Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration
HEMORRHOIDS
Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)

pandemic, saving the medical fraternity all the
while to the long-term effects that COVID-19 might
have on surviving patients and the mental trauma that
many had to face due to extended isolation periods.
While people might have accepted the changes and are
trying to live with it, the challenges continue to evolve
and nag the industry.
Medical Insurance sector also felt the effects. To start,
many, including the insurers did not know what and
how to cover the COVID-19 claims that came pouring
in. The policyholders found themselves paying an
exorbitant amount of money for treatment and the
insurance providers were looking forward to the
guidelines that could help them deal with it. Whether
health insurance is being provided by public
establishments or private players, it played another
crucial role in the way insurance money was handled.
With the economic slowdown, it was tough for
policyholders to pay their premiums in due time. So,
policy providers had to quickly reshuffle the premium
days giving extensions to handle the building capital
pressure. The categorization of what is to be paid out
and what not was another challenge. With no
standardized cost of treatment and inclusion of novel
items like PPE kits into mandatory and routine
treatment, they had a hard time figuring out the
payables from the non-payables.
On the brighter side, health insurance providers saw a
surge in demand for policies. An increase in the number
of queries also led to the development of new policies
and products that could better cater to communicable
diseases in the future. Taking a leaf out of the SARS
pandemic that earlier caused havoc, this time around
the providers actually had something to build on even
though the current data was not sufficient enough. It
also provided them with an opportunity to partner
with hospitals and corporates for providing better
health care benefits.
Overall, the medical insurance and the healthcare
sector are set for a revamp even when the pandemic is
curbed. There has to be a unison among the various
parties involved in providing healthcare starting from
patients, to the hospitals to the insurance providers. In

20. The world today is confronted with a
dangerous virus that has brought
economies, travel and life to a complete
standstill. Healthcare systems are trying their
level best to keep afloat and discover a cure as
soon as possible. This urgency to save lives has
once again highlighted the need and importance
of high quality scientific evidence and
knowledge, stable research infrastructure as
well as funds available for medical research, to
ensure better response and resilience.
Healthcare research also known as medical
research is the driving force for improving the
performance of healthcare systems and the
quality of life of individuals by finding better
preventive and proactive treatments and drugs.
This can involve testing new medicines,
vaccines, medical devices, new medical
procedures as well as examining the whole
genome sequence to find out the cause and
treatments for various diseases.
While India today is well known for its
excellent medical facilities and clinical
expertise, it really lacks behind in the field of
medical research. A study published by Ray et
al (2016) revealed that between 2005-14 out of
the 579 medical institutes and hospitals, that
were evaluated, only 25 produced more than
100 papers in a year and alarmingly 57.3% of
the medical colleges did not have a single
publication in a decade as compared to the
research output of Mayo clinic, which was
approximately 3700 studies in a year. The
reason for such a disappointing output can be
insufficient funds, poor research infrastructure,
the notion that research is done only for the
benefit of the pharmaceutical industry as well as
overburdened physicians having little time and
energy left to think about any serious research.
Apart from this the government and authorities
have also failed to integrate medical education
and research with basic sciences as well as
technology as prevalent in the United States and
most European countries. This proves to be a
major roadblock to “Make in India” based
innovations. In addition to this there is also a
strong presence of an authoritative and
hierarchal set up that discourages capable start-
ups and faculty to go beyond the routine
healthcare practices particularly in the field of
genetics.

