developed a reputation as a renowned consultant to family

Kindly tell us the source of inspiration for starting a
Medical Aesthetics company.
I began my medical career in 1977 after graduating from
Arkansas' Medical School and completing a plastic surgery
residency. I treated patients in my plastic surgery practice in
Arkansas for over twenty years. At the time, I never
envisioned starting a skincare company.
However, I wanted to provide my patients with the best skin
care products and ingredients available to help their healing
process, so I set out to develop products not available at the
time that would improve their overall skin health and
appearance.
Having majored in chemistry and from treating severely
burned patients, I was inspired to create my full line of
product solutions for restoring full function and mobility to
skin-grafted hands. This led to the development of
Beckman's Skin Care Cream, which would later become the
foundation product for Therapon Skin Health.
The steps to restoring aging skin are to clean and gently
remove damaged skin elements or cells, repair the skin,
restore skin to its healthiest condition and protect skin from
damage. In 1996, the Theraderm Skin Renewal system was
developed, and we have continued to innovate and add
breakthrough products ever since.
Tell us more about your products and service offerings
and what aspects make them stand out in the industry.
What differentiates us is that all of our products are
formulated by me as a plastic surgeon and chemistry major
to restore function to damaged skin. I have personally seen
probably every kind of trauma and skin problem

skin problems.
We first look at what is the cause of the disease or problem;
then, we look at what ingredients can possibly rectify or

the best result from the combination. This would be similar
to preparing an excellent meal with 3-4 exceptional dishes
that complement each of the others.
Please tell us about your journey and how you have
helped with the development and enhancement of the
outreach of your company.
Therapon products in the Theraderm brand enhance the
quality of life through beautiful skin and a restored youthful
appearance. We do this by controlling the factors that
contribute to aging, such as damaged collagen structure,
irregular pigment, and loss of elasticity.
Prevention of daily, cumulative, permanent damage to the
skin from UV exposure combined with restoration of
damage already present is a key element in getting the
exceptional results consistently seen.
The life/work goal of a plastic surgeon has always been to
restore full function and appearance to patients after injury
or damage from disease or trauma. Time teaches that the
skin is not simply the "painted surface" of the body. Its full-
thickness qualities are perhaps the most functional part of
one's life and self-esteem.
Over years of treating severely burned patients, I realized
my passion for developing a superior cream to restore
mobility, suppleness, and softness to skin grafted hands –
hands that could bend and grasp objects necessary for
someone to have a better quality of life.
My college degree in chemistry and passion for knowing
the details of skin anatomy, physiology, and function, and
numerous hours of research in the library led me to
laboratory formulations and clinical testing. The goal was to
find the "ultimate skin restoration cream."

21. Several years later, five women who all had excellent
results in eliminating sagging skin from facelifts asked a
familiar question within the same week: "What can erase
fine lines, wrinkles, and make skin pigment even after a
facelift removes sagging?" In the quest for a product that
would correct these issues, I discovered a unique secret
ingredient that not only enhanced moisturizer penetration
but also had the side effect of erasing fine wrinkles and
restoring youthful colouration to ageing facial skin!
I can remember the spring day in 1988 when the clear
vision came. The vision was to create a simplified daily
skincare system of products that would restore and
rejuvenate ageing facial skin to a beautiful, younger look,
as well as make skin soft and pliable again. Each
component of the system would have a special role in the
system and yet be a fully "stand-alone" product.
The result was the Theraderm Skin Health System that
restores sun-damaged or aged skin and produces youthful,
natural beauty. Taking only minutes per day, women (or
men) of any age or ethnicity and skin type could now
incorporate products into their beauty routine specifically
focused on skin health. Other plastic surgeons heard about
the products, so I began to provide these incredible new
products to their patients. It was a natural progression to
start a company to make sure it could reach the masses.
In your opinion, what could be the future of the medical
aesthetics industry post-pandemic? And how are you
strategizing your company's operations for that future?
As proven years ago, during the great depression, when
both money and goods were scarce, women continued to
spend money on skincare products. Little expense could
make their skin appearance look pretty and youthful! Our
plan is to continue providing exceptional skincare products
that are proven to diminish aging appearance while
restoring both youthful texture and coloration to skin.
As an established leader, what would be your advice to
the budding entrepreneurs and enthusiasts aspiring to
venture into the medical aesthetics industry?
My father instilled many business principles in me as I
grew up. He died suddenly in the summer after my first year
in medical school, leaving behind a business with 56
employees that I needed to manage for a year and a mother
and six sisters to care for, so I took a year off from med
school.
I luckily had good advisors as I ran the company and
quickly learned about all aspects of business, which helped
prepare me for running my future practice and business.
My advice would be to stop waiting. I encourage every
person of any age or station in life to wake up every
morning and think about who and what they could do to
effect change in their own lives and others' lives.
How do you envision scaling your company's services in
2023 and beyond?
My major in college was chemistry. This allows me to
originate essential aspects of our product formulations to
get properties that direct skin condition characteristics and
desired actions.
Each of our products is made using our own proprietary
formulations. We choose specific containers and contract
manufacturers for the custom production of our products to
our specifications. Our manufacturing partners can scale up
production to extremely large quantities if given a week or
two lead times.

