successful build with (0) errors and (0) warnings. NOTE: The build output messages appear at the bottom of your window after you click the Build menu and before you click the Start Without Debugging option. Your build output messages provide a list of compiler warnings or errors and let you know whether your program compiled successfully.
· Include the Word document as part of the zipped project file.
3. Upload each part of the lab into its corresponding weekly Dropbox.

Step 1: Create a New Project
Create a new project in VB.NET. Name your project CIS170A_Lab05.
Practically every real-world professional program in existence today uses some type of modular design—this is just the way programs are designed and built. As a result, the primary objective of this lab is for you to understand how modular programs are designed and how data are passed between modules as the program is executing. This may be one of the most important lessons of this course, so it is critical that you focus on the modular design of the program; if you have any questions, post them to the Lab Forum threaded discussion.
In this project, we are going to use the Week 4 Hockey Player Statistics program that you created last week as a starting point and make a few minor changes in the program requirements. 2. Article Summary:
“Transmission Unaccomplished” written in the Lex section of Friday’s Financial Times
offers an interesting and simplified perspective of the complex and sophisticated purpose and
workings of monetary policy. At a time when the world is reeling from the effects of
misunderstood monetary policy in the United States and other nations around the globe, this
article clearly cuts to the heart of the matter, provides a simple, easy to understand analogy
relating monetary policy to an automobile. The authors describe the key moving parts of the
economy as they correlate to their counterparts in an automobile. While he labor and
resources constitute the fuel of the economy, technology and institutions correspond to the
engine, and commerce is depicted as wheels. The financial system is the transmission,
responsible for moving the power and energy created by the fuel and engine to the wheels.
This simple analogy helps frame the context for the reason why central bankers – the
transmission mechanics – were facing increasing difficulty. In particular, the 1.6 % drop in
the dollar’s value, the lack of real economic turn-around, and the lagging increase in GDPs
around the world. The authors seem to think the US, despite an increasing saving rate and
10% deleveraging, still has long ways to go on the road to recovery, and the automobile
analogy suggests, is in need of significant repairs and rebuilding before it is truly road worthy.
3. How is the article is related to the readings and class discussions?
The concepts in the article mesh with the readings in chapters 3 – 5 as a current, real world
depiction of how monetary policy influences decisions in economics. The article highlights the
need for central bankers to properly manage monetary policy in order to maintain the
transmission of the vehicle, and keep the proper amount of power moving from the motor to the
wheels. It questions the true value of quantitative easing, and highlights the ramifications of
pursuing excessive QE as a policy. Just as was discussed in the first five chapters of the text
book, monetary policy involves a delicate balance of adjusting interest rates, setting currency
value, and establishing guidelines that enable prosperity and growth. The article also identifies
too much intervention as a possible means for enhancing the problems we face, rather than
ameliorating the problems.
4. What did I learn from this article?
This article certainly helped put monetary policy, something I seem to be familiar with only
through studying politics and economics, into a very concise, easy to understand framework that
enables a deeper understanding of greater associated issues. I learned that the US liabilities are

- 1. Anxious Anna Anna is a thirty-four year-old white female. Currently she is married with four children and works full-time as a homemaker. Her non-verbals (picking at nails and clothes, very talkative) indicated a moderate level of anxiety regarding the appointment. The client perceives her primary difficulty at this time to be marital issues including betrayal and a desire to forgive and submit. Anna was raised by a maternal grandmother. She left home at the age of sixteen, to escape her father’s sexual abuse. Anna married her current husband in 1992, although they had already had a child together before that time. Neither Joe nor Anna were Christians when they met, and she briefly described a history of personal alcohol abuse, as well her husband’s drug and alcohol abuse and an escalating pattern of pornography addiction. Both Anna and her husband were recently saved at a Pentecostal Church; a decision that Anna believes dramatically changed
- 2. their lives. ‘Recently’ (2 years ago) Anna was made aware of her husband’s affair with a seventeen year old. She states this is the first she knew of his adulterous behavior, although she describes his long standing patterns of staying out all night, finding hickeys and following him to bars where he would leave with other women. She states that she is very angry with him about this behavior, although she believes that this was an isolated incident. When discovering that she was aware of the affair Joe also became physically violent with Anna, but she states that this was an isolated incident as well. Anna’s goal for seeking counseling was to learn how to control her anger toward her husband and gain coping skills for the situation. She desires to learn how to forgive him for his past actions and learn how to submit to him. On her intake papers Anna describes herself as a ‘kind, loving, affectionate, meek shy and blessed’ person’; her husband believes she has low self-esteem and little confidence. Currently Anna is on Oxyeontin for pain management; Flexoral for muscle spasms and Relafen for arthritis.
- 3. She currently suffers from Fibromyalgia and Osteo-arthritis. The client indicated prior counseling at SMH in 89-90 for Anorexia; she also has a brief history of lesbian behavior. On the phone, as well as during our initial meeting Anna presented as a very chaotic and angry person. She arrived at the session approximately twenty minutes late after getting lost on the way – apologizing profusely when I went to greet her. Once inside the office she asked if she could start the session with prayer, then got on her knees and touching her forehead to the ground and prayed for approximately five minutes, primarily asking the Lord to help her become a more submissive wife to her husband. It was extremely difficult to end the session, with our time lasting almost twenty minutes over the hour; she was not prepared to pay at that time. She subsequently missed her follow-up appointment, and when called stated that things were much better with her husband and she would no longer need counseling at this time. What are the spiritual implications for this client? Is the use of religion functional or dysfunctional? What spiritual interventions might be appropriate?
