As an experienced management professional, this qualified individual would like to
utilize all the previous experience related to managing and critical decision-making. They would

Golden Living West Shore, Camp Hill, PA, December, 2009-present
While providing the basic nursing care services to the residents, the following were also some
responsibilities at the positions held. This is a facility that is licensed for 309 beds, but currently
the facility only fills 225 beds.
With this facility several positions have been held and job duties experienced
In 2015, worked closely with Post Acute Care Network with Pinnacle Health and meeting
their metrics, participating in quality committee meetings, orthopedic joint initiatives. Work as a
preferred provider for the River Health ACO, meeting their metrics with low length of stays.
Working with local acute organizations and providers on a CMS initiative for bundled payment
system in a model 3, dealing with risk for gain shares model.
July 2015 – Present : Assistant Director of Nursing
February, 2015 – June 2015: Interim Director of Nursing
 All duties as ADNS
 Wrote effective PIPs and QAPIs on hospital readmission reduction, falls, communication,
survey readiness, sepsis protocol
 Decrease hospital readmissions from 25% to 10% per month over 6 months
 Implementation of portions to EHR
 Decreased infections rates of cdiff, UTIs, and other common infections in SNF
April, 2013-January, 2015: Assistant Director of Nursing
 Supervises all staff for day to day care delivery 24 hours a day
 Scheduling – assist when needed
 Assist in orientation of new staff
 Interview potential new staff
 Ensures care of staff is provided in a manner that is consistent with company policies,
standards of nursing practices, and governmental regulations
 Oversees the delivery of nursing care in the department on all units (5)
 Assists with Plans, coordinates and manages the nursing department – the overall
direction, coordination and evaluation of nursing care and services provided to residents
 Assist in preparation of annual budget for the living center
 Performs rounds to monitor and evaluate quality and appropriateness of nursing care
 Develops and implements written staffing plan
 Monitors pharmacy charges and coordinates with physicians to make changes when
possible
 Daily monitoring of ratio staff to resident and makes adjustments daily
 Performs duties needed to coordinate care with nursing staff, therapy, and physicians
 Takes part in initiatives aimed towards bundling payments with CMS and Pinnacle ACO
May 2012 – April 2013: Resident Assessment Coordinator (RNAC)
 In house case manager considering all aspects of the resident’s care
 Accurate completion of MDS – which ultimate effect reimbursement for facility
 Communicating with DNS to identify regulatory risk, effectiveness of living center
systems that allow capture of the services provided to residents
 Directly educates staff on proper documentation
 Supervises all staff and ensures proper documentation (200 nurses and cnas)
August 2011 – May 2012: Shift Supervisor 3-11
 Supervision of the staff for the assigned shift – approximate 30-40 depending on census
 Supervising CNAs, LPNs, RNs, and other staff personnel
 Monitoring the care provided by staff

Grane Hospice Services, Camp Hill, PA, February 2012 – February 2013
Worked as the on call nurse for every weekend. This meant to prioritize and manage the
care of any patient calling the service for concerns they have about their conditions. Completion
of admissions as planned on patients who were being added to the hospice service. Played a role
on interdisciplinary team of case managers, social workers, and physicians.
Visiting Nurses Association, Harrisburg, PA, March 2012 – August 2012
Worked for the company as needed for seeing patients in the evening for either admission
or general visits, which include the following: wound care, chemotherapy hook up, and or
cardiopulmonary assessment.
HealthSouth Special Services (LTACH), Mechanicsburg, PA, December, 2005-January, 2007
Served as a LPN at facility with patients with high acuity. Assigned to be the nurse on an
LTACH unit with patients who had trachs, vents, IVs, extensive wounds and played a role on
interdisciplinary team of physicians, RNs, LPNs, CNAs and social workers. Actively
participated in development and implementation of individual treatment plans for patients with
broad range of medical issues. Ensured that doctors’ orders were effectively carried out,
including testing, medical procedures, consultations, medications, and treatments.
Country Meadows, Mechanicsburg, PA, May, 2003-November, 2005
Served, as a LPN at facility with a main resident needs were that of an assistant living
facility. Assigned to be the charge nurse on dementia unit. Actively participated in development
and implementation of individual treatment plans for patients with broad range of medical issues.
Ensured that doctors’ orders were effectively carried out, including testing, medical procedures,
consultations, medications, and treatments.

This is a facility that is licensed for 309 beds, but currently the facility only fills 225 beds. With this facility several positions have been held and job duties experienced In 2015, worked closely with Post Acute Care Network with Pinnacle Health and meeting their metrics, participating in quality committee meetings, orthopedic joint initiatives. Work as a preferred provider for the River Health ACO, meeting their metrics with low length of stays. Working with local acute organizations and providers on a CMS initiative for bundled payment system in a model 3, dealing with risk for gain shares model.
