Background: Tramadol induced disorder is a new problem. Objective: To explain the efficacy of low dose of clonidine, baclofen and ibuprofen (NSAID) in the treatment of severe and chronic tramadol dependency (2000 mg daily). Method: To evaluate the usefulness of non-narcotic drugs in the management of severe tramadol withdrawal symptoms. Results: Clonidine 0.3 mg, baclofen75 mg and ibuprofen 1200 mg per day is very effective in the management of severe tramadol withdrawal symptoms and craving. Discussion: Our findings indicates that low dose of clonidine, baclofen and ibuprofen (NSAID) is beneficial in the treatment of severe tramadol dependency. This is a fascinating result. Conclusion: To our knowledge dramatic effects combination of clonidine, baclofen and ibuprofen in these situations has not been published yet, and this finding is a considerable addition to the literature.

However, 6 triggers did not detect any ADRs, for a failure rate of 29%. The most common ADRs detected were hypotension, hypertension, vertigo and cough. This study demonstrates that trigger tools can effectively detect ADRs, but requires further improvement to reduce the failure rate.

Non-steroidal anti-inflammatory drugs are the most widely used "over the counter" medication all over the world despite their complications in different major organs. Present studies envisaged for knowing the occurrence and severity of adverse drug reactions from NSAIDs in different ethnic communities of Sikkim. A cross sectional study was undertaken in the medicine outpatients department of a secondary and tertiary care hospital. The patients belonging to Nepalese, Bhutias, Lepchas ethnic communities and others community (settlers from other parts of India) were included to analyzed the data based on the age and gender, ethnicity and ADRs, drugs and ADRs. Severity assessment was done using Hartwing and Siegel scale and causality assessment by Naranjo scale. Total 109 cases of ADRs, predominating in female were detected. Nepalese were the most affected and Gastrointestinal tract (GIT) being the most affected organ in them. Diclofenac showed maximum number of ADRs in all the communities. Maximum number of cases occurred on single day use (40.36%) of drugs. All the cases were belonging to the "possible category" and the maximum being the mild (72.48%) in nature. It is advisable to consider the ethnic/racial differences equally with other factors, to improve the safety and efficacy of a drug.

Patterns of Prescription Writing in Psychiatric Clinics
This study analyzed 602 psychiatric prescriptions from private clinics in Peshawar, Pakistan to evaluate prescription writing practices and prescribing trends. The key findings were:
1) On average, 3.34 drugs were prescribed per prescription. Central nervous system drugs accounted for 71% of all drugs prescribed.
2) Important prescription elements like drug strength, quantity, and dosage instructions were missing in about 30% of prescriptions evaluated.
3) The most commonly prescribed medications were fluoxetine (8.4%), sodium valproate (7.9%), and alprazolam (7.7%). Non-steroidal anti-inflammatory drugs and vitamins were also regularly prescribed.

This study evaluated the correlation between bispectral index (BIS) scores and sleep stages in 12 patients with moderate to severe obstructive sleep apnea-hypopnea syndrome (OSAHS). Polysomnography and BIS monitoring were performed simultaneously. Two patients were excluded, one for not meeting OSAHS criteria and another for lacking non-rapid eye movement stage 3 sleep. The remaining 10 patients showed mean BIS scores of 86.3 for awake, 78.4 for stage N1, 73.8 for stage N2, 61.8 for stage N3, and 83.3 for rapid eye movement sleep. This study identified and characterized drug-related problems (DRPs) in 104 elderly orthopedic patients with femoral neck fractures at an Israeli hospital. They found that 69% of patients had at least one DRP, for a total of 145 identified DRPs. The most common DRP categories were safety issues (34%) and indication problems (32%). Most recommendations were to add or discontinue medications, suggesting over- or under-treatment. Having more chronic medications was correlated with more DRPs. A clinical pharmacist was added to the orthopedic ward team based on these findings.