last two decades. Researchers world
over have analyzed enormous amounts
of DNA sequenced data to assess
genetic predisposition to diseases, to
improve diagnostics and therapeutic
treatments for cancer, diabetes,
neurological and cardiovascular issues
and for the development of tests to
improve efficacy of drugs to prevent
adverse drug reactions.
Even though genetic research and
testing has the power to transform
healthcare it is still grossly under
utilized for disease management in
India. Being home to different cultures
and genetic diversity in addition to
endogamous practices rare diseases
such as blood disorders, primary
immunodeficiency, neurodegenerative
and musculoskeletal diseases amongst
many others contribute significantly to
the medical and economic burden of
the country. According to the Council
of Scientific and Industrial Research
(CSIR) nearly 7 crore Indian’s suffer
from rare disorders of which at least
40% are genetic in nature.
Unfortunately quite a few of these
genetic disorders are either
undiagnosed or misdiagnosed due to
insufficient clinical and diagnostic
resources and even if these are known
the treatment is not always available
resulting in huge physical and
monetary costs to patients who very
often have to suffer all their lives. Rare
diseases are also more common in
children and are responsible for 30% of
the deaths during infancy. There is a
need to address these challenging
issues by investing more funds into
biomedical and genetic research to
identify the main causes of diseases
and discover novel therapeutic targets
and strategies to improve healthcare in
the country.
It has also been observed that the
genetic data of the Indian population is
grossly under represented in worldwide
genomic studies and thus clinicians
need to rely on the gene data banks
from UK, USA or the Caucasian
population. This does not serve us well
as this data is not reliable. Given the
unique diversity in the Indian
population, which is absent elsewhere,
genomic research only seems to be the
way forward to reduce the impact of
diseases. Practicing precision medicine
utilizing this indigenous data will also
help to reduce inequities in how
healthcare is financed. It is estimated
that the government expenditure was
about 1.4% of the GDP in 2017-18
while the private expenditure was three
times this number. Given this pattern
correct and early diagnosis and
removal of trial and error through
pharamacogenomics will greatly help
to reduce this cost.
The department of Biotechnology has
made a considerable breakthrough to
solve the struggle for genetic data in
the last one year by mapping the
genomes of 1008 people across
different ethnicities. While this is a step
in the right direction it is still a drop in
the ocean given the sheer diversity and
sub population of people in India. In
China and the US almost a million
people are included in research studies
therefore it is essential for the
government to scale up the sample size
and include more people as well as
reduce the cost of collecting such data.
Attention also needs to be given to
carry out newborn screening pilot
projects on a national scale to lower
the burden of rare diseases and also
train more clinicians, researchers and
councilors to correctly interpret the
results for the benefit of the common
people. The government also needs to
initiate researches on gut micro biomes
and gene editing – a group of
technologies that give the scientists the
ability to change a person’s DNA by
adding, removing or altering genetic
material at particular locations in the
genome- to help treat cancer, metabolic
and cardiovascular disease as well as
neurodegenerative conditions like
Huntington disease, Alzheimer and
Parkinson that have no specific
diagnostic approaches or treatments.
Our country is definitely active in the
field of medical research but it is still
not doing as much as it can. The
discoveries that are being made are
only a fraction as there is so much that
can be learnt from our diverse
population. India can truly become one
of the models to study genetic
variations and how these relate to
disease and drug responses. It is now
time for the government to increase
funding and formulate polices to create
a national genomic platform, which
will improve the healthcare system so
that our future generations can lead
healthier lives without having to suffer
tremendous financial burdens.

Kettering Cancer Center in New York.
Dr. Zaatari has also held a faculty
position at Emory University in Atlanta
(1982-1986) and served as associate

1995) before returning to AUB in

at the clinical level, AUBMC has
strategically utilized the size of its
medical campus to designate a
freestanding building to the care of
COVID-19 patients; it is away from
the main buildings of the Medical
Center where regular patients are
admitted. This building includes both
ambulatory and inpatient services. The
ambulatory service, also known as the
flu clinic, takes care of all patients who
are coming in with symptoms that are
suggestive of the diagnosis.
Nearby, it has a separate area to carry
tests for the individuals who are
seeking testing for the purpose of
travel or other indications such pre-
operative testing or in preparation for
procedures. Secondly, it also has a

