The chosen theorist for this discussion is Madeleine M. Leininger. According to Butts and Rich (2015), Leininger was the first nurse to obtain a PhD in cultural and social anthropology. This theory is concerned with on how children with cultural differences received a lack of or difference in healthcare. Leininger identified several concepts and theories in nursing and anthropology and then used these to develop the Cultural Care Theory of Diversity and Universality. Ray (2012) stated “Her theory is one of the few nursing theories that offer a translational/implementation science approach to transcultural care in practice” (p. 7). Relationship between the model and concepts in nursing metaparadigm are as follows, nursing- the profession acknowledges cultural considerations are utilized when care is delivered. The person; every human deserves high quality care, regardless of cultural differences. Cultural care exists to promote health across the health-wellness continuum. If all needs are met, including cultural needs, health and wellness are achieved. In regards to environment, if cultural conflict occurs, it may lead to patient noncompliance with the plan of care. “Leininger has long maintained that it is a human right to have one’s cultural values, beliefs, practices and needs respected, understood and appropriately used within any caring or curing context” (Wehbe-Alamah, 2008, p. 84). This model is useful as countries around the world, in general, are becoming multi-cultural. This model would be useful in both the educational and practical applicatoin aspect of nursing. It is critical to determine the patients’ learning needs on admission. This assists the nurse in planning educational and discharge material to be utilized in the planning process. Culture Care Theory: A Major Contribution
to Advance Transcultural Nursing
Knowledge and Practices
MADELEINE LEININGER, PhD, LHD, DS, CTN, FAAN, FRCNA
This article is focused on the major features of the Culture
Care Diversity and Universality theory as a central contrib-
uting theory to advance transcultural nursing knowledge and
to use the findings in teaching, research, practice, and consul-
tation. It remains one of the oldest, most holistic, and most
comprehensive theories to generate knowledge of diverse and
similar cultures worldwide. The theory has been a powerful
means to discover largely unknown knowledge in nursing and
the health fields. It provides a new mode to assure culturally
competent, safe, and congruent transcultural nursing care.
The purpose, goal, assumptive premises, ethnonursing
research method, criteria, and some findings are highlighted.
This article is focused on the 2001 Pittsburgh Precon-
ference theme “Major Contributions of Book Authors to
Transcultural Nursing Knowledge and Practices.” As the
founder of the discipline and author of 28 books and 220 pub-
lished articles, I hold that my Culture Care Diversity and Uni-
versality theory has made a significant contribution to estab-
lish and advance transcultural nursing research knowledge
and practice since the mid-1950s. This article is a brief synop-
sis of culture care theory with its unique features and major
contributions to support transcultural nursing as a discipline
and practice field.
In establishing this new discipline, different lines of think-
ing and practice were essential. It necessitated futuristic
vision, risk taking, commitment, patience, and leadership to
challenge many traditional nursing ideas and practices.
Unquestionably new knowledge and practices were essential
for nurses to function in a rapidly changing multicultural
world. Substantive theory-based research knowledge was
greatly needed with a global and comparative focus to care for
people of diverse cultures. Culturally based care knowledge
was the major missing area in nursing in the mid-20th century
and still is in some places in the world. I coined the construct
of culturally congruent care, which is the central goal of the
theory.
In my first two books,Nursing and Anthropology(1970)
and Transcultural Nursing: Concepts, Theories, Research,
and Practice(1978), the nature, rationale, need, and theoreti-
cal base were given to establish transcultural nursing. Nurses
needed in-depth knowledge of cultures with an anthropologi-
cal view and in-depth, culturally based care phenomena. I
held that care was the essence of nursing and had meaning
within cultural contexts. Care was not fully known and valued
in nursing, and so it was a challenge to get nurses interested in
the Culture Care theory in the 1950s and 1960s as the medical
mind-body treatments and symptoms held nurses’ interests

