Chẩn đoán tiểu đạm
– Que nhúng: phản ứng Tetrabromophenol blue + Albumin Định
tính, bán định lượng tiểu đạm khi >1(+)
Kết quả:
• 1+ 30 mg/dl
• 2+ 100 mg/dl
• 3+ 300 mg/dl
• 4+ ≥2000 mg/dl
– Đạm niệu/creatinin niệu
• ≥0.5 mg/mg = ≥50 mg/mmol (6 – 24 tháng) Bất thường
• ≥0.2 mg/mg = ≥20 mg/mmol (>24 tháng) Bất thường
• ≥2 mg/mg = ≥200 mg/mmol Ngưỡng thận hư
– Đạm niệu 24h: ≥4 mg/m2/h hoặc ≥150 mg/1.73 m2/24h

Nguyên nhân tiểu đạm
– Thoáng qua:
• Sốt, vận động, stress, co giật
• Tư thế (thường ≤1 g/l)
– Bệnh lý
• Cầu thận: 5 hội chứng bệnh cầu thận
– Bệnh cầu thận nguyên phát
– Bệnh cầu thận thứ phát
Biện luận: Lâm sàng – nước tiểu – sinh hoá, miễn
dịch – GPB
• Ống thận:
– Quá tải đạm (đa u tuỷ, tiểu Hb, Mb)
– Tổn thương ống thận: Hoại tử OT cấp, viêm OT – MK do thuốc
• Đường tiểu: TITO
(Trauma – Inflammation/Infection – Tumor – Others)

Chỉ định STT
• Tiểu đạm >1g (trừ HCTH sang thương tối
thiểu)
Tiểu đạm + cặn lắng nt bất thường
• Tiểu đạm + GFR <80 ml/ph/1.73m2 (trừ VCTC
gđ sớm)
• Giảm C3 kéo dài trên 3 tháng
• Bằng chứng bệnh hệ thống

• Loại nguyên nhân do thuốc (Rifampicin,
Chloroquine,…), thức ăn, tiểu vàng sậm/ vàng da
• Có tiểu máu không?
– TPTNT: Chất thử hoạt tính giống men peroxidase
(tetramethylbenzidine) Phản ứng Heme: Hb, Mb
– Soi cặn lắng nước tiểu:
• 5HC/ QT40 (chú ý kỹ thuật soi cặn lắng nước tiểu)
• Trụ HC, hình dạng HC
• Cặn Addis: >5000 HC/phút Chú ý kỹ thuật xn cặn Addis

Phân biệt nguyên nhân cầu thận gây tiểu máu
– Nguyên phát
– Thứ phát
• Nhiễm trùng: Liên cầu khuẩn, HBV, HCV, HIV, SR
• Miễn dịch: Lupus, Henoch – Chonlein, Goodpasture, IgA
• Chuyển hoá: $ Alport (Tiền căn gia đình suy thận, điếc, tiểu
máu)
• Ác tính:
• Khác: Thuốc, độc chất
Phân biệt dựa vào lâm sàng (TCCN, TCTT, tiền
căn), hoá sinh, miễn dịch, giải phẫu bệnh

Phân biệt các nguyên nhân tiểu máu không do
cầu thận:
– Toàn thân: Rối loạn cầm máu ban đầu, đông máu
toàn bộ, tăng Ca niệu?
– OT – MK: Viêm OT – MK, hoại tử OT cấp
– Đường niệu: TITO
Phân biệt dựa vào LS (TCCN, TCTT, tiền căn),
nước tiểu, sinh hoá, miễn dịch, hình ảnh học

Chỉ định sinh thiết thận trong tiểu máu
• Tiểu máu vi thể kéo dài (trừ VCTC hậu nhiễm LCK)
• Tiểu máu + tiểu đạm >1g/ngày
• Tiểu máu tiến triển trong 6 tháng theo dõi
• Tiểu máu kéo dài + C3 giảm kéo dài (>8 tuần)
• Tiểu máu + suy thận, cao huyết áp kéo dài
• Tiểu máu + bệnh hệ thống
• Tiền căn gia đình có bệnh thận
• Tiểu máu đại thể tái phát CRNN

1. URINE SPECIFIC GRAVITY (USG)
Purpose: urine osmolality and represnts patient's hydration status and
concentrating ability of their kidneys
• Normal: 1.003-1.030
• Increased USG: glycosuria, SIADH
• Decreased USG: diuretic use, DI, adrenal insufficiency,
aldosteronism, impaired renal fx
• NOTE: if intrinsic renal insufficiency, USG fixed at 1.010
2. URINARY PH
Purpose: urinary pH reflects serum pH except in renal tubular acidosis
(RTA), useful for diagnosis/management of UTIs and calculi
• Normal: 4.5-8.0, slightly acidic usually (5.5-6.5)
• Alkaline urine: suggest urea-splitting organism
• Acidic urine: uric acid calculi

3. HEMATURIA
Testing: for peroxidase activity, will be positive in hematuria, myoglobinuria,
hemoglobinuria
• if positive: must look at micro to confirm presence of RBCs
• Types: glomerular, renal, urologic
4. PROTEINURIA
• Criteria: >150 mg/day (10-20mg per dL)
-microalbuminuria= 30-150 mg/day (sign of early renal disease)
• Normal urinary proteins: albumin, serum globulins, proteins secreted by the
nephron
• Testing: reagant sensitive to albumin, but can miss low levels of globulins
• Dipstick results:
– 1+ = 30 mg/dL
– 2+ = 100 mg/dL
– 3+ = 300 mg/dL
– 4+ = 1,000 mg/dL

5. GLYCOSURIA
• Normal: almost completely reabsorbed in proximal
tubule
Criteria: occurs at approximately 180-200 mg/dL
Elevated: DM, Cushing's syndrome, liver and
pancreatic disease, Fanconi's syndrome
6. KETONURIA
• Normal: not present in urine
Testing: presence of acetic acid through a chemical
reaction
Causes: uncontrolled diabetes, pregnancy,
carbohydrate-free diets, starvation

7. NITRITES
• Normal: Not present in urine
Testing: bacteria that reduce urinary nitrates to nitrites
-Positive test: can be gram negatives or gram positive organism (neg > pos)
-Indicate significant number (>10,000/mL)
-Negative test: Can’t R/O UTI because non-nitrate reducing organism cause UTIs
-NOTE: nitrite stick sensitive to air exposure: after one week of exposure about
1/3 of strips give false positive
8. LEUKOCYTE ESTERASE
• Testing: presence of neutrophils
• Sterile Pyuria:
-Organisms: Chlamydia and Ureaplasma urealyticum
-Other causes: balantis, urethritis, TB, bladder tumors, viral infx, nephrolithiasis,
foreign body, exercise, glomerulonephritis, corticosteroid, cyclophosphamide use

9. BILIRUBIN
Normal Bilirubin: no detectable amounts
• -Unconjugated bili: not water soluble, doesn't pass through
glomerulus
-Conjugated bili: water soluble, indicates possible liver dysfunction
or biliary obstruction
10. UROBILINOGEN
• Normal Urobilinogen: only small amounts
-Urobilinogen: end product of conjugated bili after
metabolized by intestine, small amount reabsorbed into portal
circulation and filtered by glomerulus
• Increased urobilinogen: hemolysis, haptocellular disease
• Decreased urobilinogen: antibiotic use, bile duct obstruction
