17. Diagnostika
• Urīnceļu USG
Jāveic katram bērnam ar febrilu akūtu urīnceļu
infekciju.
Novērtē, vai ir hidronefroze vai/un ureter dilatācija.
Novērtē nieres parenhīmu un izmērus. Displastiska
niere – mazāka, gaišāka
Ja redzams nieru bojājums, kas atbilst III-V
pakāpei, veic izmeklējumus ar kontrastvielu.
Usg in utero – hidronefrozi var redzēt.
19. Diagnostika
• Mikcijas cistoureterogrāfija
Urīnpūsli pilnībā piepilda ar kontrastvielu (caur
katetru, mazākiem bērniem – pediatriskā barošanas caurule, lielākiem – Foley
katetrs) un RTG kontrolē novērtē urīna
plūsmu/refluksu tā iztukšošanas laikā.
23. Ārstēšana
• Lielā daļā gadījumu spontāna regresija (I un II
pakāpe)
• Jākontrolē, lai bērns laicīgi iztukšotu urīnpūsli
(ik pēc 3h)

Pieņemu, ka pacientiem ar urīnceļu infekcijām biežāk, jo ir iemels izmeklēt nieres un urīnpūsli. Citos gadījumos paliek nediagnosticēts.
When a UTI or pyelonephritis develops, Escherichia coli is the most frequently detected organism. P fimbriae facilitate adherence to, and colonization of, the urinary tract by E. coli. Bacterial endotoxins (especially lipid A) are virulence factors that have a paralytic effect on ureteral peristaltic activity. Once bacterial cells adhere to the urothelium, a complex series of reactions occurs within the cells, eventually leading to the generation of cytokines (IL-1, IL-6, IL-8, TNF, TGF and others). These events, in turn, cause a chemotactic response from leukocytes, lymphocytes and other immune cells, leading to a marked inflammatory reaction. During such reactions, free oxygen radicals and proteolytic enzymes are released. These agents damage tubular cells directly and cause renal ischemia by reperfusion injury. During healing and tissue regeneration following such an infection, renal fibrosis and scarring of the affected parenchyma can lead to renal scarring.[22]

Indications for antibiotic prophylaxis
Children without renal scarring at diagnosisDiagnosis made in infancy: All patients diagnosed at infancy (ie, < 1 y) with grades I-V reflux should be treated initially with continuous prophylactic antibiotics.
Diagnosis made in children aged 1-5 years: When unilateral and/or bilateral grades I-IV reflux or unilateral grades III-V reflux are diagnosed in children aged 1-5 years, they should be treated initially with continuous prophylactic antibiotics.
Diagnosis made in children aged 6-10 years: Children diagnosed at age 6-10 years with unilateral and/or bilateral grades I-II reflux and unilateral grades III-IV reflux should be treated initially with continuous antibiotic prophylaxis. However, some are advocating withholding treatment in patients with grade I or II VUR, as most of these patients are at low risk for UTIs and pyelonephritis provided they have no voiding dysfunction or constipation.
Children with renal scarring at diagnosisDiagnosis made in infancy: Infants (ie, < 1 y) with scarring at diagnosis and grades I-V reflux should be treated initially with continuous antibiotic prophylaxis.
Diagnosis made in children aged 1-5 years: Antibiotic prophylaxis is the preferred option for preschool-aged children (ie, 1-5 y) with renal scarring at diagnosis, unilateral and/or bilateral grades I-II reflux, unilateral grades III-IV reflux, and bilateral grades III-IV reflux.
Diagnosis made in children aged 6-10 years: In children diagnosed at age 6-10 years with renal scarring and unilateral and/or bilateral grades I-II reflux or unilateral grades III-IV reflux, antibiotic therapy is the preferred treatment option.
