In this chapter, we explore the antecedents and
early years of family therapy. There are two
compelling stories here: one of personalities,
one of ideas. The first story revolves around
the pioneers—visionary iconoclasts who broke
the mold of seeing life and its troubles as a
function of individuals and their personalities.
Make no mistake: The shift from an individual to
a systemic perspective was a revolutionary one,
providing those who grasped it with a powerful
tool for understanding and resolving human
problems.
The second story in the evolution of family
therapy is one of ideas. The restless curiosity of
the first family therapists led them to ingenious
new ways of conceptualizing the joys and
sorrows of family life.
As you read this history, stay open to surprises.
Be ready to reexamine easy assumptions—
including the assumption that family therapy
began as a benevolent effort to support the
institution of the family. The truth is, therapists
first encountered families as adversaries.
The undeclared War
Although we came to think of asylums as places of
cruelty and detention, they were originally built to
rescue the insane from being locked away in family
attics. Accordingly, except for purposes of footing
the bill, hospital psychiatrists kept families at arm’s
length. In the 1950s, however, two puzzling devel-
opments forced therapists to recognize the family’s
power to alter the course of treatment.
Therapists began to notice that often when a pa-
tient got better, someone else in the family got worse,
almost as though the family needed a symptomatic
member. As in the game of hide-and-seek, it didn’t
seem to matter who “It” was as long as someone
played the part. In one case, Don Jackson (1954)
was treating a woman for depression. When she be-
gan to improve, her husband complained that she was

getting worse. When she continued to improve, the
husband lost his job. Eventually, when the woman
was completely well, the husband killed himself. Ap-

Donna Little is a 39-year-old Indian woman who has a history of substance misuse and has struggled with reunification with her adolescent children over the last 6 years. She was in residential school from the age of 6 to 16 years old. She has a history of domestic violence in her previous relationships. Donna was the youngest of four children in her family. Her parents, siblings, and herself were raised in the same small northern reservation. Both her parents had gone to residential school in the early 1950s, as did her grandfathers and grandmothers on both sides of her family system in the late 1910s. Donna was raised in an environment of violence and mayhem in her early childhood, which she has talked about quite extensively in counseling. Although her parents abused alcohol, she emphasizes repeatedly that her family was quite ceremonial and participated in the big drum feast and singing within the community. When Donna was 6, an Indian agent wearing a red, white, and black checkered jacket gave her candy and took her to the residential school. She never had the opportunity to say good-bye to her mom and dad, who died of tuberculosis while she was in the residential school. Donna reflects on her residential school experience with a despondent look. While in the residential school, she had only one friend she could count on. Her siblings, who were also at the school, were older and thus not allowed to play with her or sleep near her at the residence dorms. Neither Joe nor Anna were Christians when they met, and she briefly described a history of personal alcohol abuse, as well her husband’s drug and alcohol abuse and an escalating pattern of pornography addiction.
Both Anna and her husband were recently saved at a Pentecostal Church; a decision that Anna believes dramatically changed their lives.
‘Recently’ (2 years ago) Anna was made aware of her husband’s affair with a seventeen year old.
She states this is the first she knew of his adulterous behavior, although she describes his long standing patterns of staying out all night, finding hickeys and following him to bars where he would leave with other women.
She states that she is very angry with him about this behavior, although she believes that this was an isolated incident.
When discovering that she was aware of the affair Joe also became physically violent with Anna, but she states that this was an isolated incident as well.
Anna’s goal for seeking counseling was to learn how to control her anger toward her husband and gain coping skills for the situation.
She desires to learn how to forgive him for his past actions and learn how to submit to him.
On her intake papers Anna describes herself as a ‘kind, loving, affectionate, meek shy and blessed’ person’; her husband believes she has low self-esteem and little confidence.
Currently Anna is on Oxyeontin for pain management; Flexoral for muscle spasms and Relafen for arthritis.
She currently suffers from Fibromyalgia and Osteo-arthritis.
The client indicated prior counseling at SMH in 89-90 for Anorexia; she also has a brief history of lesbian behavior.
On the phone, as well as during our initial meeting Anna presented as a very chaotic and angry person.
She arrived at the session approximately twenty minutes late after getting lost on the way – apologizing profusely when I went to greet her.
Once inside the office she asked if she could start the session with prayer, then got on her knees and touching her forehead to the ground and prayed for approximately five minutes, primarily asking the Lord to help her become a more submissive wife to her husband.
It was extremely difficult to end the session, with our time lasting almost twenty minutes over the hour; she was not prepared to pay at that time.
She subsequently missed her follow-up appointme.