Leininger explains her belief in this article that the quality of life is derived from her theory of culture care diversity and universality. She tries to demonstrate how medical attendants should adopt a transcultural nursing perception in improving the quality of life as compared to how it has been perceived in most of the traditional and patterned descriptions. It is complex when one tries to understand the quality of life, and thus, different approaches of culturally patterned care are used to describe the conditions and expressions of humans. The symbolic, expression and meaning referents in most cases are influenced by diversity in humans. According to Leininger, the quality of life should be understood from an inside culturally patterned establishment in order for the results to be accurate and reliable (p 28). The underlying challenge existing in nursing practice is to help health care providers distinguish and identify the inside and outside patterned expressions and meanings linked with the quality of life to help nurses make sound treatment decisions, behaviors and counseling.
In summary, the article revealed that the quality of life is in a large part described by culturally patterned and articulated through our way of living as well as the prevailing cultural ideas. Leininger concludes by stating that, what determines the quality of life is not universal. However, further research is needed to validate and understand more the meaning of quality of life.

TRANSCULTURAL NURSING THEORY
I. BACKGROUND: OVERVIEW
A. Born in 1925, died 2012 in Omaha, Nebraska, Madeleine Leininger founded the theory of transcultural nursing also known as the culture care theory that was developed into a nursing discipline (TNS, 2015).
B. One of the first nursing theorists, her philosophy was rooted in a Buddhist parable that proclaims that only when the observations of all are combined, does a clear image emerge (McFarland, Wehbe-Alamah, Wilson, & Vossos, 2011).
C. As a frame of reference, starting in the 1950s (before the entire nation had electricity) Madeleine Leininger had the vision that the world was becoming a global community (Madeleine-leininger.com, 2015).
D. Leininger believed that anthropology and nursing shared many aspects, and had a vision to blend these fields, leading to her theory: Diversity and Universality: A Worldwide Theory of Nursing (Florida Atlantic University (FAU) Christine E. Lynn College of Nursing, 2011).
II. BACKGROUND: THEORY DESCRIPTION
A. Transcultural nursing theory
1. The Theory:
A comparison of culture to study similarities and differences across different cultures. Culture universals are similarities across groups, and culture-specifics are differences among groups (Leininger, 1991).
2. The purpose:
a. To develop an understanding and respect, and to acknowledge the diversity of patients in the context of culture (Madeleine-leininger.com, 2015).
b. To examine the cross cultural impact on outcomes, health, illness, practice, knowledge, workforce, education, communication, technology, leadership, policy, advocacy, research and empowerment (Madeleine-leininger.com, 2015).
c. To guide nurses to reflect on our own culture to incorporate effective care of people of cultures outside of our own, and care for them in the context of culture (Madeleine-leininger.com, 2015).
d. To guide the discipline of nursing to take accountability for its fundamental impact on social justice and human rights and the cultural aspects of healing (Douglas, Pierce, Rosenkoetter, Pacquiao, Callister, Hattar-Pollara et al., 2011).
III. BACKGROUND: ORIGINS
A. In the period following world war II, many immigrants and refugees from diverse cultures were moving to the U.S and to other places worldwide (Nursing Theories, 2011).
B. This necessitated the incorporation of knowledge of culture into nursing as a sound basis for the new discipline of transcultural nursing (Nursing Theories, 2011).
C. Different cultures perceived, identified, and practiced care in different ways, yet there were some commonalities about care among all cultures of the world (Nursing Theories, 2011).

Grand Theories
Theories can be classified based on their levels of abstraction. The scope of grand theories is very broad and, therefore, they don't lend themselves easily to application and testing. Grand theories are less abstract than conceptual models, but the concepts that compose them are still relatively abstract and general, and the relationships cannot be tested empirically (Fawcett, 1995)1.
Let's begin our exploration of grand theories with Sr. Callista Roy's adaptation model. The principal premise behind this model is that individuals are adaptive systems that cope with change through adaptation, and nursing helps to facilitate this adaptation of individuals during health and illness.
Middle Range Theories
Middle-range theories originate from grand theories. They are less abstract and comparatively more focused and narrower in scope, which implies that they are made up of concepts and propositions that are testable. The development of the concept of caring as the central concept to nursing gave rise to several theories focused on caring. The most prominent of these caring theories is Madeleine Leininger's theory of culture care.

