This document discusses the role of public health in preventing violence. It argues that violence should be viewed as a public health problem and that the public health community has neglected this role. It makes three key points:
1) The public health community should be more interested in violence prevention and promote the evidence base for effective crime and violence prevention strategies.
2) Public health directors should provide leadership for local crime and violence prevention partnerships and agendas.
3) The police are interested in evidence-based approaches to crime prevention and public policy.

Abstract:
This presentation presents a model of working with traumatized children and families across culture. When it comes to trauma in children, we need to address three basic questions:
(1) why development matters, (2) why family matters, and (3) why culture matters (Di Nicola, 1992, 1996, 1997, 1998, 2012, 2018; Di Nicola & Song, forthcoming).
These three aspects of children’s lives are reviewed as key critical contexts to understand the “sequential traumatizing” (Keilson, 1992) of young people as highlighted in two clinical vignettes. In the first vignette, “A Train of Traumas,” the layers of the trauma history of an immigrant child and his family from the Maghreb are teased out as an imbricated series of triggers across developmental, cultural and family predicaments. “The Memory Clinic,” the second vignette, revisits the story of an adolescent refugee from a war-torn country in the Middle East whose quest was to forget her trauma. Exposed first to civil war and the loss of her family, then arriving in Montreal as a refugee with her extended family where she was abused, this vignette presents issues about how to create the conditions for listening to the “trauma story” (Mollica, 2009) as enlightened witnesses and the emerging understanding of traumatic memory through identity narratives (Novac, et al, 2017). Together, these vignettes highlight the conditions required for the practice of “trauma-informed care” with children and families across culture.

Domestic violence, sexual violence against women and human trafficking are the three most pervasive problems women rights activists from the Good Shepherd Sisters Congregation encounter in the 72 nations in which they operate. To better understand how women fall prey to this cycle of violence and abuse the New York-based NGO, Good Shepherd International Justice Peace Office (GSIJP) undertook a globe-spanning survey to determine who’s most at risk and how this pattern can be broken. Here are the results.
Recognition of the role local public health departments play has risen sharply since 2018, according to a national poll conducted by Public Opinion Strategies for the de Beaumont Foundation. 73 percent of voters say public health departments play an important role in making communities healthy, compared with 56 percent in 2018. And 61 percent say they'd be willing to pay more in state and local taxes to provide funding for public health services.

Harm Reduction acknowledges that drug use occurs and is potentially damaging, and attempts
to reduce and prevent these harms in ways, which are pragmatic and humanistic. Although the
harm reduction approach and specific harm reduction interventions (including needle and
syringe programmed and methadone programmed) have been implemented with resounding
success internationally (including in a number of African countries) for more than 20 years, the
South African approach to substance use and substance-related harms is still largely informed
by the doctrine and strategies of the "War on Drugs", and focuses almost exclusively on
prohibition and punishment. Because of the strong link between drug use and poverty, different
policy approaches to poverty and homelessness must also be considered. My talk will critically
examine these two approaches to drug use, poverty, and displacement (support or punish), as
they are being implemented in Cape Town. Opportunity for discussion will be provided.

Bowman’s Gland: Secretes mucus.
Stimulation of Olfactory Cells:
Odorant dissolves in mucus and attaches to receptors on olfactory cilia.
Involves a cascade effect through G-proteins and second messengers, leading to depolarization and action potential generation in the olfactory nerve.
Quality of a Good Odorant:
Small (3-20 Carbon atoms), volatile, water-soluble, and lipid-soluble.
Facilitated by odorant-binding proteins in mucus.
Membrane Potential and Action Potential:
Resting membrane potential: -55mV.
Action potential frequency in the olfactory nerve increases with odorant strength.
Adaptation Towards the Sense of Smell:
Rapid adaptation within the first second, with further slow adaptation.
Psychological adaptation greater than receptor adaptation, involving feedback inhibition from the central nervous system.
Primary Sensations of Smell:
Camphoraceous, Musky, Floral, Pepperminty, Ethereal, Pungent, Putrid.
Odor Detection Threshold:
Examples: Hydrogen sulfide (0.0005 ppm), Methyl-mercaptan (0.002 ppm).
Some toxic substances are odorless at lethal concentrations.
Characteristics of Smell:
Odor blindness for single substances due to lack of appropriate receptor protein.
Behavioral and emotional influences of smell.
Transmission of Olfactory Signals:
From olfactory cells to glomeruli in the olfactory bulb, involving lateral inhibition.

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

”But deaths are only the tip of the interpersonal violence iceberg. For every death due
to interpersonal violence there are perhaps hundreds more victims that survive. Globally,
tens of millions of children are abused and neglected each year; up to 10% of males and
20% of females report having been sexually abused as children. For every homicide
among young people there are 20–40 non-fatal cases which require hospital care. In
addition, rape and domestic violence account for 5–16% of healthy years of life lost by
women of reproductive age, and, depending on the studies, 10–50% of women experience
physical violence at the hands of an intimate partner during their lifetime.”

Providing services for victims is only one of the roles that the health sector can play
Reading the pledge and these three key publications will give many actions that can be taken now.
Just SOME of the actions that can be taken:
At an individual level
Measure your carbon footprint
Understand exactly where you use energy and how you can reduce it.
Think tonnes or kgs of Carbon dioxide rather than just KWH
Understand what low carbon and active travel really is and live it – walk, bike, public transport
Think about the carbon and climate consequences of the way we/you eat, everything from food miles, to waste, to supporting local producers an retailers to the carbon consequences ad health consequences of high meat and dairy diets
At an organisational level
Ensure your workplace really provides low carbon transport options, buses, liftshare, cycle paths, racks, showers, lockers...
Is there a sustainable development or climate change staff group that tackles things from the ground up?
Has there been a organisational carbon footprinting or auditing process done?
Ensure the head of your unit / line manager / Chief Executive, knows that MANY health professionals are seriously concerned. Numbers matter.
Set an example to other staff, patients and visitors
Is climate change on the risk register of your organisation?
Is your organisation part of the carbon Reduction Commitment from April 2010?
Is sustainability built into the measurement of performance of your organisation?
How much renewable energy does your organisation buy, and could it buy more?
Is your organisation explicit about what it’s climate impact is and what it is doing about it?
Is there a procurement policy in your organisation that reduces the climate/carbon impact?
Is the leadership team in your organisation aware of the many immediate health co-benefits that can be gained from immediate action – health, lives, money and reputation...?
At a political level
Write to your MP and get you family friends and colleagues to do the same.
Contact them well before the next round of international agreements are made in Copenhagen, Nov 2009 and ask tem where they stand.
