Engineers with service backgrounds who saw the
community’s need for quality service at affordable prices.
The company has expanded its service capabilities to
provide selected services and equipment nationally and
internationally throughout Florida.
Exemplary Leadership
John has a master’s degree in biomedical engineering. He
started working as a trainee at a Medical Imaging Sales and
Service Company. Eventually, John worked out his way
into the field and became a very proficient MRI/CT Field
Service engineer and then MRI Manager. He decided to
start his own company with another service engineer.
Together, they founded Engineering and Network Systems
(ENS) in 2005, which lasted ten years, and then they
decided to part ways. Later, John founded Bravo Imaging
Inc. in 2015 and has successfully positioned Bravo Imaging
Inc. as a significant competitor among companies providing
multi-vendor and multi-modality imaging equipment and
service to the medical community.

Amongst the major concern around the talks of
patients, a major stake is in affordable care
treatment.
This can be observed through a recent research study.
According to a study, by the year 2026, experts estimate
that out-of-pocket healthcare spending will increase by
9.9% per patient's financial information. However, when
patients in a 2018 survey were asked, “What do you value
most when getting services from a healthcare provider?”
they responded, “affordable out-of-pocket costs” and “my
provider cares about me.”
But, it is worth noting that affordable care doesn't just
comprise consultation and drug treatment. One major
segment in this context is diagnosis through Radiological
services. In a well-functioning healthcare system, radiology
has become a core contributor that guarantees clinical
information, health gains, and reduced costs in treatment.
Achieving such objectives of serving affordable care and
supporting the human cause is Marigold Hospital—a
Nigerian-based care center promoting the utilization of

Sandra Chisom Ohuabunwa, a Radiographer (Intern) at

approach to patient care. Through the hospital's modern
laboratory, incorporated with diverse modality brands and

justification of the treatment that they receive.
Let's walk through the insides of radiology procedure that is
enabling marigold Hospital achieve effective care.
What was your inspiration behind venturing into
Radiography?
At a young age, I realized that one's life could only be made
worthwhile through service to humanity.
The question of how I would be remembered by humanity
became important. Thus, I believed I could serve humanity
using my unique natural qualities and empathy alongside
my knowledge and dexterity.
Venturing into the health sector, I picked radiography,
particularly because not many people venture into this field.
Currently, there are over 217 million Nigerians and less
than or about 2500 radiographers. I became passionate
about seeing the good we can do with technology.
Please tell us about your journey, highlighting your
contribution to Marigold Hospital's success.
My journey has not been easy, knowing that I had to work
twice as hard. But looking back, there is not a single thing
that I would change.
Marigold Hospital has become a center that provides
quality care and training to mission-minded health
professionals.
Being Marigold's pioneer radiographer intern, an enormous
responsibility falls on my shoulders to set the pace for the
succeeding Radiography interns. There is also the prospect

