The task is to present a comprehensive analysis of the case which includes identifying information, symptoms and problems, hypotheses regarding the presenting problem, a multi-axial diagnosis (along with disorders that were ruled-out), the type of treatment (therapy and/or medication) the client should receive, and relevant cultural considerations. In addition, the presenters should include a slide that lists additional questions that would help to treat the client and/or that would provide clarity regarding the presenting problem.
Students will be graded on correct diagnosis(es) for each axis, including principle, deferred and/or differential, whether each diagnosis or lack of a diagnosis was clearly justified based on clinical criteria, and if important features of the client’s symptoms/behaviors were clearly identified and insightfully analyzed. The presentation will also be graded for content, quality of presentation, presentation skills (e.g., level of comfort, knowledge of subject, etc.), and number of errors in mechanics, usage, grammar, and spelling. Breakdown of points as follows:

Differential diagnosis
refers to all of the diagnostic categories that you seriously considered during the diagnostic process. Because the symptoms present in the case study suggest the possibility of several disorders, a thorough discussion of disorders that you excluded is warranted. In other words, you should discuss why you assigned the diagnoses that you did and why you ruled out others.
Multi-axial Diagnosis Format
You can have multiple diagnoses on any axis. It is also possible that there is no diagnosis on an axis. List every diagnosis for which the diagnostic criteria are met. When no diagnosis exists for a particular axis, “No Diagnosis” is entered on the line. The first diagnosis listed on Axis I is assumed to be the principal diagnosis unless otherwise specified. If the principal diagnosis is a Personality Disorder or Mental Retardation, it should be listed on Axis II, labeled as the “Principal Diagnosis” in parentheses.
Axis I:
Includes all of the disorders we will cover in class, with the exception of Personality Disorders and Mental Retardation.
Axis II:
Includes only Personality Disorders and Mental Retardation
Axis III:
Includes general medical conditions that are relevant to Axis I and Axis II diagnoses.
Axis IV:
Includes a listing of any relevant psychosocial and environmental problems or stressors.

The parasympathetic nervous system is responsible for maintaining ordinary process which is vital for a normal life. Some of the key functions of the parasympathetic nervous system include regulating digestion, sexual arousal, lowering the blood pressure and slowing the heart rate. The parasympathetic nervous system, therefore, plays an important role when it comes ensuring that the body is well monitored and it is able to carry out its normal daily activities (Colman, 2006). This paper is going to examine the importance of parasympathetic nervous system on stimulation through cuddling and massage. The parasympathetic nerves touch is some of the automatic reaction that the body undergoes when they are activated. When someone is cuddling or massaged, the physical touch activates the pressure receptors in the body and instantly, signals are sent to the vague nerves. The vague nerve is an important parasympathetic t element in our parasympathetic system, this is because it acts as a safety valve. When one is cuddled or massaged and carry out is under high pressure, the vague nerves relax and sends the message to the entire body through the nervous system, thus reducing blood pressure and one feels calm and secure.
What happens when the parasympathetic nervous system gets activated?
During cuddling from a lover hormone are released, this is through the activation of chemicals in the bodies of those cuddling. The chemicals in the brains play a crucial role, this is because they are the neurotransmitters, the chemicals include; dopamine and oxytocin. Oxytocin, is the hormone that is activated during cuddling. When lovers are cuddling the levels of oxytocin hormones tend to increase, this makes the body to be warmer, fuzzy and makes one to be in a good mood.
What benefits do we get?