spiritual care in the inpatient setting at
the end of life. The purpose of this study
was to better understand how inpatient
palliative care nurse consultants provide
spiritual care and how they acquired
these skills. Additional aims included
discovering the personhood of nurses
providing spiritual care for dying patients.
A purposeful sample of 10 experi-
enced palliative care nurse consultants
working in inpatient palliative care
in the Midwestern United States
were recruited for two recorded
semi structured interviews to produce
20 transcripts. Institutional review
board approval was obtained from the
author’s institution, and informed
consent was obtained from all partici-
pants. A historical self-awareness, nurs-
ing process, and expertise in practice
interview guide were utilized to
obtain narrative data (Benner, Tanner,
& Chesla, 2009). Transcripts were
decoded, checked for accuracy, and
analyzed by the primary investigator
(PI). Themes were identified by the
PI and verified by a team of experi-
enced interpretive phenomenological
researchers as well as validated by
participants.
Interpretive phenomenology,
which seeks holistic understanding
and illumination of phenomena, was
the methodology used for this study.
This is an appropriate research method
for examining the everyday practice
and skill of providing spiritual care.
As nurses presented narratives about
caring for patients and families at
end of life, they described the lived
experience of providing spiritual care
in concrete situations where meaning
and shared experiences were uncovered
and skills and practices explicated.

IDENTIFYING/DEMOGRAPHIC DATA:
Shanice is a 32 year old, single African American female, with no siblings. Shanice reports never being married. She works full time at the library.
CHIEF COMPLAINT/PRESENTING PROBLEM:
Shanice reported having sudden attacks and feeling anxious. She described feelings coming over her so quickly where her heart was pounding, and she couldn’t breathe. Recently when shopping, she wanted to just run out of the store because she suddenly was so dizzy she wanted to vomit. She even stopped going to the library.
HISTORY OF PRESENT ILLNESS:
Shanice notes “I like people, but I don’t deal well with them. I never seem to meet a lot of people. I guess I have always been this way. I remember rarely inviting other kids to my birthday parties. “I didn’t think they really wanted to come”.
Shanice described feeling anxiety and panic over the past three weeks when she was assigned a new boss. She believes her work is unsatisfactory and feels very dissatisfied with her job. She doesn’t like the anxiety she is now experiencing. Over the past two years, her other boss would cover up for her mistakes when her concentration was off, and she couldn’t make a decision. She may need to quit her job. She cannot believe that she ge.
Autobiography
Marie Balter suffered from clinical depression and was misdiagnosed with schizophrenia. She was placed in several foster homes and mental institutions throughout her life. In her autobiography, Marie discusses enduring hallucinations, delusions, and suicidal thoughts. She was eventually correctly diagnosed with endogenous depression but had endured improper treatments, including overmedication, due to her initial misdiagnosis. Some factors that may have contributed to Marie's depression include a difficult childhood, lack of parental support, and feeling isolated in institutions.

The Case of Angela
Angela is a 27-year-old, Caucasian female, who first came to
counseling to address her history of sexual abuse. She graduated
from college with a BS in chemistry and has since been employed
by pharmaceutical companies. After obtaining a new job, she relo-
cated to an apartment in an East Coast city where she knew no
one. Both of Angela’s parents live on the West Coast, and she has
one younger brother who also lives in a different state. Angela has
limited contact with both her mother and brother and does not
have any contact with her father. Angela is obese and disclosed
a history of struggling with her weight and eating issues. She has
few friends, and those she does have live far away.
Angela has a long history of trauma in her life. She was sexually
abused between the ages of 9 and 21 by her father, sexually assaulted
at the age of 14 by a classmate in school, and mugged as a young
adult. There was domestic violence in the home, also perpetrated by
her father. Angela’s father is considered an upstanding member of
the community, and he is well liked and respected by others. No one
in Angela’s family believes that she was sexually abused, and her
father joined a “false memory syndrome” group and is outspoken
about that issue. There has been little discussion in her family about
what took place in the home while she was growing up.
Angela struggled with daily functioning and exhibited symp-
toms of post-traumatic stress disorder (PTSD). She had a history
of cutting herself and binge eating and displayed some charac-
teristics of borderline personality disorder. Angela also mildly
dissociated when under duress. Angela suffered from depression
and anxiety and had trouble establishing new relationships, both
socially and at work. Although Angela has a stable job and was
able to complete her work each day, at times she became over-
whelmed by her emotions and retreated to the bathroom where
she cried and sometimes cut herself before returning to her work-
station. Angela relied on writing, artwork, and her cat for solace