Patterns of Prescription Writing in Psychiatric Clinics
This study analyzed 602 psychiatric prescriptions from private clinics in Peshawar, Pakistan to evaluate prescription writing practices and prescribing trends. The key findings were:
1) On average, 3.34 drugs were prescribed per prescription. Central nervous system drugs accounted for 71% of all drugs prescribed.
2) Important prescription elements like drug strength, quantity, and dosage instructions were missing in about 30% of prescriptions evaluated.
3) The most commonly prescribed medications were fluoxetine (8.4%), sodium valproate (7.9%), and alprazolam (7.7%). Non-steroidal anti-inflammatory drugs and vitamins were also regularly prescribed.

This study evaluated the correlation between bispectral index (BIS) scores and sleep stages in 12 patients with moderate to severe obstructive sleep apnea-hypopnea syndrome (OSAHS). Polysomnography and BIS monitoring were performed simultaneously. Two patients were excluded, one for not meeting OSAHS criteria and another for lacking non-rapid eye movement stage 3 sleep. Background: Methamphetamine abuse and dependence is currently a considerable problem.
Objective: To explain the efficacy of buprenorphine in the quick management of methamphetamine
related anxiety and depressive disorders and also methamphetamine withdrawal craving.
Results: 52 mg of buprenorphine is very valuable in the treatment of severe methamphetamine
related anxiety and depression and also methamphetamine craving.
Discussion: This study lightens that high dose of buprenorphine is quite effective in the lowering
of anxiety, depression, and chest pain and methamphetamine withdrawal symptoms. Hence, our
finding is a substantial addition to the literature.
Conclusions: We resulted that a single high dose of buprenorphine could quickly subside
methamphetamine induced anxiety, depression, psychogenic pain and withdrawal symptoms.
It looks that buprenorphine may be a safe and valuable drug for the treatment of the majority of

This study aimed to evaluate the prevalence and pattern of potentially inappropriate medication use in elderly patients based on the Beers criteria 2003. The study analyzed prescriptions from 407 elderly patients at a tertiary care hospital in India. The results found that 23.59% of patients received at least one inappropriate medication, and 7.42% of total drugs prescribed were inappropriate. The most common inappropriate medications belonged to category A of the Beers criteria, which are drugs that should generally be avoided in elderly patients. Antihistamines, anticholinergics, sedatives, and cardiac glycosides were among the most frequently inappropriate prescribed drug classes. The high prevalence of inappropriate medication use suggests the need for improved prescribing practices for elderly patients

This dissertation examines the clinical relevance of drug immunogenicity. The author conducted a systematic review and meta-analysis of 17 studies involving 936 patients. The analysis found that anti-drug antibodies (ADAb) significantly reduce therapeutic responses by around 80% and concomitant immunosuppression decreases ADAb production by 64%. Nearly half of ADAb-positive patients developed infusion-related adverse events. A bridging ELISA assay was found to be a practical method for routinely assessing ADAb. A new algorithm was proposed integrating immunogenicity testing, and when applied in a clinical trial it led to better outcomes than the standard approach, with ADAb-positive patients having higher disease activity. The dissertation concludes ADAb have an important clinical impact.

Since the discovery of the first effective antipsychotic medication (APM) in the mid 1950s, efforts to enhance their efficacy have been limited, despite improvements in tolerability. This stagnation is evident in effectiveness trials conducting in Europe and the United States. Several factors contribute to the failure to develop more effective APMs, including the absence of appropriate assessment tools for core symptoms domains in schizophrenia, reliance on the dopamenergic hypothesis, and the prolifration of “me too” drugs. The classification of APMs is also convoluted, grouping together second-generation, partial agonists, and multimodel APMs despite significant differences in their mechanism of action. Chllenges such as inadequate sample sizes, lack of statistical measures correlating with clinical significanse, and the high cost of newer APMs further hinder drug development. Additionally, there is lack of early predictors of antipsychotic response and tools to optimize APM efficacy. Suboptimal APM use by mental health providers, including excessive maintenance doses and irrational polypharmacy, exacerbates effectivness and medication adherence issues. Despite these challenges,there have been advancaments in APM tolerability and the development of long-acting injectables to address medication nonadherence. This critical review examines 70 years of antipsychotic development, identifies reasons for the failure to develop more effective APMs , and suggestes future directions in this field.