There are several benefits that can be derived from parasympathetic nervous system when two lover cuddle or touch. The benefits include: Improving self-esteem and reducing stress and anxiety. Improving self-esteem occurs when the hormones are released and they calm the body, therefore, the body is able to sense security and safety. Studies have been able to show that lovers who often cuddle and hug each other tend to have high self-esteem (Colonnello, 2016). Improved self-esteem can be observed from social interaction and connection and image that is portrayed within the public. Reduced stress and anxiety, on the other hand occur when one is hugged or cuddle to feel better. The brain will naturally produce dopamine which makes the body to relax, dopamine will reduce the pressure and make one feel relaxed (The Autonomic Nervous System, 2016). The purpose of the Case Write-Up Assignment is for your instructor to "see" what you are doing in clinical and "see" how you are making clinical decisions. For these write-ups, you will select a patient seen in your current clinical rotation. You will “write-up” the visit, omitting any identifying patient factors. Ensure your write-ups demonstrate comprehensive advanced practice thinking and not just the new skills of ordering and prescribing.
Make sure to start “fresh”. Do not copy and paste from any examples, templates, other students work or even your own work. Put all your old case write-ups away and give your brain a chance to formulate the note so that it really becomes a part of what you know. THAT will make you a competent NP.
Be honest in your write up. If you realize that you have forgotten to assess something or forgot a certain part of the teaching, add an addendum at the bottom of the write-up saying what should have been done. Your clinical faculty do not expect perfect write-ups, but do expect that you use every patient encounter and subsequent write-up as a time to learn and to evaluate and improve your own practice.
If your preceptor orders something that is not appropriate or fails to order something that you believe should have been part of the plan, write an addendum at the end of the write-up to let your instructor know that you are aware and what you would have done. You are not responsible for what your preceptor orders, but you are responsible for knowing the appropriate plan of care and you are responsible for knowing if a plan of care is inappropriate. You and your faculty are the only ones that see the write-up, so no feelings will be hurt. We all get set in our ways and tend to order the same thing over and over. If your readings and research indicate that another plan is more appropriate, write it as an addendum.
You are learning to practice evidence-based practice. Support the assessment AND plan with research. This can be your textbook and/or other class readings. The best way to support your write up is using a research article.
Make sure that the article is current (5 years or less old). The article can be used to support the use of the medication (or other therapy) for the presumptive diagnosis. When using an article, please attach the article along with the write-up into the appropriate assignment category. Subjective and Objective (S and O from a SOAP note)
Most weeks have more than one body system
A separate write up is required for each body system
Sample write ups appear in your textbook
Subjective information = questions to assess symptoms in text
Objective information = complete assessment in text
3. Format for weekly Write Ups
Perform a focused assessment on a volunteer
OK to use a pt as long as they give permission
Write up your assessment and submit by midnight on
Sunday at midnight of the week.
Format = 2 paragraphs
1st – subjective data – what the pt tells you. Use your
textbook – “questions to assess symptoms”
2nd- Objective Data –Your findings- Use your
textbook
Samples are in your text book
4. Tips
Be sure to follow the correct format. Only include the
subjective information you would gather from the review
of systems in the history and the objective data gathered
from the physical assessment. See the examples posted
under “class resources”.
Don’t use “Appears or Seems” in your write ups. They
make you look uncertain of your findings and are old
fashioned.
Don’t use “normal or WNL”. Fully describe your findings
even if they are normal.
5. Tips
Don’t use descriptive words that can mean different
things to different examiners like “good, average or
moderate”. Give precise information.
Don’t use lay terms like “ear drum, belly button or arm
pit”
Be careful of words that sound similar. Ie “Ancillary and
axillary or palpable and palatable”.
Stay away from descriptive words that can be interpreted
as judgmental such as “nice, pretty, God fearing”.
Be sure to fully describe any abnormal findings.
6. Tips
Measure anything that can be measured such as “scars,
edema or birthmarks”.
Be complete and comprehensive in your write up.
Don’t include interventions in the focused write up.
Don’t use assessments taken from patient charts.
You don’t have to write in full sentences but make sure
your write up is easy to read by using formatting such as
separate paragraphs, underlining, bolding or bullet
points.
7. Tips
Stay in third person in write ups. Don’t use “I” to refer to
yourself.
Put pt. comments in quotations.
Don’t narrate the assessment
