This study analyzed phenotypic virulence factors and antibiotic resistance patterns in 156 uropathogenic E. coli (UPEC) isolates from patients with urinary tract infections (UTIs). The researchers found that 85.3% of isolates produced biofilm and 34% produced hemolysin. 62.8% exhibited mannose-sensitive hemagglutination (MSHA) and 37.2% exhibited mannose-resistant hemagglutination (MRHA). Biofilm formation correlated with infection type. Total 453 urine samples were included in this study. Urinary isolates from symptomatic UTI cases were identified by conventional methods. Of the 453 processed samples 166 samples showed significant colony count of pathogens among which the most prevalent were E. coli (49.39%) followed by Klebsiella species (7.83%). The majority of the isolates were from female (68.67%) while the remaining was from male (31.32%). Dysuria was the most common clinical presentation followed by fever and abdominal pain. Diabetes and urogenital instrumentation were the major risk factors for UTI. Among the 166 urine samples which showed significant colony count, 152 (91.56%) of specimen showed pus cells in wet film examination. Among the gram-negative enteric bacilli high prevalence of resistance was observed against Ampicillin, Cefotaxime, Ciprofloxacin, Nalidixic acid and co-trimoxazole. 44% of isolates were detected to produce ESBL among the gram negative bacteria. Carbapenemase production was seen in 13 (11.71%) isolates. Among the 32 Enterococcus isolates 14 (43.75%) were resistant to High level Gentamicin, 2 (6.25%) were resistant to High level Streptomycin while 12 (37.50%) of isolates were resistant to both of the antimicrobial drugs. Among the 16 Staphylococcus species, 8 (50%) were MRSA.

Multidrug Resistance Pattern of Staphylococcus Aureus Isolates in Maiduguri M...Scientific Review
Multi drug-resistant (MDR) isolates of Staphylococcus aureus are on rise and are becoming a challenge for timely and appropriate treatment. The present study was carried out with an objective to isolate Staphylococcus aureus from clinical samples and determine their sensitivity. Out of 110 samples collected, 44 were shown to contained S. aureus. The isolates were subjected to antibiotic sensitivity tests using 10 different and commonly used antibiotics by modified Kirby- Bauer disc diffusion technique. Out of the total isolates (42) tested, only 7.1% were susceptible to all the antibiotics. Multiple resistance was eminent in over 92% with highest occurrence in 4.8% where the entire antibiotics were resisted. Multiple antibiotic resistance indixes (MAR index) indicated that 0.6 index occurred most (23.8%) followed by 0.5 (19.0%). On the other hand, 0.1 and 0.8 indexes were the lowest with 0.0% and 1.0% occurrence respectively. Ciprofloxacin was resisted by most of the organisms (64.3%) while amoxicillin (64.3%) and streptomycin (61.9%) were most efficacious. With over 90% isolate having MAR index ≥ 0.2, the multiple drug resistance by the S. aureus is quite alarming and might suggest inappropriate antibiotic usage by the sampled population. Therefore, the need to strategize the nature of antibiotic treatment against S. aureus and massive campaign on indiscriminate antibiotic use is urgent.
A total number of 74 coagulase negative Staphylococci were isolated from orthopaedic patients in Ahmadu Bello University Teaching Hospital, Zaria, Nigeria. They were further characterized into various Staphylococci species using API STAPH identification kit: Staph xylosus (31.1%), Staph lentus (10.8%), Staph hominis (10.8%), Staph cohnii cohnii (5.4%), Staph epidermidis (4.1%) others were Staph cohnii ureal., Staph hyicus, Staph lugdunensis (2.7% each) Staph caprae , Staph capitis, Staph haemolyticus, Staph scuiri, Staph chromogenes and Staph warneri (1.4% each). Microcossus spp was 8.2% while 13.5% isolates were undetermined. Kirby Baurer disk method was used for the antibiotics susceptibility test, the result showed gentamicin and ciprofloxacin to be most active (96.6%), followed by vancomycin (93.1) and pefloxacin (87.9). The isolates were resistant to ampicillin (96.6), amoxicillin clavulanic acid (65.5%), clindamycin 41.4%). The study compared the prevalence and antibiogram of bacterial isolates from the urinary and
genital tracts of pregnant women attending ante-natal clinics in Imo State. Urine and High vaginal swab (HVS)
samples were collected from across the three geopolitical zones of Imo State (Owerri, Orlu and Okigwe).
Federal Medical Centre (FMC) Owerri, Imo State University Teaching Hospital (IMSUTH) Orlu and General
Hospital Okigwe (GHO) were used as focal points. A total of 1197 samples were obtained from women and
used. Infection was significantly more with the urine samples than the HVS samples (P < 0.05) while
polymicrobial growth was more observed with the HVS samples. Escherichia coli was the predominantly
isolated organism (38.3%) from the urine samples while Staphylococcus aureus (29.1%) was the predominant
bacterial isolates in HVS. Other commonly isolated bacterial species include; Enterococcus faecalis and
Staphylococcus epidermidis, Klebsiella pneumoniae, Proteus mirabilis and Bacteriodes were solely isolated
from urine while Lactobacillus was solely isolated from HVS. Overall antibiogram showed ciprofloxacin to be
the most effective antibiotic followed by nalidixic acid and pefloxac in for both specimens. Generally, multidrug
resistance was more in urine isolates (55.7%) than vaginal isolates (53.6%) with many showing the same
resistance patterns. The rate of multi/drug resistance in both samples is high (>50%) and worrisome. These call
for routine HVS as well as urine culture to be carried out on all antenatal women to ensure holistic antenatal care/ management.

