who sustained cumulative trauma due to repetitive movement caused by delivering
fuel. Per OMNI, the patient was diagnosed with cervical spondylosis, myalgia,
testicular dysfunction and depression. The patient is status post right shoulder
surgeries on 7/26/04 and on 7/06/06 and left elbow surgery on 01/05/10 as per
OMNI notes. Per medical report dated 2/5/16 by Dr. Wilson, the patient s medications
include Protonix, Lasix, trazodone, Paxil, lisinopril, Norvasc, albuterol, Percocet,
cephalexin, Lidoderm 5%, and Benadryl. Per medical report by Dr. Wilson dated 2
/18/16, the patient presents for monitoring and management of the implanted spinal

Pain is associated with numbness, weakness, pins and needles and skin sensitivity
to both arms and hands and both legs and feet. There is loss of bowel control. He
continues to use Percocet to manage his pain (30% pain relief) and allow him to
function throughout the day which includes walking and getting to appointments.
He does require albuterol at times. He takes Paxil which stabilizes his moods. He
reports the Benadryl was added to his regimen to help with withdrawals as he
comes off his pain medication. It was noted that adverse reactions which include
trouble sleeping and constipation are relieved by stool softener. Patient has a
history of drug abuse. He is currently pending inpatient detoxification and this has
allowed lowering the dose on his pain pump. He also uses Lidoderm patches along
the midline of his lower back. This helps to relieve pain which is residual to the
Percocet and Paxil. MD has been reducing his spinal infusion pump. He is doing
well with the reductions. He has received his new walker and a power wheelchair.