Culture plays an important role in every healthcare setting. Integrating culture with care is an effective way of guiding thought processes, actions, and decisions within that culture.
Leininger's theory takes into account the cultural beliefs and values of individuals and groups to provide satisfying and culturally congruent nursing care. Culture exhibits both diversity and universality. While diversity includes different ways of practicing care, universality refers to the common elements in care. While adopting this model, it is important to incorporate cultural differences into nursing assessments, interventions, and care plans.
The culture care theory stresses the importance of cultural care and the need to be sensitive to the role of culture in health and healing. Consider this case. A South East Asian female patient admitted to a U.S. hospital for gynecological complaints felt that she was in a hostile healthcare environment, in the midst of caregivers who were not friendly or sensitive to her needs. Everything, starting from the admission procedure, routine questioning, and physical assessment to the diet she was served, sent out negative signals to the patient. Had the attending nursing and other healthcare professionals been sensitive to her cultural differences and taken the trouble to make her feel comfortable, this turn of events could easily have been avoided. Culturally competent nurses can create positive environments where patients feel accepted and their cultural norms, beliefs, and practices are respected.

Special Feature
Some may question the role of nursing theory and related
paradigms in the age of sustainable development. True, what
is needed now is action—measurable change with a focus on
outcomes showing improved health and wellbeing and a
demonstrable commitment to equity, inclusion, universal
respect, and environmental thriving. The profession needs
leaders to advance nursing’s political voice and harness
mutually beneficial, long-term partnerships with stakehold-
ers in the economic, financial, agricultural, energy, and busi-
ness sectors. One tangible goal, being promoted through
campaigns such as Nursing Now (n.d.), is to have a nursing
presence at all decision-making tables related to health pol-
icy that addresses the myriad determinants of health. With
2020 claimed as the Nurse and the Midwife (World Health
Organization [WHO], 2019), we must not only honor our
skills to adapt, collaborate, and progress but also celebrate
our historical roots that establish nurses as sojourners with
society and lightkeepers in a chronically dehumanizing
healthcare system.
Why is talk of caring, integrality, and a humanistic nursing
lens essential in the context of the 17 United Nations (UN)
Sustainable Development Goals (SDGs) (United Nations,
2016) (see Figure)? Theory-guided practice continues to
inform discipline-specific care as well as agendas of
advocacy. Nurses across settings and in all nations worldwide
play pivotal roles in the global attainment of the SDGs
through acts of concerned citizenship and professional lead-
ership (Dossey, Rosa, & Beck, 2019; Rosa, 2017a; Rosa,

One of the theorists that speaks to my nursing practice is Madeleine Leininger’s Theory of Culture Care Diversity and Universality otherwise known as transcultural nursing. The theory of transcultural nursing promotes understanding of both the universally held and common understandings of care among humans and the culture-specific caring beliefs and behaviors that define any particular caring context of interaction (Butts & Rich, 2018, p. 572). Culture is the specific pattern of behavior that distinguishes any society from others and gives meaning to human expressions of care (McFarland & Wehbe-Alamah, 2015). This theory is intended to view the patient as a whole being. As an APN, this theory will allow me to be considerate of my patient’s needs. In turn, they will feel respected and listened to and hopefully this will promote a long-lasting relationship.
Leininger discovered that inadequate knowledge of cultural factors represented a missing link in her ability to provide care (Nelson, 2006). As a future APN, I recognize the need and sometimes missing component of culture care nursing in our women’s health practice today. For example, in South Dakota we have a lot of Native American reservations. Since we see a number of Native American women, I think it would be beneficial for me to better understand their culture and beliefs related to healthcare in order to offer these women expert care.
Also, with Leininger’s transcultural nursing theory she believes seeing the patient as a whole and to appreciate that the patient represents a complex constellation of physical, emotional, psychological, spiritual, social, environmental, and economic factors that interact and contribute to the patient’s unique wholeness and influence their ability to participate in health promotion, health maintenance, and disease modifying activities (McFarland & Eipperle, 2008). As an APN, by viewing the patient as a whole and recognizing their diversities, I will provide better patient care in which caring will be the main focus and recognizing that their different beliefs and cultures play a part in their decision making to the different treatment options.