Major depressive disorder affects many people and treatment-resistant depression is defined as failing to respond to two antidepressants. Emerging evidence suggests abnormalities in glutamate signaling may be involved in depression. Ketamine, a glutamate receptor antagonist, shows promise in treating treatment-resistant depression based on three studies. The studies found ketamine provided rapid relief of depressive symptoms for up to 2 weeks in 70-71% of patients. While side effects like elevated blood pressure and dissociative symptoms occurred, ketamine may be a safe and effective option for treatment-resistant depression.

Introduction: Lithium is the most effective long-term therapy for bipolar affective disorder (BPAD). Its therapeutic benefits may be restricted by frequent adverse drug reactions (ADRs) and low therapeutic index. We evaluated the pattern of ADR of lithium monotherapy in patients with BPAD. Materials and Methods: An observational study was conducted in 213 patients presenting to psychiatric outpatient department and diagnosed with BPAD and taking lithium monotherapy at least for 3 months. Data collection was done from December 15, 2018, to December 14, 2019. Sociodemographic profile, relevant laboratory investigations, and ADRs were recorded on a self-designed proforma. Descriptive statistics were used to analyze the data. Chi-square test was used to correlate the association between ADR and baseline variables using Statistical Package for the Social Sciences (Version 22.0). P < 0.05 was considered statistically significant. Results: Out of 213 patients, 110 (51.6%) were male. Mean age was 32.06 ± 1.80 years. Sixty-nine (32.4%) patients were substance user. The prescribed dose of lithium ranged from 300 to1200 mg/day. At least one ADR was observed in 139 (65.3%) patients. Tremor (45%) was the most common ADR followed by nausea (9.5%) and sedation (7.8%). Discussion: Daily administration of lithium 1000 mg or more was found to be a significant risk factor for occurrence of the ADRs. Weight gain might result in lithium non-compliance and hence the patients must be counseled regarding this to improve medication adherence. Conclusion: The prevalence of ADR was high in the patients taking maintenance dose of lithium. The patients need regular follow-up to detect and manage ADR which help to increase the medication adherence. This randomized, double-blind, placebo-controlled clinical trial tested whether a genetically-informed biomarker of nicotine metabolism (nicotine metabolite ratio or NMR) could predict responses to nicotine patch or varenicline for smoking cessation. 1246 smokers were randomized to receive placebo, nicotine patch, or varenicline. The primary outcome was 7-day point abstinence at end of treatment. For normal metabolizers, varenicline was more effective than nicotine patch, but for slow metabolizers there was no difference. Action research on drug safety assestment

Describing and Interpreting an Immersive Learning Case with the Immersion Cub...
Current descriptions of immersive learning cases are often difficult or impossible to compare. This is due to a myriad of different options on what details to include, which aspects are relevant, and on the descriptive approaches employed. Also, these aspects often combine very specific details with more general guidelines or indicate intents and rationales without clarifying their implementation. In this paper we provide a method to describe immersive learning cases that is structured to enable comparisons, yet flexible enough to allow researchers and practitioners to decide which aspects to include. This method leverages a taxonomy that classifies educational aspects at three levels (uses, practices, and strategies) and then utilizes two frameworks, the Immersive Learning Brain and the Immersion Cube, to enable a structured description and interpretation of immersive learning cases. The method is then demonstrated on a published immersive learning case on training for wind turbine maintenance using virtual reality. Applying the method results in a structured artifact, the Immersive Learning Case Sheet, that tags the case with its proximal uses, practices, and strategies, and refines the free text case description to ensure that matching details are included. This contribution is thus a case description method in support of future comparative research of immersive learning cases. We then discuss how the resulting description and interpretation can be leveraged to change immersion learning cases, by enriching them (considering low-effort changes or additions) or innovating (exploring more challenging avenues of transformation). The method holds significant promise to support better-grounded research in immersive learning.