Septic arthritis is the purulent invasion of a joint by an infectious agent which produces arthritis. The main organisms having great potential to infect human beings as well as other mammals are Staphylococcus aureus, Streptococcus pneumoniaeand Streptococcus pyrogens. In Silico comparative analysis ofall the pathways of host Homo sapiens and pathogens was performed by using KEGG and Protein BLAST. 25, 20 and 16 unique pathways were identified for Staphylococcus aureus, Streptococcus pyrogensand Streptococcus pneumonia respectively. Out of these we identified 3 enzymes for Staphylococcus aureus, 4 for Streptococcus pneumoniae and 1 for Streptococcus pyrogens, which are non-homologous to Homo sapiens proteins. The enzymes essential for survival of the pathogens were found out by DEG database. Further CELLO analysis results showed that 50% enzymes are found to be Extracellular, 25% to be cytoplasmic and 25% to be membranous for Staphylococcus aureus. For Streptococcus pneumoniae, 50% enzymes are found to be Extracellular, 12% cytoplasmic, 13% membranous and 25% as cell wall proteins. 100% enzymes were found to be membranous for Streptococcus pyrogens. Finally the enzymes from DEG were submitted in Drug Bank database to identify approve drug targets. This Data Mining approach found that mostly the enzymes which can act as targets belong to extracellular level in Staphylococcus aureus, Streptococcus pneumoniae and membranous in Streptococcus pyrogens. This findings gives an understanding of these enzymes interaction with human protein protein interaction at extracellular and membrane level.
Background and study aim: During last two decades, there has been a world-wide trend in increasing occurrence of enterococcal infections in the hospitals. The aim of present study was to determine the spectrum of enterococcal infections, species prevalence, antimicrobial and characteristics of vancomycin resistant enterococci (VRE) in a tertiary care hospital, Eastern India.
Patients and Methods: Between January 2013 and July 2014, 152 Enterococcus species were obtained from clinical samples. Enterococci were identified using standard biochemical tests. Antimicrobial susceptibility was tested by Kirby-Bauer disk diffusion according to Clinical resistance
& Laboratory Standards Institute (CLSI) guidelines.VRE agar base was used to screen VRE isolates. Minimum inhibitory concentration (MIC) values of VRE isolates were determined using Epsilometer-test. VRE isolates were also examined by PCR to detect vanA gene.
Results: From 1602 clinical samples, 961 (60%) were culture positive and 152 (15.8%) enterococcal isolates were obtained. Most common species isolated was E. faecalis (63.8%) followed by E. faecium (35.5%). Majority of enterococcal infections were detected from ICUs and surgical wards and clinically presented as UTIs. Disk diffusion method showed 67.1% were resistant to penicillin, 61.2% ampicillin, 58.5% ciprofloxacin, 46.7% high-level gentamicin, 42. 8% high-level streptomycin, 7.9% teicoplanin and none to linezolid. Twenty (13.2%) enterococcal isolates were vancomycin resistant in VRE screen and disk diffusion method. Epsilometer-test of VRE isolates showed 8 (40%) isolates were resistant and 9 (45%) were intermediately resistant. From 20 VRE isolates, six showed VanA and two VanB phenotypes and all six VanA phenotypes had vanA gene cluster.
Conclusion: More accurate and reliable MIC determination tests should be performed in all suspected VRE isolates. Confirmatory PCR is required for identifying resistant gene cluster.

Background: Extended-Spectrum Beta-Lactamases - producing Enterobacteriaceae are common in hospitals. This study aims to describe the antibiotic resistance of these bacteria and their associated demographic and clinical factors. Methods: It was a prospective study of 73 isolates of Extended-Spectrum Beta-Lactamases - producing Enterobacteriaceae for a period of six months from July to December 2019 in the laboratory of Befelatanana. Results: This study showed 73 (6.3%) isolates of Extended-Spectrum Beta-Lactamases- producing Enterobacteriaceae, represented by 25 (34.2%) isolates of Klebsiella spp, 24 (32.9%) isolates of Escherichia coli, 22 (30.1%) isolates of Enterobacter spp and 2 (2.7%) isolates of Proteus spp. The antibiotic resistance of these bacteria varied from 0% to 100% for all of the antibiotics tested. Resistance to aminoglycosides ranged from 0% (amikacin) to 69.9% (gentamycin). Resistance to quinolones ranged from 43.8% (levofloxacin) to 76.7% (nalidixic acid). Similarly, 60 (82.2%) isolates were resistant to cotrimoxazole and 25 (34.2%) isolates to chloramphenicol. Patients under 20 years (57.1%) (p=0.03), men (52.2%)(p=0.11; NS), patients with respiratory samples (83.3%)(p=0.004), with pus (61.9%)(p=0.02) and hospitalized in surgery and intensive care units (68.4%)(p=0.0009) were the most affected by these enterobacteria. Conclusion: Extended-Spectrum Beta-Lactamases - producing Enterobacteriaceae are responsible for severe infections and the majorities are multi-resistant bacteria.

using content analysis was employed wherein the Automated Sensitivity Testing Machine data obtained from the

revealed that based on the percentage distribution of the specimens, the highest number of isolates for

Meropenem; Cirpofloxacin and Levofloxacin for Enterobacter cloacae; Imipenem for Escherichia coli;
Amikacin for Klebsiella pneumoniae; Ertapenem, Imipenem and Pip/Tazo for Proteus spp.; Pip/Tazo for
Pseudomonas aeruginosa, and Imipenem for Raoultella (K.) ornithinolytica. The most resistant antibiotics for
Enterobacter aerogenes, Escherichia coli, Klebsiella pneumonaie and Raoultella (K.) ornithinolytica was
Ampicillin; Amox/K Clav, Cefazolin and Cefoxitin for Enterobacter cloacae; Cefotaxime for Pseudomonas
aeruginosa and for Proteus spp. was Cefazolin. The researchers recommend for the conduct of an annual
antibiogram study by institution, disseminate the information through seminars/fora especially to clinicians,
conduct another study to include other hospitals and other types of bacteria.

