Role Of A Care Coordinator At The Community Care Access...
Healthcare workplaces are very challenging in these ever changing times. Many factors contribute to
the ability to affect change, and if not utilized precisely, will act as obstacles. The home care
community is experiencing many concerning issues, and restructuring has begun to unfold. Poor
communication, unwillingness to share information, and the inability to manage the effectively have
created a work place that does not promote interactions between front line staff and management.
Instead of opening the lines of communication and encouraging feedback from the care
coorindators, walls have grown where bridges are needed. Information sharing about the importance
of home care coordinators and the valuable role that Community Care Access Centres (CCAC)
provide to their patients is imperative to develop a stronger home care system. Can we rebuild a
home care system that addresses the concerns, and focuses on the patients that we provide care to?
The Role of a Care Coordinator: My work environment is case management at the Community Care
Access Centre of Erie St. Clair. The work involves assessment of patients to determine if they are
eligible for home care services. These assessments can be face to face or over the telephone, and the
patient population ranges from infants to the elderly. The services that are contracted from the

Research Design and Methodology
4.1Introduction
The purpose of this study is to find out what are the causes of the high compliance rate in property
taxes in the municipal city of Portmore, Jamaica. Taxes are of essential importance to small
developing countries as they do not have much needed resources. As further developments takes
place in the municipal city property values will increase, hence the amount of property tax collected
by the Portmore municipal council will also increase. This increase however, is not a guarantee that
the tax collection will be sufficient in the maintenance of the community due to the rising cost of
goods and services. Hence continuous voluntary compliance is essential and important as it most
efficient for both the tax payer and the tax authorities.
This section gives the hypothesis or research perspectives, the design of the research and the
methods that were used to gather the data.

Isolation And Social Isolation
Canada's aging population is growing. One of the number one emerging issue facing seniors is
social isolation. Statistics Canada's 2008/09 Canadian Community Health Survey found that 19% of
individuals aged 65 or over felt a lack of companionship, left out, or isolated from others (National
Seniors Council, 2014). Social isolation is not the same as loneliness. It is a form of emotion that
makes people depression. Social isolation impacts on seniors' quality of life, mental and physical
health. To reduce the social isolation is the way seniors are connected socially. For example, there
are exercise programs and community–engaged art programs for the elderly in the community. This
program is effective for seniors' physical and mental health. Although such programs have beneficial
effects on seniors' health, some studies argue that there are barriers to reducing the social isolation of
older people in such programs. Community programs and community participation programs benefit
from the seniors' physical, emotional and social well–being. The purpose of the Community exercise
program is a socialization–based exercise program to improve the seniors' health. Community arts
programs aim to promote optimal health for seniors. However, it is difficult to reduce the social

Social environments influence the health and well–being of a population by incorporating norms,
values, and broader community characteristics. Healthy social environments include freedom from
discrimination and prejudice (Potter and Perry, 2014 ). In particular, populations such as Aboriginal

they turned it into a coffee shop instead, and then leased it out. There is a gym, Sports hall, library,

Setting
The population data used for the modelling was obtained from a Community Health Centre in
Kensington, an inner–suburb of Melbourne which provides health services predominantly to migrant
communities. Studies of cardiovascular disease in migrants in Australia have included mostly
participants of European and Mediterranean origin and shown that they have lower rates of
cardiovascular diseases,17 18 compared to the Australian–born populations. The small amount of
available data among migrants in Australia from Africa and the Middle East suggest that these
newly arrived migrants have a high prevalence of cardiovascular disease risk factors.19
Data collection
Electronic medical records of the centre were accessed. Data was collected pertaining to age, sex,
migrant status, country of origin, most recent total and HDL cholesterol levels, status of diabetes
and smoking, vitamin D level and blood pressure of individuals who were vitamin D deficient and
free from CVD while maintaining the patient anonymity.
Inclusion and exclusion criteria
Adult (19–99 years) migrants (African origin) who visited the medical centre at least once during
the period from 1st January 2010 to 31st December 2012 (three years) were included in the study.
The information in the last visit was used from those who visited more than once during the
specified period. Data cleaning was undertaken to get the most appropriate sample for the purpose.