The demographic profile of the countries suggests that countries are rapidly becoming heterogeneous, multicultural societies. So it is imperative that nurses develop an understanding about culture and its relevance to competent care. Transcultural nursing represents and reflects the need for respect and acknowledgement of the wholeness of all human beings.

With reference to health informatics, a systematic review of literature, (Eikey, Reddy, & Kuziemsky, 2015), methods of collaborative work in health setting include telemedicine application, EMR, HER, EPR, CPOE, mobile technology, (PDAs, pagers, mobile phones), Web 2.0 applications, internet, online health communities, picture archives, and communication system, medicine dispensing devices and barcode systems, HIT and any other specific application. The authors further presented the "collaboration space model", to help researchers study collaboration and technology in healthcare.
In my healthcare setting, we use several health informatics methods such as pagers, mobile phones, communication system, medicine dispensing devices and barcode systems, but one that has been much applauded during this COVID 19 era is Telemedicine. I work in a psych/detox hospital.
We found telemedicine very relevant based on research results indicating that people with mental health problems are twice more likely to be frequent users of hospital emergency departments (Hunt, Weber, Showstack, Colby, & Callaham, 2006). The use of the emergency department for routine clinical care poses a huge strain on the health system because of the high cost of treatment, restricting use of the emergency department for admission of patients who really require admissions. This is also because of the associated complexity and pressure to admit patients for inpatient care (Institute of Medicine (U.S.), 2007).
Recent technological advancements have led to easy access to videoconferencing utilizing personal computers or tablets. A few research work has also been carried out using telemedicine. Patients are encouraged to check their vital signs using personal devices. Also, the introduction of asynchronous store-and-forward approaches enable clinicians review their patient without direct interaction. Also, telehomecare enables clinicians monitor their patients in their own homes (Yellowlees et al., 2010). A. Born in 1925, died 2012 in Omaha, Nebraska, Madeleine Leininger founded the theory of transcultural nursing also known as the culture care theory that was developed into a nursing discipline (TNS, 2015).
B. One of the first nursing theorists, her philosophy was rooted in a Buddhist parable that proclaims that only when the observations of all are combined, does a clear image emerge (McFarland, Wehbe-Alamah, Wilson, & Vossos, 2011).
C. As a frame of reference, starting in the 1950s (before the entire nation had electricity) Madeleine Leininger had the vision that the world was becoming a global community (Madeleine-leininger.com, 2015).
D. Leininger believed that anthropology and nursing shared many aspects, and had a vision to blend these fields, leading to her theory: Diversity and Universality: A Worldwide Theory of Nursing (Florida Atlantic University (FAU) Christine E. Lynn College of Nursing, 2011).

A comparison of culture to study similarities and differences across different cultures. Culture universals are similarities across groups, and culture-specifics are differences among groups (Leininger, 1991).

a. To develop an understanding and respect, and to acknowledge the diversity of patients in the context of culture (Madeleine-leininger.com, 2015).
b. To examine the cross cultural impact on outcomes, health, illness, practice, knowledge, workforce, education, communication, technology, leadership, policy, advocacy, research and empowerment (Madeleine-leininger.com, 2015).
c. To guide nurses to reflect on our own culture to incorporate effective care of people of cultures outside of our own, and care for them in the context of culture (Madeleine-leininger.com, 2015).

III. BACKGROUND: ORIGINS
A. In the period following world war II, many immigrants and refugees from diverse cultures were moving to the U.S and to other places worldwide (Nursing Theories, 2011).
B. This necessitated the incorporation of knowledge of culture into nursing as a sound basis for the new discipline of transcultural nursing (Nursing Theories, 2011).

Grand Theories
Theories can be classified based on their levels of abstraction. The scope of grand theories is very broad and, therefore, they don't lend themselves easily to application and testing. Grand theories are less abstract than conceptual models, but the concepts that compose them are still relatively abstract and general, and the relationships cannot be tested empirically (Fawcett, 1995)1.
Let's begin our exploration of grand theories with Sr. Callista Roy's adaptation model. The principal premise behind this model is that individuals are adaptive systems that cope with change through adaptation, and nursing helps to facilitate this adaptation of individuals during health and illness.
Middle Range Theories
Middle-range theories originate from grand theories. They are less abstract and comparatively more focused and narrower in scope, which implies that they are made up of concepts and propositions that are testable. The development of the concept of caring as the central concept to nursing gave rise to several theories focused on caring. The most prominent of these caring theories is Madeleine Leininger's theory of culture care.

