As a graduate social worker and a licensed nursing home administrator, I look forward to continuing my
career in nursing home administration. I will be bringing with me 11 years of experience in the gerontology
field of social work practice and well-rounded generalist perspective training, as well as a Bachelor social work

I have a great passion for the aging community and I feel that I am able to make a great difference in this

Ridgeway Nursing & Rehabilitation Facility (Simpson Lane Consulting) July 2015-Currently
• I was employed as the Executive Director of Ridgeway Nursing and Rehabilitation. I have worked closely with
each department to make sure the running of the facility is correct and in order. I was in charge of all the day to
day operations for Ridgeway. I was to ensure that all standards of our corporation were met and in compliance.
During my time I have had survey experience and have excelled in that process evidence by past survey results. I
work diligently to make sure the expenses do not exceed our profits and that all costs are reasonable. I lead, coach
and train roughly 138 staff members in total as well as their supervisors. We have seen improvement and
excellent outcomes during my time as the administrator for Ridgeway Nursing and Rehabilitation. I work closely
with our corporate employees on a daily basis as well as our care staff. I am familiar in the quality measures,
MDS, case mix, life safety, CQI, safety, marketing, aging and other aspects of facility life to operate a 99 bed
facility such as ours. I was the SRNA supervisor for a period of 6 months when I learned to create schedules,
worksheets, assignment sheets, etc. I grew a wealth of knowledge in learning to utilize the strongest nurse aides
where they were needed most. It became even more important in my day to day to realize the staffing to acuity of
the residents on the halls. I have a strong background in marketing and public relations. I constantly tried to come
up with new ideas for our facility to bring the community into our environment in hopes to gain referrals as well

3. as quality employees. I actively participate in the resident and employee activities in and outside of the facility.
As a State Registered Nursing Assistant I would help out the floor when needed and at activities when they were
out in the community. I worked diligently to make sure we had adequate and competent staff within our facility
by helping with the hiring process when necessary and interviewing applicants. I also made sure that if the
employees did not meet the standards of our facility they were no longer employees of Ridgeway. When an issue
arises with an employee or staff member I would thoroughly investigate the concern to find the root cause of the
problem.

• Employed as the Director of Social Services and Admissions for a 99 bed facility. I worked closely with the
current administrator to learn the processes of the building that I was not as familiar with in my social services
training. Completed monthly reports that were sent to the corporate office to explain the operations of the building
as a whole. I complete monthly maintenance round with the maintenance director to ensure the overall appearance
of the building as well as the integrity of its structure is at facility standards. Assisted with the facility newsletter
and activities department calendars. I worked closely to complete MDS assessments and CAA
documentation for each resident. I also worked to improve social services documentation and forms
within the company. I am the CQI environmental leader to facilitate audits and overview the daily needs
for the building; as well as currently a committee member on the safety, clinical, and infection control
committees as within the facility to also seek audits to improve care within the building. I am solely
responsible for the admissions that come into our building. These responsibilities include admission
paperwork completed with the family and/or resident, coordinating with hospital caseworkers and
families in regards to discharge from current location, planning with facility staff to accommodate each
potential residents needs within our care. Worked closely with the bookkeeper and business office to
assist with payment as well as the debt accumulated by previous residents. Worked daily with insurance
companies and families to ensure the benefits and coverage by each available plan purchased by the
residents. Conducted authorizations and precertification for insurance companies based on specific
residents needs.
The Terrace Nursing & Rehabilitation Facility (Simpson Lane Consulting) March 2013-Febuary 2014
• Employed as the Administrator in Training for the building of 102 beds. Having a background in social
work I worked closely to assist with the social services of the building. I worked closely to complete
MDS assessments and CAA documentation for each resident. I also worked to improve social services
documentation and forms within the company. Facilitated the Alzheimer’s support group each month.
Coordinated the resident and family council meetings for the facility. I assisted with the admissions
process and learned the proper policies for the procedure to admit residents into the facility. Became the
CQI operations coordinator to audit procedures throughout the facility. I was a committee member on
the safety, clinical, and infection control committees as within the facility to also seek audits to improve
care within the building. I worked as the marketing coordinator for the building which included
purchasing products to market to other facilities, hospitals and doctor offices within the area to boost
community relations. Completing the facilities wide newsletter for each employee as well as
constructing several resident and event articles in the highly awarded facility newsletter. On occasion
assisting with activities for the residents including a cognitively impaired group to target their specific
needs and interests. Upon new hire of all staff I assist in the education of resident rights, abuse, HIPPA,
Marketing in the community and hospitality. I’ve created flyers and marketing tools for events within
the facility such as fundraisers for the Walk to End Alzheimer’s, CEMP bowling, food bank donation
drive, etc.
Cambridge Place June 2012-March 2013
• Employed as a social services/admissions coordinator for a facility of 118 beds. I was responsible for
those residents, their family concerns, care needs, placement, plan of care and discharges when required.
As the social services coordinator I would do assessments for each resident as well as abide by the MDS
schedule as necessary. BIMS and PHQ-9 assessments were completed daily as well as social service