and comfort. She was also very active outdoors, often hiking,
biking, and going on camping trips by herself. Her goals in life
were to own her own home, lose weight, enjoy relationships with
others, and find peace with her traumas.
As a result of the abuse she experienced, it was necessary to
begin treatment focusing heavily on establishing trust and a rela-
tionship with the client. After 1 year of therapy, deeper process
work was being done around her traumas, and she was able to
open up much more. She disclosed more painful experiences to
the therapist and began expressing her feelings, including intense
anger at her family members.
Angela also joined a group for survivors of sexual violence in
the same program where she was receiving individual therapy. .

Deontology
The ethical position to do what is right out of duty or obligation. It is often called rule-based ethics.
Deontology has been described as "absolutist," "universal," and "impersonal" (Kant, 1785/1959). It prioritizes absolute obligations over consequences. In this moral framework, ethical decision making is the rational act of applying universal principles to all situations irrespective of specific relations, contexts, or consequences. This reflects Immanuel Kant's conviction that ethical decisions cannot vary or be influenced by special circumstances or relationships. Rather, a decision is "moral" only if a rational person believes the act resulting from the decision should be universally followed in all situations. For Kant, respect for the worth of all persons was one such universal principle. A course of action that results in a person being used simply as a means for others' gains would ethically unacceptable.
With respect to deception in research, from a deontological perspective, since we would not believe it moral to intentionally deceive individuals in some other context, neither potential benefits to society nor the effectiveness of participant debriefing for a particular deception study can morally justify intentionally deceiving persons about the purpose or nature of a research study. Further, deception in research would not be ethically permissible since intentionally disguising the nature of the study for the goals of research violates the moral obligation to respect each participant's intrinsic worth by undermining individuals' right to make rational and autonomous decisions regarding participation (Fisher & Fyrberg, 1994).
Utilitarianism
The ethical position depends on the consequences of the action with the goal being producing the most good.
Utilitarian theory prioritizes the consequences (or utility) of an act over the application of universal principles (Mill, 1861/1957). From this perspective, an ethical decision is situation specific and must be governed by a risk-benefit calculus that determines which act will produce the greatest possible balance of good over bad consequences. An "act utilitarian" makes an ethical decision by evaluating the consequences of an act for a given situation. A "rule utilitarian" makes an ethical decision by evaluating whether following a general rule in all similar situation would create the greater good. Like deontology, utilitarianism is impersonal: It does not take into account interpersonal and relational features of ethical responsibility. From this perspective, psychologists' obligations to those with whom they work can be superseded by an action that would produce a greater good for others (Fisher, 1999).