A comparative study of various diagnostic techniques for CryptosporidiosisIOSR Journals
Diarrhoeal disease is a common complication of infection with HIV. Cryptosporidium has gained importance as an AIDS indicator disease and a cause of intractable diarrhoea in immunosuppressed individuals. This warranted a study of stool specimens of HIV positive patients with (n=60) and without (n=60) diarrhoea along with their HIV negative counterparts (n=200). Microscopic examination for ova and cysts were done using wet mount and Lugol’s iodine preparation. Smears were stained with Kinyoun Cold Acid Fast (KCAF) and Auramine ‘O’ fluorochrome (AOF) staining methods to identify Cryptosporidium oocysts. ELISA using Cryptosporidium microplate assay (alexon Inc) for detection of Cryptosporidium antigen was conducted on all stool specimens. By KCAF staining detection of Cryptosporidium in HIV positive subjects with diarrhoea was 20%, by AOF it was 7.5% and by ELISA the detection rate went up to 30%. All the detailed result were statistically compared taking KCAF staining as gold standard which revealed AOF staining to have sensitivity of 36.67% and specificity of 99.31% while ELISA was found to have sensitivity of 83.88% and specificity of 96.55%. Keeping in mind the present scenario of HIV infection in India and more so in Goa, it is recommended to include detection of Cryptosporidium oocysts in routine parasitological examination of stool specimens and an urgent need to standardize a gold standard for various diagnostic tests presently available

Septic arthritis is the purulent invasion of a joint by an infectious agent which produces arthritis. The main organisms having great potential to infect human beings as well as other mammals are Staphylococcus aureus, Streptococcus pneumoniaeand Streptococcus pyrogens. In Silico comparative analysis ofall the pathways of host Homo sapiens and pathogens was performed by using KEGG and Protein BLAST. 25, 20 and 16 unique pathways were identified for Staphylococcus aureus, Streptococcus pyrogensand Streptococcus pneumonia respectively. Out of these we identified 3 enzymes for Staphylococcus aureus, 4 for Streptococcus pneumoniae and 1 for Streptococcus pyrogens, which are non-homologous to Homo sapiens proteins. The enzymes essential for survival of the pathogens were found out by DEG database. Further CELLO analysis results showed that 50% enzymes are found to be Extracellular, 25% to be cytoplasmic and 25% to be membranous for Staphylococcus aureus. For Streptococcus pneumoniae, 50% enzymes are found to be Extracellular, 12% cytoplasmic, 13% membranous and 25% as cell wall proteins. 100% enzymes were found to be membranous for Streptococcus pyrogens. Finally the enzymes from DEG were submitted in Drug Bank database to identify approve drug targets. This Data Mining approach found that mostly the enzymes which can act as targets belong to extracellular level in Staphylococcus aureus, Streptococcus pneumoniae and membranous in Streptococcus pyrogens. This findings gives an understanding of these enzymes interaction with human protein protein interaction at extracellular and membrane level.
Background and study aim: During last two decades, there has been a world-wide trend in increasing occurrence of enterococcal infections in the hospitals. The aim of present study was to determine the spectrum of enterococcal infections, species prevalence, antimicrobial and characteristics of vancomycin resistant enterococci (VRE) in a tertiary care hospital, Eastern India.
Patients and Methods: Between January 2013 and July 2014, 152 Enterococcus species were obtained from clinical samples. Enterococci were identified using standard biochemical tests. Antimicrobial susceptibility was tested by Kirby-Bauer disk diffusion according to Clinical resistance
& Laboratory Standards Institute (CLSI) guidelines.VRE agar base was used to screen VRE isolates. Minimum inhibitory concentration (MIC) values of VRE isolates were determined using Epsilometer-test. VRE isolates were also examined by PCR to detect vanA gene.
Results: From 1602 clinical samples, 961 (60%) were culture positive and 152 (15.8%) enterococcal isolates were obtained. Most common species isolated was E. faecalis (63.8%) followed by E. faecium (35.5%). Majority of enterococcal infections were detected from ICUs and surgical wards and clinically presented as UTIs. Disk diffusion method showed 67.1% were resistant to penicillin, 61.2% ampicillin, 58.5% ciprofloxacin, 46.7% high-level gentamicin, 42. 8% high-level streptomycin, 7.9% teicoplanin and none to linezolid. Twenty (13.2%) enterococcal isolates were vancomycin resistant in VRE screen and disk diffusion method. Epsilometer-test of VRE isolates showed 8 (40%) isolates were resistant and 9 (45%) were intermediately resistant. From 20 VRE isolates, six showed VanA and two VanB phenotypes and all six VanA phenotypes had vanA gene cluster.
Conclusion: More accurate and reliable MIC determination tests should be performed in all suspected VRE isolates. Confirmatory PCR is required for identifying resistant gene cluster.

Background: Extended-Spectrum Beta-Lactamases - producing Enterobacteriaceae are common in hospitals. This study aims to describe the antibiotic resistance of these bacteria and their associated demographic and clinical factors. Methods: It was a prospective study of 73 isolates of Extended-Spectrum Beta-Lactamases - producing Enterobacteriaceae for a period of six months from July to December 2019 in the laboratory of Befelatanana. Results: This study showed 73 (6.3%) isolates of Extended-Spectrum Beta-Lactamases- producing Enterobacteriaceae, represented by 25 (34.2%) isolates of Klebsiella spp, 24 (32.9%) isolates of Escherichia coli, 22 (30.1%) isolates of Enterobacter spp and 2 (2.7%) isolates of Proteus spp. The antibiotic resistance of these bacteria varied from 0% to 100% for all of the antibiotics tested. Resistance to aminoglycosides ranged from 0% (amikacin) to 69.9% (gentamycin). Resistance to quinolones ranged from 43.8% (levofloxacin) to 76.7% (nalidixic acid). Similarly, 60 (82.2%) isolates were resistant to cotrimoxazole and 25 (34.2%) isolates to chloramphenicol. Patients under 20 years (57.1%) (p=0.03), men (52.2%)(p=0.11; NS), patients with respiratory samples (83.3%)(p=0.004), with pus (61.9%)(p=0.02) and hospitalized in surgery and intensive care units (68.4%)(p=0.0009) were the most affected by these enterobacteria. Conclusion: Extended-Spectrum Beta-Lactamases - producing Enterobacteriaceae are responsible for severe infections and the majorities are multi-resistant bacteria.

