The Waterfront Community Connection
Outline
Initially we want to focus on assisting in the recovery from
mental and behavioral health problems, helping consumers cope
with addictions, find housing, and recover from the impacts of
trauma and violent victimization.
We also want to include the delivery of services to children,
families, transition-age youth and a focus on treating high-risk
individuals and finding supported employment for those ready
to work.
We will eventually dedicate an area to providing integrated
primary and behavioral care, which will allow consumers to
receive all of their care without disruption and with the greatest
possible continuity.

Our mission is to provide behavioral health, residential
services, and primary health care coordination for marginalized
and disenfranchised women, men, youth, and children living in
Jefferson County, many of whom are coping with challenges
including mental illness, addiction, and the aftermath of trauma
and abuse.

lead every aspect of the clinical part of the center as well as
projects that focus on consumer empowerment, trauma recovery,
homelessness, addictions, de-institutionalization, and person
centered care, mental health recovery, transition aged youth,
and justice system involvement.
· Chief Human Resources Officer leads our Human Resources
Department, and recruiting, hiring, and talent development
efforts for staff.
· Chief Administrative Officer will lead the administrative and
administration aspect of The Waterfront Community Connection
Center. This person will oversee the Revenue Cycle
Management, Information Technology, Plant Management, and
the Practice Management Software.
· Chief Compliance Officer will lead the Compliance Program,
Quality Improvement Department, Medical Records Department,
& Consumer Benefits Program.
Job Roles and Responsibilities
Chief Executive Officer:
· Increases management’s effectiveness by recruiting, selecting,
orienting, training, coaching, counseling, and disciplining
managers; communicating values, strategies, and objectives;
assigning accountabilities; planning, monitoring, and appraising
job results
· Creating, communicating, and implementing the organization’s
vision, mission, and overall direction – i.e. leading the
development and implementation of the overall organization’s
strategy.