Her non-verbals (picking at nails and clothes, very talkative) indicated a moderate level of anxiety regarding the appointment.
The client perceives her primary difficulty at this time to be marital issues including betrayal and a desire to forgive and submit.
Anna was raised by a maternal grandmother.
She left home at the age of sixteen, to escape her father’s sexual abuse.
Anna married her current husband in 1992, although they had already had a child together before that time.
Neither Joe nor Anna were Christians when they met, and she briefly described a history of personal alcohol abuse, as well her husband’s drug and alcohol abuse and an escalating pattern of pornography addiction.
Both Anna and her husband were recently saved at a Pentecostal Church; a decision that Anna believes dramatically changed their lives.
‘Recently’ (2 years ago) Anna was made aware of her husband’s affair with a seventeen year old.
She states this is the first she knew of his adulterous behavior, although she describes his long standing patterns of staying out all night, finding hickeys and following him to bars where he would leave with other women.
She states that she is very angry with him about this behavior, although she believes that this was an isolated incident.
When discovering that she was aware of the affair Joe also became physically violent with Anna, but she states that this was an isolated incident as well.
Anna’s goal for seeking counseling was to learn how to control her anger toward her husband and gain coping skills for the situation.
She desires to learn how to forgive him for his past actions and learn how to submit to him.
On her intake papers Anna describes herself as a ‘kind, loving, affectionate, meek shy and blessed’ person’; her husband believes she has low self-esteem and little confidence.
Currently Anna is on Oxyeontin for pain management; Flexoral for muscle spasms and Relafen for arthritis.
She currently suffers from Fibromyalgia and Osteo-arthritis.
The client indicated prior counseling at SMH in 89-90 for Anorexia; she also has a brief history of lesbian behavior.
On the phone, as well as during our initial meeting Anna presented as a very chaotic and angry person.
She arrived at the session approximately twenty minutes late after getting lost on the way – apologizing profusely when I went to greet her.
Once inside the office she asked if she could start the session with prayer, then got on her knees and touching her forehead to the ground and prayed for approximately five minutes, primarily asking the Lord to help her become a more submissive wife to her husband.
It was extremely difficult to end the session, with our time lasting almost twenty minutes over the hour; she was not prepared to pay at that time.
She subsequently missed her follow-up appointme.

An Exploration of Spiritual Care Expertise
ABSTRACT: This interpretive phenomenological study explored
how inpatient palliative care nurse consultants provide spiritual care
and how they acquired these skills. Humbled experts describes the

A purposeful sample of 10 experi-
enced palliative care nurse consultants
working in inpatient palliative care
in the Midwestern United States
were recruited for two recorded
semi structured interviews to produce
20 transcripts. Institutional review
board approval was obtained from the
author’s institution, and informed
consent was obtained from all partici-
pants. A historical self-awareness, nurs-
ing process, and expertise in practice
interview guide were utilized to
obtain narrative data (Benner, Tanner,
& Chesla, 2009). Transcripts were
decoded, checked for accuracy, and
analyzed by the primary investigator
(PI). Themes were identified by the
PI and verified by a team of experi-
enced interpretive phenomenological
researchers as well as validated by
participants.

IDENTIFYING/DEMOGRAPHIC DATA:
Shanice is a 32 year old, single African American female, with no siblings. Shanice reports never being married. She works full time at the library.
CHIEF COMPLAINT/PRESENTING PROBLEM:
Shanice reported having sudden attacks and feeling anxious. She described feelings coming over her so quickly where her heart was pounding, and she couldn’t breathe. Recently when shopping, she wanted to just run out of the store because she suddenly was so dizzy she wanted to vomit. She even stopped going to the library.
HISTORY OF PRESENT ILLNESS:
Shanice notes “I like people, but I don’t deal well with them. I never seem to meet a lot of people. I guess I have always been this way. I remember rarely inviting other kids to my birthday parties. “I didn’t think they really wanted to come”.
Shanice described feeling anxiety and panic over the past three weeks when she was assigned a new boss. She believes her work is unsatisfactory and feels very dissatisfied with her job. She doesn’t like the anxiety she is now experiencing. Over the past two years, her other boss would cover up for her mistakes when her concentration was off, and she couldn’t make a decision. She may need to quit her job. She cannot believe that she ge.

Marie Balter suffered from clinical depression and was misdiagnosed with schizophrenia. She was placed in several foster homes and mental institutions throughout her life. In her autobiography, Marie discusses enduring hallucinations, delusions, and suicidal thoughts. She was eventually correctly diagnosed with endogenous depression but had endured improper treatments, including overmedication, due to her initial misdiagnosis. Some factors that may have contributed to Marie's depression include a difficult childhood, lack of parental support, and feeling isolated in institutions.