Culture plays an important role in every healthcare setting. Integrating culture with care is an effective way of guiding thought processes, actions, and decisions within that culture.
Leininger's theory takes into account the cultural beliefs and values of individuals and groups to provide satisfying and culturally congruent nursing care. Culture exhibits both diversity and universality. While diversity includes different ways of practicing care, universality refers to the common elements in care. While adopting this model, it is important to incorporate cultural differences into nursing assessments, interventions, and care plans.
The culture care theory stresses the importance of cultural care and the need to be sensitive to the role of culture in health and healing. Consider this case. A South East Asian female patient admitted to a U.S. hospital for gynecological complaints felt that she was in a hostile healthcare environment, in the midst of caregivers who were not friendly or sensitive to her needs. Everything, starting from the admission procedure, routine questioning, and physical assessment to the diet she was served, sent out negative signals to the patient. Had the attending nursing and other healthcare professionals been sensitive to her cultural differences and taken the trouble to make her feel comfortable, this turn of events could easily have been avoided. Culturally competent nurses can create positive environments where patients feel accepted and their cultural norms, beliefs, and practices are respected.

Special Feature
Some may question the role of nursing theory and related
paradigms in the age of sustainable development. True, what
is needed now is action—measurable change with a focus on
outcomes showing improved health and wellbeing and a
demonstrable commitment to equity, inclusion, universal
respect, and environmental thriving. The profession needs
leaders to advance nursing’s political voice and harness
mutually beneficial, long-term partnerships with stakehold-
ers in the economic, financial, agricultural, energy, and busi-
ness sectors. One tangible goal, being promoted through
campaigns such as Nursing Now (n.d.), is to have a nursing
presence at all decision-making tables related to health pol-
icy that addresses the myriad determinants of health. With
2020 claimed as the Nurse and the Midwife (World Health
Organization [WHO], 2019), we must not only honor our
skills to adapt, collaborate, and progress but also celebrate
our historical roots that establish nurses as sojourners with
society and lightkeepers in a chronically dehumanizing
healthcare system.
Why is talk of caring, integrality, and a humanistic nursing
lens essential in the context of the 17 United Nations (UN)
Sustainable Development Goals (SDGs) (United Nations,
2016) (see Figure)? Theory-guided practice continues to
inform discipline-specific care as well as agendas of
advocacy. Nurses across settings and in all nations worldwide
play pivotal roles in the global attainment of the SDGs
through acts of concerned citizenship and professional lead-
ership (Dossey, Rosa, & Beck, 2019; Rosa, 2017a; Rosa,

One of the theorists that speaks to my nursing practice is Madeleine Leininger’s Theory of Culture Care Diversity and Universality otherwise known as transcultural nursing. The theory of transcultural nursing promotes understanding of both the universally held and common understandings of care among humans and the culture-specific caring beliefs and behaviors that define any particular caring context of interaction (Butts & Rich, 2018, p. 572). Culture is the specific pattern of behavior that distinguishes any society from others and gives meaning to human expressions of care (McFarland & Wehbe-Alamah, 2015). This theory is intended to view the patient as a whole being. As an APN, this theory will allow me to be considerate of my patient’s needs. In turn, they will feel respected and listened to and hopefully this will promote a long-lasting relationship.
Leininger discovered that inadequate knowledge of cultural factors represented a missing link in her ability to provide care (Nelson, 2006). As a future APN, I recognize the need and sometimes missing component of culture care nursing in our women’s health practice today. For example, in South Dakota we have a lot of Native American reservations. Since we see a number of Native American women, I think it would be beneficial for me to better understand their culture and beliefs related to healthcare in order to offer these women expert care.
Also, with Leininger’s transcultural nursing theory she believes seeing the patient as a whole and to appreciate that the patient represents a complex constellation of physical, emotional, psychological, spiritual, social, environmental, and economic factors that interact and contribute to the patient’s unique wholeness and influence their ability to participate in health promotion, health maintenance, and disease modifying activities (McFarland & Eipperle, 2008). As an APN, by viewing the patient as a whole and recognizing their diversities, I will provide better patient care in which caring will be the main focus and recognizing that their different beliefs and cultures play a part in their decision making to the different treatment options.