Behavioral Health Services:
· Assertive Community Treatment:
The Assertive Community Treatment (ACT) teams provide
comprehensive, community-based, psychiatric treatment,
rehabilitation, and support to persons with serious and
persistent mental illness. Act clients often have co-occurring
problems with addiction, homelessness, criminal justice
involvement, physical illness, and violent victimization. ACT
teams embrace the nationally established evidence based
practice model for act and achieve a high level of fidelity to the
model. Teams are composed of a psychiatrist, nurse, licensed
team supervisor, a peer support specialist, and rehabilitation
support staff. Staff may develop sub-specialties in group
services, substance abuse treatment, forensic services, parenting
interventions, and vocational rehabilitation. ACT teams have
small caseloads and work as a tight knit interdisciplinary unit to
create opportunities for growth and recovery.
· Community Support TREATMENT:
We will provide comprehensive and integrated mental health
services for clients who have mental health problems and may
have co-occurring problems of addiction, homelessness, medical
illness, and violent victimization. Integrated clinical services
include diagnostic assessment, community support, care
coordination, therapy, and liaison with psychiatric services.
Community Support Specialists will provide direct care and
coordinate services to help clients achieve their recovery goals.
· Crisis Services:
The Waterfront Community Connection Crisis Service Team is
responsible for providing comprehensive crisis services after 5
pm, on weekends, and holidays to all clients receiving services
at The Waterfront. This allows us to respond to crises as they
happen, ensuring the safety of all our clients. Our main goals
are to prevent crises, stabilize emergent situations, and reduce
use of city emergency services such as 911 or hospital
emergency rooms. The Crisis Service Team currently consists of
trained mental health clinicians, supervised by a licensed
clinical supervisor.
· Day Rehabilitation Services program:
The Day Rehabilitation Services Program (DSVCP) is a skills
training and motivation enhancement program that supports
participants in their efforts to change lifestyle, overcome
obstacles, recover their lives, and experience empowerment. We
believe people with a serious mental illness can recover and are
capable of living independent, satisfying lives. Central to that
process is the development of skills that will empower the
individual to make positive life choices. Program participants
work together in a variety of activities to gain or rebuild
confidence and the skills necessary for living independently.
Participants can expect integrated care with their treatment
teams within and outside of The Waterfront.
· Outpatient Behavioral Health Clinic:
The Outpatient Behavioral Health Clinic (OBHC) is an
interdisciplinary team that serves as the main access point for
adult services, and provides psychotherapy and low -intensity
community support for individuals who are seeking assistance
with addressing life stressors, managing mental health
symptoms more effectively, or healing from trauma.
Recognizing that clients need a diverse spectrum of support, the
OBHC uses a patient-centered approach to treatment planning
and program referral, and lengthens the enrollment and
engagement process to better match individuals with programs,
interventions, and the appropriate level of care.
· Supported Employment:
Supported employment is an evidence-based program
encompassing recovery principles and recognizes that
employment can be an integral part of a person’s recovery.
Employment Specialists develop relationships with community
business owners and hiring managers to help the client become
employed in the job of their choice. Working together,
employment specialists and program participants develop an
individual work plan, create a resume, and practice interviewing
skills. Employment Specialists coordinate with the
participant’s treatment team; provide job coaching, and on-
going support to assist the individual in maintaining
employment.
INTEGRATED CARE SERVICES:
· Health Homes
Health Homes is an integrated model of care addressing an
individual’s behavioral and physical health needs. The health
home team includes nurse care managers, a primary care liaison,
and care coordinators who work together to meet each clients'
physical and mental health needs, as well as psychosocial goals.
The teams work with a individual and their network, including
family members and medical providers, to help clients improve
self-management of chronic illness and promote total wellness.
Comprehensive care management, care coordination,
transitional care, health promotion, individual/family support,
and accessing resources via referrals are the six services that
comprise the Health Home Service Delivery Model.
· PSYCHIATRY CLINIC
The Community Connections Psychiatric Clinic provides access
to Board Certified psychiatrists and nurses. The clinic provides
integrated psychiatric care to all interested Communi ty
Connections clients (adults, children, and youth) and works
closely with their treatment teams. Services provided in the
clinic include assessment, medication management, lab work,
and nursing services. Psychiatrists and nurses working with the
Assertive Community Treatment teams often meet clients in the
community.
HOMELESSNESS & HOUSING:
· The Waterfront…we want to support individuals and families
with establishing and maintaining safe and affordable homes as
a part of their whole health recovery. We want to be the leader
in Jefferson County as an effort to develop and implement
effective Coordinated Assessment and Housing Placement
processes. We want to connect with government partners and
local coalitions of community providers to expand the
availability of housing resources, especially for the city’s most
vulnerable individuals and families. In all of these activities, we
demonstrate a comprehensive understanding and commitment to
the states strategic plan to end homelessness.
We want to offer a comprehensive range of outreach and
residential supportive services to promote housing stability and
prevent homelessness – especially with people experiencing
mental illness, addiction, and trauma.
Our Homeless Services Program will provide outreach to
women and men living outside and “in reach” to those staying
in emergency and transitional shelters. We want to offer
extensive navigation assistance (e.g., obtaining identification,
completing applications, mitigating housing barriers, etc.).
OUTPATIENT ADDICTION SERVICES:
· The Waterfronts Outpatient Addiction Services (OAS) will be
an ARC/DBH approved substance use treatment provider.
Recognizing the complexity of addiction, OAS utilizes
Community Connections' expertise in mental health and trauma
recovery to provide sophisticated clinical services geared
toward empowering clients. Clients will be able to discover the
roots of their substance addiction and how they can attain
sustained recovery to meet their goals. Addiction and recovery
are not just about a substance, but about a complex person with
multiple concerns including the impact of mental health
symptoms, developmental traumas, family relationships,
environmental factors, and other obstacles to recovery. OAS
will incorporate a holistic approach to recovery. We will teach
relapse prevention skills, emphasize lifestyle modifications, and
encourage network and family involvement. OAS will provide
person-centered and curriculum-based treatment and a diverse
staff that brings a wide range of life experience, work
experience, and education to the recovery process.

The Center for Families and Children
Youth Centered Family Focused Community BASED
The Waterfront Community Connection will be pleased to offer
services to children and youth who suffer from mental illness,
substance abuse, or a history of trauma. Our goal is to provide
high quality, clinical mental health services focused on the
strengths of each individual child and family. Family support
and skill building will be offered through individual, group, and
family-based interventions. We will utilize the focus and
creativity of the entire team to develop flexible and targeted
interventions. Problems are recognized in their proper context
and addressed through collaborative partnerships with families
and their formal and informal support networks.

