This document summarizes the case of a 62-year-old man who was admitted due to cognitive decline and gait difficulty over the past 2.5 years. On examination, he had increased tone, frontal release signs, and a cognitive syndrome affecting attention, abstraction, and perseveration. Brain MRI was normal. The differential diagnosis discussed included frontotemporal dementia (FTD), corticobasal syndrome (CBS), and vascular causes like CADASIL, with the need to exclude structural lesions.