This research study focused on the antibiogram of bacterial isolates at Hail General Hospital,
Kingdom of Saudi Arabia. It sought to answer the following questions: (1) What is the percentage distribution of
the isolates on the specimens when classified according to the following bacteria? Enterobacter aerogenes,
Enterobacter cloacae, Escherichia coli, Klebsiella pneumoniae, Proteus spp., Pseudomonas aeruginosa,
Raoultella (K.) ornithinolytica. (2) What is the antibiogram result of the bacterial isolates? Descriptive research
using content analysis was employed wherein the Automated Sensitivity Testing Machine data obtained from the
Microbiology section of Hail General Hospital. The statistical tools were frequency and percentage. The results
revealed that based on the percentage distribution of the specimens, the highest number of isolates for
Enterobacter aerogenes, Enterobacter cloacae, Klebsiella pneumoniae and Proteus spp. was from throat swab,
urine for Escherichia coli and Psedomonas aeruginosa, ward swab and high vaginal swab for Raoultella (K.)
ornithinolytica. On antibiogram result, the most sensitive antibiotics for Enterobacter aerogenes was
Meropenem; Cirpofloxacin and Levofloxacin for Enterobacter cloacae; Imipenem for Escherichia coli;
Amikacin for Klebsiella pneumoniae; Ertapenem, Imipenem and Pip/Tazo for Proteus spp.; Pip/Tazo for
Pseudomonas aeruginosa, and Imipenem for Raoultella (K.) ornithinolytica. The most resistant antibiotics for
Enterobacter aerogenes, Escherichia coli, Klebsiella pneumonaie and Raoultella (K.) ornithinolytica was
Ampicillin; Amox/K Clav, Cefazolin and Cefoxitin for Enterobacter cloacae; Cefotaxime for Pseudomonas
aeruginosa and for Proteus spp. was Cefazolin. The researchers recommend for the conduct of an annual
antibiogram study by institution, disseminate the information through seminars/fora especially to clinicians,
conduct another study to include other hospitals and other types of bacteria.

A comparative study of various diagnostic techniques for CryptosporidiosisIOSR Journals
Diarrhoeal disease is a common complication of infection with HIV. Cryptosporidium has gained importance as an AIDS indicator disease and a cause of intractable diarrhoea in immunosuppressed individuals. This warranted a study of stool specimens of HIV positive patients with (n=60) and without (n=60) diarrhoea along with their HIV negative counterparts (n=200). Microscopic examination for ova and cysts were done using wet mount and Lugol’s iodine preparation. Smears were stained with Kinyoun Cold Acid Fast (KCAF) and Auramine ‘O’ fluorochrome (AOF) staining methods to identify Cryptosporidium oocysts. ELISA using Cryptosporidium microplate assay (alexon Inc) for detection of Cryptosporidium antigen was conducted on all stool specimens. By KCAF staining detection of Cryptosporidium in HIV positive subjects with diarrhoea was 20%, by AOF it was 7.5% and by ELISA the detection rate went up to 30%. All the detailed result were statistically compared taking KCAF staining as gold standard which revealed AOF staining to have sensitivity of 36.67% and specificity of 99.31% while ELISA was found to have sensitivity of 83.88% and specificity of 96.55%. Keeping in mind the present scenario of HIV infection in India and more so in Goa, it is recommended to include detection of Cryptosporidium oocysts in routine parasitological examination of stool specimens and an urgent need to standardize a gold standard for various diagnostic tests presently available

Multi drug-resistant (MDR) isolates of Staphylococcus aureus are on rise and are becoming a
challenge for timely and appropriate treatment. The present study was carried out with an objective to isolate
Staphylococcus aureus from clinical samples and determine their sensitivity. Out of 110 samples collected, 44
were shown to contained S. aureus. The isolates were subjected to antibiotic sensitivity tests using 10 different
and commonly used antibiotics by modified Kirby- Bauer disc diffusion technique. Out of the total isolates (42)
tested, only 7.1% were susceptible to all the antibiotics. Multiple resistance was eminent in over 92% with
highest occurrence in 4.8% where the entire antibiotics were resisted. Multiple antibiotic resistance indixes
(MAR index) indicated that 0.6 index occurred most (23.8%) followed by 0.5 (19.0%). On the other hand, 0.1
and 0.8 indexes were the lowest with 0.0% and 1.0% occurrence respectively. Ciprofloxacin was resisted by
most of the organisms (64.3%) while amoxicillin (64.3%) and streptomycin (61.9%) were most efficacious. With
over 90% isolate having MAR index ≥ 0.2, the multiple drug resistance by the S. aureus is quite alarming and
might suggest inappropriate antibiotic usage by the sampled population. Therefore, the need to strategize the
nature of antibiotic treatment against S. aureus and massive campaign on indiscriminate antibiotic use is urgent.

Pseudomonas aeruginosa is a ubiquitous bacterium that causes various hospital- acquired and community-acquired infections. It has been reported that the clinical isolates of P. aeruginosa are difficult to treat because of their virulence factors and antibiotics resistances. The aim of present study was to screen the antibiotic resistance patterns and the prevalence of virulence factor genes in a set of Pseudomonas aeruginosa isolated from Ogbomoso, and to determine whether a correlation exists between the prevalence of virulence factors and antibiotic resistance of P. aeruginosa. A total of 100 P. aeruginosa isolates were collected from various types of clinical specimens. Antimicrobial susceptibility testing was performed using the Kirby-bauer method. In addition, PCR assays were used for screening four virulence encoding genes (OPRL, LasB, PLCH and ToxA). The results showed that OPRL (79%) and LasB (62%) were the most frequent virulence genes in P. aeruginosa strains, followed by PLCH (41%) and ToxA (35%). The highest resistance was detected towards Piperacillin (42%) and Tetracycline (42%). Moderate rate of resistance (12-39%) were detected towards the other antibiotics. The virulent factors identified in this study provide valuable information regarding the prevalence of resistance genes of P. aeruginosa isolates in Ogbomoso, Nigeria and their potential impact on treatments that exploit the unique physiology of the pathogen. This will be useful for the health workers to improve infection control measures and to establish a surveillance system.

