PALANTE, práctica presentada por Ana Mª Carriazo en el IV Innovando en Jueves sobre Telemedicina (celebrado el 24 de Mayo en Sevilla). PALANTE es un proyecto de I+D+i de ámbito europeo cuyo fin es facilitar a los pacientes crónicos el acceso on-line y seguro a su historia clínica mediante la implantación generalizada de servicios de telemedicina. PALANTE (PAtient Leading and mANanaging their healThcare through EHealth) pretende así ayudar a los pacientes a desempeñar un rol más activo en el cuidado de su salud, de forma conjunta con el equipo médico que lo atiende, a través de las nuevas tecnologías de la información y la comunicación.
La iniciativa, con un presupuesto de 6,3 millones de euros, tiene un plazo de ejecución de 36 meses y se enmarca dentro de los proyectos europeos deI+D+i “Competitiveness and Inoovation Framework Programme (CIP)”. El proyecto contempla la implantación de pilotos en España (Andalucía y País Vasco), Italia, Turquía, Noruega, Austria y República Checa así como de dos experiencias adicionales en Francia y Dinamarca.
El piloto de Andalucía constituye uno de las grandes aportaciones al proyecto y está basado en la integración en el Sistema Sanitario andaluz de dos prototipos –Clic Salud y Tratamiento 2.0-
Clic Salud es un nuevo servicio al ciudadano que permite consultar su historia clínica resumida recogida en Diraya por los profesionales del sistema sanitario público andaluz. Por su parte, Tratamiento 2.0 es una plataforma middleware genérica de servicios diseñada para la aplicación y gestión inteligente del tratamiento de enfermedades crónicas. El pilotaje se realizará con 7.000 ciudadanos con diabetes, tanto de tipo I como II.
Sus objetivos serán permitir a los pacientes el acceso y la gestión de su información sanitaria registrada en Diraya así como poner a su disposición los servicios sanitarios de telegestión y monitorización que mejoren su adherencia al tratamiento mediante la educación, los hábitos saludables y la accesibilidad a la información sanitaria.
En la práctica, un paciente que disponga de certificado digital podría acceder a los datos registrados en su historia digital para la consulta de informes clínicos, tratamientos o alergias y contraindicaciones, entre otros. Del mismo, por ejemplo, un diabético tendría la posibilidad de interactuar con su expediente sanitario informando directamente de los datos registrados por su glucómetro de forma que se puedan generar alertas en caso de que éstos se encuentren fuera de los rangos de normalidad.

Tele-Electroencefalografía. Práctica presentada por Claudio Sempere (Servicio de TICs del HU Virgen de las Nieves, de Granada) en el IV Innovando en Jueves celebrado el 24 de Mayo de 2012 en Sevilla.
La necesidad que propicia el proyecto es la siguiente:
• Numerosos pacientes pediátricos o adultos tienen que desplazarse más de 100-150 kms para realizar EEG o cualquier prueba neurofisiológica.
• Con frecuencia son lactantes, prematuros o críticos
• Durante el invierno son frecuentes los cortes de carreteras por problemas meteorológicos.
Los objetivos de la teleElctroencefalografía
• Puesta en marcha de una Unidad de EEG Baza-Granada para el registro electroencefalográfico en los pacientes referidos del Hospital de Baza y su Comarca y envío a través de la red de la señal EEG. Recepción y análisis en el Servicio de Neurofisiología Clínica y posterior envío de Informe
• Asistencia a un grupo de pacientes que necesitan realizarse un EEG y que presentan una serie de limitaciones para desplazarse
• Dificultades de niños y ancianos para trasladarse a nuestro Hospital desde la Comarca de Baza para la realización de EEG
• Pacientes en coma y en muerte cerebral no acceden a la valiosa información del EEG por carecer de especialistas en NFC en Hospital de Baza.
• Pacientes en status convulsivo o pacientes encamados con sospecha de encefalopatías no pueden acceder al EEG por carecer de especialistas en NFC en Hospital de Baza.
