This document discusses the combination drug salmeterol/fluticasone propionate (Advair Diskus) which is used to treat asthma and chronic obstructive pulmonary disease (COPD). It works by fluticasone reducing airway inflammation while salmeterol acts as a long-acting bronchodilator to open the airways. The drug is indicated for asthma in patients 4 years and older and to reduce COPD exacerbations. Dosages vary based on age and maximum dosage is listed. Contraindications include acute asthma/COPD episodes. Common side effects involve the eyes, lungs and skin. More serious effects can impact the heart, brain and gastrointestinal system.
3. 1. introduction
salmeterol/fluticasone propionate is a combination in (Advair Diskus ) for
treat ashma and maintenance of airflow in patients with COPD(choronic
obstruction pulmonary disease.
( Advair Diskus
4. Fluticasone Propionate / Salmeterol
Fluticasone Propionate: it is anti inflammatory that belongs to a class of drugs known as
corticosteroids. It works by reducing the irritation and swelling of the airways
Salmeterol:
Salmeterol belongs to the class of drugs known as long-acting beta 2 agonists. It works by
opening airways in the lungs to make breathing easier. Controlling symptoms of breathing
problems can decrease time lost from work or school
5. Mechanism of action
Fluticasone:Inhibits multiple cell types (eg, mast cells)
and mediator production or secretion (eg, histamine)
involved in the asthmatic response.
Salmeterol:Produces bronchodilation by relaxing
bronchial smooth muscle through beta-2 receptor
6. Indication and uses
or the treatment of asthma in patients 4 y of patients and older
maintenance treatment of airflow in patients with COPD(chroni,
including chronic bronchitis and/or emphysema to reduce
exacerbations of COPD in patients with a history of exacerbations
7. dosage
4 years to 11 years:
-Fluticasone 100 mcg-salmeterol 50 mcg (1 inhalation) orally twice a day
12 years or older:
-1 inhalation of fluticasone 100 mcg-salmeterol 50 mcg, fluticasone 250 mcg-salmeterol 50
mcg, or
fluticasone 500 mcg-salmeterol 50 mcg orally twice a day
-Maximum dose: Fluticasone 500 mcg-salmeterol 50 mcg orally twice a day
8. contrindication
Primary treatment of status asthmaticus or other acute
episodes of asthma or COPD in which intensive
measures are required; hypersensitivity to any
component of the product; severe hypersensitivity to
milk proteins ( Advair Diskus
9. Side effect
blurred vision
cough
difficulty with breathing or swallowing
eye pain
fever
skin itching, rash, or redness
sore throat
fast, slow,, or irregular heartbeat or pulse
plapition
11. Question
Why combine b/w salmeterol/fluticasone propionate ?
For treatment copd has muscle contract and inflammatory
Why use salmeterol with fluticasone ,why not use other beta 2 agonist?
Because COPD is hard to reverse for this use long action beta agonist
Duration T ½- >12 hourse
Salmeterolol has more side effect for this con not use alone
