In the United States (U.S.), chronic diseases are the leading cause of death and disability. Chronic disease activities are sparsely delivered in the U.S.
despite the fact that 7 out of 10 deaths are caused by chronic diseases, every year; and 75% of our health care costs 75% of health care costs are
attributable to preventable diseases.1,2 One main reason for this imbalance is that funding for chronic disease prevention activities is limited. In fact,
only 3% of the U.S. total health expenditure is devoted to public health.3 The U.S. Centers for Disease Control and Prevention, the leading federal

The first way to share the information is to simply begin putting it into practice consistently. By modeling these powerful impacts and showing

The project will be about sunflowers. Home Depot will be the main selection for buying the seeds. I will buy everything I need on 9/23/17. The reason
why I will be using sunflowers is because they are inexpensive and it will only take 2–3 weeks to grow. My plan is to have everything set up by the

I will try to see if anything will be happening while it grows, or if anything different will happen with this plant then with others. I plan to see how
long its life span is and with the proper care the plant may exceed what most people estimated it may live. Overall, there's only so much that
happens for this type of plant. Possibly I will learn something that I didn't expect to find out from this plant on this project. The health benefits it has
is that the seeds are very high in vitamin A and E. It also has a natural source of enzymes. A person is able to easily absorb the nutritional benefits
that the seeds gives off. If someone were to have symptoms of asthma the plant would be able to reduce the symptoms of it. There was plenty more
medical uses for the plant but, that's just some of what it does.
The problems I may face is bugs or my dog trying to dig it up and destroy my project. I will most likely have the plant inside my house to try and
prevent something bad from happening to it. I will try to avoid as many problems as I can. I will have to worry about because if I keep it inside by a

Class Improvement Plan
We focus on two goals on our improvement plan which are increasing the graduation rate and student achievement.
I was surprised when I learned the main reason which put us in the 5% of the lowest achieving schools in the state. The reason was mainly our
graduation rate in the state report card which reflects the results from two years back. As it is shown in the graphic chart below there is a dramatic
drop in the graduation rate in 2013. According to my simple calculations, the graduation rate should have been more than 46%, we had 36 seniors and

According to my observations, one of the main problems is collecting and charting the data. Our school has a very useful database system. We are
able to see the scores of EOC, NWEA, Explore, PLAN, and ACT practice tests. We are also able to pull out the reports including item analysis,
performance index score, individual reports, proficiency scores, student error report, and growth target report. I heard several teachers saying that they
don't know how to access those reports or they don't feel comfortable with what to do with the data. Training about using the data would help to solve
this problem. Teachers who are using the data effectively can help to other teachers. The ways about how to train teachers should be determined in the

After much needed thought, I decided to develop the c.i.p. around my nonverbal behaviors in hopes to identify the underlining cause(s). The
following sections detail my personal experience in the process and results of implementing a c.i.p. over the course of a semester starting with the