of introducing first-class and innovative ideas, new
perspectives, and a unique dexterity to Marigold Hospital.
As a student radiographer, please shed light on your
experience at Nnamdi Azikiwe University and Foremost
Radiology Consultants Limited. At present, how does
that serve your interests and the role that you play at
Marigold Hospital?
At Nnamdi Azikiwe University Teaching Hospital
(NAUTH), I was exposed to a range of modalities such as
Computed Tomography, Fluoroscopy. Although NAUTH
was not my first clinical experience with such modalities, it
was an eye-opener.
There are quite a number of rare radiology procedures, and
sometimes we only have theoretical knowledge. But at
NAUTH, I witnessed and gained first-hand experience.
In addition, my journey at Foremost Radiology Consultants
Ltd began in May 2020. Training with different senior
radiographers coming from diverse knowledge backgrounds
was exciting. There, my experience ranged from Magnetic
Resonance Imaging to X-rays. My journey at FRCL ended
in June 2021, and I became a better radiographer while also
creating a professional network.
Looking back, my experiences at FRCL and NAUTH have
equipped me for my role at Marigold. I was trained in
Magnetic Resonance Imaging, Computed Tomography,
Ultrasound, Fluoroscopy, Mammography, Special
Investigations, and X-ray at both places. Although my
experience is not limited to the mentioned modalities, these
experiences have proven to be relevant at Marigold as the
Radiology department is similar in most establishments.
Can you elaborate upon the core values on which
Marigold Hospital is built and the company's mission?
The core values of Marigold are to provide patient-centered
care and quality and innovative healthcare delivery. In
addition, their mission is simply to provide the most
appropriate care in a setting that supports and responds to
each patient's unique needs.
Please elaborate on the technological innovation that
Industrial Radiography should incorporate.
When it comes to the field of Industrial Radiography, the
area of focus should be on creating awareness and making
training accessible.
Several studies carried out within Industrial Radiography
have demonstrated a poor level of awareness. What I
propose could be a solution to this challenge is digitalizing
the training for Industrial Radiography. This solves the
problem of awareness amongst radiography students and
would gradually extinguish the notion that Industrial
Radiography is an entirely different field.
While serving for Diagnostic Professional Imaging
Center, what challenges did you face? How did you turn
them into opportunities for growth?
Diagnostic Professionals Imaging Center was a wonderful
center, but no establishment is without its shortcomings.
My only challenge while working there was prolonged
downtime of the Computed radiography system. This
brought about a temporary revert to Manual (dark room)
processing. The exposed radiographic film is processed by
hand through a series of timed steps in manual processing.
For these reasons, many radiographers do not like the
manual processing method. On the other hand, I decided to
use this as an opportunity to experience working with the
manual processing system.
An encounter with both extremes of the profession (manual
and digital) made me appreciate the technological
advancements in radiography and how far we've come.

Please shed light on the state of Radiography in the
African market and what can be done better.
I am proud of Radiography in the African market and our
achievements. However, specialization in radiography is
important. With specialization, there is room for
advancement and further training. This would increase the
quality of radiographers in the African market, enable them
to follow the career path they believe is best suited to their
capabilities and goals, create career opportunities,
encourage research in areas needing development, and
allow growth in the profession.
How do you envision further strengthening Marigold
Hospital's stronghold in 2023 and beyond?
I envision strengthening Marigold Hospital's stronghold by
increasing the value of my input in delivering quality health
service and employing extreme creativity when working.
I see growth as a necessity. Thus, I would continue
passionately in professional development by acquiring new
skills and global knowledge through attending conferences,
updating courses, and webinars, and learning globally.
These skills and knowledge gained will be applied when
working to ensure Marigold remains one of the top
hospitals.
Identifying challenges and working together with the
radiology team and administration to tackle them to ensure
the values and mission of Marigold Hospital are sustained.

hat does it mean to live well with
Wa serious illness? Just because
someone is or might be dying,
does not mean that they are done living. Our
healthcare systems must be equipped to not
only strive for health but also to strive for
quality of life at every stage of life and
illness. We can’t deny patients the
opportunity to experience joy, feel peace,
bring meaning to their last chapter, and
importantly live while they are dying.
Ultimately, this is what palliative care
provides to those who are experiencing
health-related suffering – whether they are
dying or not, whether they are still pursuing
curative treatment or not – though too few in
Canada receive that care.
Palliative care could benefit 89% of those
who die in Canada. However, less than 6 in
10 Canadians who died in 2021-2022
received care from the discipline that
specializes, in part, in caring for the dying.
The gap in access is significant and bridging
it will require a concerted effort across health
professions, all levels of government and
communities. We can’t afford to not invest
the effort.
The Case for Investing in Palliative Care
Access to palliative care in Canada has
increased by 6% over 5 years (52% in 2016-
17 to 58% in 2021-22). While this is moving
in the right direction, we can’t forget that
Canada has a rapidly aging population. By
2030, there will be 2.2 million more seniors
in Canada than there were in 2022. Since
seniors, as a population group, have higher
palliative care needs, a 6% growth in access
to palliative care over the last 5 years is a
drop in the proverbial bucket. This is a
demographic timebomb heading straight for
our healthcare systems. These systems will
inevitably implode under this exponential
growth in the need for health care for
Canadian seniors, including palliative care
unless significant changes are made.