Sexual Abuse and Trauma:
The Case of Angela
Angela is a 27-year-old, Caucasian female, who first came to
counseling to address her history of sexual abuse. She graduated
from college with a BS in chemistry and has since been employed
by pharmaceutical companies. After obtaining a new job, she relo-
cated to an apartment in an East Coast city where she knew no
one. Both of Angela’s parents live on the West Coast, and she has
one younger brother who also lives in a different state. Angela has
limited contact with both her mother and brother and does not
have any contact with her father. Angela is obese and disclosed
a history of struggling with her weight and eating issues. She has
few friends, and those she does have live far away.
Angela has a long history of trauma in her life. She was sexually
abused between the ages of 9 and 21 by her father, sexually assaulted
at the age of 14 by a classmate in school, and mugged as a young
adult. There was domestic violence in the home, also perpetrated by
her father. Angela’s father is considered an upstanding member of
the community, and he is well liked and respected by others. No one
in Angela’s family believes that she was sexually abused, and her
father joined a “false memory syndrome” group and is outspoken
about that issue. There has been little discussion in her family about
what took place in the home while she was growing up.
Angela struggled with daily functioning and exhibited symp-
toms of post-traumatic stress disorder (PTSD). She had a history
of cutting herself and binge eating and displayed some charac-
teristics of borderline personality disorder. Angela also mildly
dissociated when under duress. Angela suffered from depression
and anxiety and had trouble establishing new relationships, both
socially and at work. Although Angela has a stable job and was
able to complete her work each day, at times she became over-
whelmed by her emotions and retreated to the bathroom where
she cried and sometimes cut herself before returning to her work-
station. Angela relied on writing, artwork, and her cat for solace

and comfort. She was also very active outdoors, often hiking,
biking, and going on camping trips by herself. Her goals in life
were to own her own home, lose weight, enjoy relationships with
others, and find peace with her traumas.
As a result of the abuse she experienced, it was necessary to
begin treatment focusing heavily on establishing trust and a rela-
tionship with the client. After 1 year of therapy, deeper process
work was being done around her traumas, and she was able to
open up much more. She disclosed more painful experiences to
the therapist and began expressing her feelings, including intense
anger at her family members.
Angela also joined a group for survivors of sexual violence in
the same program where she was receiving individual therapy. .

Deontology
The ethical position to do what is right out of duty or obligation. It is often called rule-based ethics.
Deontology has been described as "absolutist," "universal," and "impersonal" (Kant, 1785/1959). It prioritizes absolute obligations over consequences. In this moral framework, ethical decision making is the rational act of applying universal principles to all situations irrespective of specific relations, contexts, or consequences. This reflects Immanuel Kant's conviction that ethical decisions cannot vary or be influenced by special circumstances or relationships. Rather, a decision is "moral" only if a rational person believes the act resulting from the decision should be universally followed in all situations. For Kant, respect for the worth of all persons was one such universal principle. A course of action that results in a person being used simply as a means for others' gains would ethically unacceptable.
With respect to deception in research, from a deontological perspective, since we would not believe it moral to intentionally deceive individuals in some other context, neither potential benefits to society nor the effectiveness of participant debriefing for a particular deception study can morally justify intentionally deceiving persons about the purpose or nature of a research study. Further, deception in research would not be ethically permissible since intentionally disguising the nature of the study for the goals of research violates the moral obligation to respect each participant's intrinsic worth by undermining individuals' right to make rational and autonomous decisions regarding participation (Fisher & Fyrberg, 1994).
Utilitarianism
The ethical position depends on the consequences of the action with the goal being producing the most good.
Utilitarian theory prioritizes the consequences (or utility) of an act over the application of universal principles (Mill, 1861/1957). From this perspective, an ethical decision is situation specific and must be governed by a risk-benefit calculus that determines which act will produce the greatest possible balance of good over bad consequences. An "act utilitarian" makes an ethical decision by evaluating the consequences of an act for a given situation. A "rule utilitarian" makes an ethical decision by evaluating whether following a general rule in all similar situation would create the greater good. Like deontology, utilitarianism is impersonal: It does not take into account interpersonal and relational features of ethical responsibility. From this perspective, psychologists' obligations to those with whom they work can be superseded by an action that would produce a greater good for others (Fisher, 1999).