Background: The widespread use of antibiotics has resulted in emergence of community-acquired antibiotic resistance among uropathogens in outpatient’s population. This constitutes an impediment in the management of urinary tract infection (UTI) in both community and hospital settings. Objective: The aim of this study was to determine the current antibiotic resistance trends, extended spectrum beta-lactamase (ESBL) production and plasmid profile of uropathogens from outpatients. Methods: A total of 370 mid-stream urine samples were collected and cultured by standard methods. Isolated uropathogens were identified using appropriate biochemical methods. The modified Kirby Bauer disk method was used for antibiotic susceptibility test. The ESBL-producing uropathogens were identified and their plasmid DNA extraction and curing were carried out by standard methods. Results: About 35.7% and 32.7% of uropathogens were multi-drug resistant and ESBL-producing respectively. There was higher prevalence of ESBL-production among isolates from female patients (62.5%) when compared to that from male patients (37.5%). The isolated uropathogens were most resistant to Cefotaxime, and most sensitive to Imipenem. Resistance to antibiotics by ESBL-producing uropathogens was found to be plasmid-mediated. Conclusion: Community acquired Uropathogens from outpatients were multidrug resistant due to ESBL production localized on plasmids, a probable cause of treatment failures experienced in Uyo.
PREVALENCE AND CHARACTERIZATION OF VIRULENCE PROPERTIES OF PSEUDOMONAS AERUGI...SUS GROUP OF INSTITUTIONS
Pseudomonas aeruginosa is the epitome of an opportunistic pathogen of humans that cause urinary tract infections, respiratory system infection, particularly in victim of severe burns, cancer and AIDS patient who are immunocompromised. Most Pseudomonas infections are both invasive and toxigenic. The particular bacterial determinants of virulence mediate different stages of infection and are ultimately responsible for the characteristic syndromes that accompany the disease. In the present study P. aeruginosa was found to be more prevalent in burn patients (100%) followed by urinary tract infection samples (71%), sputum samples (66%) and wound samples (59%). 85% isolates recovered from clinical samples were mucoid. A total of 35% isolates were strong siderophore producers, 19% isolates were strong protease producers while 52% were strong phospholipase producers. Isolates from burns, sputum and environment sample were strong rhamnolipid producers. Elevated level of hemolysin production was observed in burn, urine and wound isolates. The prominence of haemagglutination ability in environmental isolates followed by burns isolates provided evidence for its being a nosocomial pathogen. The association between virulence determinants and disease can indicate the precise role played by the determinant in estabilishing the disease. Isolates were maximally sensitive towards lactam antibiotics.

The prevalence of extended spectrum β-lactamases among 246 clinical isolates from Abia State University teaching Hospital patients was investigated. The isolates were made up of 134 Escherichia coli and 112 Klebsiella species. Antimicrobial susceptibility of the isolates was determined by the disc diffusion method. ESBL phenotypes were determined by the double disc synergy method using ceftazidime, cefotaxime, ceftriaxone and co-amoxiclav. Out of the 246 isolates, 125 (50.8%) were ESBL producers, made up of 62(50.8%) E. coli and 63 (50.4%) Klebsiella isolates. Seventeen (54.8%) of the ESBL producing E. coli isolates were from in-patients while 45 (47.9%) were from out-patients. For the ESBL positive Klebsiella spp., 14 (45.2%) and 49 (52.1%) were from in-patients and out-patients respectively. ESBL producing isolates were also found to be more prevalent among the female patients (72.8%) than among the male patients (27.2%). The isolates also expressed high rates of resistance to other classes of antibiotics tested. However, Amikacin was found to have excellent performance against the urinary isolates tested and therefore is recommended for the treatment of infections caused by Escherichia coli and Klebsiella species. This study shows high prevalence of ESBL producing E. coli and Klebsiella isolates clinical samples of patients attending the Abia State University Teaching Hospital Aba, Abia State Nigeria.

Abstract
Objectives: Urinary tract infection caused by uropathogenic Escherichia coli
(UPEC) strains is one of the most important infections in the world. UPEC encode
widespread virulence factors closely related with pathogenesis of the bacteria.
The purpose of this study was to evaluate the presence of different phenotypic
virulence markers in UPEC isolates and determine their correlation with antibi-
otic resistance pattern.
Methods: UPEC isolates from patients with different clinical symptoms of UTI
were collected and screened for biofilm and hemolysin production, mannose
resistant, and mannose sensitive hemagglutination (MRHA and MSHA, respec-
tively). In addition, antimicrobial resistance pattern and ESBL-producing isolates
were recorded.
Results: Of the 156 UPEC isolates, biofilm and hemolysin formation was seen in
133 (85.3%) and 53 (34%) isolates, respectively. Moreover, 98 (62.8%) and 58
(37.2%) isolates showed the presence of Types 1 fimbriae (MSHA) and P fimbriae
(MRHA), respectively. Our results also showed a relationship between biofilm
formation in UPEC isolated from acute cystitis patients and recurrent UTI cases.
Occurrence of UTI was dramatically correlated with the patients’ profiles. We
observed that the difference in antimicrobial susceptibilities of the biofilm and
nonbiofilm former isolates was statistically significant. The UPEC isolates showed
the highest resistance to ampicillin, tetracycline, amoxicillin, and cotrimox-
azole. Moreover, 26.9% of isolates were ESBL producers.

