General principles of periodontal surgery

Surgical periodontal procedures are usually performed in the dental office.
The following findings may indicate the need for a surgical phase of therapy:
1. Areas with irregular bony contours, deep craters, and other defects usually require surgical approach.
2. Pockets on teeth in which a complete removal of root irritants is not considered clinically possible may call for surgery. This occurs frequently in molar and premolar areas.
3. In cases of furcation involvement of grade II or III, a surgical approach ensures the removal of irritants; any necessary root resection or hemisection also requires surgical intervention.
4. Intrabony pockets on distal areas of last molars, frequently complicated by mucogingival problems, are usually unresponsive to nonsurgical methods.
5. Persistent inflammation in areas with moderate to deep pockets may require a surgical approach. In areas with shallow pockets or normal sulci, persistent inflammation may point to the presence of a mucogingival problem that needs a surgical solution.
Patient Preparation
Reevaluation after Phase I Therapy.
Almost every patient undergoes the so-called initial or preparatory phase of therapy, which basically consists of thorough scaling and root planing and removing all irritants responsible for the periodontal inflammation. These procedures (1) eliminate some lesions entirely; (2) render the tissues more firm and consistent, thus permitting a more accurate and delicate surgery; and (3) acquaint the patient with the office and the operator and assistants, thereby reducing the patient’s apprehension and fear.
Premedication
For patients who are not medically compromised, the value of administering antibiotics routinely for periodontal surgery has not been clearly demonstrated. However, some studies have reported reduced postoperative complications, including reduced pain and swelling, when antibiotics are given before periodontal surgery and continued for 4 to 7 days after surgery.
Smoking
The deleterious effect of smoking on healing of periodontal wounds has been amply documented. Patients should be clearly informed of this fact and asked to quit or stop smoking for a minimum of 3 to 4 weeks after the procedure. For patients who are unwilling to follow this advice, an alternate treatment plan that does not include more sophisticated techniques (e.g., regenerative, mucogingival, esthetic) should be considered.

2. Кариес зубов – патологический процесс
неясной этиологии, возникающий после
прорезывания зубов, заключающий в
деминерализации твердых тканей с
образованием полости и протекающий при
непременном участии микроорганизмов.
7. Основной метод лечения кариеса зубов –
препарирование кариозной полости и
реставрация коронки зубов в соответствии
с анатомо-функциональными и
эстетическими требованиями.
8. Основным и самым востребованным
на сегодняшний день способом
препарирования является применение
вращающихся инструментов с

пневматического привода.
9. Препарирование кариозной полости
включает ряд последовательных этапов,
которые выполняются борами различной
формы. Боры, используемые для
препарирования, должны быть острыми,
прочно фиксироваться в наконечнике, по
форме и размеру соответствовать

Работать бором следует на высокой скорости
его вращения и прерывисто. Одно из
важнейших правил препарирования —
надежная фиксация руки врача,
удерживающей наконечник.

Метод предусматривает широкое иссечение
кариесвосприимчивых участков с созданием
полости ящикообразной формы (жевательные
зубы).
Метод показан при использовании
пломбировочных материалов, не обладающих
адгезией к тканям зуба, - амальгама, силикатные
и силико-фосфатные цементы, а также вкладки.

кариеса, стандартный подход к препарированию полости
«-» большой объем иссекаемых здоровых тканей зуба,

Метод предусматривает щадящее отношение к
непораженным тканям, иссекаются только
пораженные кариесом ткани

