Rhinoplasty, or nose surgery, is a plastic surgery procedure that involves restructuring the nasal passage to change its shape for medical or aesthetic reasons. It is one of the most popular cosmetic procedures for women ages 18 to 35. During rhinoplasty, the surgeon will either make a closed or open incision inside the nose to access and modify the underlying nasal structures. Revision rhinoplasty poses more challenges, as it aims to correct issues from a previous nose surgery, such as nasal obstruction or external valve collapse caused by unaddressed weaknesses in the lateral nasal wall cartilage.

• Rhinoplasty or nose surgery is one of the many choices for

• Almost all surgeons interview patients to determine if they are
emotionally healthy before their procedure, and have realistic
expectations of surgery. They may also show the prospective
patient before and after photos of previous work and give
them an overview of what to expect with the procedure.

• Rhinoplasty is performed under a general or local anesthetic.
There are two possible approaches to the nose: closed
approach and open approach. Skin incision for an open
rhinoplasty may be v shaped or a stair step shaped incision.
This helps the surgeon attain a precise closure and
camouflaging of the resulting scar. The incisions for a
rhinoplasty are hidden inside the nose with a small incision
across the base of the nose, depicted by a dotted line.

6. The Process Part 3
• Revision rhinoplasty surgery is the most challenging, and
often, ungratifying procedure that facial and general plastic
surgeons perform. I am sure you have all experienced the pit
in your stomach when one of your primary rhinoplasty
patients enters your office with postoperative nasal
obstruction and ovbious iatrogenic external valve collapse. You
are overwhelmed with feelings of disappointment and
bewilderment, wondering how this happened when all you
did was perform a conservative cephalic trim (leaving 7 mm)
and place domal sutures. Why did this happen? Because you
did not diagnose the flaccid, weak lateral crura. As a result,
and manipulation of the cartilage without placement of lateral
crural strut grafts might cause this easily preventable mishap.