2. Conclusion: This study indicated that there is a relationship between the
phenotypic virulence traits of the UPEC isolates, patients’ profiles, and antibiotic
resistance. Detection of the phenotypic virulence factors could help to improve
understanding of pathogenesis of UPEC isolates and better medical intervention.
1. Introduction
Urinary tract infections (UTIs) including cystitis and
pyelonephritis are considered to be the second most
common type of infections in humans. They account for
w150e250 million cases globally per year [1,2]. More
than 50% of women will have at least one episode of
UTI during their lifetime [3,4]. Furthermore, after an
initial episode of UTI, women are more likely
(20e40%) to get recurrent UTIs [4,5]. UTI patients are
classified as either asymptomatic or symptomatic cases.
Asymptomatic bacteriuria (ABU) occurs in a small
number of healthy individuals and may not need treat-
ment, which makes it different from symptomatic
bacteriuria. It often affects pregnant women, with
varying prevalence among different populations,
depending on factors such as age, sex, sexual activity,
and the presence of genitourinary abnormalities [3,4,6].
Uropathogenic Escherichia coli (UPEC) is the most
common cause of urinary tract infections (UTIs) both in
community and hospital settings with significant
morbidity and mortality worldwide [7e9]. Previous in-
vestigations have shown that UPEC strains encode
widespread virulence factors closely related to coloni-
zation, persistence, and pathogenesis of bacteria in the
urinary tract [8,10]. The most important of these factors
include adhesins or fimbriae, biofilm formation, and
toxins such as hemolysin [8,10]. Fimbriae are catego-
rized serologically by their hemagglutination pattern and
receptor specificities as mannose sensitive (MSHA) or
mannose resistance hemagglutination (MRHA) [7,11].
Despite the vast subclass of adhesins that have been
reported in UPEC, Type I (MSHA) and P (MRHA) are
the most common fimbriae found in UPEC strains. They
play an important role in binding and invasion to
bladder (cystitis) and kidney (pyelonephritis) epithelial
cells [12,13].
Biofilm of UPEC provides a nutrient-rich environ-
ment which promotes growth and persistence of mi-
croorganisms at the site of infection, and protects
bacteria from antimicrobial substances [1,14]. More-
over, UPEC strains often express and secrete a labile
pore-forming toxin known as a- hemolysin production
that is mainly associated with more virulent UPEC
strains [15,16].
Emergence of drug resistance to broad-spectrum beta
lactams mediated by extended spectrum beta lactamases
(ESBLs) and especially multi-drug resistant (MDR)
clonal groups among UPEC strains increase the serious
threat to global public health [1,17]. Therefore, to
optimize the use of effective antibiotics for appropriate
treatment of UTI patients, it is important for physicians
to be aware of the etiological agents and antimicrobial
resistance trends of UTI pathogens in their geographic
area.
This investigation is aimed to determine the preva-
lence and correlation of phenotypic virulence traits and
antibiotic resistance profile among the UPEC isolated
from UTIs, with regard to patients’ profiles in Tehran,
Iran.
2. Materials and methods
2.1. Organism collection and identification
The present study was conducted in the Department
of Molecular Biology, Pasteur Institute of Iran, Tehran,
Iran. Random samples of 156 clinical isolates of UPEC
were collected from urine samples of symptomatic and
asymptomatic cases of UTIs with significant counts (!

CFU/mL) in various hospitals of Tehran, Iran be-
tween March 2013 and February 2014. Only patients
aged ! 20 years who were not on antimicrobial therapy
at sample collection or had not taken antibiotic drugs 1
month prior to sampling time, were included in this
study. Isolated organisms were identified and charac-
terized on the basis of standard microbiological tests
such as Gram staining, catalase, indole, methyl red,
voges-proskauer, citrate utilization, motility, fermenta-
tion, and utilization of glucose, lactose, and sucrose. All
isolates were suspended in 15% glycerol-supplemented
Luria-Bertani (LB) medium and preserved frozen
at À80
C.
2.2. Patient demographics
Clinical symptoms, infection history, treatment
summary, and all of the necessary relevant information
from patients were obtained from hospital records, lab-
oratory reports, and interviewing patients.
2.3. Biofilm formation assay
We analyzed the ability of the UPEC isolates to
produce the biofilm according to the protocols described
by O’Toole and Kolter [18] and Dusane et al [19]. We
used the E. coli ATCC 25922 strain as a positive control.
Briefly, UPEC isolates were grown in LB broth at 37
C
overnight, then the cultures were diluted 1:100 with
Tryptic Soy Broth (Merck, Darmstadt, Germany) and
incubated in a 96-well polyvinyl chloride (PVC) mi-
crotiter plate at room temperature (RT) for 48 hours.

3. Wells were washed thoroughly with double-distilled
water (DDW) and 150 mL of 0.1 % (w/v) aqueous so-
lution of crystal violet was added to each well. After
15e20 minutes of incubation at RT, the excess crystal
violet dye was washed three times with DDW. Then,
200 mL of 33% acetic acid was added to each well and
absorbance was measured at optical density 590 nm
(OD590) with an enzyme-linked immunosorbent assay
(ELISA) reader. Each assay was performed in triplicate
and the mean values of crystal violet absorbance Æ SD
was calculated for all repetitions of the experiments.
2.4. Hemolysin production
The ability of the UPEC isolates to induce hemolysis
on blood agar (Merck) was evaluated to detect the he-
molysin producer isolates. The bacteria were inoculated
into 5% sheep blood agar and incubated overnight at
37
C. Hemolysin production was detected by the pres-
ence of a complete clearing zone of the erythrocytes
around the colonies.
2.5. Hemagglutination assay (HA)
UPEC isolates were screened for MRHA and MSHA
by using 96-well round bottom plates as described by
Hultgren et al [20] and Snyder et al [21]. Approximately,