The benefits of investing in increased access to palliative
care are huge. This includes cost efficiencies by preventing
patients from being hospitalized and occupying expensive
acute care beds that aren’t appropriate for their needs. There
are thousands of patients in acute care beds waiting for
availability in alternate level care (ALC) every day, causing
a significant burden on our health care systems. Many
provinces are struggling to address the issue, including
Ontario, where nearly 40% of those ALC patient days were
for patients at end-of-life. There are thousands of days
when high-cost acute care beds were occupied by a patient
who could have been better served in a palliative care
setting if only those beds were available.
Palliative care is more cost-efficient, requires fewer
resources overall, can be provided in any setting – including
at home where most Canadians want to die, reports higher
satisfaction with care from patients and caregivers, and
provides caregiver and grief support. Which begs the
question: Why aren’t we already doing it for everyone?
Public Awareness
Despite decades of awareness building, most Canadians
remain unaware of palliative care, or mistakenly believe
that palliative care is only relevant in the last few weeks or
days of life. This myth is perpetuated in part because
palliative care is often only accessible to people in Canada
in those last weeks of life. We don’t have the workforce or
resources to provide this care earlier in the illness
progression in a consistent way. The public can hardly be
blamed for believing this myth since they rarely see any
evidence to the contrary.
A recent survey of Quebecers on palliative care showed that
despite having the right to access palliative care enshrined
in Quebec law since 2015, less than half believed that
palliative care could improve quality of life (48%), and
79% believed that palliative care is “comfort care provided
to dying patients”. These numbers show the deep
disconnect between the heart of palliative care – providing
a better quality of life – and its public image, even in a
province where it is a legal right.
Regardless of where they live, whether they have a specific
legal right to palliative care or not, people in Canada
deserve to know what palliative care can provide for them,
if or when they need it, so they can make informed
decisions about their health care.
Training Health Care Professionals
While it’s important for caregivers and patients to
understand palliative care, we can’t expect them to know
when a palliative approach to care is clinically appropriate.
As we educate the public, it’s important to also have well-
trained healthcare workers across all disciplines who
understand the palliative approach to care. Healthcare
professionals can’t be expected to provide or educate their
patients on this approach if they don’t have the training to
do so.
Every healthcare worker will care for someone with a life-
limiting illness at some point in their career. Yet, training on
basic competencies in palliative care isn’t consistently
integrated into the core curriculum of all healthcare
professions. In fact, some medical schools dedicate less
than 10 hours of instruction to palliative care. It’s hardly
surprising then that only 40% of family physicians in
Canada feel prepared to care for patients with palliative
care needs. We can’t afford to continue to under-prepare our
healthcare workers on this issue. Medical professionals
must know how to integrate the palliative approach into
their practices so they can provide the best care for their
seriously ill patients.
Moving forward
This issue impacts all of us – none of us are getting out of
this life alive. We need to hold ourselves accountable to
have these challenging conversations and make changes so
we can better live and die across Canada. Governments
need to come to the table to increase awareness of palliative
care, educate healthcare workers, invest in capacity-
building, and incentivize healthcare institutions to provide
timely and comprehensive palliative care to every patient
with a life-limiting illness. Palliative care is not a luxury,
and our healthcare systems cannot hope to survive the
demographic timebomb of an aging population without it.

X-rays
The year 1895 marked a pivotal moment in medical history
with Wilhelm Conrad Roentgen's discovery of X-rays. This
breakthrough enabled the visualization of internal structures
within the human body without invasive procedures. The
ability to see bones, foreign objects, and even certain soft
tissues was a revolutionary leap forward. However, the
early X-ray technology had limitations regarding image
quality and potential radiation risks, which paved the way
for refinements and further innovations.
Radiography and Fluoroscopy
The early 20th century witnessed the refinement of X-ray
technology, leading to the development of radiography and
fluoroscopy. Radiography, or the capturing of static images,
became a cornerstone for diagnosing fractures, assessing
lung conditions, and identifying foreign bodies. Conversely,
fluoroscopy allowed real-time visualization of moving
structures, enabling procedures such as barium studies and
angiography. These innovations transformed medical
practice by offering non-invasive insights into the body's
internal dynamics.