well–being of new mothers by implementing a new mothers walking group in the Hillside–Quadra
area.

amount of mothers walking their infants down the sidewalks and roads of the community.
Unexpectedly, these women were mainly walking in the morning or evening, by themselves and

After contacting several organisations in the Donegal area, from the Active Retirement Association,
the Family Resource Centre, the local public health nurse, and the neighbouring community centre
that caters for all age groups, I became aware of a group of over 65 years old adults who has
experienced some level of loneliness. The local public health nurse outlined the need to address the
issue of loneliness in older people in order to improve their overall physical and mental wellbeing.
After consulting a committee member in my local community hall, I became aware of how few
social activities are organised for this age group. The committee member outlined the need for a
physical activity programme that would reach out to older people, as there is currently no
programme in place. The local community hall provides an excellent service for the youth but little
has been done with the retired members of the locality. This proves disappointing as positive ageing
is in everyone's best interest.

New Nashville
Our city is called New Nashville, with a population of about 13,000. Our city is loosely based on
Nashville, TN; so our climate is hot and humid, as well as the geography being low and flat. We also
have a river that goes through the the upper part of town. With the geography being so perfect for
parks, our economy is largely spent on parks for all ages, community centers, and other buildings
that usually go into residential areas. Our city has a very peculiar set up; where our commercial and
residential are together, but our industrial buildings are as far away as possible. We decided to set it
up this way because we wanted to ensure that no pollution would harm our residents. Our residential
and commercial areas consist of houses, shops, religious centers, schools, libraries, parks, and
entertainment places. Meanwhile, the industrial area is full of many successful companies, as well as
solar power buildings, water towers, and low–pollution garbage and sewage buildings.
The infrastructure in Nashville isn't what one would call the most unique at first; because we have
the normal facilities that most cities have. However, when you look closer, you'll discover that we
have spent much money on police, hospitals, and fire stations; to ensure the safety of our residents.
After, we decided to add as many schools and ways of transportation as we could; to make sure our
city's residents are educated and can get around. Then we added the entertainment centers, like
parks, malls, etc. The way we set up our infrastructure was very important for our city, as we tried to
make sure that the number one priority was safety. This took a large chunk out of our budget; but it
greatly reduced our crime, fire, and death rate; and brought up the schooling rates; we had to make
sure that our residents were okay. In the end, taxes was low, people's needs were filled, and
everyone was very happy with New Nashville. However, not all people's needs were filled. The
elderly population still goes through lots of trouble everyday.
Our city will incorporate a variety of futuristic events. The first of which is a virtual reality display
that is meant for elder people. Although this seems a frivolous detail, it would in fact

Many factors contribute to the ability to affect change, and if not utilized precisely, will act as obstacles. The home care community is experiencing many concerning issues, and restructuring has begun to unfold. Poor communication, unwillingness to share information, and the inability to manage the effectively have created a work place that does not promote interactions between front line staff and management. Instead of opening the lines of communication and encouraging feedback from the care coorindators, walls have grown where bridges are needed. Information sharing about the importance of home care coordinators and the valuable role that Community Care Access Centres (CCAC) provide to their patients is imperative to develop a stronger home care system. The infrastructure in Nashville isn't what one would call the most unique at first; because we have the normal facilities that most cities have. However, when you look closer, you'll discover that we have spent much money on police, hospitals, and fire stations; to ensure the safety of our residents. After, we decided to add as many schools and ways of transportation as we could; to make sure our city's residents are educated and can get around. Then we added the entertainment centers, like parks, malls, etc. The way we set up our infrastructure was very important for our city, as we tried to make sure that the number one priority was safety. This took a large chunk out of our budget; but it greatly reduced our crime, fire, and death rate; and brought up the schooling rates; we had to make sure that our residents were okay. In the end, taxes was low, people's needs were filled, and everyone was very happy with New Nashville. However, not all people's needs were filled.