HEART
The Healing, Empowering, Advancing and Recovering,
Together (Heart) program is an innovative 5-year project that
will be funded by SAMSHA to grow their national child
traumatic stress initiative. HEART’S ambitious mission is to
improve early access to integrated high-quality behavioral
health and trauma specific treatment for children, youth, and
their families. The goal of the program is to disrupt the
transmission of intergenerational trauma. Family peers and
clinicians will develop and implement creative strategies to
provide coordinated services that will allow for integrated child
and adult services. HEART will utilize Community-Based
Intervention and Family Team Meetings, with trauma-specific
evidence based practices offerings: Trauma Focused Cognitive
Behavioral Therapy for 1:1 child-specific recovery work,
Strengthening Families Coping Resources for multi-family
group recovery work, and Trauma Recovery and Empowerment
for adult women’s recovery work.
· Peer Empowerment & Advancement
Peer services will play a crucial role in helping to advance
wellness and recovery for clients living in the community. Peers
are uniquely suited to use their own experience and personal
knowledge to engage clients in treatment and recovery.
Additionally, peer staff will help guarantee that our clients'
voices are heard to help inform and shape agency practices and
policies.
Our peers will be utilized in several ways across the agency.
Some are employees; others are participants in grant-funded
programs. Some will serve as interns and still others act as
volunteers. In every case, peers help clients understand how to
take charge of their own treatment, how to cope with crises, and
how to navigate an often-confusing health care system.

Behavioral Health Services:
· Assertive Community Treatment:
The Assertive Community Treatment (ACT) teams provide
comprehensive, community-based, psychiatric treatment,
rehabilitation, and support to persons with serious and
persistent mental illness. Act clients often have co-occurring
problems with addiction, homelessness, criminal justice
involvement, physical illness, and violent victimization. ACT
teams embrace the nationally established evidence based
practice model for act and achieve a high level of fidelity to the
model. Teams are composed of a psychiatrist, nurse, licensed
team supervisor, a peer support specialist, and rehabilitation
support staff. Staff may develop sub-specialties in group
services, substance abuse treatment, forensic services, parenting
interventions, and vocational rehabilitation. ACT teams have
small caseloads and work as a tight knit interdisciplinary unit to
create opportunities for growth and recovery.
· Community Support TREATMENT:
We will provide comprehensive and integrated mental health
services for clients who have mental health problems and may
have co-occurring problems of addiction, homelessness, medical
illness, and violent victimization. Integrated clinical services
include diagnostic assessment, community support, care
coordination, therapy, and liaison with psychiatric services.
Community Support Specialists will provide direct care and
coordinate services to help clients achieve their recovery goals.
· Crisis Services:
The Waterfront Community Connection Crisis Service Team is
responsible for providing comprehensive crisis services after 5
pm, on weekends, and holidays to all clients receiving services
at The Waterfront. This allows us to respond to crises as they
happen, ensuring the safety of all our clients. Our main goals
are to prevent crises, stabilize emergent situations, and reduce
use of city emergency services such as 911 or hospital
emergency rooms. The Crisis Service Team currently consists of
trained mental health clinicians, supervised by a licensed
clinical supervisor.
· Day Rehabilitation Services program:
The Day Rehabilitation Services Program (DSVCP) is a skills
training and motivation enhancement program that supports
participants in their efforts to change lifestyle, overcome
obstacles, recover their lives, and experience empowerment. We
believe people with a serious mental illness can recover and are
capable of living independent, satisfying lives. Central to that
process is the development of skills that will empower the
individual to make positive life choices. Program participants
work together in a variety of activities to gain or rebuild
confidence and the skills necessary for living independently.
Participants can expect integrated care with their treatment
teams within and outside of The Waterfront.
· Outpatient Behavioral Health Clinic:
The Outpatient Behavioral Health Clinic (OBHC) is an
interdisciplinary team that serves as the main access point for
adult services, and provides psychotherapy and low-intensity
community support for individuals who are seeking assistance
with addressing life stressors, managing mental health
symptoms more effectively, or healing from trauma.
Recognizing that clients need a diverse spectrum of support, the
OBHC uses a patient-centered approach to treatment planning
and program referral, and lengthens the enrollment and
engagement process to better match individuals with programs,
interventions, and the appropriate level of care.
· Supported Employment:
Supported employment is an evidence-based program
encompassing recovery principles and recognizes that
employment can be an integral part of a person’s recovery.
Employment Specialists develop relationships with community
business owners and hiring managers to help the client become
employed in the job of their choice. Working together,
employment specialists and program participants develop an
individual work plan, create a resume, and practice interviewing
skills. Employment Specialists coordinate with the
participant’s treatment team; provide job coaching, and on-
going support to assist the individual in maintaining
employment.
INTEGRATED CARE SERVICES:
· Health Homes
Health Homes is an integrated model of care addressing an
individual’s behavioral and physical health needs. The health
home team includes nurse care managers, a primary care liaison,
and care coordinators who work together to meet each clients'
physical and mental health needs, as well as psychosocial goals.
The teams work with a individual and their network, including
family members and medical providers, to help clients improve
self-management of chronic illness and promote total wellness.
Comprehensive care management, care coordination,
transitional care, health promotion, individual/family support,
and accessing resources via referrals are the six services that
comprise the Health Home Service Delivery Model.
· PSYCHIATRY CLINIC
The Community Connections Psychiatric Clinic provides access
to Board Certified psychiatrists and nurses. The clinic provides
integrated psychiatric care to all interested Community
Connections clients (adults, children, and youth) and works
closely with their treatment teams. Services provided in the
clinic include assessment, medication management, lab work,
and nursing services. Psychiatrists and nurses working with the
Assertive Community Treatment teams often meet clients in the
community.
HOMELESSNESS & HOUSING:
· The Waterfront…we want to support individuals and families
with establishing and maintaining safe and affordable homes as
a part of their whole health recovery. We want to be the leader
in Jefferson County as an effort to develop and implement
effective Coordinated Assessment and Housing Placement
processes. We want to connect with government partners and
local coalitions of community providers to expand the
availability of housing resources, especially for the city’s most
vulnerable individuals and families. In all of these activities, we
demonstrate a comprehensive understanding and commitment to
the states strategic plan to end homelessness.
We want to offer a comprehensive range of outreach and
residential supportive services to promote housing stability and
prevent homelessness – especially with people experiencing
mental illness, addiction, and trauma.
Our Homeless Services Program will provide outreach to
women and men living outside and “in reach” to those staying
in emergency and transitional shelters. We want to offer
extensive navigation assistance (e.g., obtaining identification,
completing applications, mitigating housing barriers, etc.).
OUTPATIENT ADDICTION SERVICES:
· The Waterfronts Outpatient Addiction Services (OAS) will be
an ARC/DBH approved substance use treatment provider.
Recognizing the complexity of addiction, OAS utilizes
Community Connections' expertise in mental health and trauma
recovery to provide sophisticated clinical services geared
toward empowering clients. Clients will be able to discover the
roots of their substance addiction and how they can attain
sustained recovery to meet their goals. Addiction and recovery
are not just about a substance, but about a complex person with
multiple concerns including the impact of mental health
symptoms, developmental traumas, family relationships,
environmental factors, and other obstacles to recovery. OAS
will incorporate a holistic approach to recovery. We will teach
relapse prevention skills, emphasize lifestyle modifications, and
encourage network and family involvement. OAS will provide
person-centered and curriculum-based treatment and a diverse
staff that brings a wide range of life experience, work
experience, and education to the recovery process.