blood flow restriction (BFR) training.
What is known today as the BFR training method stems
from the pivotal works of its inventor Dr. Yoshiaki Sato.
Just about fifty years ago, he coined the name “KAATSU,”
which could be roughly translated from Japanese as “added

it actually is? Yet, it was not until recently that researchers,
practitioners, and other professionals were made to take a
closer look at this unique technique as a result of its vast
potential.
What Is BFR Training?
BFR integrates low-intensity (~20%–40% of 1-RM)
resistance training along with partial blood flow restriction
to the working muscles using inflatable air cuffs fixed in the
proximal region of the limbs exercised. Most applications
include the complete occlusion of venous flow, while the
recommended pressure used to block the arterial flow
ranges from 40 to 80%, and only in passive BFR, the cuff
pressure is set between 80 to 100% for a short period.
Although BFR is based on very low intensities, it has been
shown to stimulate increases in muscle size and strength
that are comparable to those seen following conventional
high-intensity resistance training.
BFR Devices in a Nutshell
Many different devices are commonly used to facilitate
BFR training, including elastic wraps or straps, inflatable
pneumatic cuffs, and tourniquets. Since it is difficult to
determine the amount of applied pressure, caution is
advised when using some of them. Apropos of this, BFR
devices could be generally divided into two groups. The
first one refers to practical blood flow restriction (pBFR)
training which was created in 2009. As an alternative to the
traditional BFR, this type of training practice suggests the
use of rigid elastic bands or knee wraps designed for
occlusion training which are also affordable and practical.
The second group involves inflatable pneumatic cuffs and
tourniquets where the needed occlusion is created by
inflating the air through an automatic device or handheld
pump. This group can be further divided into two subgroups
according to devices that automatically calculate the LOP
(limb occlusion pressure) and the ones that require a
Doppler ultrasound. The percentage of occlusion is
calculated based on LOP, which is the minimum pressure
needed to completely occlude the arterial blood flow.
Dimensions of the applied device, the site of restriction, and
individual physiology, such as limb circumference and
diastolic blood pressure, all have an impact on the process
of arterial blood restriction.
Breaking Down the Benefits of BFR
A growing body of evidence has shown that BFR can be
used in physical therapy to aid in the recovery of elderly
patients after knee arthroscopy or even more complex knee
surgeries. Recent studies have also demonstrated that, when
compared with low-load training, low-load BFR training is
more effective, tolerable, and, therefore, a potential clinical
rehabilitation tool. The current literature suggests that a
significant increase in strength and sports performance is
associated with the use of BFR training.

can be used in everyday training as well as in rehabilitation

continued research is more than vital if further advances are
to be made.
The First BFR Certification in the UAE
Since its increasing popularity is undeniable, it is important
to emphasize that if not performed under the supervision of
a trained professional, BFR can cause severe health issues.
Health, Exercise, and Research Center (HERC) is a Dubai-
based company that recognized the importance of creating a
course that can build one’s competence to lead people to
change successfully. Among many other accredited courses
within HERC’s Education department, this is the world’s
first one that divides BFR usage according to its practical
applicability to BFR Training and BFR Therapy. BFR
Training Certification is aimed at personal trainers, sports
coaches, and strength and conditioning coaches working
with a healthy population in the gym environment. BFR
Therapy Certification is aimed at physiotherapists, sports
medicine specialists, and other healthcare providers who
can benefit from BFR use in a clinical setting working with
patients with low to moderate health impairment. Therefore,
the course is organized in a way that enables individuals
from different fields to further their careers and support
those in need. All participants have the chance to practice
with different BFR devices, from pBFR straps to inflatable
cuffs. HERC’s Research Department is currently involved
in a BFR research project collaborating with international
researchers. The idea is to investigate the LOE (levels of
evidence) within PRISMA’s guidelines for Systematic
Reviews. The main objective is officially to register BFR as
a method that can be used by physiotherapists and sports

