Inspite of more than 60 years history of orthopedics in India we have not been able to define optimum and the current state of bone health in our population. We have not identified the factors responsible for poor bone health and evolve state and national level strategies to improve the bone health of the nation. The poor bone health affects the work potential and performance thereby reducing productivity and adds a tremendous musculoskeletal disease burden in late youth due to weak bones. It is not only a clinical burden needing treatment but also causes a huge loss of earnings/man-day. Surgeons in India want to improve the healing times in their patients while patients in turn are willing to spend money on technologies that work. Ultimately, identifying the optimal fracture healing technology for the Indian population is of significant interest to the health care community.

The consequence of disproportionate distribution and placement of Doctor in Indonesia is affecting people who live in rural area. Patient have to travel to city to receive medical treatment and must encounter another different problem such as: patients often have to wait a long time in the doctor's office even sometimes do not get the service because of holiday or rejected because the queue is full. Medical record in some cases may lost due in Indonesia mostly medical record recorded manually (paper based). Therefore doctor treatment is not optimal because doctor can no longer inspect patient illness history and any treatment that have been conducted before. This research proposes a new concept to help people who live in rural area to get better medical treatment. People could register and monitor doctor service queue via smart phone. System expanded with medical record management facilities to improve service quality of patient. Research object was doctor service in Sulawesi, Indonesia. This research indicates system could increase time efficiency, energy, and cost efficiency for patient and also the doctor. Additionally current system will be optimal if supported with stable internet network.

The first-round seat allotment data of the National Eligibility-cum-Entrance Test-Post Graduation (NEET-PG) 2020 was published in April 2020 [1]. Apart from a few institutions having their own selection processes, this competitive examination has acted as the sole entrance to the majority of residency positions in India, since NEET-PG 2017. Additionally, since NEET-PG 2019, ranks here have served as the basis of Diplomate of National Board (DNB) counseling, adding even more significance to the outcomes of this exam. As of 2020, there are 299 institutions in India imparting the training and degree of Master of Surgery in Otorhinolaryngology [MS (ENT)] across 1151 seats [2]. We note with concern that students with the highest all-India ranks (AIRs) in the NEET-PG, a metric that only takes examination score into account, have continued to completely forsake ENT, similar to the previous years (Fig. 1). Amongst top-100 AIRs, not a single student chose ENT, as has been true for the past years. Over 89% of the top-100 AIRs this year gravitated towards ‘medical’ specialties. Similarly, amongst the top-500 AIRs, only one examinee chose ENT, with 78.8% of candidates preferring these four ‘medical’ fields: internal medicine, radio-diagnosis, pediatrics, and dermatology. Even amongst the top-2000 AIRs, merely four candidates chose ENT [1].

Accordingly, researchers at the Milken Institute undertook a comprehensive, quantitative documentation of medical technology's impact on the economic burden of disease. The study also projects how future innovation in this sector would affect the health care system and the larger economy -- a positive benefit of more than $23 billion a year for the United States.
The study takes a systematic approach to documenting the full costs and broader economic benefits of health care investments by examining innovations pertaining to four prevalent causes of disability and death: heart disease, diabetes, colorectal cancer, and musculoskeletal disease. The report considers therapeutics and diagnostic devices that are widely used and have substantially affected the lives of patients as well as the overall U.S. economy. Among the 10 devices or device-based procedures studied are pacemakers, insulin infusion pumps, colonoscopies, and joint replacement surgery.
