Visiting Physicians Association-U.S. Medical Management
Practice Manager December 2014 – Present
Currently I manage 6 physicians,3 Nurse Practitioners, 2 podiatrists, 9 medical assistants and a team of
office scheduling staff. Our care is focused on delivering excellent medical care to home bound patients.
My role as a Practice Manager is to provide direct leadership to the site employees. I monitor daily
productivity, staffing and scheduling,and address patient/providerneeds quickly with a strong internal and
external customer service mind set. In addition to growing site census,monitoring financial and managing
our vehicle fleet, I am charged with identifying areas of clinical improvement and developing processes
and procedures to improve the quality of care being delivered by our providers. I have daily interaction
with home health and hospice Administrators to aid in identifying care needs, referral process
improvement, and provider relationship building.
Visiting Physicians is a part of a large healthcare continuum including physician practice, podiatry care,
hospice and home health care, laboratory and diagnostic care.
Generation Hospice
Administrator September 2013-October 2014
Generation Hospice is part of a larger healthcare continuum that consists of a skilled nursing home, home
health company, outpatient therapy services, and independent living. I passionately accepted this unique
challenge of rebuilding this organization and team, by developing organizational standards of practice
compliant with Hospice Conditions of Participation. Based on a thorough comprehensive assessment,
policies/procedures, and clinical documentation tools were developed and implemented allowing a
standardized day to day operation to occur. CAP liability for 2015 was eliminated. Census growth occurred
from an ADC of 7 to 32 by August. We had a recertification survey from the Ohio Department of Health
which was deficiency free directly as a result from the practice changes implemented by myself since my
arrival. I provided a significant amount of education not only to the front line clinical Hospice staff, but to
the entire continuum, increasing the overall understanding of Hospice care, standards of practice and state
and federal conditions of participation.
Additional responsibilities included the direct oversight of the Hospice Team, implementation of corporate
and Hospice policies, rebuilding and implementation of the QAPI program, implementation of new
mandated data collection, budget oversight, evaluation of all staff, development and care of organizational
contracts.
2. Grace Hospice-U.S. Medical Management
Administrator May 2011 – September 2013
Responsible for implementing and supervising the administrative policies and the provision of all agency
services. Including but not limited to the planning of our agency’s overall development as set by the
Conditions of Participation and the coordination of care based on state and federal regulations. I continued
to be responsible for the implementation and management of our compliance program and state survey
preparedness. I initiated and maintained all business contracts, our site budget, and the review and approval
of patient and facility invoices. I continued to be responsible for the direct oversight, supervision, and
yearly evaluations of the clinical and administrative staff. I was directly responsible for our census growth.
I was responsible for a deficiency free recertification survey by The Ohio Department of Health in 2013.
Grace Hospice-U.S. Medical Management
Director of Clinical Services November 2010-May 2011
Responsible for the direct oversight of the clinical staff. I monitored daily schedules of all staff to ensure
visit compliance and productivity. I coordinated all admissions and had a collaborative relationship with
fellow leadership to maintain budgets. I was responsible for the implementation and management of our
compliance program. I coached and mentored staff on a daily basis, teaching Hospice care, theory and
regulatory practice.
Intellatriage
Self Employed Contract Registered Nurse February 2009-November 2010
Responsible for triaging after hour calls for various hospice and homecare programs throughout the United
States. Managed calls for approximately 800 patients per night. Duties included assessing needs of patients
and families, and dispatching employees as appropriate. Proficient in many types of clinical software
including; Suncoast, Homecare Homebase, and Misys.
Zusman Community Hospice, Columbus, Ohio
Program Director 2008-2009
Responsible for direct oversight of all clinical and administrative staff within our home Hospice program
and private 5 bed inpatient unit. Collaboration with fellow leadership to maintain budgets and compliance
in accordance with NHPCO and Conditions of Participation. Implementation and management of QI
program. Directly responsible for program stability, compliance, and growth. Continued to serve as Vice
President of Licking County Aging Taskforce.
VistaCare, Columbus Ohio
Patient Care Manager 2006-2008
Managed an interdisciplinary team of clinical staff, including the management of the site medical director.
Coordinated the admissions department, processed referrals, assigned staff and screened patients for
Hospice eligibility. Elected Vice President of Licking County Aging Taskforce.

Team Coordinator 2003 to 2006
Managed an interdisciplinary team of clinical staff including: RNs, LPNs, Home Health Aides, Social
Workers and Chaplains. Coordinated and monitored monthly/daily schedules and caseloads of clinical staff
to ensure that National Hospice and Palliative Care Organization (NHPCO) standards were maintained,
completed all staff performance evaluations, facilitated IDT meetings, responsible for staff mentoring,
coaching and discipline, monitored compliance with TB testing for assigned staff, facilitated monthly
Home Health Aide meetings and provided education and in-services as needed, responded to staff and
patient calls in a timely manner, provided hospice education to healthcare facilities and community at large,
major responsibility for marketing/customer service for community healthcare partners, participated in
chart audits for quality improvement purposes, served as hospice representative for the Licking County

Managed a seven bed hospice inpatient care center located in the community hospital, supervised a staff of
RNs and Nursing Technicians, ensured all Joint Commission accreditation requirements were met on a
daily basis, completed all quality improvement indicators, completed monthly staff schedules, provided
staff education and in-services, responsible for complaint management/follow-up, maintained
positive/professional relationship with hospital staff, committee member of Quality and Education
Councils.
Triage RN 1999-2002
Provided clinical support triaging all incoming calls from staff, patients/families, and outside healthcare
professionals, coordinated monthly and daily schedules of all clinical staff, provided supervision to clinical
staff as Administrator on-call, member of interdisciplinary council to develop a drug formulary which
resulted in a significant decrease in our daily drug cost, bereavement performance improvement team
