INTRODUCTION
Patient classification systems are essential

patients based on various factors such as their clinical
conditions, resource requirements, and level of care
needed. These systems provide a standardized
framework for healthcare professionals to assess and
classify patients, ensuring appropriate care delivery,
resource allocation, and effective management.
PURPOSE
The primary purpose of patient classification systems is to streamline healthcare
processes and optimize patient care delivery. Some key objectives and purposes of these systems
include:
Standardization: Patient classification systems provide a standardized approach to categorize
patients, ensuring consistency and uniformity in the assessment and management of healthcare
needs.
Resource Allocation: By classifying patients based on their care requirements, these systems
help in determining the appropriate allocation of healthcare resources such as hospital beds,
nursing staff, medical equipment, and other necessary facilities.
Workload Management: Patient classification systems assist in workload management by
providing an objective and systematic method to distribute patient assignments among
healthcare providers, ensuring equitable distribution of workload.
Reimbursement: In many healthcare systems, patient
classification systems are utilized for reimbursement purposes.
They help determine the reimbursement rates or funding levels
based on the complexity and severity of patients’ conditions.
Quality Improvement: These systems contribute to quality
improvement initiatives by enabling healthcare professionals to
identify trends, patterns, and variations in patient populations,
leading to targeted interventions, improved outcomes, and
enhanced patient safety.
THE JOHNSON PATIENT CLASSIFICATION SYSTEM
The Johnson Patient
Classification System typically
categorizes patients into different
levels or categories based on their
care needs. Here are some common
categories that are often included:

Class I: Self-care patients
These are patients who
require minimal assistance
with activities of daily living
(ADLs) and have stable health
conditions. They can perform
most tasks independently.
Class II: Low-care patients
Patients in this category
require some assistance with ADLs
but generally have stable
conditions. They may need
occasional monitoring and basic
nursing interventions.
Class III: Moderate-care patients
Patients in this category
have moderate care needs and
require more frequent monitoring
and nursing interventions. They
may have chronic illnesses or
conditions that require regular
medication administration or
specialized treatments.
Class IV: High-care patients
Patients in this category have
complex care needs and require
intensive nursing care. They may
have multiple chronic illnesses,
require frequent monitoring, and
need assistance with ADLs. They
may also require specialized
treatments or interventions.
Class V: Intensive-care patients
These are critically ill
patients who require constant
monitoring and intensive nursing
care. They may be unstable, have
acute conditions, or be recovering
from major surgeries or
procedures.

Therapeutic Intervention Scoring System (TISS) :is primarily used in intensive care units
(ICUs) to assess patient acuity and determine the level of care required. TISS focuses on the
intensity and complexity of therapeutic interventions needed for each patient. Some key points
about TISS include:

workload, monitor
patient progress, and
allocate appropriate
resources within the
ICU.
SCORING METHODS:
Assigns numerical
values to interventions
performed, such as
procedures,
medications, and life
support measures.
CALCULATIONS:
Scores are summed up
for all interventions over
a specific time period,
usually 24 hours

Nursing Intensity Classification System
(NICS)
Nursing Intensity Classification System (NICS) is a
patient classification system used in general nursing
units to assess nursing workload. NICS takes into
account patient dependency factors and nursing care
activities. Key details about NICS include:

  • 5. THE JOHNSON PATIENT CLASSIFICATION SYSTEM The Johnson Patient Classification System typically categorizes patients into different levels or categories based on their care needs. Here are some common categories that are often included: 5 Class I: Self-care patients These are patients who require minimal assistance with activities of daily living (ADLs) and have stable health conditions. They can perform most tasks independently.
  • 6. Class II: Low-care patients Patients in this category require some assistance with ADLs but generally have stable conditions. They may need occasional monitoring and basic nursing interventions. Class III: Moderate-care patients Patients in this category have moderate care needs and require more frequent monitoring and nursing interventions. They may have chronic illnesses or conditions that require regular medication administration or specialized treatments.
  • 7. Class IV: High-care patients Patients in this category have complex care needs and require intensive nursing care. They may have multiple chronic illnesses, require frequent monitoring, and need assistance with ADLs. They may also require specialized treatments or interventions. TISS focuses on the intensity and complexity of therapeutic interventions needed for each patient. Some key points about TISS include: PURPOSE: To determine nursing workload, monitor patient progress, and allocate appropriate resources within the ICU. SCORING METHODS: Assigns numerical values to interventions performed, such as procedures, medications, and life support measures. CALCULATIONS: Scores are summed up for all interventions over a specific time period, usually 24 hours BENEFITS: Helps in workload management, staffing decisions, and equitable resource allocation in ICUs 8
  • 9. Nursing Intensity Classification System (NICS) Nursing Intensity Classification System (NICS) is a patient classification system used in general nursing units to assess nursing workload. NICS takes into account patient dependency factors and nursing care activities. Key details about NICS include: 9

(NICS) PURPOSE: To determine the required level of nursing care, assist in staffing decisions, and manage workload in general nursing units. a numerical scoring system to quantify nursing workload for each patient. Patient dependency factors (physiological stability, mobility, cognitive status) and nursing care activities (assessments, treatments, interventions).   • 11. All India Institute of Medical Sciences (AIIMS) System Developed by the All India Institute of Medical Sciences, New Delhi. Primarily used in tertiary care hospitals and medical colleges. Classifies patients into categories based on disease severity, complexity, and resource utilization Helps in determining the level of care required, resource allocation, and planning
  • 12. CASE MIX GROUPING (CMG)  Employed in private hospitals and some government hospitals.  Classifies patients based on their principal diagnosis, surgical procedures, age, gender, and comorbidities.  Assists in predicting patient outcomes, resource allocation, and reimbursement.