Computed Tomography (CT)
The 1970s saw the advent of computed tomography (CT), a
groundbreaking development that introduced cross-
sectional imaging. CT scans revolutionized medical
imaging by generating detailed three-dimensional images
that unveiled previously hidden anatomical structures. This
capability was precious for diagnosing complex conditions
like tumors, internal bleeding, and neurological disorders.
CT scans provided unprecedented clarity and were pivotal
in guiding surgical interventions.
Magnetic Resonance Imaging (MRI)
In the 1980s, magnetic resonance imaging (MRI) emerged
as a transformative addition to the medical imaging toolkit.
Unlike X-rays or CT scans, MRI harnesses strong magnetic
fields and radio waves to create detailed images of soft
tissues, such as the brain, muscles, and organs. MRI's
ability to differentiate between various soft tissues with
remarkable clarity opened up new avenues for diagnosing
previously challenging conditions. This technology proved
indispensable in neurology, oncology, and musculoskeletal
imaging.
Ultrasound
Ultrasound imaging, or sonography, introduced a non-
invasive and radiation-free method for visualizing internal

Nuclear Medicine and Positron Emission Tomography
(PET)
Advancements in medical imaging were not limited to
anatomical visualization; functional insights also became
crucial. Nuclear medicine techniques, such as single-photon
emission computed tomography (SPECT) and positron
emission tomography (PET), utilized radioactive tracers to
monitor metabolic and physiological processes. PET scans,
in particular, enabled the visualization of disease activity at

staging, brain studies, and cardiovascular assessments.
Artificial Intelligence (AI) in Medical Imaging
AI algorithms powered by machine learning have
demonstrated remarkable capabilities in analyzing medical
images, identifying patterns, and assisting radiologists in
making accurate diagnoses. These algorithms can sift
through vast datasets to identify subtle anomalies that might
elude human eyes. AI's role extends beyond diagnostics; it
aids in treatment planning, predicting disease progression,
and enhancing image quality.
Telemedicine and Remote Imaging
In recent years, the evolution of medical imaging solutions

to the rise of telemedicine and remote imaging. Emergence

Back in 2020, Google DeepMind AI unit published
a study that claimed that artificial intelligence can
detect breast cancer with greater accuracy, fewer
false positives, and fewer false negatives than experts.
Three years later, in 2023, the innovations in imaging AI
technologies that promise endless possibilities in supporting
physicians look less like something out of sci-fi, and more
like everyday reality.
In this article, we will explore AI's advancements in the
diagnostics field, take a glimpse at the ongoing projects
across Europe, and peer into the future of AI governance as
developers, regulators, and policymakers chart the way for
responsible and ethical utilisation of this technology.
Diagnostic Renaissance: Exploring the Full Potential of
AI
Across Europe, several noteworthy AI healthcare projects
are currently underway. In 2020, The Danish Centre for
Clinical Artificial Intelligence (CAI-X) and the
Department of Radiology at Odense University Hospital
started their AI project called MAGIC - AI for breast cancer
diagnostics. Today, the project is steadily reaching its end
date of December 2023. MAGIC aims to improve patient
outcomes by assessing 2 major points: the potential of AI in
detecting clinically significant cancers, and the impact of
deep learning-based workflow for cancer screening on
Danish clinical practice.
Within CAI-X, it's not the only project based on artificial
intelligence. Other AI projects by CAI-X include PEACE
(improved prediction of dysplasia in the cervix with AI),
HjerteRo (identifying individuals with ischaemic heart
disease), AICE (AI supported picture analysis in large
bowel camera capsule endoscopy), and more.
Last year, Humanitas' Cancer Centre and AI Centre
announced the SYNTHEMA project that won a €7 million
grant from the European Commission to develop new data
analysis systems within the field of haematological
diseases, fighting the critical issues of data scarcity and
fragmentation. The project focuses on rare haematological
diseases such as sickle cell anaemia and acute myeloid
leukaemia and began in early 2023.
With the ever-increasing amount of AI projects currently in
work, the safety, efficacy, and regulatory approval of AI
diagnostics stand under close scrutiny. As we progress
through research and development in AI technologies, it is
essential to strike a balance between leveraging its benefits
and addressing the associated challenges.
Balancing Benefits and Challenges in Healthcare
Diagnostics
As we delve deeper into the integration of AI in healthcare,
it becomes apparent that certain challenges must be
carefully addressed.
One such challenge lies in the lack of transparency
surrounding AI algorithms, making it difficult to fully
comprehend how these systems arrive at their conclusions.
Given the potential consequences at stake, accountability
becomes a crucial factor, especially in the medical field
where errors can have significant implications.
AI-based systems also raise concerns regarding data
security and privacy. Patient data, comprising medical
records, personal identification, and payment information,
is subject to strict regulations such as GDPR and HIPAA.
AI systems use a lot of data, which raises the risk of data
leaks.