Se concluye, tras la puesta en funcionamiento de este servicio, que:
• La Telemedicina permite el acceso de la Neurofisiología Clínica a todos los pacientes.
• La Telemedicina puede dar respuesta rápida y puntual a pacientes pediátricos, ancianos y críticos
• La Telemedicina duplica la eficacia e eficiencia de los servicios de Neurofisiología Clínica
• El uso del sistema sigue una evolución creciente, incluyéndose mayor número de paciente, demostrando su utilidad y eficacia.
TELEDERMATOSCOPIA EN EL CRIBADO DE CÁNCER CUTÁNEO fue presentado por Concepción Martínez Díaz de Argandoña (Médico de Atención Primaria. UGC Huerta del Rey/Mallén. Distrito Sevilla) y Rosa Corbí Llopis (Especialista en Dermatología. UGC Dermatología. Hospital Universitaro Virgen del Rocío, Sevilla) en el IV Innovando en Jueves sobre TELEMEDICINA
En 2007 el Centro de Salud Huerta del Rey/Mallén y la UGC Dermatología del Hospital Universitario Virgen del Rocío iniciaron la teledermatoscopia diferida para el cribado de cáncer cutáneo. La introducción de esta técnica obecede a varios motivos. El principal es el aumento en la indicencia del cáncer del piel atribuido fundamentalmente al el envejecimiento de la población junto con los cambios en los hábitos de exposición solar. Por otra parte, existe una demanda asistencial elevada en Atención Primaria a causa de tumores cutáneos benignos que motivan dudas diagnósticas en los médicos de familia.
La teledermatoscopia diferida consiste en el envío telemático de imágenes dermatoscópicas junto a imágenes clínicas en caso de sospecha de cáncer de piel, desde Atención Primaria a las consultas hospitalarias de Dermatología. La dermatoscopia es una técnica diagnóstica no invasiva que permite observar estructuras de la epidermis y la dermis, y puede considerarse a mitad de camino entre la inspección clínica y el estudio histopatológico. En los últimos años, tres meta-análisis y dos estudios randomizados han probado definitivamente que la dermatoscopia mejora la sensibilidad diagnóstica en el melanoma comparada con el examen a “ojo desnudo”, sin reducir la especificidad. Cada vez mas frecuentemente los médicos de atención primaria se suman a la utilización de la dermatoscopia como método de cribado para seleccionar aquellos casos que precisan valoración por el dermatólogo por una sospecha de cáncer cutáneo. Para su inicio fue determinante la publicación en 2006 de los resultados de un estudio realizado Argenziano y cols con médicos de familia, basado en un método simplificado de dermatoscopia y orientado a facilitar la selección de pacientes que deberían derivarse al especialista. Los resultados obtenidos, con un un incremento del 25% en la sensibilidad para el cáncer cutáneo sin cambios en la especificidad, motivaron que nos decidiésemos a implantar la teledermatoscopia diferida.
El objetivo fundamental de la teledermatoscopia diferida es cribado de cáncer cutáneo, fundamentalmente carcinoma basocelular y melanoma. El médico de familia entrenado en dermatoscopia puede realizar un primer filtro y enviar mediante teledermatoscopia aquellas lesiones con sospecha de malignidad o dudosas. Al recibir las imágenes, el dermatólogo realiza un segundo filtro. En caso de tumores cutáneos malignos se gestionará la valoración hospitalaria directamente. Si se trata de un tumor benigno no será preciso el desplazamiento del usuario a las

In precedenza ha lavorato a migrazioni e corsi di formazione su LibreOffice per diverse amministrazioni pubbliche e privati. Da gennaio 2020 lavora in SUSE come Software Release Engineer per Uyuni e SUSE Manager e quando non segue la sua passione per i computer e per Geeko coltiva la sua curiosità per l'astronomia (da cui deriva il suo nickname deneb_alpha).
