2. Program Significance
Major Depressive Disorder has been estimated to
affect roughly 6.7% of American adults. It has a
median onset of 32 years and it generally affects
women more than men (Ford-Martin, Odle, 2011).
Most people are treated with antidepressants and
psychosocial therapy to evaluate the reasons for the
depression. Studies have shown that aerobic exercise
can also be a positive treatment in depressive
disorders (Hoffman, et al., 2010).

Works in marketing, as well as part time in the restaurant

4. Health Promotion Focus
After using the Belloc and Breslow’s & Healthy Habits
Assessment Tool it was determined that the focus of
this health promotion project would be on using
exercise to decrease the effects of depression.
This will be accomplished by adding an exercise
regimen of 30-45 minutes of cardiovascular exercise 3-
4 days per week for a two month period.
5. Healthy People 2020 Goals
Goal: “Improve mental health through prevention and by ensuring
access to appropriate, quality mental health services.”
Mental health disorders are one the most common causes of
disability in the United States and Canada.
Connection between mental health and physical health are closely
linked.
Many mental disorders, including depression, affect individuals’
ability to participate in health promoting behaviors and activity.

6. Healthy People 2020 Goals
Goal: “Improve health, fitness, and quality of life through
daily physical activity.”
More than 80% of Americans do not follow or meet the
guidelines for exercise set forth by the 2008 publication
Physical Activity Guidelines for Americans.
Positive benefits of exercise include; lowering the risk of stroke,
heart disease, high blood pressure, colon and breast cancer,
and type 2 diabetes. Benefits also include improved
cardiovascular fitness and reduced symptoms of depression.

7. Trans-Theoretical Model of
Change
Evaluates a person’s readiness for change through a
five stage process; pre-contemplation, contemplation,
preparation for action, action, and maintenance
(Simmons-Morton, Melroy, & Wendell, 2012)
8. Learning Theory
The Health Promotion Model proposed by Pender is an effective tool that can
be used when trying to change a behavior. Several factors make up this
model and are important when evaluating the way to proceed with behavior
change.
Prior related behavior: has enjoyed being active in the past
Personal factor: currently engaging in counseling and other healthy behaviors
Perceived benefits of action: will be less depressed and in better physical shape
Perceived barriers to action: work schedule, weather, and lack of energy and
motivation
Perceived self efficacy: knows that she can achieve this but is concerned she will
lack the motivation
Interpersonal influences: supportive social network
Situational influences: supportive work environment and living situation

9. Program Evaluation
Green and Kreuter’s PRECEDE-PROCEDE model was
used to evaluate the program from beginning to end.
The PRECEDE portion of the model is used to evaluate
the best ways to construct and implement a program.
10. Program Evaluation
The PROCEDE portion of this model actually involves
the implementation of the program and the evaluation
of the program both during the program and at the
conclusion.
11. Evaluation Tool
A survey that assessed depression like behavior was
given to CM at the beginning and end of the program.
Topics included feelings of sadness, energy level, sleep
patterns, participation in social activities, and
enjoyment of activities.

14. Conclusion
Research and evidence supports that exercise
improves mood, increases energy levels, and promotes
an overall healthier person

----- Meeting Notes (4/14/14 17:21) -----The person I chose to work withfor this project is CM. She is 5'6" and weighs 140 pounds which puts her with aBMI of 22.6. CM is college educated and works in marketing at an outdoor biking office. She also works part time at a restaurant which makes her average work week around 50-60 hours. She does not smoke and has an average of one glass of wine per day. She exercises occasionally. She is in good overall health, although she does suffer from depression.
Using weekly planner to manage schedule, exercise journal to follow progress and hold herself accountable, participate in activities like biking and rock climbing with friends. Find activities that she would enjoy and be more likely to continue participation in ----- Meeting Notes (4/14/14 17:21) -----The focus of this project will be to use regular exercise to help lessen depression symptoms
----- Meeting Notes (4/14/14 18:03) -----Healthy people 2020 is a national program dedicated to imporving the health of all americans. Through scientific research they have set out certain goals, two of which are applicable to this program.. The first goal is .... Several of the reaons this goal is imporant include ...
At the beginning of the program CM was in the preparation for action stage, followed by the action stage. The preparation stage involves the participant identifying the need for changes and making the necessary steps toward implementing the change. The action stage is the participant actually making the changes in their life. CM is still in the action stage at this point. Once these behaviors have successfully continued and become an integrated part of her life she will continue with the maintenance process.
----- Meeting Notes (4/14/14 18:29) -----The prcede model started with the social diagnosis which included speaking with CM to describe the research of the positive effects of exercise in reference to depression. Through our conversstion she weas able to choose types of exercise that she would enjoy which will help maximaze enjoyment and participation. THe second phase is the epdimiological diagnosis which measures the level depression along with her current treatment. This was done through the use of a survey asking about depression symptoms, energy level, sleep disturbances, involvement in social activities, and overall enjoyment of life. This same survey was filled out at the end of the program and compared to measure the program's effectiveness. The educational and ecological diagnosis was used to gauge the need for education on exercise and. CM is very knowledgeable of ways to be active as well as safe practices to use in exercise. The administration diagnosis showed that CM has access to both indoor and outdoor areas for exercise, both of which had activities that she would enjoy participating in
----- Meeting Notes (4/14/14 19:28) -----The procede model begins with program implementation. CM agreed on a date for the program to begin and conclude and mapped out a tentativve schedule to follow..This is followed by the porcess evaluation. Throughout the program we met once a week to follow her progress and evaluate any needs to help continue and maintain the program. The next step of the proced model is to evaluate the effectiveness in relation to the initial objective with an evaluation tool
At the conclusion of the two month program CM completed the survey again. She had less depressed thoughts throughout the day and reported a more positive outlook on life. She also reported having a higher energy level which led to her exercising more and an increased activity level. CM also spent more time participating in social activities at the end of the program.
Program maintenance will be promoted by the continued use of an exercise journal and participating in exercise with friends. She will also use a planner to review her schedule and make time for exercise activities.
----- Meeting Notes (4/14/14 19:28) -----in conclusion the research and evidence of exercises poistive effects on individuals were mirrored in this health promotion project. CM continues to be more engaged in positive behaviors and thought due to the addition of exercise into her life. This was a positive study for her and I am encouraged that she wants to continue with the program. I hope to report even more improvement in the future