These challenges, along with others, necessitate a
meticulous and thoughtful approach when implementing
AI. The absence of standardised guidelines for the use of AI
and ML in healthcare has become a growing concern.
Since April 2021, the European Commission has been
working on the first EU regulatory framework for AI.
Bringing this initiative one step closer to reality, Members
of the European Parliament approved their negotiating
stance on the AI Act — the first law on AI by a major
regulator anywhere — on June 14, 2023. With this
milestone achieved, discussions will now commence with
EU countries in the Council, and the results remain to be
seen.
The ultimate goal is to secure an agreement by the end of
this year, as stakeholders seek to strike a balance between
fostering innovation and ensuring ethical and responsible
use of AI. As the talks progress, the final shape and scope of
the EU's AI regulatory framework will become clearer,
potentially shaping the future of AI governance not only
within the European Union but also serving as a global
reference point.
In this era of unprecedented technological advancements,
the responsible integration of AI in healthcare has the
power to transform lives. The trajectory we choose today
will shape the path to a better future for healthcare
tomorrow.

What was your inspiration behind venturing into
Radiography?
My source of inspiration recognised the problems that exist
within medical imaging and how they impact health
outcomes globally. There is a huge backlog and bottleneck
as pictures are continuously taken, but there are very few
people who can read and report. Even when the scan is
done, the report doesn't come for a long time, which causes
a delay in treatment. By background, I am a clinician, and
so I am keenly aware of the importance and value of
diagnostics and early diagnosis especially. This is a global
problem, not just a UK problem, and if we want to improve
the quality of care across the globe, it is hugely important
that we diagnose diseases as early as possible. It was my
recognition of this problem and my desire to provide a
solution that could accelerate the diagnostics and diagnosis
process and therefore provide better care to the public
which acted as my inspiration to venture into the medical
imaging niche.
Please tell us about your journey, highlighting your
contribution to Vital Radiology Service's success.
Although I am qualified as a GP, I have also moved to
technology. Hence my profile is a combination of
healthcare and IT. I've now been working in teleradiology
and diagnostics for over 20 years. When I began working in
teleradiology, I saw that there was a lot of inefficiency in
the system, and I could see that this was due to technology
not being used optimally. I started on a small scale and have
increasingly worked towards a global view of the market
and its problems, and then began to think about a solution
that could influence global issues. I started Vital Radiology
and got together a very good team in the UK and India, and

Please elaborate on the technological innovation that the

I strongly believe that healthcare and technology work hand
in hand, and that organisations that are at the forefront of

leading the healthcare space going forward. This is the key
focus that we have at Vital Radiology.
Through our innovative RadiCube technology, we provide
a fully optimised solution to ensure every step of the
reporting process happens seamlessly. RadiCube displays
the status of all studies sent i.e., the number sent, the
number reported, urgent findings, etc. Ultimately, this helps
to reduce the time administration staff need to spend on
chasing up this type of information. By utilising a fully
automated system we reduce the need for human
intervention across the reporting pathway, an approach
which helps to reduce the clinical risks involved in
radiology reports – providing more accurate and timely
results for patients.
In addition to this, our innovative RadiCube technology
integrates directly with NHS systems, providing the
reporting radiologist with secure access to the patient's prior
images. With the full content of the patient's history
available, Vital supports more informed clinical decisions
and the creation of higher-quality reports.
What challenges does the company face when delivering
services? How does it turn them into opportunities for
growth?
We are experiencing a unique situation where the
challenges radiology services are currently facing are also
those which are providing a significant opportunity for
growth. According to the most recent census released by
the Royal College of Radiologists, the UK is currently short
of 1,669 clinical radiologists, with 200 vacancies being
unfilled for over a year. Though this problem is not unique
to the UK. Across the globe, we are experiencing a growing
shortage of radiologists and unless urgent action is taken it
is problem which is projected to worsen. Ultimately, this
means slower results for patients and further delays in
starting treatment.
The technological innovation Vital incorporates across its
platform means we can help to address some of these
problems. For example, we have extended our services to
provide full out-of-hours emergency reporting offered
through the Moonlight service (runs from 20:00 to 08:00 7
days a week). In under a 30-minute time frame, we can
have a consultant radiologist available and ready to be a
part of the service offering providing reports within 30
minutes for stroke and trauma and 60 minutes for all other
examination types. When a scan is received, Vital's
advanced technology auto-filter directs scans to the most
appropriate radiologist with training and expertise in that
area. This step ensures that high-quality and efficient
reports are provided to patients, equipping them with the
answers they need to move along the care pathway.
Apart from your journey, how do you envision further
strengthening Vital Radiology's stronghold in 2023 and
beyond?
We have the intention of expanding geographically beyond
the 7 countries that we currently cater to. We plan to expand
to new continents such as Africa, Europe, and Asia. We also
have the intention of developing different types of services,
for example, specialist services, such as lung and heart
screening programs, specialist cancer reporting services,
and nuclear medicine. This will involve bringing in lots of
new specialist radiologists, and we are building a team to
meet these needs.