The Center for Families and Children
Youth Centered Family Focused Community BASED
The Waterfront Community Connection will be pleased to offer
services to children and youth who suffer from mental illness,
substance abuse, or a history of trauma. Our goal is to provide
high quality, clinical mental health services focused on the
strengths of each individual child and family. Family support
and skill building will be offered through individual, group, and
family-based interventions. We will utilize the focus and
creativity of the entire team to develop flexible and targeted
interventions. Problems are recognized in their proper context
and addressed through collaborative partnerships with families
and their formal and informal support networks.

HEART
The Healing, Empowering, Advancing and Recovering,
Together (Heart) program is an innovative 5-year project that
will be funded by SAMSHA to grow their national child
traumatic stress initiative. HEART’S ambitious mission is to
improve early access to integrated high-quality behavioral
health and trauma specific treatment for children, youth, and
their families. The goal of the program is to disrupt the
transmission of intergenerational trauma. Family peers and
clinicians will develop and implement creative strategies to
provide coordinated services that will allow for integrated child
and adult services. HEART will utilize Community-Based
Intervention and Family Team Meetings, with trauma-specific
evidence based practices offerings: Trauma Focused Cognitive
Behavioral Therapy for 1:1 child-specific recovery work,
Strengthening Families Coping Resources for multi-family
group recovery work, and Trauma Recovery and Empowerment
for adult women’s recovery work.
· Peer Empowerment & Advancement
Peer services will play a crucial role in helping to advance
wellness and recovery for clients living in the community. Peers
are uniquely suited to use their own experience and personal
knowledge to engage clients in treatment and recovery.
Additionally, peer staff will help guarantee that our clients'
voices are heard to help inform and shape agency practices and
policies.
Our peers will be utilized in several ways across the agency.
Some are employees; others are participants in grant-funded
programs. Some will serve as interns and still others act as
volunteers. In every case, peers help clients understand how to
take charge of their own treatment, how to cope with crises, and
how to navigate an often-confusing health care system.