CFU/mL of the UPEC bacteria was serially diluted in
phosphate-buffered saline (PBS; pH 7.4) and bacterial
suspensions were standardized at OD600 nm of 1.0 and
added to the wells. An equal volume of 3% (v/v) solution
of guinea pig erythrocytes was added to each well with or
without 2% D-mannose (Sigma, St. Louis, MO, USA)
and gently mixed with the bacterial suspensions. After
incubation at RT for 10 minutes, the hemagglutination
results were observed according to the clumping of
erythrocytes. Hemagglutination was designated as
MRHA when it occurred in the presence of D-mannose
and MSHA when it was inhibited by D-mannose. Wells
with only the suspension of erythrocytes with or without
D-mannose served as negative control and E. coli ATCC
25922 was used as a positive control for MRHA.
2.6. Antimicrobial susceptibility patterns
Antibiotic susceptibility patterns of 20 traditional and
conventional antibiotics against UPEC isolates was
interpreted as recommended by the Clinical and Labo-
ratory Standards Institute (CLSI) by the disk-diffusion
method [22]. The following antibiotics (Mast, Co.,
Merseyside, UK) were included in this study: piper-
acillin (100 mg), imipenem (10 mg), neropenem (10 mg),
piperacillinetazobactam (100/10mg), ampi-
cillinesulbactam (10/10 mg), amikacin (30 mg), cotri-
moxazole (25 mg), ciprofloxacin (5 mg), ceftazidime
(30 mg), nitrofurantoin (300 mg), gentamicin (10 mg),
ceftriaxone (30 mg), nalidixic Acid (30 mg), norfloxacin
(10 mg), cefotaxime (30 mg), cefixime (5 mg), cefepime
(30 mg), amoxicillin (25 mg), tetracycline (30 mg), and
ampicillin (10 mg). Inhibition zone diameter (mm) of
each antimicrobial disc was measured, and the isolates
were classified as resistant and susceptible. The E. coli
ATCC 25922 was used as control strain.
2.7. Detection of ESBL producers
All of the UPEC isolates that were resistant to third-
generation cephalosporins (3GCs) by disc diffusion test,
ceftazidime (zone diameter of 22 mm), ceftriaxone
(zone diameter of 25 mm), or cefotaxime (zone
diameter of 27 mm) were selected for confirmation of
ESBL production by double disk synergy test (DDST) as
described by CLSI guidelines [22]. E. coli ATCC 25922
and Klebsiella pneumoniae ATCC 700603 were used as
nonESBL and ESBL producing organisms, respectively.
2.8. Statistical analysis
Data analysis was performed by using SPSS software
version 19.0 for windows (IBM, Chicago, IL, USA).
Chi-square test, independent sample t test, odd ratio
(OR) analysis, two-tailed Fisher’s exact tests, and one-
way ANOVA analysis test were used to compare the
occurrence of phenotypic markers in UPEC and describe
the associations of the potential virulence factors with
other described factors. A p value 0.05 was consid-
ered as statistical significant.
3. Results
3.1. Prevalence of UTI in relationship with
profiles of patients
Of the 156 UTI patients, the rate of UTI in females
(79.5%) was higher than males (20.5%). The incidence
of UTI in female patients with age group of 31e40 years
(45.2%) and 51e60 years (29.8%) was highest while the
age group 41e50 years had the lowest incidence of UTI
(8.9%). Among male patients, the highest prevalence of
UTI was observed in the age group of 51e60 years
(46.9%) and the lowest was seen in the age group of
31e40 years (12.5%). The prevalence of UTI in relation
to other important patients’ profiles is shown in Table 1.
Our results showed that occurrence of UTI were
dramatically correlated with the socioeconomic status,
educational level, and sexual activity (Table 1).
3.2. Virulence characteristics of UPEC isolates
3.2.1. Biofilm formation
On the basis of our investigations, biofilm formation
capacity of all UPEC isolates was classified into four
groups, strong biofilm producers (17.3%), moderate
biofilm producers (18.6%), weakly biofilm producers
(49.4%), and nonbiofilm producers (14.7%).
3.2.2. Hemolysin production
According to our results, hemolysin production was
observed in 34% of the UPEC isolates. The remaining
66% isolates showed no hemolysis.

4. 3.2.3. Prevalence of Type 1 (MSHA) and P
fimbriae (MRHA)
The ability of UPEC isolates to cause agglutination
of erythrocytes is an indirect evidence of the presence of
fimbriae [23]. In the present study, 62.8% and 37.2%
isolates showed the presence of Type 1 (MSHA) and P
fimbriae (MRHA), respectively.
3.3. Antimicrobial resistance profile of UPEC
The resistance pattern of UPEC isolates to the
different antimicrobial agents is shown in Fig. 1. Among
the antibiotics tested, ampicillin resistance prevalence
was the highest (77.6%), followed by tetracycline
(60.3%), amoxicillin (59%), cotrimoxazole (58.3%), and
piperacillin (55.8%). The isolates showed the highest
sensitivity to antibiotics such as imipenem and mer-
openem (100%), amikacin (96.8%), piperacillin/tazo-
bactum, and nitrofurantion (94.9%). Double disc
synergy test (DDST) showed that 26.9% of the UPEC
isolates were positive for ESBL production. Totally, 123
isolates tested (79%) were multidrug-resistant (isolates
with resistance to three or more different classes of
antibiotics).
3.4. Relation of antimicrobial resistance with
virulence determinants of UPEC and
patients’ profiles
According to our results, overall resistance rates of
the antibiotics tested among the UPEC isolated from
male patients were higher than those of female patients
(p Z 0.04), although the differences in distribution of
antimicrobial resistance in different age groups of UTI
patients were not statistically significant (p 0.05). We
also found that overall resistance rates were higher
among inpatients isolates compared to outpatients
(p Z 0.039). Among the isolates, resistance to ampi-
cillin, cefepime, and ceftazidime were significantly
more prevalent in inpatients than outpatients
(p Z 0.037, p Z 0.008, and p Z 0.047, respectively).
There was also a significant correlation between higher
levels of resistance to multiple antibiotics such as nali-
dixic acid (p Z 0.038), ceftriaxone (p Z 0.041), cotri-
moxazole (p Z 0.029), and acute cystitis patients
compared with the pyelonephritis and asymptomatic
bacteriuria patients (Table 2).
In addition, our results showed that there was no
significant difference between resistance to the antibi-
otics, other than cotrimoxazole (p Z 0.015), and history
of recurrent in the UTI patients. Relationship between
antimicrobial resistance and phenotypic virulence
markers of the UPEC isolates is shown in Table 3. As
shown in Table 3, there was a significant correlation
between intensity of biofilm formation and resistance to
antibiotics such as ampicillin, norfloxacin, and cotri-
moxazole (p 0.05). Moreover, hemolysin producing
isolates were significantly less resistant to tetracycline,
nalidixic acid, and cotrimoxazole than in nonhemolysin
isolates (p 0.05). No relationship was found between

