–What is the title and job description for the social worker? Mr. David Benjamin is a Clinical Social
Worker/Therapist with his own private practice and also works for the Fairfax county government
community service board of mental health. His job description has such a wide range of roles. He
works with children, adolescents, and adults. He helps teens with issues involving school, peer
relationships, depression, anxiety, divorce, sexual trauma, and bullying as well. He also does case
management, and his main focus and children ages 7–19. –Describe the agency or organization with
whom he/she works? What type of agency is it? What is the mission of the agency? Mr. Benjamin
has his own private practice but also works for Fairfax county government in the community service

Depending on the behavioral health center the social workers may or may not provide case
management because the main purpose of a behavioral health center is to assist the client with their
behavior. Even though there are dozens of different settings that a social worker can choose to work
in, malpractice of supervisor is a risk in each one. Whenever the DSM is involved, social workers
should be mindful of the diagnosis. A client's diagnosis can have a result of a claim being filed for
malpractice. For example, a client could be a psychopath and decide to lie about the social worker in
order to seek revenge because of something the social worker rightfully did. It doesn't matter if the

Nurse Led Case Management Program for Older Adults with Co–Morbidities
In a study conducted by Chow and Wong (2014), they analyzed a case management team led by
nurses and the effectiveness of the care coordination regarding older adults whom are diagnosed
with co–morbidities. Utilizing a randomized controlled trial during 2010 to 2012, the researchers
analyzed older adults who suffered from at least two chronic illnesses (Chow & Wong, 2014). Those
chosen to be analyzed were then further divided into three groups, including one control groups and
two study groups (Chow & Wong, 2014). Baseline data was gathered and collected at four weeks
and then again at twelve weeks from the two hundred and eighty one patients participating (Chow &
Wong, 2014). The two study groups in which interventions in which nurse led case management
occurred, resulted in lower readmission rates within an eighty four day frame after discharge from
hospitalization. Additionally these participants rated their health and self– sufficiency at a higher
level than the control group (Chow & Wong, 2014). Conclusions gathered from research indicates
that care coordination after discharge led by nurses whom empower patients are effective in
improving clinical outcomes for older patients with co–morbidities (Chow & Wong, 2014). This
research conducted is crucial to my selected topic as is provides evidence that coordination of care
after discharge can improve outcomes.

Problem: The intervention plan aims at Social Adult Day center (SADC), under the network of
Hamilton Madison House (HMH). This program was designed to assist seniors who have physical
movement problems or cognitive impairments in managing emotional and physical challenges.
However, the problem is that the organization has inadequate professional social workers to provide
qualified services. To be specific, while in general around 40 seniors come to the organization every
day, only two social workers work in the agency currently–the Director Rita who is over 60 years
old and the Program Coordinator Ping who works part–time. Relevant policies regulate that only
licensed social workers have access to paperwork and certain services such as case management. As
a result, two social workers, particularly Rita, are seriously overwhelmed by significant workloads;
meanwhile, limited number of seniors has opportunities to acquire alternatives for their adversities
from social workers.
Objective: The purpose of this intervention plan is to have more professional social workers
involved in SADC to offer enhanced services for seniors.

My Future Learning Needs For A Joint Health And Social Care
In this analysis of professional practice I shall examine the work I have undertaken in my current
work setting and also evidence the progression in my practice. The names of the service users
discussed has been changed in order to protect identity and to follow confidentiality policy (HCPC
SOCPE: point 2). To conclude I shall identify and discuss my future learning needs.

promotes joint working between health and social care and these principles have been further
embedded in the new Care Act 2014. Cameron et al (2012) argue the Government intention is to
enable joint working between the NHS and local authorities in order to make it easier for
commissioners and providers to adopt partnership arrangements to provide joint local services. The
authors argue that joint working ethos have been further embedded in current policies due to the
rising demand for services from an ageing population joined with the need to reduce public
spending. Moreover, the integration of health and social care services offers further resources to

their friend overdose on drugs. This friend happened to be their only support system. They are
having a hard time coping with this tragedy and attended counseling in hopes of gaining a new

become too stressful for them. As a final point, they wish to contact their family again, but they are

Making An Initial Visit After Receiving A Referral Within...
Summary The aim of this session was to introduce us to the process of making an initial visit after
receiving a referral within the role of a social worker. This was done with a small group exercise
involving role play. We were placed into groups of four and once in our groups we had to divide
ourselves into roles. There were three roles consisting of Service user, Social worker and observer.
We were then taken to one side with people from other groups whom had be given the same role as
ourselves and briefed on what it was we had to do. I was given the role of social worker, therefore i
was told that I had to plan and undertake an initial visit and given the receipt of a referral. The
referral consisted of concerns regarding a young boy, Max aged 2. The referral was made by the
child's nursery. They had concerns about his behavior and presentation, for example, Max is often
very tired, wears the same clothes for a number of consecutive days, is greedy when given food and
aggressive towards his peers. Alongside this, questions regarding his mother, sally, were also raised.
When staff at the nursery had spoken to her she seemed to show littler concern regarding her son's
behavior, she often smells strongly of alcohol and bruises have been seen on her face. It was
therefore my responsibility to take this information from this receipt of referral on board before then