We are proud that Vital's high-quality service offering has
led to us being accredited and recognised by multiple
agencies. Currently, we are CQC regulated, approved by the
Information Commissioner's Office and ISO certified. As a

At present, how does that serve your interests and the role that you play at Marigold Hospital? At Nnamdi Azikiwe University Teaching Hospital (NAUTH), I was exposed to a range of modalities such as Computed Tomography, Fluoroscopy. Although NAUTH was not my first clinical experience with such modalities, it was an eye-opener. There are quite a number of rare radiology procedures, and sometimes we only have theoretical knowledge. But at NAUTH, I witnessed and gained first-hand experience. In addition, my journey at Foremost Radiology Consultants Ltd began in May 2020. Training with different senior radiographers coming from diverse knowledge backgrounds was exciting. There, my experience ranged from Magnetic Resonance Imaging to X-rays. My journey at FRCL ended in June 2021, and I became a better radiographer while also creating a professional network. Looking back, my experiences at FRCL and NAUTH have equipped me for my role at Marigold. I was trained in Magnetic Resonance Imaging, Computed Tomography, Ultrasound, Fluoroscopy, Mammography, Special Investigations, and X-ray at both places. Although my experience is not limited to the mentioned modalities, these experiences have proven to be relevant at Marigold as the Radiology department is similar in most establishments. When it comes to the field of Industrial Radiography, the area of focus should be on creating awareness and making training accessible. Several studies carried out within Industrial Radiography have demonstrated a poor level of awareness. What I propose could be a solution to this challenge is digitalizing the training for Industrial Radiography. This solves the problem of awareness amongst radiography students and would gradually extinguish the notion that Industrial Radiography is an entirely different field. While serving for Diagnostic Professional Imaging Center, what challenges did you face? How did you turn them into opportunities for growth? Diagnostic Professionals Imaging Center was a wonderful center, but no establishment is without its shortcomings. My only challenge while working there was prolonged downtime of the Computed radiography system. This brought about a temporary revert to Manual (dark room) processing. The exposed radiographic film is processed by hand through a series of timed steps in manual processing. For these reasons, many radiographers do not like the manual processing method. On the other hand, I decided to use this as an opportunity to experience working with the manual processing system. An encounter with both extremes of the profession (manual and digital) made me appreciate the technological advancements in radiography and how far we've come. How do you envision further strengthening Marigold Hospital's stronghold in 2023 and beyond? I envision strengthening Marigold Hospital's stronghold by increasing the value of my input in delivering quality health service and employing extreme creativity when working. I see growth as a necessity. Thus, I would continue passionately in professional development by acquiring new skills and global knowledge through attending conferences, updating courses, and webinars, and learning globally. Our healthcare systems must be equipped to not only strive for health but also to strive for quality of life at every stage of life and illness. We can’t deny patients the opportunity to experience joy, feel peace, bring meaning to their last chapter, and importantly live while they are dying. Ultimately, this is what palliative care provides to those who are experiencing health-related suffering – whether they are dying or not, whether they are still pursuing curative treatment or not – though too few in Canada receive that care. Palliative care could benefit 89% of those who die in Canada. However, less than 6 in 10 Canadians who died in 2021-2022 received care from the discipline that specializes, in part, in caring for the dying. The gap in access is significant and bridging it will require a concerted effort across health professions, all levels of government and communities. We can’t afford to not invest the effort. The Case for Investing in Palliative Care Access to palliative care in Canada has increased by 6% over 5 years (52% in 2016- 17 to 58% in 2021-22). While this is moving in the right direction, we can’t forget that Canada has a rapidly aging population. By 2030, there will be 2.2 million more seniors in Canada than there were in 2022. Since seniors, as a population group, have higher palliative care needs, a 6% growth in access to palliative care over the last 5 years is a drop in the proverbial bucket. This is a demographic timebomb heading straight for our healthcare systems. These systems will inevitably implode under this exponential growth in the need for health care for Canadian seniors, including palliative care unless significant changes are made. Defusing the Demographic Timebomb by Investing in Palliative Care Laurel Gillespie is the CEO of the Canadian Hospice Palliative Care Association (CHPCA). She holds an MBA (Master of Business Administration) and is a Certified Health Executive (CHE).   • 26. The benefits of investing in increased access to palliative care are huge. This includes cost efficiencies by preventing patients from being hospitalized and occupying expensive acute care beds that aren’t appropriate for their needs. There are thousands of patients in acute care beds waiting for availability in alternate level care (ALC) every day, causing a significant burden on our health care systems. Many provinces are struggling to address the issue, including Ontario, where nearly 40% of those ALC patient days were for patients at end-of-life. There are thousands of days when high-cost acute care beds were occupied by a patient who could have been better served in a palliative care setting if only those beds were available. Palliative care is more cost-efficient, requires fewer resources overall, can be provided in any setting – including at home where most Canadians want to die, reports higher satisfaction with care from patients and caregivers, and provides caregiver and grief support. Which begs the question: Why aren’t we already doing it for everyone? Public Awareness Despite decades of awareness building, most Canadians remain unaware of palliative care, or mistakenly believe that palliative care is only relevant in the last few weeks or days of life. This myth is perpetuated in part because palliative care is often only accessible to people in Canada in those last weeks of life. We don’t have the workforce or resources to provide this care earlier in the illness progression in a consistent way. The public can hardly be blamed for believing this myth since they rarely see any evidence to the contrary. A recent survey of Quebecers on palliative care showed that despite having the right to access palliative care enshrined in Quebec law since 2015, less than half believed that palliative care could improve quality of life (48%), and 79% believed that palliative care is “comfort care provided to dying patients”. These numbers show the deep disconnect between the heart of palliative care – providing a better quality of life – and its public image, even in a province where it is a legal right. Regardless of where they live, whether they have a specific legal right to palliative care or not, people in Canada deserve to know what palliative care can provide for them, if or when they need it, so they can make informed decisions about their health care. Training Health Care Professionals While it’s important for caregivers and patients to understand palliative care, we can’t expect them to know when a palliative approach to care is clinically appropriate. As we educate the public, it’s important to also have well- trained healthcare workers across all disciplines who understand the palliative approach to care. Healthcare professionals can’t be expected to provide or educate their patients on this approach if they don’t have the training to do so. Every healthcare worker will care for someone with a life- limiting illness at some point in their career. Yet, training on basic competencies in palliative care isn’t consistently integrated into the core curriculum of all healthcare professions. In fact, some medical schools dedicate less than 10 hours of instruction to palliative care. It’s hardly surprising then that only 40% of family physicians in Canada feel prepared to care for patients with palliative care needs. We can’t afford to continue to under-prepare our healthcare workers on this issue. Medical professionals must know how to integrate the palliative approach into their practices so they can provide the best care for their seriously ill patients. Moving forward This issue impacts all of us – none of us are getting out of this life alive. We need to hold ourselves accountable to have these challenging conversations and make changes so we can better live and die across Canada. Governments need to come to the table to increase awareness of palliative care, educate healthcare workers, invest in capacity- building, and incentivize healthcare institutions to provide timely and comprehensive palliative care to every patient with a life-limiting illness. Three years later, in 2023, the innovations in imaging AI technologies that promise endless possibilities in supporting physicians look less like something out of sci-fi, and more like everyday reality. In this article, we will explore AI's advancements in the diagnostics field, take a glimpse at the ongoing projects across Europe, and peer into the future of AI governance as developers, regulators, and policymakers chart the way for responsible and ethical utilisation of this technology. Diagnostic Renaissance: Exploring the Full Potential of AI Across Europe, several noteworthy AI healthcare projects are currently underway. In 2020, The Danish Centre for Clinical Artificial Intelligence (CAI-X) and the Department of Radiology at Odense University Hospital started their AI project called