4. documentation and social history assessments. When needed I would also complete CAA documentation
on specific residents. Being the admissions coordinator I helped to boost admissions into the building
and complete the process of new residents. These duties included speaking to caseworkers at the
hospitals as well as families for referrals coming from home. I would then complete necessary
paperwork with the resident/family to achieve admission into the facility. Attended daily clinical
meeting as well as behavior meetings and care plans as scheduled. Once a month I would coordinate the
marketing meeting to arrange with the department heads of the facility and discuss ideas to boost
admissions of residents.
Owenton Manor Care and Rehabilitation- Sunbridge Healthcare August 2011- June 2012
• Employed as the Social Services Director of a 100-bed facility being responsible for all resident needs concerning
family/ resident adjustment to placement, plan of care and illness. Achieving outstanding skills in the categories
of time management, prioritizing and multi tasking. Attending several KAHCF meetings and seminars as well as
their last annual awards presentation. Gained a wealth of knowledge on state and federal regulations of long-term
care facilities. Became quite familiar with the processes and requirements of Medicaid and Medicare part A and
part B. Organized and facilitated the accuracy of the Map 350 forms and regulations. Educated other staff on the
importance of resident rights, the 7 kinds of abuse and the importance of grievances and reportables; also
conducted numerous reportables and even more grievances to address the issues the staff, families and residents
brought to myself and the management teams attention. Attended daily clinical and bi-weekly Care Plan meetings
as well as correlated Care Conference meetings to speak one on one with residents and families to address their
concerns. Conducted BIMS/PHQ-9 resident interviews as well as completed Section E (behavior) and CAA of the
MDS categories. Performed Mini-Mental Exams on residents when necessary. Had an in depth knowledge of the
admission process of residents as well as organized and composed all residents discharge plans. Coordinate with
the therapy department to develop the most beneficial individualized plan of care for each resident.
Bluegrass Area Development District-Area on Aging January-May 2011
• Performed over 390 internship hours experiencing all forms of social work practice in a macro level setting.
Greatly achieving a skill in public speaking to any audience while doing several presentations over the Older
Americans Act and participating in over 20 policy based agency meetings. Attended state meetings to discuss
issues with programs, such as DAIL, that the Bluegrass Area Development District and other A.D.D.’s in the state
of Kentucky. Maintained critical thinking skills essential to providing competent and dignified individual care by
assessing each individual’s needs that called the help line. Gaining individual characteristics to demonstrate
aspects of hard work and an energetic attitude; as well as a flexibility to adapt easily to change of environment
and work schedule. Gained a micro level of practice knowledge by being greatly involved in the Chronic Disease
Self-Management Program based out of the University of Stanford. The agency was very involved in this program
and therefore I was able to gain many more opportunities for micro level growth than had been expected.

they are issuing are lawful based on regulations. Gained insight on how the Consumer Directed Option program
based out of the Bluegrass Area Development District is preformed by each of the C.D.O. workers. Supervising

• Oversaw all aspects of the Kentucky government monitoring an elected official on her day to day through the
capitol. Gained insight on the role of policy in the social work practices, how to construct new statutes and how
detrimental policy change is in this profession. Acquired a deeper communication competence and a great deal of
leadership skills that many politicians possess. Gained professional insight into the conferences that interested
Representative Westrom by attending several and participating in several of her interviews with the media,
