1. Required Documents List
PRINCIPALS
We require any and all owners, partners, officers and stockholders owning 20% or more of the business to apply as a
co-borrower and personally guarantee the note. All documents below apply for co-borrower as well, if applicable.
 Recent Paystub or other proof of income - Provide proof of all income indicated on your application.
 Personal Tax Returns for two (2) prior years (or signed extension) - please include all schedules and pages
 Personal Bank Statements (last 3 months) - please include all pages
 Copy of Driver’s License
 Copy of Recent Utility Statement from Home- to confirm address
SMALL BUSINESS CONCERN
 Proof of Business Ownership – If Corporation is not to be set up by the date of closing, the following is needed: Federal Employment
Identification Number and Business License with names of owners
 Collateral Description Summary - please list any unencumbered business assets or personal assets that you are willing to pledge and
copies of titles to any vehicles, etc. that are pledged
 Year-to-Date Profit and Loss and Balance Sheet
 Business Plan
 Business Tax Returns for two (2) prior years (or signed extension)- please include all schedules and pages
 Business Bank Statements (last 3 months)- please include all pages
 Copy of Recent Utility Statement from Business- to confirm address
 Debt schedule- attachment included
 Corporate Borrowing Resolution – attachment included
 Loan Request Detail– attachment included