Mission Our mission is to provide behavioral health, residential services, and primary health care coordination for marginalized and disenfranchised women, men, youth, and children living in Jefferson County, many of whom are coping with challenges including mental illness, addiction, and the aftermath of trauma and abuse.   • 4. · Chief Operating Officer will lead the operational aspect of the center as well as assist in the advancement of The Waterfront Community Connection as a leader in the field of behavioral health and residential services by developing strategic community partnerships and launching new initiatives. Additionally, the COO will oversee the Clinical Housing Department · Chief Clinical Officer and Chief of Clinical Innovation will lead every aspect of the clinical part of the center as well as projects that focus on consumer empowerment, trauma recovery, homelessness, addictions, de-institutionalization, and person centered care, mental health recovery, transition aged youth, and justice system involvement. · Chief Human Resources Officer leads our Human Resources Department, and recruiting, hiring, and talent development efforts for staff. · Chief Administrative Officer will lead the administrative and administration aspect of The Waterfront Community Connection Center. This person will oversee the Revenue Cycle Management, Information Technology, Plant Management, and the Practice Management Software. · Chief Compliance Officer will lead the Compliance Program, Quality Improvement Department, Medical Records Department, & Consumer Benefits Program. Job Roles and Responsibilities

Chief Executive Officer: · Increases management’s effectiveness by recruiting, selecting, orienting, training, coaching, counseling, and disciplining managers; communicating values, strategies, and objectives; assigning accountabilities; planning, monitoring, and appraising job results · Creating, communicating, and implementing the organization’s vision, mission, and overall direction – i.e. leading the development and implementation of the overall organization’s strategy. · Responsible for fixing prices and signing business deals · Responsible for providing direction for the business · Responsible for signing checks and documents on behalf of the company · Evaluates the success of the organization · Reports to the board. Chief Operating Officer · Responsible for overseeing the smooth running of HR and administrative tasks for the organization · Regularly hold meetings with key stakeholders to review the effectiveness of HR Policies, Procedures and Processes · Chief Administrative Officer Ensures operation of equipment by completing preventive maintenance requirements; calling for repairs. • Responsible for arranging travel, meetings and appointments Oversee the smooth running of the daily home activities · Defining job positions for recruitment and managing interviewing process · Carrying out induction for new team members · Responsible for training, evaluation and assessment of employees

Chief Clinical Officer Addiction Treatment Specialist/Psychiatrist · Providing residential alcoholism rehabilitation · Providing residential drug addiction rehabilitation · Providing treatment at convalescent homes or hospitals for psychiatric patients · Providing treatment at halfway houses for patients with mental health illnesses and substance abuse problems · Providing treatment at homes and hospitals for emotionally disturbed or psychiatric convalescent adults or children · Develop a treatment plan in consultation with other professionals, such as doctors, therapists, and psychologists · Create rehabilitation or treatment plans based on clients’ values, strengths, limitations, and goals · Assist clients in creating strategies to develop their strengths and adjust to their limitations · Monitor clients’ progress and adjust the rehabilitation or treatment plan as necessary · Advocate for the rights of people who are just coming out of drug addictions and alcoholism to live in the community and work in the job of their choice. Marketing and Sales Executive · Identify, prioritize, and reach out to new clients, and business opportunities et al · Identifies development opportunities; follows up on development leads and contacts · Writing winning proposal documents, negotiate fees and rates in line with organizations’ policy · Responsible for handling business research, market surveys and feasibility studies for clients · Responsible for supervising implementation, advocate for the customer’s needs, and communicate with clients · Document all customer contact and information
  • 7. · Represent The Waterfront Community Connection Community Connection in strategic meetings · Help increase sales and growth for The Waterfront Community Connection Community Connection® Addiction Treatment Center, Inc. Accountant/Cashier · Responsible for preparing financial reports, budgets, and financial statements for the organization · Provides managements with financial analyses, development budgets, and accounting reports · Responsible for financial forecasting and risks analysis. · Performs cash management, general ledger accounting, and financial reporting for the organization · Responsible for developing and managing financial systems and policies · Responsible for administering payrolls · Ensuring compliance with taxation legislation · Handles all financial transactions for The Waterfront Community Connection Community Connection · Serves as internal auditor for The Waterfront Communi ty Connection Community Connection Chief Operational Officer · Ensure that the facility is secured at all time · Control traffic and organize parking · Give security tips to staff members from time to time · Patrols around the building on a 24 hours basis · Any other duty as assigned by the facility administrator Cleaners: · Responsible for cleaning the facility at all times · Ensure that toiletries and supplies don’t run out of stock · Assist our inpatients and outpatients when they need to take their bath and carry out other tasks · Handle any other duty as assigned by the facility manager

