This document discusses the possibilities of e-Health and personal health applications. It describes how a personal health application can create a personal health profile, provide tailored health advice and interventions, and facilitate follow-up care providers. Evaluation results are presented showing positive effects on health behaviors, cardiovascular risk reduction, and decreased absenteeism from work.
