Information Process
1. Collection of information of accidents from OHS and co-workers of
injured.
(Safety inspectors are posted round the clock in OHS center to obtain
first hand information about the accident as soon as the message
orinjured person approaches / received at OHS)

2. Logging information in shift log book and filling the format “Daily
accident report”

3. Collecting Injury on Works (IoW) forms from OHS -Do-
4. Sending Daily accident report along with IoW forms to safety office -Do-
5. If the accident is serious in nature, inform HoD and safety
immediately.

Investigation Process
6. IoW form and Daily Accident report is sent to statistics cell /safety Shift safety
inspector
7. Safety representative visits the site of accident / incident Safety officer
8. Observes the site condition:
− Equipment -do-
− Procedures
− House Keeping
9. Safety representative Interrogates:
− Injured Person
− Eye-witnesses (If any)
− Co-Workers
− Supervisor
