Target Demographics
• Our program aims to serve adults
over the age of 18 who are
diagnosed with substance use or
co-occurring substance use and
mental health disorders.
5. Advanced Recovery, Care, and
Wellness have two goals:
1. To promote the recovery of adults struggling to manage
substance use disorders or co-occurring substance use and
mental health disorders.
a. Objective: By the end of 2024, 90% of the
clients served by Advanced Recovery, Care, and Wellness will
be actively engaged in their recovery by collaborating with their
providers to establish effective and personalized treatment
plans that meet clients where they are.
2.Never refuse services to persons seeking them by referring
folks to outside providers when our agency reaches capacity
while also offering a waitlist.
a. Objective: By the end of 2024, Advanced
Recovery, Care, and Wellness will have a full roster of clientele,
established a waitlist for future clients, and have an extensive
list of alternative resources compiled so staff may refer those
who may need urgent services or need more comprehensive
services.

Political, economic, social, and technological
factors
that impact our clients
The staff at Advanced Recovery, Care, and Wellness
understand that various external factors impact the
well-being of our clients. Politically, Danbury, CT is a
republican city with frequent opposition to
developments of new mental health, substance use,
and low-income housing programs. Despite this, the
Danbury citizens and surrounding community members
have come together time and time again to raise funds
for these types of programs in the years prior, which
shows community support. There is still a prevalent
social stigma for the substance use and mental health
disorders communities in this area, evident by the
frequent calls to the authorities on these individuals.
Technology has come to be a tremendous, supportive
factor for our clients. As a result of the pandemic,
Alcoholics and Narcotics Anonymous meetings went
online and appointments. These services remain
available as a supplemental resource to our clients.

diversity of Advanced
Recovery, Care, and
Wellness
At Advanced Recovery, Care, and Wellness, we believe that inclusion, authenticity, and diversity are
at the core. We allow these principles to guide us throughout all our work. Because all staff hired is
on their journey of recovery and wellness, we have a distinct and personal understanding of the
substance use and co-occurring substance use and mental health disorder culture. This diverse staff
and personal cultural connection benefit the clients immensely. They can make a more authentic
connection to their treatment, knowing that the people working with them understand their
experience firsthand.

We aim to be the first agency in Fairfield county to
offer inpatient and outpatient psychiatric,
therapeutic, rehabilitation, and case management
services. Advance Recovery, Care, and Wellness will
offer services through a well-structured team, as
outlined in the next slide, using the whole-person
approach.

12. Evaluation Process for Advanced Recovery, Care, and Wellness
Program evaluation outcome measures:
Evaluate data on adults with co-
occurring mental health and
substance use disorders who
accessed services for the first time
through our agency.

engaged in treatment to the point
of short-term recovery and those
who remained active in their

13. At Advanced Recovery, Care, and Wellness, we
stand by our mission statement. We believe in
providing a high level of care with compassion,
empathy, and authenticity. Our excellent staff,
who have firsthand experience with substance
use or co-occurring substance use and mental
health disorders, are finely tuned into the
nuances of recovery. These specialists in their
field will be divided into two branches of service,
inpatient and outpatient, both receiving
psychiatric, psychological, and case management
care as needed.