As someone with Attention Deficit Hyperactivity Disorder (ADHD), I have struggled with short-term memory issues for my entire life. However, since I was diagnosed later in life, I spent the bulk of my school-age years developing strategies to deal with what I thought were typical memory issues that everyone faced. By the time I entered university I knew that unless I physically wrote out my notes with a pen on paper, I would not remember them because it was too easy for me to type notes while my mind was ‘elsewhere’ not paying attention. If I needed to remember a list of things, I needed to write it somewhere I would see it repeatedly, like the back of my hand or the leg of my jeans; a list on paper in my pocket was quickly forgotten – out of sight out of mind.
The working memory model, developed by Baddeley and Hitch in 1974, focuses on the idea that working memory has a limited capacity to store information temporarily as it processes where the information is to go, such as into long-term memory to action (Baddeley, 2003). The information that is gathered falls into different subsets of the overall system, determined by the central executive: the phonological loop (spoken or written information) or visuospatial sketchpad (visual or spatial information) or the episodic buffer (a general category) (Mcleod, 2012).
Issues with short-term memory loss as related to ADHD and the working memory model comes from reported deficits in the ability to control attention and an oversaturation of information input into the central executive (Ortega et al., 2020; Mcleod, 2012). This results in information input being disorganized and not always making it to the long-term memory. Meaning, my central executive may not function in the manner in which Baddeley (2003) suggests it should and it can get easily overwhelmed, resulting in the interference of informational input and encoding. Issues with short-term memory are often considered to be as a result of decay or interference, though decay is difficult to prove due to issues with testability and overall understanding of the cause (Jonides et al., 2008).
It was explained to me that many people who have unknowingly existed with ADHD throughout their lives have developed both conscious and unconscious strategies to manage any cognitive processing issues they deal with. I mentioned writing things down as a conscious strategy that I have implemented over the years. Another strategy that I’ve used and continue to practice is repeating someone’s name back to them when being introduced. The act of saying their name out loud forces my attention to be focused on that singular piece of information, allowing for a better chance of it being appropriately encoded and therefore retrieved. Another option is to use a multisensory strategy when possible, allowing for auditory and visual input of the same information. Saying someone’s name out loud and reading it on a name tag for example. I’ve also asked people in the past .

- 1. The chosen theorist for this discussion is Madeleine M. Leininger. According to Butts and Rich (2015), Leininger was the first nurse to obtain a PhD in cultural and social anthropology. This theory is concerned with on how children with cultural differences received a lack of or difference in healthcare. Leininger identified several concepts and theories in nursing and anthropology and then used these to develop the Cultural Care Theory of Diversity and Universality. Ray (2012) stated “Her theory is one of the few nursing theories that offer a translational/implementation science approach to transcultural care in practice” (p. 7). Relationship between the model and concepts in nursing metaparadigm are as follows, nursing- the profession acknowledges cultural considerations are utilized when care is delivered. The person; every human deserves high quality care, regardless of cultural differences. Cultural care exists to promote health across the health-wellness continuum. If all needs are met, including cultural needs, health and wellness are achieved. In regards to environment, if cultural conflict occurs, it may lead to patient noncompliance with the plan of care. “Leininger has long maintained that it is a human right to have one’s cultural values, beliefs, practices and needs respected, understood and appropriately used within any caring or curing context” (Wehbe-Alamah, 2008, p. 84). This model is useful as countries around the world, in general, are becoming multi- cultural. This model would be useful in both the educational and practical applicatoin aspect of nursing. It is critical to determine the patients’ learning needs on admission. This assists the nurse in planning educational and discharge material to be utilized in the planning process.