Context. With a mass exceeding several 104 M⊙ and a rich and dense population of massive stars, supermassive young star clusters
represent the most massive star-forming environment that is dominated by the feedback from massive stars and gravitational interactions
among stars.
Aims. In this paper we present the Extended Westerlund 1 and 2 Open Clusters Survey (EWOCS) project, which aims to investigate
the influence of the starburst environment on the formation of stars and planets, and on the evolution of both low and high mass stars.
The primary targets of this project are Westerlund 1 and 2, the closest supermassive star clusters to the Sun.
Methods. The project is based primarily on recent observations conducted with the Chandra and JWST observatories. Specifically,
the Chandra survey of Westerlund 1 consists of 36 new ACIS-I observations, nearly co-pointed, for a total exposure time of 1 Msec.
Additionally, we included 8 archival Chandra/ACIS-S observations. This paper presents the resulting catalog of X-ray sources within
and around Westerlund 1. Sources were detected by combining various existing methods, and photon extraction and source validation
were carried out using the ACIS-Extract software.
Results. The EWOCS X-ray catalog comprises 5963 validated sources out of the 9420 initially provided to ACIS-Extract, reaching a
photon flux threshold of approximately 2 × 10−8 photons cm−2
s
−1
. The X-ray sources exhibit a highly concentrated spatial distribution,
with 1075 sources located within the central 1 arcmin. We have successfully detected X-ray emissions from 126 out of the 166 known
massive stars of the cluster, and we have collected over 71 000 photons from the magnetar CXO J164710.20-455217.

The technology uses reclaimed CO₂ as the dyeing medium in a closed loop process. When pressurized, CO₂ becomes supercritical (SC-CO₂). In this state CO₂ has a very high solvent power, allowing the dye to dissolve easily.
The debris of the ‘last major merger’ is dynamically young
The Milky Way’s (MW) inner stellar halo contains an [Fe/H]-rich component with highly eccentric orbits, often referred to as the
‘last major merger.’ Hypotheses for the origin of this component include Gaia-Sausage/Enceladus (GSE), where the progenitor
collided with the MW proto-disc 8–11 Gyr ago, and the Virgo Radial Merger (VRM), where the progenitor collided with the
MW disc within the last 3 Gyr. These two scenarios make different predictions about observable structure in local phase space,
because the morphology of debris depends on how long it has had to phase mix. The recently identified phase-space folds in Gaia
DR3 have positive caustic velocities, making them fundamentally different than the phase-mixed chevrons found in simulations
at late times. Roughly 20 per cent of the stars in the prograde local stellar halo are associated with the observed caustics. Based
on a simple phase-mixing model, the observed number of caustics are consistent with a merger that occurred 1–2 Gyr ago.
We also compare the observed phase-space distribution to FIRE-2 Latte simulations of GSE-like mergers, using a quantitative
measurement of phase mixing (2D causticality). The observed local phase-space distribution best matches the simulated data
1–2 Gyr after collision, and certainly not later than 3 Gyr. This is further evidence that the progenitor of the ‘last major merger’
did not collide with the MW proto-disc at early times, as is thought for the GSE, but instead collided with the MW disc within
the last few Gyr, consistent with the body of work surrounding the VRM.

- 3. Abstract Background: Tramadol induced disorder is a new problem. Objective: To explain the efficacy of low dose of clonidine, baclofen and ibuprofen (NSAID) in the treatment of severe and chronic tramadol dependency (2000 mg daily). Method: To evaluate the usefulness of non-narcotic drugs in the management of severe tramadol withdrawal symptoms. Results: Clonidine 0.3 mg, baclofen75 mg and ibuprofen 1200 mg per day is very effective in the management of severe tramadol withdrawal symptoms and craving. Discussion: Our findings indicates that low dose of clonidine, baclofen and ibuprofen (NSAID) is beneficial in the treatment of severe tramadol dependency. This is a fascinating result. Conclusion: To our knowledge dramatic effects combination of clonidine, baclofen and ibuprofen in these situations has not been published yet, and this finding is a considerable addition to the literature.
