Minnesota's Health Care Home Initiative aims to improve coordination of care, quality of care, and control costs through care redesign and payment reform. The initiative certifies clinics as Health Care Homes and provides per-member per-month payments on a tiered structure based on patient complexity. It takes a statewide approach and involves patients and families in clinic practices and policy work. Looking ahead, officials see opportunities to build on the initiative through federal health reform programs.