First, trauma has become nationally recognized and appreciated in the last two decades. The nation's
perspective has changed for the better due to 9/11, Hurricane Katrina, and the influx of veterans
returning from Iraq and Afghanistan with Post Traumatic Stress Disorder. Citizens are now
recognizing how trauma impacts individuals, families, and communities (Reardon, 2011).
A second trend in social services include the Mental Health Parity and Addiction Equity Act of
2008. The Act provides that large companies provide equal insurance coverage for mental health
services when compared to medical services. In other words, insurance companies are to provide the
same medical treatment standards to insurers and their families for mental health services. Insurance
companies are no longer able to discriminate against people with mental health issues and are
required to close the disparity gaps between people with mental health concerns and people with
only medical concerns. Hence, families will be able to receive mental health services at an
affordable price. Social workers need to be aware of the "carve" out benefits. "Carve–out
arrangements with managed behavioral health organizations are still allowed, as long as mental
health and substance use disorder benefits are being covered and managed in a manner that is no

had a lot of people giving me suggestions and advice to choose the right profession. My
psychologist convinced me to pursue my master in social work. During my undergrad, I was
interested in psychology, but social work seemed like an easy path to become a therapist. However,
I have learned that social work is not as easy as I thought it would be, and that it is the main key for
many opportunities to succeed and to make the difference.
As a social work student, I have discovered that social work is an overlooked profession. Many
people used to ask me "what can you do with a social work degree?" when the real question is "what
can you not do with a social work degree?" Nowadays, we are surrounded by social workers; they
work in the schools, universities, hospitals, courts, prisons, different community based organizations
and more. People also said that "social work doesn't pay enough". However, what they do not know
is that every day as a social worker is rewarding; knowing that you have made the difference in
someone's life is priceless.

Completing the initial contact with the client showed me both areas in which I need improving and
areas I did well. After speaking with classmates and receiving feedback from my supervisor I was
able to better understand what others thought of my interview. Seemingly, main areas of concern
during the interview were to maintain professionalism, practice self–reflection, show empathy,
conduct the interview in a thorough manner, and avoid any personal biases during the interaction.

1. By participating in this whole CARES program, I have learned a lot about myself. I know that I
want to help people out as a profession, but never truly sure what I was going to major in. Social
work was always a possibility, since I love helping people out. However, after taking part in this

Eventually, we got our break and were able to help out about five other clients.
2. There were so many different skills that were involved when working with our clients. Many of
the skills we used were considered basic ones that people should have. For example, we practiced
our listening skills. Having the ability to listen to someone is like finding gold. It's hard to help the
client out, if the advocate is not being attentive/listening to them.
Another skill that is very useful and that goes hand in hand with listening is being able to be patient
with the client/participants. Two times in the DPSS office, I caught myself wanting to stop the client
from giving me more information. The two clients, we helped were giving us information that didn't
line up with the evidence we had from their case/paperwork.
My partner and I had one long case that went on for at least an hour or longer from start to finish.
This client is a 64 year old African American women. Her name is Regina Armstrong. When we first
approached her, she didn't hesitate to ask us questions about her application. Kasey and I asked
several questions regarding what she needed help with. Regina needed help figuring out why she
wasn't receiving money into her account.
There were several loops that we had to jump through in order to get to the bottom of this issue.

Factors that can affect feelings of satisfaction include quality and level of supervision, income,
location, education, case load, experience, autonomy, organization, and promotion (Arches, 2009;

Care managers work with various populations with extra support in those that are considered

The objective of this composition is to discuss the association between my social work classes and

This is good news for me, as the potential to start my career right out of college is exponentially
higher. On the other hand, it is also saddening to think there are that many families and children who
require that amount of specialized help. While overall employment is expected to increase, the OOH
did say that there could be limited available funding on the federal, state, and local levels. When you
are working in an often times turbulent atmospheric environment like social work, one has to expect
that a straight 9–5er is not going to happen. The OOH states that working evenings, weekends, and
holidays should be expected. Should the need arise, doing house calls when clients are available

During this session four service users came into our lecture to talk openly about their thoughts,
feelings and experiences of the social work profession. The service users spoke honestly and openly
about these experiences, some of them negative but many positive. I found that many of the
underlying feelings of the service users where in regard to social workers as individuals, such as
their approachability, professionalism empathy and the ability to engage with them to meet their
needs. It is with this in mind that I have decided to reflect upon these issues, I will do this from a
personal perspective whilst also laying reference of these personal feelings to social work values

based facility that has more than 800 clients currently receiving a daily regime of methadone; this is
an agonist substance used in the treatment of opiate addiction. My interview today is with Lakisha
Russell, a Licensed Social Worker: she also carries a MHT degree and a LCDC III. Furthermore
acquiring a portion of this education at Sinclair. Lakisha is currently employed by Project Cure, as a
chemical dependency counselor. The hierarchy at PC. Starts with the CEO Virgil, Deputy Director
Denice, Outpatient Supervisor Dr Erving, Lead Counselor Supervisor Rena, CDC Lakisha, with
several other CDC counselors, and also a medical staff. Lakisha explains: that a typical work day
starts at 8am usually with some kind of impromptu staff meeting; which generally sets her back for

Nurse Led Case Management Program for Older Adults with Co–Morbidities In a study conducted by Chow and Wong (2014), they analyzed a case management team led by nurses and the effectiveness of the care coordination regarding older adults whom are diagnosed with co–morbidities. Utilizing a randomized controlled trial during 2010 to 2012, the researchers analyzed older adults who suffered from at least two chronic illnesses (Chow & Wong, 2014). Those chosen to be analyzed were then further divided into three groups, including one control groups and two study groups (Chow & Wong, 2014). Baseline data was gathered and collected at four weeks and then again at twelve weeks from the two hundred and eighty one patients participating (Chow & Wong, 2014). The two study groups in which interventions in which nurse led case management occurred, resulted in lower readmission rates within an eighty four day frame after discharge from hospitalization. Additionally these participants rated their health and self– sufficiency at a higher level than the control group (Chow & Wong, 2014).
