Fractures and Soft Tissue Injuries

Fractures
A fracture occurs when a bone is broken, either partially
or completely. There are different types of fractures,
however a first aider is most likely to come across and
“open” fracture where the skin is pierced of there is a
wound leading to the broken bone and “closed” when
here skin has not been pierced. Both open and closed
fractures can be “complicated”.
This means there has been damage to the surrounding
nerves, tissues or organs caused by the broken ends of
the bones. A complicated fracture and a fracture of a
large bone can cause major internal or external blood loss
and sever shock.

Signs and Symptoms
• Pain or tenderness
• “Snap” of the bone can be felt or heard
• Swelling, inflammation or bruising
• Bleeding
• Loss of strength or movement of injured area
• Deformity
• Signs and symptoms of shock

Steps to assist casualty
• Control bleeding
• Call ambulance
• Avoid moving injured part
• Support and immobilise injured part
• Reassure the casualty
• Treat for shock
• Do not try to straighten the broken bones
• Do not move casualty unless in danger
• Apply a splint to stabilise the suspected fracture
• Immobilise the fracture
• Use a well-padded splint well possible
• Immobilise injured area and joints either side of the injury
• Check circulation before and after immobilising the injury

Dislocations
A dislocation occurs when a bone is displaced
from a joint. It can be difficult to determine if a
body joint has been dislocated or fractured. If
there is any doubt treat the injury as a fracture.

Signs and Symptoms
• Pain or tenderness
• Deformity
• Swelling, bruising or inflammation
• Unable to move joint normally
• Signs and symptoms of shock

Steps to assist the casualty
• Support and immobilise injured area
• Elevate injured area if possible
• Apply ice pack or cold compress
• Treat for shock
• Do not try to relocate the injured part
• Seek medical aid
• Call an ambulance 000

Bruising
A bruise, also known as a contusion occurs when
there has been soft tissue damage and bleeding has
occurred under the skin. This is often caused by a
blow of some kind of impact. Bruising is usually red
or purple at first but changes in colour over time as
the tissues heal and as the blood is reabsorbed by
the body.
Bruising can also indicate more serious internal or
underlying injury.

Signs and Symptoms
• Pain or tenderness
• Swelling
• Discolouration under the skin
• Loss of strength
• Signs and symptoms of shock

Steps to assist the casualty
• If associated with a strain or sprain provide first
aid to this injury
• Rest
• Ice pack
• Compression Bandage
• Elevation of injured area
• Referral to medical aid
• If you suspect bruising is caused by a fracture or
other serious injury call ambulance 000

Sprains and Strains
• A strain is caused when a casualty
overstretches muscles or tendons.
• A sprain is caused when a casualty
overstretches or tears ligaments around a
joint.
Sprains and Strains are referred to as ‘soft tissue’
injuries. The signs and symptoms of both of
these injuries are similar and therefore the first
aid for these injuries is similar.

Signs and Symptoms
• Pain or tenderness around injury site
• Swelling, bruising or discolouration
• Loss of power movement in area
• Signs and symptoms of shock

Sprains and Strains
Steps to assist casualty
• Have casualty stop any activity immediately
• Rest – rest and support the injured area
• Ice – Apply a cold compress or wrapped ice pack to injured area.
Use an ice pack for 10 – 20 minutes every 2 hours for the first 48-72
hours. Remove ice pack if it starts to become painful.
• Do not apply ice directly to the skin
• Compression – Apply a firm bandage to in the injured area. Ensure
it does not restrict circulation.
• Elevation – Raise the injured area, above heart level if possible.
• Referral - Casualty should seek medical aid to make sure there is
not any serious damage and seek advice on treatment

  • 2. Fractures A fracture occurs when a bone is broken, either partially or completely. There are different types of fractures, however a first aider is most likely to come across and “open” fracture where the skin is pierced of there is a wound leading to the broken bone and “closed” when here skin has not been pierced. Both open and closed fractures can be “complicated”. This means there has been damage to the surrounding nerves, tissues or organs caused by the broken ends of the bones. A complicated fracture and a fracture of a large bone can cause major internal or external blood loss and sever shock. CSF_V1_0316 2
  • 3. Fractures Signs and Symptoms • Pain or tenderness • “Snap” of the bone can be felt or heard • Swelling, inflammation or bruising • Bleeding • Loss of strength or movement of injured area • Deformity • Signs and symptoms of shock CSF_V1_0316 3

Fractures Steps to assist casualty • Control bleeding • Call ambulance • Avoid moving injured part • Support and immobilise injured part • Reassure the casualty • Treat for shock • Do not try to straighten the broken bones • Do not move casualty unless in danger • Apply a splint to stabilise the suspected fracture • Immobilise the fracture • Use a well-padded splint well possible • Immobilise injured area and joints either side of the injury • Check circulation before and after immobilising the injury CSF_V1_0316 4
  • 5. Dislocations A dislocation occurs when a bone is displaced from a joint. It can be difficult to determine if a body joint has been dislocated or fractured. If there is any doubt treat the injury as a fracture. CSF_V1_0316 5
  • 6. Dislocations Signs and Symptoms • Pain or tenderness • Deformity • Swelling, bruising or inflammation • Unable to move joint normally • Signs and symptoms of shock CSF_V1_0316 6
  • 7. Dislocations Steps to assist the casualty • Support and immobilise injured area • Elevate injured area if possible • Apply ice pack or cold compress • Treat for shock • Do not try to relocate the injured part • Seek medical aid • Call an ambulance 000 CSF_V1_0316 7
  • 8. Bruising A bruise, also known as a contusion occurs when there has been soft tissue damage and bleeding has occurred under the skin. This is often caused by a blow of some kind of impact. Bruising is usually red or purple at first but changes in colour over time as the tissues heal and as the blood is reabsorbed by the body. Bruising can also indicate more serious internal or underlying injury. CSF_V1_0316 8

Bruising Steps to assist the casualty • If associated with a strain or sprain provide first aid to this injury • Rest • Ice pack • Compression Bandage • Elevation of injured area • Referral to medical aid • If you suspect bruising is caused by a fracture or other serious injury call ambulance 000 CSF_V1_0316 10
  • 11. Sprains and Strains • A strain is caused when a casualty overstretches muscles or tendons. • A sprain is caused when a casualty overstretches or tears ligaments around a joint. Sprains and Strains are referred to as ‘soft tissue’ injuries. The signs and symptoms of both of these injuries are similar and therefore the first aid for these injuries is similar. CSF_V1_0316 11
  • 12. Sprains and Strains Signs and Symptoms • Pain or tenderness around injury site • Swelling, bruising or discolouration • Loss of power movement in area • Signs and symptoms of shock CSF_V1_0316 12
  • 13. Sprains and Strains Steps to assist casualty • Have casualty stop any activity immediately • Rest – rest and support the injured area • Ice – Apply a cold compress or wrapped ice pack to injured area. Use an ice pack for 10 – 20 minutes every 2 hours for the first 48-72 hours. Remove ice pack if it starts to become painful. • Do not apply ice directly to the skin • Compression – Apply a firm bandage to in the injured area. Ensure it does not restrict circulation. • Elevation – Raise the injured area, above heart level if possible. • Referral - Casualty should seek medical aid to make sure there is not any serious damage and seek advice on treatment CSF_V1_0316 13