5. MRHA, MSHA isolates, and resistance to the antibiotics
used (p 0.05). As shown in Fig. 2, although all of the
antibiotic resistance occurred at a higher rate among
ESBL producer isolates than nonESBL producer iso-
lates, only resistance to nalidixic acid, piperacillin,
ampicillin, and cotrimoxazole was statistically signifi-
cant (p 0.05).
3.5. Phenotypic virulence patterns of UPEC
isolates in relation to clinical symptoms
Based on the distribution of the phenotypic virulence
traits, all of the UPEC isolates exhibited 16 virulence
patterns, referred to as UPEC patterns (UP) in Table 4.
The majority of the isolates shared some similarities in
most virulence markers, but several differences were
also observed. UP 12 was characterized by the presence
of the biofilm and MSHA phenotypic markers only, and
was the most common pattern found in 46 isolates.
Moreover, some of the UPEC patterns were only
observed in single type of clinical symptoms in UTI
patients (Table 4). Occurrence of multiple urovirulence
markers (isolates with three or more virulence markers)
observed in 58 (37.2%) of the UPEC isolates (Table 4).
It has been observed that pyelonephritis and cystitis
cases were more associated with multiple urovirulence
markers compared to asymptomatic bacteriuria cases
(p Z 0.022 and p Z 0.038, respectively; Table 4).
4. Discussion
Phenotypic characteristics of UPEC isolates and their
correlation with antibiotic resistance patterns in patients
with UTI are not well known and few data have been

6. reported in Iran. Identification of virulence markers of
UPEC will help to understand their contribution with the
antimicrobial resistance [1,2].
Our findings in accordance with other studies indi-
cated that females have a notable frequency of UTI
versus males [3,24]. This difference in frequency could
be due to several clinical factors, including anatomic
differences, hormonal effects, and behavioral patterns
[3,24]. Moreover, our investigations reveal that overall
resistance rates of antimicrobial drugs were higher in
men than women (p Z 0.04). These observations were
similar to the findings of Yilmaz et al [25]. In the current
study, most recurrent UTIs in women presented as acute
cystitis, as occurred in nonrecurrent women.
We observed that resistance rates were higher among
antimicrobials that have been used for a long time as
empirical therapy such as ampicillin and cotrimoxazole.
Similar findings were reported in Iran and other coun-
tries [2,26]. This may be due to increased consumption
of these antibiotics, self-medication, and transfer of
resistant isolates [27]. Thus, the use of other antibiotics
such as nitrofurantoin and imipenem is recommended
for treatment of UTI patients.
Our results showed that the percentage of nalidixic acid
resistant isolates causing acute cystitis is more than those
causing acute pyelonephritis and asymptomatic baceriuria
(p Z 0.038). A similar observation was reported by
Velasco et al [28]. These data suggest that the nalidixic
acid resistant UPEC isolates may have lost the ability to
attach the kidney epithelial cells. Moreover, UPEC iso-
lates that produced hemolysin or biofilm were less resis-
tant to nalidixic acid than in nonhemolysis (p Z 0.011)
and nonbiofilm formers (p Z 0.043). These findings in
accordance with other studies suggest that resistance to
nalidixic acid may be associated with marked reduction in
the virulence severity of UPEC isolates [29e31]. Also,
this is in accordance with the observation of Johnson et al
[32] that UPEC strains with greater antibiotic resistance
tended to express lower virulence traits.
The present study showed that hemolysin producer
isolates had a higher ability of biofilm formation. This
finding is similar to the study of Soto et al [29] and in
contrary to observations of Marhova et al [33].
According to our results, some urovirulence markers
were closely associated with a specific anatomical site of
infection. In this regard, we observed that hemolysin
production is more frequent in UPEC isolated from
patients with pyelonephritis than in isolates from pa-
tients with other clinical symptoms of UTI (p Z 0.01).
This finding reveals that hemolysin may be contributing

7. to the severity of UPEC infections. Moreover, our re-
sults showed a higher potency of biofilm production
among UPEC isolates causing acute cystitis than isolates
from acute pyelonephritis and, in particular, those
causing asymptomatic bacteriuria (p Z 0.04). In fact,
biofilm formation seems to be one of the most important
virulence factors among the UPEC isolated from pa-
tients with acute cystitis. Also, these biofilm producing
UPEC isolates showed a significantly greater expression
of Type 1 fimbriae (MSHA) than nonbiofilm producing
isolates (p Z 0.021). This could be explained by the
important role of Type 1 fimbriae in the first steps of
biofilm formation [12]. One-way analysis of variance
indicated that the difference in antimicrobial suscepti-
bilities of the biofilm and nonbiofilm producing isolates
was statistically significant (p 0.05). Murugan et al
[34] have reported the correlation between biofilm for-
mation and multiple drug resistance among UPEC
strains. We also found a remarkable correlation between
biofilm and ESBL producing UPEC.
In this study, production of MRHA and MSHA was
in relation to clinical symptoms of UTI patients. MRHA
isolates were significantly higher among the UPEC
isolated from pyelonephritis patients (p Z 0.025),
whereas MSHA isolates were more prevalent in patients
with cystitis and asymptomatic bacteriuria (p 0.05).
Association of P fimbriae (MRSA) of UPEC with acute
pyelonephritis in UTI patients suggests that P fimbriae
are required for colonization and invasion of the human
upper urinary tract [35,36].
Our results showed that UPEC isolated from patients
with symptomatic bacteriuria are characterized by
higher virulence characteristics than those isolated from
patients with asymptomatic UTI (Table 4). This obser-
vation reflects the important role of virulence factors of
UPEC in severity of clinical symptoms of UTI patients.
In our study, biofilm producing UPEC isolates were
strongly linked to recurrent UTI (p Z 0.001). These data
suggest that the tropism of UPEC isolates for the recur-
rent infections may depend on the biofilm formation.
In conclusion, the present study provides novel
epidemiological information relevant to the UPEC iso-
lated from UTI patients. These findings indicate that
there is a correlation between the most important
phenotypic virulence traits of the UPEC isolates and
antibiotic resistance. Also, we believe that detection of
the phenotypic virulence factors could be valuable in
investigations on the pathogenesis of UPEC isolates and
management of UTI therapy. However, further studies
of genotypic and phenotypic characteristics of UPEC
isolates may help us to get novel insights into patho-
genesis of UPEC isolates.