  • 10. services we will provide: · Behavioral health · integrated care · homelessness & housing · outpatient addiction services · families & children · peer empowerment & advancement · inpatient addiction services Behavioral Health Services: · Assertive Community Treatment: The Assertive Community Treatment (ACT) teams provide comprehensive, community-based, psychiatric treatment, rehabilitation, and support to persons with serious and persistent mental illness. Act clients often have co-occurring problems with addiction, homelessness, criminal justice involvement, physical illness, and violent victimization. ACT teams embrace the nationally established evidence based practice model for act and achieve a high level of fidelity to the model. Teams are composed of a psychiatrist, nurse, licensed team supervisor, a peer support specialist, and rehabilitation support staff. Staff may develop sub-specialties in group services, substance abuse treatment, forensic services, parenting interventions, and vocational rehabilitation. ACT teams have small caseloads and work as a tight knit interdisciplinary unit to
  • 11. create opportunities for growth and recovery. · Community Support TREATMENT: We will provide comprehensive and integrated mental health services for clients who have mental health problems and may have co-occurring problems of addiction, homelessness, medical illness, and violent victimization. Integrated clinical services include diagnostic assessment, community support, care coordination, therapy, and liaison with psychiatric services. Community Support Specialists will provide direct care and coordinate services to help clients achieve their recovery goals. · Crisis Services: The Waterfront Community Connection Crisis Service Team is responsible for providing comprehensive crisis services after 5 pm, on weekends, and holidays to all clients receiving services at The Waterfront. This allows us to respond to crises as they happen, ensuring the safety of all our clients. Our main goals are to prevent crises, stabilize emergent situations, and reduce use of city emergency services such as 911 or hospital emergency rooms. The Crisis Service Team currently consists of trained mental health clinicians, supervised by a licensed clinical supervisor. · Day Rehabilitation Services program: The Day Rehabilitation Services Program (DSVCP) is a skills training and motivation enhancement program that supports participants in their efforts to change lifestyle, overcome obstacles, recover their lives, and experience empowerment. We believe people with a serious mental illness can recover and are capable of living independent, satisfying lives. Central to that process is the development of skills that will empower the individual to make positive life choices. Program participants work together in a variety of activities to gain or rebuild
  • 12. confidence and the skills necessary for living independently. Participants can expect integrated care with their treatment teams within and outside of The Waterfront. · Outpatient Behavioral Health Clinic: The Outpatient Behavioral Health Clinic (OBHC) is an interdisciplinary team that serves as the main access point for adult services, and provides psychotherapy and low -intensity community support for individuals who are seeking assistance with addressing life stressors, managing mental health symptoms more effectively, or healing from trauma. Recognizing that clients need a diverse spectrum of support, the OBHC uses a patient-centered approach to treatment planning and program referral, and lengthens the enrollment and engagement process to better match individuals with programs, interventions, and the appropriate level of care. · Supported Employment: Supported employment is an evidence-based program encompassing recovery principles and recognizes that employment can be an integral part of a person’s recovery. Employment Specialists develop relationships with community business owners and hiring managers to help the client become employed in the job of their choice. Working together, employment specialists and program participants develop an individual work plan, create a resume, and practice interviewing skills. Employment Specialists coordinate with the participant’s treatment team; provide job coaching, and on- going support to assist the individual in maintaining employment.
  • 13. INTEGRATED CARE SERVICES: · Health Homes Health Homes is an integrated model of care addressing an individual’s behavioral and physical health needs.
