result in no credit being given.Case StudyLiz is employed in a long-term-care center for
ventilator-dependent patients. One of her patients is Mike, a 50-year-old man who suffered
severe injuries in a motor vehicle accident three years ago. Initially, he recovered well
enough to be transferred to a rehabilitation facility, but he then suffered a stroke and
became paralyzed and ventilator-dependent. Mike has been in long-term care for a year and
a half. His physician, in consultation with the neurologist, has recently determined that he is
in a permanent vegetative state (PVS) and will not recover consciousness.Liz has been
working with Mike’s wife, children, and parents to help with their adjustment to his initial
injury and now to his diagnosis of PVS. The patient’s wife and parents have a fragile
relationship. Mike is an only child, and his parents objected to his marriage. Mike’s wife has
maintained the children’s relationships with their grandparents out of respect for her
husband’s wishes. She and Mike made all their decisions together, and now she feels very
alone in decision-making. She has come to depend on her husband’s former boss for
direction. Mike’s parents resent this dependence and do not understand why the wife does
not consult them. With the new diagnosis of PVS, the physician reviewed a variety of care
options, including a terminal wean from the ventilator for the patient. In reflecting on the
options, Mike’s wife and Mike’s boss both agree that the patient would choose to be
removed from the ventilator if there was no hope of recovery. The parents are absolutely
adamant that there is always hope and have told the social worker and the facility’s
administrator that they will fight the decision. The administrator has asked the social
worker to arrange for an ethics consultation. The wife and parents have agreed to the
consultation.