Assigned Case Study Problem:
You will create a case study for a microbial infection selected from the current pathogen list. Your case
study will be assembled using a detailed rubric (see below). Upon completion, you will submit your
case study to the Blackboard gradebook in Unit 5 and to SafeAssign.
How to create a case study
The case studies are meant to be an enjoyable, interesting, and informative assignment. This is your
chance to show that you understand the key teaching points about a microbe and to communicate
these points in a written format.
What information belongs in my case study?
Have at least 3-4 key referenced points in each of the five areas shown in the Case Study Information
Chart (see below). The left-hand heading in the chart suggests the type of information requested for the
pathogen. Outlines can be in whatever form you prefer (bullets/charts/outlines/diagrams or a mix). Be
sure to include two discussion questions (and provide complete answers) that you can incorporate
into your case study (place them at the end of your write-up). These questions should help connect your
case to other material in the course. For example, what other microbes have an A-B toxin? What other
viruses are transmitted by fecal-oral spread?
How much information should I provide for my case study?
For the Case Study, you are asked to provide at least the information requested in the chart below. The
boxed questions are suggestions for the minimum amount of information within each category. The
more detailed the information, the better the study. You may consult your textbook, CDC, WHO, Access
Medicine, Google Scholar, NCBI, WebMD, etc. to find the information. For example, if you perform a
Google search using the name of the pathogen and the word ‘vaccine’, you will find information on
current vaccines (if any), those in clinical trials, vaccines used only in animals, etc.

This Dissertation explores the particular circumstances of Mirzapur, a region located in the
core of India. Mirzapur, with its varied terrains and abundant biodiversity, offers an optimal
environment for investigating the changes in vegetation cover dynamics. Our study utilizes
advanced technologies such as GIS (Geographic Information Systems) and Remote sensing to
analyze the transformations that have taken place over the course of a decade.
The complex relationship between human activities and the environment has been the focus
of extensive research and worry. As the global community grapples with swift urbanization,
population expansion, and economic progress, the effects on natural ecosystems are becoming
more evident. A crucial element of this impact is the alteration of vegetation cover, which plays a
significant role in maintaining the ecological equilibrium of our planet.Land serves as the foundation for all human activities and provides the necessary materials for
these activities. As the most crucial natural resource, its utilization by humans results in different
'Land uses,' which are determined by both human activities and the physical characteristics of the
land.
The utilization of land is impacted by human needs and environmental factors. In countries
like India, rapid population growth and the emphasis on extensive resource exploitation can lead
to significant land degradation, adversely affecting the region's land cover.
Therefore, human intervention has significantly influenced land use patterns over many
centuries, evolving its structure over time and space. In the present era, these changes have
accelerated due to factors such as agriculture and urbanization. Information regarding land use and
cover is essential for various planning and management tasks related to the Earth's surface,
providing crucial environmental data for scientific, resource management, policy purposes, and
diverse human activities.
Accurate understanding of land use and cover is imperative for the development planning
of any area. Consequently, a wide range of professionals, including earth system scientists, land
and water managers, and urban planners, are interested in obtaining data on land use and cover
changes, conversion trends, and other related patterns. The spatial dimensions of land use and
cover support policymakers and scientists in making well-informed decisions, as alterations in
these patterns indicate shifts in economic and social conditions. Monitoring such changes with the
help of Advanced technologies like Remote Sensing and Geographic Information Systems is

Why can a sensitive Christian counselor not just automatically and quickly confront obvious sin in the life of the counselee? Of the cautions mentioned by the class sources, which ones do you think counselors most often overlook? Why? Now, consider the following case. What forms of confrontation do you think would work best in this scenario? Why did you answer as you did? Case Study: Jean (pseudonym) was diagnosed with schizophrenia. She reported having very little memory of her childhood. Her mother was severely depressed and, on several occasions throughout Jane’s early childhood, the mother spent several months at a time hospitalized for her depression. When her mother returned home, she would at times “lose control” and start beating on the children.
- 2. Jean’s father left her mother when Jean was only three years old. Jean was eventually removed from the home and placed in foster care. She moved around from one foster care parent to another, and finally came to live with a pastor and his wife who raised her in a warm, loving family. Unfortunately, Jane started “looking for love in all the wrong places.” Jane became promiscuous and eventually got pregnant. This was a great disappointment to the pastor and his wife. Jane had been attending church regularly before the pregnancy and had made a profession of faith in Christ. She felt extremely guilty about the pregnancy. The pastor’s wife told Jean that she was extremely disappointed with her and that she was embarrassed to have her in her home. Not long thereafter, Jean secretly got an abortion. She then ran away with a young man to a hippie commune in California (during the late 60s). Jean is now 55 years old and has been in and out of counseling her entire life. Recently, she was referred for therapy following an attempted suicide.
