It is a written order by registered physician to the
pharmacist with the direction for the preparation or
dispensing of the prescribed drug and their use by the
patients.

 Prescription for already prepared drug
 Medicine is already available in a form suitable for
administration
 98% of the prescriptions are of this type
Compounded prescription
 In which the physician select the drug, dose and pharmaceutical
form and Pharmacist prepare the medicine.
 Less than 2% prescription
All Written Prescription Should Contain
 Full name and address of the Patients
 Prescribers full name, address, telephone number, and registration
numbers
 Date of issue
 Dosage form, Name, Strength, Dose interval, Duration for which drug is
to be taken
 Direction for the pharmacist
 Direction of use
 Refill instruction

Superscription
 is said to be an abbreviation for the Latin
word ‘’recipe’’, meaning "take" or "take this
 Also considered as sign of Greek gods of healing.

 Body of prescription
 It gives the name and quantities of the ingredients
 Dosage form{Cap/Tab/Inj/mix/syrup}
 Name of drug
 Strength
 Dose interval
 Duration for which drug is to be taken
 Basis :- Main drug responsible for desired effect
 Adjuvant:- What should be added to promote the effect of main drugs
 Corrective:- What to be added to minimize the undesirable effect of the main
drugs/adjuvant drug.
 Vehicle:- Preparing the drug in requisite amount
Part of Inscription
 In case of Pre-compounded prescription
 Drugs can be written in brand/proprietary
name/generic name
Subscription
 Provide direction to pharmacist regarding fulfilling the
inscription.
 Usually subscription is brief and included the quantity and
dosage form of drug to be dispensed.
Direction to the patients/Transcription
 Direction to the patient contains instruction about the amount of drugs, time,
frequency of dose to be taken
 Special Instruction Like-
 For external use only
 Not to be gulped
 Keep away from child
 Shake well before use
Signature of the prescriber
 Prescription should be signed by the prescriber
 Doctor is expected to write his/her registration number
 Usually below the direction to the patients on the right side

Prescription writing

  • 1.
  • 2. Prescription It is a written order by registered physician to the pharmacist with the direction for the preparation or dispensing of the prescribed drug and their use by the patients.
  • 4. Pre-compounded prescription:-  Prescription for already prepared drug  Medicine is already available in a form suitable for administration  98% of the prescriptions are of this type
  • 5. Compounded prescription  In which the physician select the drug, dose and pharmaceutical form and Pharmacist prepare the medicine.  Less than 2% prescription
  • 6. All Written Prescription Should Contain  Full name and address of the Patients  Prescribers full name, address, telephone number, and registration numbers  Date of issue  Dosage form, Name, Strength, Dose interval, Duration for which drug is to be taken  Direction for the pharmacist  Direction of use  Refill instruction

  • 8. Particulars of Doctors :- Name - Address - Mobile no - Qualification - Reg. no - Date - Particulars of patient :- Name – Age – Sex - Weight – Address – Occupation - Superscription
  • 9.  is said to be an abbreviation for the Latin word ‘’recipe’’, meaning "take" or "take this  Also considered as sign of Greek gods of healing.
  • 10. Inscription  Body of prescription  It gives the name and quantities of the ingredients  Dosage form{Cap/Tab/Inj/mix/syrup}  Name of drug  Strength  Dose interval  Duration for which drug is to be taken

  • 12.  In case of Pre-compounded prescription  Drugs can be written in brand/proprietary name/generic name
  • 13. Subscription  Provide direction to pharmacist regarding fulfilling the inscription.  Usually subscription is brief and included the quantity and dosage form of drug to be dispensed.
  • 14. Direction to the patients/Transcription  Direction to the patient contains instruction about the amount of drugs, time, frequency of dose to be taken  Special Instruction Like-  For external use only  Not to be gulped  Keep away from child  Shake well before use
  • 15. Signature of the prescriber  Prescription should be signed by the prescriber  Doctor is expected to write his/her registration number  Usually below the direction to the patients on the right side
  • 16. WHEN CHOOSING DRUGS TO USE CONSIDER THE FOLLOWING  What is the diagnosis ?  What are you trying to achieve ?  Make a list of possible drug classes  Compare them according to safety and efficacy suitability and cost  Select a drug class for this situation.  Compare drugs within the class.  Select a drug for this situation.