Steyn's extensive experience in chemistry and
biotechnology, has led to the formation of companies that
are focused on topical probiotics and products that target
the skin microbiome. His entire portfolio of companies
provides certified organic, cruelty-free, fair trade, and
carbon-neutral personal care products.
In 1996, Trevor finished his master's degree in organic
chemistry after studying the subject. Prof. Siegfried
Drewes, his mentor, sparked his interest in the chemistry of
African plants. Trevor developed and established Esse
Skincare after comprehending the possibilities of several of
these botanicals for skin care. Esse's development started in
the year 2000 with a study of the African Sausage Tree,
Kigelia Africana. Esse was born in 2002, two years after
she had been in the making.
Below are the highlights of the interview, which shed light
on Trevor Steyn's work on the application of new probiotic
species and delivery methods to combat ageing and
optimise skin health.
Kindly tell us the source of inspiration and what led you
to serve the Medical Aesthetic Industry?
We were early movers in the skin microbiome space, and
we developed a strong understanding of the options
available to shift the microbiome towards healthy diversity.
Medical aesthetics is dominated by treatments that are fairly
invasive.
These treatments often disrupt the skin's microbiome,
leading to persistent inflammation and slower healing times.
This causes suboptimal results for the client. We see this as
an opportunity to apply new biotech techniques, like live
biotherapeutics, to reduce the risk of complications for
practitioners and optimize results for their clients.
Brief us about yourself and shed some light on your
journey at Esse Skincare.
I did a postgraduate degree in organic chemistry and
initially moved into drug discovery before starting Esse in
2002. In 2009, the Human Microbiome Project caught my
attention. I realized that the findings would fundamentally
change health care and skin care, so I started the process of
rebuilding myself as a microbiologist. The intersection of
chemistry and microbiology led to new techniques to shift
the microbial ecology of the skin. Over time, these have
yielded strong anti-aging results due to our ability to
optimize barrier function and reduce inflammation.
Can you elaborate on the science upon which your
products and services are based? How are they different
from other skincare products out in the market?
In February 2015, Esse became the first brand to use live
bacteria in a retail product. It isn't easy keeping them alive
in an off-the-shelf product at room temperature. We couldn't
use preservatives, and we had to develop encapsulation
technology that kept them in a dormant state until the
product was applied to the skin. We launched our probiotic
serum with one billion colony-forming units per ml, and
this product has become our hero product in more than 40
countries worldwide. We showed its efficacy in independent
trials in Bonn, Germany, and this brought the credibility
that we needed in the Medical Aesthetics channel. There are
currently only a handful of companies in the world that are
able to offer live topical probiotics.
Since then, we have launched a few new actives and
techniques that have the ability to nudge the microbial
ecology on the skin toward a more balanced state.

36. What areas, in your opinion, do the skincare products in
the market fail to address, when it comes to healthy
skincare?
The skin microbiome is largely in control of barrier
function, and barrier function is the skin's most important
role. Many of the products that we see on the market fail to
understand that high-foam surfactants and many other
synthetic ingredients are detrimental to tight junctions and
commensal skin microbes, so they create “leaky” skin with
a poor barrier function.
We hope to reduce inflammation by improving barrier
function and strengthening the skin microbiome, allowing it
to protect itself through the competitive exclusion of
pathogens. This takes around two months, but the long-term
results are worthwhile.

37. In what way do you consider technological advances to
be a pathway in conducting your operations?
We're largely a group of scientists that use the company as a
way to fund the research that we find exciting. The skincare
industry has been treating skin in a hopelessly
oversimplified way, and there are so many ways to improve
upon current approaches to long-term skin health.
Understanding the deep complexity of the skin's interaction
with microbes drives us to avoid disrupting it, instead using
technological advances to mimic the conditions for which
we've evolved. This will allow us to reduce chronic
inflammation in the skin and return it to its ancestral state,
free of the inflammatory conditions that plague modern
humans.
What R&D advances are you leading at Esse Skincare
currently? How will they be beneficial to the customers?
We're doing a lot of work on mimicking skin lipids because
these are the primary nutrient source for commensal
microbes. Oils are very high in energy and come at a
substantial metabolic cost. However, our skin still produces
large amounts of unique oils that are found nowhere else in
the animal kingdom and are also absent from our internal
tissues.
Sapienic acid is named for our species, Sapiens, and it is the
most abundant fatty acid on human skin. We have managed
to find a source of this critically important fatty acid and
have included it in products designed to protect the skin
after invasive procedures.
There are no other brands that have access to this active, so
we're excited to pioneer another first in skincare. We are
also using UV light to modulate the skin microbiome and
boost Vitamin D levels in the skin.
What endeavours are you currently pursuing to
improve your service measures for the clients?
We are currently working on a personalized service for
clients that will reduce the number of products that they
need to purchase and provide only the actives that are
relevant to their skin concerns (and budget).
What are some of the challenges you face when
conducting projects, and how do you turn them into
opportunities for growth?
In our experience, we find new and interesting challenges in
every project. Usually, these are not the challenges that we
anticipated at the outset, but we generally keep a buoyant
attitude toward resolving them. Each challenge is an
opportunity to differentiate ourselves from our competition.
We find that most people give up too early.
What advice would you like to give to budding
entrepreneurs and enthusiasts who desire to venture
into the medical aesthetic industry?
Don't give up too early.
How do you envision scaling your services and
operations in 2023 and beyond?
We've recently expanded into Canada, and this opens up the
opportunity to drive sales in the USA, which will be new
territory for us. The market is structured differently in North
America, and there will be plenty for us to learn.
What are some of the testimonials or recognition that
accurately highlight your position in the market?
We've won awards around the world, but, to be honest, we
don't place much emphasis on those. Individual testimonials
are also too easy to rely on. A sample size of one doesn't
give us much information. We prefer statistically relevant
results as drivers of innovations that are effective. It is very
difficult to discern signal from noise in the skin microbiome
space. We will continue to run our sequencing studies, but
we understand that the client doesn't care about their
microbiome; they only care about visible improvements in
skin condition, so we will be sure to deliver those first.