facility. As for inpatient service, this
dedicated building has four floors,
which include intensive care units to
admit patients with severe
complications as well as those with
less severe illness. The building houses
an operating room, dedicated to those
COVID patients who need to go for
surgery.
In worst-case scenario of the
pandemic, especially with
unpredictable surges in the country,
AUBMC may have to utilize the
regular hospital beds in the main
building; in the meanwhile, we are
striving to avoid such situation by
coordinating with other healthcare
facilities in the city.
For the educational programs it
undertakes, AUB & AUBMC are
relying on online and remote education
to its medical students and trainees.
However, there are situations where we
need to bring them for face-to-face
instruction and education, especially
for those require direct patient
exposure for adequate education.
To cope up with financial crisis of the
country, AUBMC is going through
belt-tightening measures, and it is
restricting capital expenditures to the
most needed and required equipment.
The leadership has to be very selective
and prioritize the new equipment and
technology that the Medical Center
needs to acquire during this critical
period
Being a private non-profit organization,
AUB & AUBMC have some
independent funding and financial
resources that enable it to carry on with
much of its routine work, but right
now, it is in a determined survival
mode that would allow it to carry on
with its mandate of offering best care
possible and also sustaining its mission
for the future.

U
nlike most animals (not counting some like dogs and cats),
humans don’t really have a system of regulating their food
intake! Tastier the food, more the intake! Given that most
so-called tasty foods are processed with more than their share of
starch, carbs, salts and sugar, it’s but natural that most humans
suffer the most when it comes to health.
To obviate the same and live a beautiful life for all times, all this is
called for, is a change to healthy immunity-boosting superfoods
which in most cases are abundantly found all over the planet in
some form or the other, and in almost all cases, are packed with
body-nourishing nutrients. We give you a list of such foods as also
explain what they do when you consume them in the required
quantities.
Citrus Fruits (lemons, oranges and lime)
Citruses are packed with Vitamin C. The latter is needed in
abundant quantities throughout our lives to help us create and
maintain WBCs which itself help us grow and repair body tissues
be it of the skin, ligament or even the blood vessels. How well our
bodies help themselves heal wounds is a function of Vitamin C. The
tricky part is that it is water-soluble which means unlike fats, our
body does not store them and you need to regularly replenish

including tobacco smoke. Free radicals on their part are
known to exacerbate conditions like cancer, heart
disease and even diabetes.
Eat these the way they are or add then liberally to a
dollop of yogurt or ice-cream to make the latter more
interesting. But, eat you MUST!
Tea (green and black)
Tea of any kind is known as a storehouse of flavonoids,
another kind of antioxidant that remain our main
defence against free-radicals that we are exposed to
almost daily. Of these, green tea is particularly potent
with its ingredient, epigallocatechin, a variant of
antioxidant known to boost human immunity strongly.
If tea isn’t on your list of drinks on a daily basis, try
bringing it actively by substituting other including
coffee, milk and of course alcohol. Besides making you
feel good with its faint traces of caffeine, it has a lot
more to offer to in terms of nutrients.
Ginger
Aside of its ability to add an essential zing to most
foods, ginger also has amazing properties that has seen

round of garlic-rich foods, remember that the pungent
smell is due to the mix of sulphur and allicin, both of
which boost WBC production and functioning. WBC if

that does not command nutritional value. A tablespoon
of this immunity-boosting super-food contains
manganese, phosphorus and potassium in traces
besides of course the star, curcumin that research
shows has the ability to prevent and treat various kinds
of cancer.
Curcumin, as per research is also great at fighting
inflammation that come about due to conditions
including cancer, heart-disease, diabetes and arthritis.
Curcumin being difficult to absorb on its own, it makes
sense to add it to curries and lentils. Turmeric lattes are
the latest fad in the west, and apparently quite effective
in curing common colds. Green-tea laced with turmeric
too is a great way to consume this great compound. And
for those with time and patience in hand, there are also
pickles and chutneys made out of turmeric!