«+» сохранение непораженных тканей зуба,
простота, малые трудозатраты, меньшие затраты
времени
«-» недолговечность пломб из-за высокого риска
развития вторичного кариеса
20. Метод «профилактического пломбирования»
Метод был разработан после появления композитов и
стеклоиономерных цементов
Предполагает минимальное иссечение здоровых тканей и
пломбирование до «иммунных зон», т.е. сочетает
оперативное лечение кариеса, пломбирование полости,
профилактическое запечатывание фиссур и местную
флюоризацию эмали зубов.
Метод предусматривает пять возможных вариантов
(подходов) к препарированию и пломбированию
кариозной полости
21. Метод «профилактического пломбирования»
Вариант №1
ART-методика (atraumatic restorative treatment).
Предусматривает пломбирование полости без
препарирования материалами, простыми в применении и
обладающими противокариозным действием, в первую
очередь, стеклоиономерными цементами.
Техника выполнения: кариозная полость очищается
экскаватором, высушивается и пломбируется СИЦ.
ART-методика рекомендована ВОЗ (1994) для оказания
помощи жителям бедных регионов, беженцам,
эмигрантам и т.п.
22. Метод «профилактического пломбирования»
Вариант №2
В случае, если на жевательной поверхности
имеются «открытые» непораженные фиссуры и
небольшая кариозная полость, производится
щадящее препарирование согласно принципам
«биологической целесообразности» Лукомского и
пломбирование композитом и (или) СИЦ.
Методика особенно применима у пациентов с
низким индексом КПУ и высоким уровнем
гигиены полости рта.
23. Метод «профилактического пломбирования»
Вариант №3
В случае, если на жевательной поверхности
имеются «закрытые» глубокие непораженные
фиссуры и небольшая кариозная полость,
производится профилактическое пломбирование
с неинвазивным закрытием фиссур. Полость
формируется до видимо здоровых тканей,
пломба накладывается не только в полость, но и
на фиссуры.
24. Метод «профилактического пломбирования»
Вариант №4
На жевательной поверхности имеются «закрытые»
глубокие, труднодоступные пигментированные
фиссуры, возможно – с начальным поражением, и
небольшая кариозная полость. В этом случае
производится профилактическое пломбирование
с инвазивным закрытием фиссур. Полость
формируется до видимо здоровых тканей,
фиссуры раскрываются бором в пределах эмали,
полость получается «неклассической» формы со
ступенчатым дном.
25. Метод «профилактического пломбирования»
Вариант №5
На жевательной поверхности имеется обширная
кариозная полость со значительным поражением
эмали и дентина. В этом случае препарирование
и пломбирование производятся в соответствии с
классическими принципами Black. Разница лишь в
том, что используются материалы, обладающие
адгезией к тканям зуба.
26. Варианты препарирования кариозных полостей в
зависимости от восстановительного материала
1. Препарирование кариозных полостей под
металлсодержащие материалы
2. Препарирование кариозных полостей под
силикатные и силико-фосфатные цементы.
3. Препарирование кариозных полостей под
композитные материалы и стеклоиономерные
цементы.
27. Этапы препарирования кариозной полости
1. Раскрытие кариозной полости:
Цель данного этапа: обеспечение доступа для дальнейших
манипуляций и хорошего обзора полости.
Инструментарий: алмазные шаровидный, цилиндрический
или обратноконусный боры.
Критерии: отсутствие нависающих краев.
28. Этапы препарирования кариозной полости
2. Некрэктомия (некротомия):
Цель данного этапа: полное удаление размягченного и
пигментированного дентина из кариозной полости.
Инструментарий: экскаватор, шаровидный твердосплавный
бор
Критерии: дентин стенок полости светлый, плотный; на дне
полости допускается оставление пигментированного, но
плотного дентина, особенно при большой глубине
полости.
29. Этапы препарирования кариозной полости
3. Формирование кариозной полости:
Цель данного этапа: придание кариозной полости формы,
которая обеспечит пломбе достаточную устойчивость и
фиксацию.
Инструменты: цилиндрический, шаровидный,
обратноконусный боры.
Критерии: ящикообразная форма полости.
30. Этапы препарирования кариозной полости
4. Финирование краёв эмали:
Цель данного этапа: создание скоса эмали, удаление
трещин и неровностей эмали, образующихся в процессе
манипуляций предыдущего этапа
Инструменты: финишный твердосплавный бор или
мелкозернистая алмазная головкой на малой скорости
без давления с обязательным водно-воздушным
охлаждением
Критерии: наличие скоса эмали под углом 45 градусов
31. Альтернативные методики препарирования
кариозных полостей
Воздушно-абразивная методика: воздействие
фокусированного потока абразивного средства (порошок
оксида алюминия или бикарбоната натрия с размерами
частиц 25-50-100 мкм) на твердые ткани зубов.
Хемомеханическая методика: последовательная
химическая и инструментальная (ручными
инструментами) обработка полостей. С помощью
химических агентов (5-10% молочная кислота,
«Каридекс» и др.) патологически измененные твердые
ткани кариозной полости размягчаются и легко могут быть
удалены ручными инструментами.

кариозных полостей
Лазерная методика
Использование твердотельных импульсных лазеров
для препарирования твердых тканей зубов.