MAGIC - AI for breast cancer diagnostics. Today, the project is steadily reaching its end date of December 2023. MAGIC aims to improve patient outcomes by assessing 2 major points: the potential of AI in detecting clinically significant cancers, and the impact of deep learning-based workflow for cancer screening on Danish clinical practice. Within CAI-X, it's not the only project based on artificial intelligence. Other AI projects by CAI-X include PEACE (improved prediction of dysplasia in the cervix with AI), HjerteRo (identifying individuals with ischaemic heart disease), AICE (AI supported picture analysis in large bowel camera capsule endoscopy), and more. Last year, Humanitas' Cancer Centre and AI Centre announced the SYNTHEMA project that won a €7 million grant from the European Commission to develop new data analysis systems within the field of haematological diseases, fighting the critical issues of data scarcity and fragmentation. The project focuses on rare haematological diseases such as sickle cell anaemia and acute myeloid leukaemia and began in early 2023. With the ever-increasing amount of AI projects currently in work, the safety, efficacy, and regulatory approval of AI diagnostics stand under close scrutiny. As we progress through research and development in AI technologies, it is essential to strike a balance between leveraging its benefits and addressing the associated challenges. Balancing Benefits and Challenges in Healthcare Diagnostics As we delve deeper into the integration of AI in healthcare, it becomes apparent that certain challenges must be carefully addressed. One such challenge lies in the lack of transparency surrounding AI algorithms, making it difficult to fully comprehend how these systems arrive at their conclusions. Given the potential consequences at stake, accountability becomes a crucial factor, especially in the medical field where errors can have significant implications. AI-based systems also raise concerns regarding data security and privacy. It was my recognition of this problem and my desire to provide a solution that could accelerate the diagnostics and diagnosis process and therefore provide better care to the public which acted as my inspiration to venture into the medical imaging niche. Please tell us about your journey, highlighting your contribution to Vital Radiology Service's success. Although I am qualified as a GP, I have also moved to technology. Hence my profile is a combination of healthcare and IT. I've now been working in teleradiology and diagnostics for over 20 years. When I began working in teleradiology, I saw that there was a lot of inefficiency in the system, and I could see that this was due to technology not being used optimally. I started on a small scale and have increasingly worked towards a global view of the market and its problems, and then began to think about a solution that could influence global issues. What challenges does the company face when delivering services? How does it turn them into opportunities for growth? We are experiencing a unique situation where the challenges radiology services are currently facing are also those which are providing a significant opportunity for growth. According to the most recent census released by the Royal College of Radiologists, the UK is currently short of 1,669 clinical radiologists, with 200 vacancies being unfilled for over a year. Though this problem is not unique to the UK. Across the globe, we are experiencing a growing shortage of radiologists and unless urgent action is taken it is problem which is projected to worsen. Ultimately, this means slower results for patients and further delays in starting treatment. The technological innovation Vital incorporates across its platform means we can help to address some of these problems. For example, we have extended our services to provide full out-of-hours emergency reporting offered through the Moonlight service (runs from 20:00 to 08:00 7 days a week). In under a 30-minute time frame, we can have a consultant radiologist available and ready to be a part of the service offering providing reports within 30 minutes for stroke and trauma and 60 minutes for all other examination types. When a scan is received, Vital's advanced technology auto-filter directs scans to the most appropriate radiologist with training and expertise in that area. This step ensures that high-quality and efficient reports are provided to patients, equipping them with the answers they need to move along the care pathway. Apart from your journey, how do you envision further strengthening Vital Radiology's stronghold in 2023 and beyond? We have the intention of expanding geographically beyond the 7 countries that we currently cater to. We plan to expand to new continents such as Africa, Europe, and Asia. We also have the intention of developing different types of services, for example, specialist services, such as lung and heart screening programs, specialist cancer reporting services, and nuclear medicine. This will involve bringing in lots of new specialist radiologists, and we are building a team to meet these needs.
