Osteochondroses are a group of diseases involving fragmentation and sclerosis of epiphyses or apophyses in immature skeletons. They are categorized as conditions characterized by osteonecrosis, conditions related to trauma/stress without osteonecrosis, or normal variations in ossification. Common examples include Legg-Calvé-Perthes disease of the femoral head in boys ages 4-8, Kienböck's disease affecting the lunate bone in adults ages 20-40, and Osgood-Schlatter disease causing tibial tuberosity apophysitis in active adolescents. Imaging can demonstrate pathognomonic findings that help diagnose and classify these self-limiting bone growth disorders.
This document discusses osteonecrosis and osteochondrosis. It defines osteonecrosis as bone death due to interruption of blood supply and describes its common causes and radiographic features including MRI appearance. Specific types discussed include Kienbock's disease, Preiser's disease, and Legg-Calve-Perthes disease. Osteochondrosis is defined as abnormal bone and cartilage development, with subtypes including Scheuermann's disease, Osgood-Schlatter disease, and Blount's disease caused by repetitive stress without necrosis, as well as osteonecrosis conditions. Radiographic features of each condition are provided.
Blount's disease, also known as tibia vara, is a progressive orthopedic condition that causes bowing of the legs in children. It results from a growth disturbance in the proximal medial tibial growth plate. The deformity includes varus alignment of the knees, internal tibial torsion, and leg length discrepancy. Radiographs show a characteristic wedging and irregularity of the proximal medial tibial epiphysis. Treatment depends on age and severity, ranging from bracing to corrective osteotomies. Congenital metatarsus adductus is another common pediatric foot deformity involving inward twisting of the forefoot. It is often mild and resolves with stretching, but more severe cases may require
This document provides guidelines for planning speed training for team sports. It discusses understanding the speed requirements of different sports, acknowledging the physical stimulus of games while knowing games are not optimal for fitness. It outlines considering individual factors when planning. The document recommends annual planning while writing plans in pencil, emphasizing recovery. It discusses applying the correct training tools and following best practice session guidelines. It also notes the importance of logistics, balance, and considering speed training as an ongoing process with windows of opportunity.
This document discusses hip prosthesis replacements and issues related to them. It includes a link to information on dislocated hip prostheses from an orthopedic radiology site. It also links to a Fortune article about a $1 billion verdict against Johnson & Johnson related to faulty hip implants.
Minimally invasive total hip replacementTunO pulciņš
Minimally invasive total hip replacement (MITHR) uses a smaller incision of 6-10 cm compared to the standard incision. While MITHR results in less soft tissue damage and shorter hospital stays, it also has a longer learning curve for surgeons. Early in the learning curve, complication rates are higher for MITHR. With increased experience, surgeons can perform MITHR with outcomes equivalent to conventional THR. However, the evidence does not clearly support making MITHR the standard procedure over the conventional approach. Patient characteristics and surgeon experience should guide the choice of surgical approach.
Cemented versus uncemented fixation in total hip replacementTunO pulciņš
The document discusses and compares cemented versus uncemented fixation techniques in total hip replacement (THR). Cemented fixation uses acrylic polymer to lock the bone and implant together, while uncemented implants have rough, porous coatings to allow bone ingrowth. Some advantages of cemented fixation are that it is more suitable for obese patients, has a better outcome for displastic hips, and is better for patients with osteoporosis. However, uncemented fixation has a lower revision rate within 10 years, a low risk of femoral loosening once stable fixation occurs, and does not commonly cause osteolysis. Both techniques have benefits depending on the individual patient's needs.
Anterior cruciate ligament reconstruction- allograft versus autograftTunO pulciņš
1. The document compares allograft versus autograft options for anterior cruciate ligament (ACL) reconstruction surgery. Allografts use donor tissue while autografts use the patient's own tissue.
2. There are several factors to consider for each graft including patient characteristics, surgical factors, biological incorporation, and the risk of disease transmission. Younger, high-demand athletes often due better with autografts which incorporate faster and have lower re-tear rates.
3. However, allografts can be preferable for older, lower-demand patients due to benefits like avoiding donor site morbidity and faster return to activities of daily living. Overall graft selection requires weighing these various patient and graft-specific
Presacral venous plexus bleeding in patients with pelvic fracturesTunO pulciņš
1. The document discusses presacral venous plexus bleeding in patients with pelvic fractures and various methods to stop the bleeding.
2. Techniques used to stop bleeding include pelvic packing, use of sterile metallic thumbtacks, cauterization, suturing, and ligating bleeding vessels.
3. Damage control resuscitation principles are also described, including permissive hypotension, blood product transfusion in a 1:1:1 ratio, and rewarming.
Damage control surgery is a technique used for seriously injured patients that prioritizes short-term physiological recovery over anatomical reconstruction. It involves abbreviated laparotomy to control bleeding and contamination, followed by intensive care resuscitation to correct hypothermia, coagulopathy, and acidosis, and then definitive surgery within 36-48 hours. The goal is to prevent the "lethal triad" of hypothermia, acidosis, and coagulopathy that can lead to multiple organ failure and death in trauma patients.

One of the main tests for meniscus tears is the McMurray test. Your doctor will bend your knee, then straighten and rotate it. This puts tension on a torn meniscus. If you have a meniscus tear, this movement will cause a clicking sound. Your knee will click each time your doctor does the test.
PARLASIī info avotu par th
The outside one-third of the meniscus has a rich blood supply. A tear in this "red" zone may heal on its own, or can often be repaired with surgery. A longitudinal tear is an example of this kind of tear.
In contrast, the inner two-thirds of the meniscus lacks a blood supply. Without nutrients from blood, tears in this "white" zone cannot heal. These complex tears are often in thin, worn cartilage. Because the pieces cannot grow back together, tears in this zone are usually surgically trimmed away.
Along with the type of tear you have, your age, activity level, and any related injuries will factor into your treatment plan.

Laterālās (ārējās) kolaterālās saites bojājumi biežāk sastomai sporta aktivitātēs, kad pārāk liels spēks iedarbojas uz ceļa locītavas iekšpusi, radot ārējo saišu bojājumus.
Mediālo (iekšējo) kolatērālo saišu bojājumi visbiežāk sastopami piemēram futbolā, kad trieciens pa kājas ārpusi rada iekšējo saišu plīsumu.
Klāt ārstēšanu klāt par klasēm? Vai vajag klasifikāciju? Iedala līdzīgi kā acl, th vienāda, atšķirīgs izveseļošanās laiks.

The cause of Osgood-Schlatter disease (OSD) is unknown; however, theories suggest that this condition is a result of repeated knee extensor mechanism contraction that causes partial microavulsions of the chondrofibro-osseous tibial tubercle. This proposed pathophysiology is supported by the repetitive runner, jumper athletic patient population OSD. Osgood-Schlatter disease occurs most often in children who participate in sports that involve running, jumping and swift changes of direction — such as soccer, basketball, figure skating and ballet.
