Yearly employee performance evaluation
It is an evaluation of an individual employees performance of specific work related tasks and skills
development for the past 12 months.

Performance review forms are tailored to meet the needs of each individual company in
assessing the performance of an employee based on the criterion the company selects.

The form I use is divided into several sections:
Safety
Environment
Quality
Capital
Competitive Position
Employee Development
External Relations
Additional Observations
Performance Improvement Plan
•

The employee performance is evaluated by his/her supervisor on specific areas
within each section on a scale from 1 to 5.

•

1 = Distinguished
2 = Commendable
3 = Competent
4 = Adequate
5 = Marginal

•

The employees strengths and areas needing improvement are documented in each
section of the performance appraisal form.
An overall rating is also given using the same 1 -5 scale.

•

Once the supervisor has completed his evaluation of the employee he/she will
meet with the employee to review and discuss the evaluation. A section is
provided for the employee to add his comments about his/her review on the form
and for him/her to sign and date the form.
The form is then sent to the appropriate reviewer. After review the form is sent to
HR and filed.

Yearly employee performance evaluation

  • 2. It is an evaluation of an individual employees performance of specific work related tasks and skills development for the past 12 months. Performance review forms are tailored to meet the needs of each individual company in assessing the performance of an employee based on the criterion the company selects. The form I use is divided into several sections: Safety Environment Quality Capital Competitive Position Employee Development External Relations Additional Observations Performance Improvement Plan
  • 3. • The employee performance is evaluated by his/her supervisor on specific areas within each section on a scale from 1 to 5. • 1 = Distinguished 2 = Commendable 3 = Competent 4 = Adequate 5 = Marginal • The employees strengths and areas needing improvement are documented in each section of the performance appraisal form. An overall rating is also given using the same 1 -5 scale. • Once the supervisor has completed his evaluation of the employee he/she will meet with the employee to review and discuss the evaluation. A section is provided for the employee to add his comments about his/her review on the form and for him/her to sign and date the form. The form is then sent to the appropriate reviewer. After review the form is sent to HR and filed.
